Cerebral endothelial regeneration following experimental brain injury. Variation in the regeneration process according to the severity of injury.

It is still unknown when and in which area endothelial regeneration occurs after brain injury, and to what extent such changes depend on the severity of the injury. We have, therefore, studied bromodeoxyuridine (BrdU) uptake by regenerating endothelial cells in two different groups of rats given cold lesions using immunohistochemistry employing anti-BrdU monoclonal antibody, anti-factor VIII-related antigen antibody and anti-glial fibrillary acidic protein antibody. The earliest evidence for the presence of BrdU-positive endothelial cells (BrdU + end) was observed at 2 days after injury, the injured endothelial cells regenerating from the edge toward the center of the lesion in both groups. We considered that edema fluid could act as an important factor, since at 2 days post-injury BrdU + end were not in contact with macrophages and were always found in Evans blue-stained areas. Study of endothelial cell kinetics also confirmed that the repair of injured endothelial cells was intimately involved in the reconstruction of the blood-brain barrier, since the time of disappearance of BrdU + end coincided with the disappearance of Evans blue-stained areas. The difference in the process of endothelial regeneration was first apparent on the 3rd day, there being no difference at 2 days.

Definition of carcinoma of the gastric cardia.

This study concerns the definition of carcinoma of the gastric cardia. The topography of the esophagogastric mucosal junction (mucosal EGJ) was investigated with an endoscope in 182 patients who were free of hiatal hernias, ulcers, and neoplasms in the esophagus and stomach. The relationship between the EGJ and the cardiac gland area was then examined histologically in 56 resected specimens containing intact EGJs and cardia gland areas. Furthermore the cancerous center was determined; the shortest distance between the cancerous center and the EGJ and the amount of esophageal invasion were measured in 102 resected carcinomas located close to the junction; the carcinomas contained the EGJ and were good enough for pathohistological examination. The EGJ was located 0.5 - 1.0 cm proximal to the His angle (the gastric cardia) in radiological and endoscopic examinations. Histologically the cardiac gland area was found to straddle the EGJ at a range of about 1 cm proximal and 2 cm distal to the junction. Among the upper stomach carcinomas, most of the tumors (87.5%) whose center was located within 2 cm from the EGJ invaded the esophagus. In conclusion, carcinoma of the gastric cardia is defined as a lesion with its center located within 1 cm proximal and 2 cm distal to the EGJ.

Postoperative results of distal partial gastrectomy, selective vagotomy plus antrectomy, and selective proximal vagotomy for duodenal ulcers.

We investigated the postoperative results of distal partial gastrectomy, selective vagotomy plus antrectomy, and selective proximal vagotomy, to evaluate their effectiveness in the treatment of duodenal ulcers. The operative mortality of selective vagotomy plus antrectomy and selective proximal vagotomy seemed to be lower when compared to distal partial gastrectomy, although each procedure showed a sufficiently low mortality. The acid reduction rate was significantly lower after selective proximal vagotomy than after the other procedures (p less than 0.01). However, the rate of ulcer recurrence following selective proximal vagotomy tended to be higher compared with the other procedures. All three procedures showed good results according to Visick's grading and postoperative symptoms occurred in about 50 per cent of all patients, no matter what the procedure. The regaining of physical ability was significantly greater following selective proximal vagotomy than following distal partial gastrectomy (p less than 0.05) and the capacity to work was also better after vagotomy, particularly selective vagotomy plus antrectomy (p less than 0.05). Thus, although distal partial gastrectomy and selective vagotomy plus antrectomy proved superior regarding the low ulcer recurrence rate and acid reduction, while selective proximal vagotomy proved superior for improving the quality of life, on the whole, the three operations promise almost equivalent results.

Electron spin resonance spectrum of Tyr-151 free radical formed in reactions of sperm whale metmyoglobin with ethyl hydroperoxide and potassium irridate.

A five-line ESR spectrum was observed at room temperature in reactions of sperm whale metmyoglobin with ethyl hydroperoxide (EtOOH) at pH 9.5 and with potassium irridate at pH's 7.0 and 9.5. A spectrum with the same g value and hyperfine splitting constant appeared in a reaction of sperm whale apomyoglobin with potassium irridate and was assigned to a tyrosyl radical on the basis of optical spectrum data obtained under the same reaction conditions. It was concluded that this radical arose from Tyr-151 for the following reasons. (i) This ESR spectrum could not be observed in the reaction of horse heart metmyoglobin, which lacks Tyr-151. (ii) Sperm whale metmyoglobin no longer gave this spectrum when treated with tetranitromethane (TNM) under conditions in which approximately one tyrosine is lost in sperm whale metmyoglobin but none is lost in horse heart metmyoglobin. (iii) A complex ESR spectrum observed in the reaction of sperm whale metmyoglobin with EtOOH at neutral pH was found to be a mixture of this five-line spectrum and one arising from an unidentified free radical formed in the reaction of horse heart metmyoglobin with EtOOH. The TNM-treated sperm whale metmyoglobin gave the same ESR spectrum as that observed in the reaction of horse heart metmyoglobin with EtOOH.

A new procedure for the analysis and purification of naturally occurring anatoxin-a from the blue-green alga Anabaena flos-aquae.

Anabaena flos-aquae produces at least two neurotoxins termed anatoxin-a (ANTX-a) and -a(s) [ANTX-a(s)]. ANTX-a is a potent postsynaptic depolarizing neuromuscular blocking agent. Its structure is the secondary amine, 2-acetyl-9-azabicyclo[4.2.1]non-2-ene. Known isolation and analysis methods have not always given satisfactory results. Therefore, an improved procedure was developed for both isolation and analysis of ANTX-a which involves four steps; extraction, clean-up, separation and determination. Extraction was efficiently done with 0.05 M acetic acid, and a reversed phase. ODS (octadecylsilanized) and a cation exchanger (COOH) organosilans bonded to silica gels were applied to a clean-up step. Separation and determination were then performed using two TLC and HPLC chromatographies.

Effects of a novel calcium antagonist, KB-2796, on neurologic outcome and size of experimental cerebral infarction in rats.

Effects of a novel calcium 2+ antagonist, KB-2796, on neurologic outcome and size of infarction were studied in the rat model of focal cerebral ischemia. Neurologic deficits were examined from 1 to 24 hours after occlusion of the middle cerebral artery and size of infarction was measured at 24 hours postocclusion. When KB-2796 (10 mg/kg, IP) was given immediately after, or 1 hour after, middle cerebral artery occlusion, marked improvement in neurologic score was seen 1 to 3 hours postocclusion. When given immediately after middle cerebral artery occlusion, size of infarction was also markedly smaller when compared with the control rat. The present study suggests that the calcium 2+ antagonist KB-2796 has brain-protective effects against ischemia even with its administration after induction of ischemia.

Focal brain ischemia model in rats. An experimental study.

Using 74 male rats, anatomical variations of branching of the proximal segment of the middle cerebral artery were studied. In 36 rats (28 Sprague-Dawley rats and eight spontaneously hypertensive rats), the middle cerebral artery and/or its branches were occluded at various sites. Occlusion of the middle cerebral artery at the lateral border of the olfactory tract with or without simultaneous ligation of the ipsilateral common carotid artery rarely caused cerebral infarction in Sprague-Dawley rats. Occlusion of the olfactory branch in addition to the trunk of the middle cerebral artery caused large infarction of the pallium in five of eight Sprague-Dawley rats. Occlusion of the middle cerebral artery at the medial border of the olfactory tract caused infarction in the pallium and/or basal ganglia in five of six rats, and neurologic deficits were severe and persistent. Occlusion of the middle cerebral artery at the lateral border of the olfactory tract in spontaneously hypertensive rats caused large infarction with severe neurologic deficits in all eight animals. Possible factors responsible for such diverse differences and the relative value of each group as an experimental model of focal cerebral ischemia in rats were discussed.

Restriction map and alpha-amylase activity variation among Drosophila mutation accumulation lines.

The specific activities of alpha-amylase were measured for two sets of mutation accumulation lines, each set having originated from a different lethal-carrying second chromosome and SM1(Cy) chromosome and having been maintained by a balanced lethal system for about 300 generations. Significant variation was found to have accumulated among lines of both sets. Because of dysgenic crosses in the early generations of mutation accumulation, insertions or deletions of transposable elements in the Amy gene region were suspected of being the cause of this variation. In order to test this possibility, the structural changes in the 14 kb region of these chromosomes that includes the structural genes for alpha-amylase were investigated by restriction map analysis. We found that most part of the activity variation is due to replacements of a chromosomal region of SM1(Cy), including the structural genes for alpha-amylase, by the corresponding regions of the lethal chromosomes. One line also contained an insertion in this region but this line has an intermediate activity value. Thus, insertions of transposable elements into the Amy gene region were not found to be responsible for the new variation observed in alpha-amylase activity. If we remove those lines with structural changes from the analysis, the genetic variance of alpha-amylase specific activity among lines becomes non-significant in both sets of chromosomes.

Noninvasive study of critical thresholds of intracranial pressure and cerebral perfusion pressure for cerebral circulation and brain function.

To ascertain the critical thresholds of intracranial pressure (ICP) and cerebral perfusion pressure (CPP) for cerebral circulation and brain function, the extra- and intracranial haemodynamics and electrical brain responses were evaluated noninvasively with Doppler ultrasonography and multimodality evoked potentials (MEP's) in 50 patients with severe head injury. Both extra- and intracranial blood flow velocities changed monotonically depending on the changes in ICP and CPP. They were decreased when ICP increased to 20-30 mmHg and when CPP decreased to 40-50 mmHg. The changes in elasticity index of the pulse wave of the common carotid artery was proportional to those of blood flow velocities. The frequency and degree of abnormalities of MEP's were proportionally increased with the rise of ICP and reduction of CPP. When ICP increased to higher than 31 mmHg, MEP's were classified as moderately or severely abnormal in more than 76% of the recordings. These results indicate that noninvasive study by use of Doppler ultrasonography and MEP's can provide valuable information on critical brain ischaemia and brain dysfunction in patients with acute intracranial hypertension.

Early diagnosis of esophageal cancer. Analysis of 11 cases of esophageal mucosal cancer.

We reviewed 11 patients with esophageal mucosal carcinoma in various aspects to improve the early diagnosis of the disease. Eighteen lesions measuring 0.5 to 5.0 cm were confirmed histologically in the 11 cases. Histologically 10 of the 18 lesions were carcinomas in situ (ep cancer), and the other 8 lesions were carcinomas confined to the mucosa other than ep cancer (mm cancer); all 18 lesions were squamous cell carcinomas. Six (85.7%) of the seven mm cancers showed abnormal radiographic findings regardless of the size. Similarly these findings were noted on four of five (80%) ep carcinomas 2 cm or larger in size. All 15 lesions diagnosed before operation showed abnormal findings on conventional endoscopy regardless of the size and depth of transmural invasion. Morphologic change was observed in 9 lesions (53.3%), while 13 (86.7%) showed color change; most of the lesions (80%) were manifested as redness. Dyeing of the resected specimen with Lugol solution (Katayama Chemical Industries, Osaka, Japan) showed all 18 cancerous lesions as unstained areas. Among the 18 lesions, two lesions were unstained areas, which agreed with the areas determined histologically. An additional lesion was visible with dye endoscopy as an unstained area but it was not visible with radiography or conventional endoscopy. Dye endoscopy using Lugol solution is very important because it allows detection and evaluation of the extent of esophageal mucosal cancer.

Two populations of mouse lymphokine-activated killer cells separated by use of soybean agglutinin.

We separated lymphokine-activated killer (LAK) cells induced from spleen cells of BALB/c mice by culturing with recombinant interleukin-2 (rIL-2) into soybean agglutinin-positive (SBA+) and soybean agglutinin-negative (SBA-) fractions with a cell sorter at the time when LAK activity reached a maximum (day 3). We found that the cells with LAK activity were enriched in the SBA+ fraction. Analysis of cell surface phenotypes revealed that the SBA+ cells are of non-T cell origin, while the SBA- fraction consists of T cells. We also found that the large granular lymphocyte (LGL) fraction of spleen cells obtained with a Percoll gradient became SBA+ after culture for 3 days with rIL-2, whereas cells of high density did not. The change in SBA binding sites was also examined on C57BL/6 mouse spleen cells and we found that NK1.1+ non-T cells selectively acquire SBA binding sites at an early stage of activation with rIL-2. On the other hand, sorted SBA- cells gradually acquired SBA binding sites on extended culturing with rIL-2. These results suggest that the expression of SBA binding sites is related to the stage of cell activation with rIL-2, though cells of the NK-lineage became SBA+ at an earlier period of a culture with rIL-2 than cells of the T-lineage. Utilizing these findings, we separated NK-derived LAK cells by means of SBA-binding, and used the separated cells for adoptive immunotherapy for experimental pulmonary metastasis. We found that SBA(+)-NK cell-derived LAK cells showed stronger activity for the inhibition of experimental pulmonary metastasis than SBA(-)-T cell-derived LAK cells.

Glucose metabolism and rate constants in Alzheimer's disease examined with dynamic positron emission tomography scan.

Glucose metabolic rate constants in patients with Alzheimer's disease were analyzed using dynamic positron emission tomography with [18-F] fluoro-2-deoxyglucose (FDG). The cerebral metabolic rate of glucose (CMRG) was calculated using obtained rate constants, as well as the autoradiographic method. The half-life and distribution volume of FDG between the blood and the brain tissue were also calculated from obtained metabolic rate constants. The most severely affected metabolic step was the phosphorylation of glucose in the parietotemporal regions. The distribution volume of FDG showed no remarkable deviation from normal controls, while the half-life of FDG was found to be longer in the parietal and temporal regions. The CMRG from rate constants and that from the autoradiographic method showed no remarkable differences.

[Epidemiological survey of group A hemolytic streptococci isolated in Osaka during the 21-year period between 1967 and 1987. Part I. Annual change of epidemic serotype].

During the 21-year period between 1967 and 1987, a total of 6157 strains of group A streptococci isolated in Osaka were type-classified by T-agglutination method. The results were summarized as follows. 1. Sixteen serotypes were differentiated by the T-method during the course of investigation. The most common serotype was T-12 (37.1%), followed by T-4 (13.4%) and T-1 (10.4%). 2. The most dominant serotype was T-12 during 1968-1978, and other relatively frequent serotypes were T-4 in 1971, T-1 in 1972 and T-3 in 1974 throughout the period. Though T-12 was the main serotype in the isolates thereafter, other serotypes such as T-4 in 1979 and T-3 in 1985 also became prevalent, and they occupied the highest percentage. But they declined rapidly, and their duration period was relatively short. After 1976, no definite serotype showed the long-term epidemics, while several types (T-3, T-4 and T-12) showed short-term epidemics lasting a few years. 3. Group A streptococci were isolated most frequently from the patients aged 4-8 years, whereas the isolation rates were extremely lower from the 0- to 2-year-old children. Therefore, younger children seemed to have something which made it difficult to be infected with group A streptococci.

[Epidemiological survey of group A hemolytic streptococci isolated in Osaka during the 21-year period between 1967 and 1987. Part II. Antibiotic susceptibility].

During the 21-year period between 1967 and 1987, a total of 4678 strains of group A hemolytic streptococci isolated in Osaka were examined for drug sensitivity. The results were summarized as follows. 1. All strains were sensitive to the beta-lactam group of antibiotics (benzylpenicillin and cephaloridine). Resistant strains (M.I.C. greater than or equal to 25 micrograms/ml) to tetracycline (TC), chloramphenicol (CP) and erythromycin (EM) were isolated in this study period. The incidence of TC-resistant strains in the whole isolates has not much varied through the whole period, in spite of the variation among several T-type. But those of CP- and EM-resistant strains varied greatly by year. 2. Before 1971, a small number of resistant strain was found in 1969 and 1970. Since 1972, increase of such strains had been seen and the incidence of CP-resistant strains reached the highest peak (32.5%). It showed a gradual decrease since 1976 and was 1.1% in 1987. 3. None of EM-resistant strains was isolated before 1972. The incidence of EM-resistant strains exhibited a steep rise in percent since 1973 and reached the highest value showing a two-peak-pattern in 1975 (36.1%) and in 1980 (36.0%). After reaching its peak it decreased gradually and was 2.9% in 1987. 4. Remarkable increase of the multiple drug resistant strains began to be observed since 1972 to 1973, and mainly T-12 and T-4 showed EM. TC. CP- and TC. CP-resistance, respectively. The phenomenon of multiple drug-resistance culminated during 1975-1977, followed by gradual decrease thereafter.(ABSTRACT TRUNCATED AT 250 WORDS)

[Studies on bacterial agents in acute diarrheal disease (1985-1987)].

A three-year epidemiological study (from January 1985 to December 1987) was carried out on sporadic cases of acute diarrhea. A total of 2889 fecal specimens in Cary-Blair transport medium were examined for bacterial enteric pathogens, and 832 strains of fifteen species were isolated from 739 specimens, 73 patients having two or more pathogens. C. jejuni shared 51.7%, Salmonella spp. 18.3%, V. parahaemolyticus 10.3%, and Aeromonas spp. 15.7% of total fecal specimens. Isolation rates of C. jejuni and Salmonella spp. in children under the age of fifteen years (19.3%, 6.4%) were higher than those of older years (9.8%, 3.9%), respectively. Isolation of C. jejuni decreased to 24% (12/50) during 2-4 days storage at room temperature in Cary-Blair transport medium, which showed the necessity of rapid plating for isolation of C. jejuni from fecal specimens. Incidence of A. caviae in children up to ten years of age was significantly higher as compared with those of other Aeromonas species. Desoxycholate-hydrogen sulfide-xylose-agar (DHXA) was used for direct plating technique and for plating after enrichment with alkaline peptone water (without NaCl), which was found suitable as an enrichment medium for Aeromonas spp. Enterohemorrhagic E. coli (EHEC) O157:H7 was isolated from 3 patients by using desoxycholate-hydrogen sulfide-sorbitol-agar (DHSA).

Toxicity of Microcystis species isolated from natural blooms and purification of the toxin.

Microcystis strains (2 toxic and 18 nontoxic to mice) were isolated from toxic waterblooms that had been collected from Lake Kasumigaura, Ibaraki Prefecture, Japan, in August 1985. Thirteen of the strains (2 toxic and 11 nontoxic) were Microcystis aeruginosa, 2 (nontoxic) were Microcystis wesenbergii, and the other 5 were difficult to identify. Six (1 toxic and 4 nontoxic M. aeruginosa and 1 M. wesenbergii) of these 20 strains were established as axenic cultures. A toxic and axenic strain of M. aeruginosa, K-139, was used to study the relationship between growth conditions and toxicity. Cells in early-to-mid-log phase showed the highest toxicity (50% lethal dose, 7.5 mg of cells per kg of mouse), and maximum toxicity was not affected by growth temperatures between 22 and 30 degrees C. Purification and characterization of the toxins from K-139 cells were also conducted, and at least two toxins were detected. One of the toxins (molecular mass, 980 daltons) has not been reported previously. The main target of the toxin in mice was the liver. Marked congestion and necrosis in the parenchymal cells around the central veins of the liver were observed microscopically in specimens that had been prepared from the mice with acute toxicity after injection with the toxin.
