[Two cases of interstitial pneumonitis with marked increase of tumor-associated carbohydrate antigens in serum].

Tumor-associated carbohydrate antigens, CA19-9 and SLEX have been used clinically as markers for malignancy. However, it is also known that these antigens are frequently elevated in the serum of patients with benign lung diseases. We have experienced two cases of interstitial pneumonitis with marked increase of carbohydrate antigens in serum, the level of which changed according to their clinical course. Case 1. A 64-year-old woman was admitted because of a cough and exertional dyspnea. Despite treatment with various antibiotics and prednisolone, she died of respiratory failure approximately two months after admission. Her CA19-9 and SLEX in serum elevated from 500 U/ml to 5506 U/ml and 167 U/ml to 1187 U/ml respectively in accordance with clinical deterioration. Autopsy revealed no malignancy. Case 2. A 57-year-old woman, who had been suffering from interstitial pneumonitis associated with rheumatoid arthritis, was admitted with a cough and fever. She responded to prednisolone therapy, however two years later she readmitted because of exacerbation and died of respiratory failure. The initial CA19-9 level in serum was 876 U/ml and dropped to 42 U/ml with prednisolone therapy. The serum antigen level again increased during the period of exacerbation, and showed 133 U/ml immediately before death. An immunohistochemical study of CA19-9 and SLEX in various tissues obtained by autopsy was performed in case 1. The distribution of these antigens in tissue was similar to that of normal individuals. The exceptions were the expression of these antigens on epithelial cells of microscopic honeycombing and on mucinous exudates in air spaces.

[Observation of bleeding sites by fiberoptic bronchoscopy in a case of vasculo-Behçet's disease with pulmonary involvement].

A 45-year-old man complained of fever and bloody sputum. Chest X-ray films showed a round opacity in the left hilum. He developed massive hemoptysis (500 ml) and emergency embolization of the bronchial artery was performed. Pulmonary artery aneurysm and thrombosis were detected angiographically, and fiberoptic bronchoscopy revealed a reddish irregular eminence of the left main bronchus and lingulate++ ++ bronchus. These sites could have been the bleeding sites. Because of recurrent hemoptysis, left pneumonectomy was performed 26 days after the massive hemoptysis. Pathological findings showed panarteritis, luminal thromboembolism of the pulmonary artery and markedly dilated vessels of the bronchial wall. He developed genital ulceration after the operation and these findings suggested incomplete vasculo-Behçet's disease with rare pulmonary involvement. The disease is now well-controlled by corticosteroid therapy 27 months after the operation.

[The comparison of drug susceptibility of antituberculous agents against colonizing M. intracellulare and infectious M. intracellulare].

The susceptibility of antituberculous agents against colonized M. intracellulare which were isolated from patients with pulmonary disease and infectious M. intracellulare (pathogens) isolated from atypical mycobacterial culture was investigated. Aminoglycosides were more potent against colonized organisms than against pathogens. Isoniazid, ethambutol and rifampicin showed less potent antimicrobial activity against both colonized organism and pathogens as compared to aminoglycosides. On the contrary susceptibility of cycloserine against colonized organisms was as potent as against pathogens.

[A clinical phase I study on intramuscular imipenem/cilastatin sodium].

The safety and pharmacokinetics of imipenem/cilastatin sodium (IPM/CS) were evaluated in comparative studies using single intramuscular injection, intravenous infusion or multiple intramuscular administration. The studies were done employing 30 healthy volunteers. Adverse effects were observed in 5 of 18 volunteers in the single intramuscular dose study. One of them complained of mild itching and mild pain at the injection site, and the other 4 volunteers had mild pain at the injection sites. With the other methods of administration, no adverse effects were observed. No other abnormal physical findings nor abnormal laboratory test values were observed in any of the studies. Imipenem (IPM) was absorbed rather slowly through the muscles, resulting in a low maximum plasma concentration (Cmax) and a prolonged half life (T1/2) upon intramuscular injection compared to the results obtained upon intravenous infusion. The total areas under the curves obtained with these 2 methods were similar, however. Cilastatin (CS) was absorbed rapidly after intramuscular injection, and the Cmax obtained was higher than that obtained for IPM. The T1/2 and the AUC of CS obtained with intramuscular injection were similar to those obtained with intravenous infusion. Cumulative urinary recovery rates obtained with these 2 different routes of administration were not different. Detectable urinary levels of IPM were maintained much longer upon intramuscular injection than upon intravenous infusion. In the multiple dose study, neither IPM nor CS showed tendency to accumulate.

[A case of herpes simplex encephalitis with periodic lateralized epileptiform discharges].

A case of a 50-year-old male with herpes simplex encephalitis was reported. An EEG examination revealed periodic lateralized epileptiform discharges (PLEDs) on the right hemisphere on April 17, 1990. Another EEG performed on April 26 showed PLEDs on the left hemisphere, dominantly on the temporal lobe. On and after May 2, PLEDs have not been detected. SPECT showed hyperperfusion in the right temporal and occipital lobe son April 26. T1 weighted MRI examined on April 18 showed low signal intensity and T2 and proton weighted imagings revealed high signal intensity in the greater part of the right temporal lobe and a similar abnormality in a part of the left temporal lobe. Although the origin of PLEDs is still controversial, these findings suggest that, at least in the case of herpes simplex encephalitis, PLEDs appear in the early progressive stage of the inflammatory lesion.

The development of a treatment model for lymph node metastasis in nude mice transplanted with rat fibrosarcoma clone A.

We have tried to establish a model for lymph node metastasis in nude mice transplanted with clone A of methylcholanthrene-induced rat fibrosarcoma. Tumor cells transplanted either subcutaneously or intrafootpad metastasized exclusively to the lymph nodes in nude mice treated with antiasialo GM1 antiserum. In the experiments of intrafootpad transplantation, all the mice died of disseminated lymph node metastasis if the leg was not resected, or was resected more than 3 weeks after the transplantation of clone A. Some mice whose legs were amputated 1 or 2 weeks after transplantation could be cured by amputation alone. The maximum effective dose of in vitro active drug could improve the survival in mice, and 3/5, 4/5, and 3/5 mice treated with MMC, ACNU and CDDP respectively were cured even if their legs were amputated 3 weeks after transplantation. This tumor-bearing animal system was considered to provide a valuable experimental model for lymph node metastasis.

[Analysis of cellular and biochemical contents of broncho-alveolar lavage fluid from patients with pulmonary tuberculosis].

To evaluate the local immune response in lung and activity of pulmonary tuberculosis. Broncho-alveolar lavage (BAL) was performed on 15 patients with pulmonary tuberculosis. Patients consisted of nine in the active stage and six in the inactive stage. Acid-fast bacilli (AFB) in BAL fluid (BALF) were found in six out of nine active stage patients (BAL-AFB positive group). The results obtained were as follows; 1) The number of total cells in BALF increased significantly in BAL-AFB positive group. BALF from two patients of this group, who were in the very early stage, revealed that the lymphocytes increased predominantly whereas neutrophils were dominant in BALF from the other four patients. 2) No case showed increase of B cells in BALF. Lymphocytes surface markers were compared between BAL-AFB positive and negative groups. No difference was found in the numbers of OKT3, OKT4 and OKT8. The positive rates of OKIal, and OKT4/8 ratio were both increased in BALF compared to the BAL-AFB positive group. 3) The numbers of OKT4 and OKT4/8 ratio in peripheral blood, which were taken simultaneously with BAL, were decreased in patients in the active stage. 4) Protein, albumin, IgG, IgA, lysozyme and alpha 1-antitrypsin levels in BALF were all increased in tuberculous patients. No correlation between the activity of tuberculosis and levels of protein, albumin, IgG and IgA was observed. Lysozyme level increased in the inactive stage of patients. On the other hand alpha 1-antitrypsin level increased in the active stage. Fibronectin level in BALF from patients was lower than that from normal controls. 5) Smoking habit had no influence on the above results. 6) Only one case showed the appearance of new lesions after BAL procedure as a complication.

[A case of malignant lymphoma arising from chest wall in chronic empyema].

A 70-year-old male was admitted with complaints of anterior chest pain and tumor formation. He has a history of pulmonary tuberculosis and was performed artificial pneumothorax therapy 37 years ago. The diagnosis on admission was perforation of empyema cavity into the chest wall. But thoracotomy revealed that the chest tumor arising from chronic empyema wall was a malignant lymphoma. Decortication and extirpation of the tumor were performed with combined partial resection of chest wall. Immunohistological examination showed that the histology was diffuse large T cell type of Non-Hodgkin's malignant lymphoma. The patient has been alive and well for 24 months postoperatively.

[Right ventricular function evaluated by thermodilution method in patients with chronic pulmonary diseases].

The right ventricular ejection fraction (RVEF) is an objective evaluation of the right ventricular systolic function. Recently a rapid-response thermister pulmonary artery catheter which measures RVEF and stroke volume (SV) was introduced. With this new method, RVEF and the right ventricular volumes (RVV) were measured in patients with chronic pulmonary diseases (CPD) and normal subjects. In addition, to evaluate the validity of this method, the data was compared with Kr81m perfusion method. The RVEF and the RVV of CPD patients were also compared with pulmonary hemodynamic data. There was a good correlation between the RVEFs obtained by the Kr81m perfusion method and the TD method. (r = 0.71, p less than 0.001). There was a tendency towards a low RVEF in patient with CPD with pulmonary hypertension (PAm greater than 20 mmHg) or high pulmonary vascular resistance (PAR greater than 160 dyne/sec/cm-5) and the RVV increased in the same group. These results suggested that the right ventricle was unable to respond to the increase of afterload in the CPD group and the evaluation of RVEF and RVV in patients with CPD using the new TD method was valuable for assessing the right ventricular function.

Interleukin-1 mRNA-expressing macrophages in human chronically inflamed gingival tissues.

Interleukin-1 (IL-1)-producing cells in inflamed gingival tissues obtained from patients with periodontitis were examined by combined immunohistochemistry and in situ hybridization. Macrophages, T cells, B cells, and endothelial cells were visualized in the lesions by the immunoperoxidase method using specific antibodies to each cell type. Subsequent in situ hybridization using 35S-labeled synthetic oligonucleotide probes complementary to human IL-1 alpha and IL-1 beta mRNA showed IL-1 transcripts in macrophages predominantly but not in T cells or B cells. Neither fibroblasts nor keratinocytes contained the IL-1 message to any significant extent. Endothelial cells also were essentially negative for IL-1 messages. These findings indicate that IL-1 is produced principally by macrophages in inflamed gingival tissues of humans.

[The usefulness of pleural biopsy in benign or malignant pleurisy].

Retrospective studies of pleural biopsy, cytology and ADA in pleural effusion were performed in 116 patients with pleural effusion between 1980 and 1988. Pleural malignant disease was diagnosed in 25 patients (75.8%) by cytology, in 19 patients (57.6%) by pleural biopsy. Thus, cytology should be performed first in patients with pleurisy. Both of cytologic study and CEA in pleural effusion were negative in 3 cases of squamous cell carcinoma. Tuberculous pleuritis was diagnosed in 24 patients (50.0%) by pleural biopsy, in 5 patients (10.4%) by isolation of Mycobacterium tuberculosis. Both pleural biopsy and adenosine deaminase activity (ADA) were examined in 19 cases of tuberculous pleuritis and ADA was elevated in 16 patients (84.2%). These data suggested that pleural biopsy was useful for diagnosis of pleuritis and the combination of cytology, tumor markers and ADA with biopsy improved diagnostic rates of pleuritis.

Stretch may enhance the release of endothelium-derived relaxing factor in rabbit aorta.

The effect of vascular stretch on the release of EDRF was studied by measuring tissue cGMP levels of rabbit. Aortic rings of rabbit were quick-frozen in liquid nitrogen during varying resting tensions, and cGMP contents were determined by radio-immunoassay. The tissue cGMP levels significantly elevated with the increase in resting tension in endothelium-intact rings, but not in endothelium-denuded rings. Deprivation of extracellular calcium abolished the stretch-induced elevation of tissue cGMP levels in endothelium-intact segments. These stretch-induced endothelium dependent tissue cGMP elevations were unaffected by Ca2+ channel blockers, nicardipine and diltiazem. Data suggest that vascular stretch may release EDRF via mechanism dependent on extracellular calcium, but probably not through voltage-dependent calcium channel.

Sandwich ELISA assay for quantitative measurement of SP-40,40 in seminal plasma and serum.

SP-40,40 was purified from human plasma by PEG fractionation, DEAE-Sephacel, Phenyl-Toyopearl 650M, Bio-Gel A-0.5m and hydroxylapatite chromatographies. Three monoclonal antibodies (IF12, IID9 and IVF4) to this protein were prepared: IF12 and IID9 were specific for the beta subunit and IVF4 for the alpha subunit. The concentrations of SP-40,40 in seminal plasmas and sera were determined using a sandwich ELISA method. The results showed that the average concentrations of SP-40,40 were 438 +/- 285 micrograms/ml in seminal plasmas and 111 +/- 50 micrograms/ml in sera of normal donors. SP-40,40 concentrations in seminal plasmas of Klinefelter and excretory azoospermia patients were similar to those of normal donors. However, those of oligozoospermia and idiopathic azoospermia patients were about half the normal value.

[Stereoscopic MR angiography of the abdomen].

We have developed a new method of stereoscopic MR angiography of the abdomen using gradient echo images. Consecutive coronal scanning of the abdomen was performed by fast scan during breath hold. These images were overlapped by means of MR angiography. Then two slightly different views that can give a true stereoscopic effect were constructed. The more precise spatial relationship of the abdominal vessels can be visualized with our new method.

Roles of CD4+ and CD8+ cells, and the effect of administration of recombinant murine interferon gamma in listerial infection.

Studies were made on the effects of in vivo administration of anti-CD4 mAb, anti-CD8 mAb, or a combination of both mAbs on multiplication of bacteria, the levels of serum transaminases, and mortality in mice infected with Listeria monocytogenes. Results showed that in sublethal infection, CD8+ cells enhanced the peak of bacterial multiplication and liver cell necrosis, and CD4+ cells suppressed CD8+ cell-mediated enhancement. Results also showed that either CD4+ or CD8+ cells were necessary for, and capable of, mediating clearance of the bacteria. CD8+ cells were more efficient than CD4+ cells, but for optimal clearance both were necessary. In lethal listeriosis, treatment of mice with anti-CD8 mAb or a combination of both anti-CD4 and anti-CD8 mAbs, but not anti-CD4 mAb only, protected mice from death by decreasing multiplication of bacteria in the liver and spleen after a peak of approximately 10(8) CFU, and lowering the elevated serum levels of transaminases. These findings indicated that CD8+ cells were responsible for causing irreversible systemic Listeria infection and severe liver necrosis. In lethal listeriosis, administration of rMuIFN-gamma markedly prolonged survival by decreasing multiplication of bacteria and promoting recovery from liver necrosis.
