[A case of rapidly grown pulmonary blastoma].

A 78-year-old woman who had been in a local hospital with a complaint of cough and chest pain was referred to our hospital because a mass 12 cm in size was found in her right lung by a chest X-ray and CT. Within 3 weeks, the tumor rapidly developed to 19 cm in size. Malignant schwannoma of the lung was suspected by a percutaneus lung biopsy and the right upper and middle lobectomy was performed. Histological analysis of the tumor showed a biphasic structure with epithelial and mesenchymal component which was diagnosed pulmonary blastoma. Pulmonary blastoma is very rare, but it may be of benefit for the thoracic surgeon to establish methods of diagnosis and treatment of this disease.

Isolation and identification of a diuretic hormone from the mealworm Tenebrio molitor.

A diuretic hormone of unusual structure was isolated from extracts of whole heads of the mealworm Tenebrio molitor. The hormone is a 37-aa peptide of 4371 Da, with the sequence SPTISITAPIDVLRKTWEQERARKQMVKNREFLNSLN. This peptide increases cAMP production in Malpighian tubules of T. molitor. The amino acid sequence reveals that this peptide is a member of the family of sauvagine/corticotropin-releasing factor/urotensin I-related insect diuretic hormones. The C-terminal sequence of this peptide is quite different from other members of this family, which have a hydrophobic C terminus (isoleucinamide or valinamide). When aligned comparably, T. molitor diuretic hormone has a more hydrophilic C terminus, leucylasparagine (free acid). In contrast to all other known diuretic hormones of this family, this peptide has exceptionally low stimulatory activity on cAMP production in Malpighian tubules of Manduca sexta. However, at nanomolar concentrations it stimulates cAMP production in Malpighian tubules of T. molitor. Diuretic hormones of this family have been isolated previously from Lepidoptera, Orthoptera, Dictyoptera, and Diptera. This appears to be the first diuretic hormone isolated from a coleopteran insect.

Spinal cord monitoring of the ventral funiculus function. Analysis of spinal field potentials after galvanic vestibular stimulation.

STUDY DESIGN: This study was designed to examine the possibility of a new spinal cord monitoring method using galvanic vestibular stimulation to monitor the function of the ventral funiculus of the spinal cord. OBJECTIVES: To settle the problems of previous monitoring methods by using galvanic vestibular nerve stimulation, which is highly selective for monitoring the function of the ventral funiculus of the spinal cord. SUMMARY OF BACKGROUND DATA: Although various spinal cord monitoring methods have been used, there are still problems because potentials recorded by these methods do not reflect selectively the function of the ventral funiculus of the spinal cord, which is vulnerable during anterior spinal surgeries. METHODS: In anesthetized cats, field potentials evoked by galvanic stimulation of the labyrinth were recorded from the epidural space of the spinal cord. The origin of these potentials was determined by mapping field potentials from within the upper cervical spinal cord using a micropipette and by examining the effect of sectioning the brainstem on the evoked potentials. RESULTS: The spinal cord potentials evoked by galvanic stimulation between the bilateral labyrinths could be recorded from the epidural space, and these potentials mainly originated from the ventral and ventromedial funiculus of the spinal cord. The latency and intraspinal distribution of the evoked potentials and the effect of sectioning the medial longitudinal fascicle on the evoked potentials indicated that the earliest component of the evoked potentials reflects mainly the activity of the vestibulospinal tract. CONCLUSIONS: Recording spinal cord potentials evoked by galvanic vestibular stimulation from the epidural space appears to be a potential technique to monitor the functional state of the ventral funiculus during anterior spinal surgeries.

A new method of multisegment motor pathway monitoring using muscle potentials after train spinal stimulation.

STUDY DESIGN: Evoked muscle and nerve action potentials after spinal stimulation for intraoperative monitoring were investigated using a modified stimulation technique. Animal experiments and clinical application were performed. OBJECTIVES: To contrive a useful method of intraoperative motor pathway monitoring under inhalation anesthesia. SUMMARY OF BACKGROUND DATA: Many different kinds of procedures have been reported. No reliable method that reflects pure motor tract function has been established. METHODS: Characteristic of our stimulating technique was the use of numbered consecutive pulses ("train stimulation"). In 16 cats, optimum condition of train stimulation, effects of anesthetic agents, and conductive pathway were examined. In 35 patients, muscle potentials evoked by train stimulation were recorded, and clinical usefulness was evaluated. RESULTS: In the experimental study, the optimum stimulus condition was determined 1 ms interstimulous interval train of five pulses. Conductive pathway of this method was identified as a lateral column by selective spinal cord transection. In the clinical application, by using train stimulation, multisegmental muscle potentials were obtainable even using inhalation anesthetics. CONCLUSIONS: The facilitative effects of train stimulation, attributed to temporal summation, are considered to overcome the suppression of inhalation anesthesia. The evoked muscle potentials by train spinal stimulation reflect the functions of pure motor tract and is the only, extremely efficient method for intraoperative motor pathway monitoring.

[Reevaluation of hypothermia as protective measures against cerebral ischemia].

Two patients underwent carotid artery ligation for the purpose of surgical hemostasis either under hypothermia or under normothermia. One was 56-year-old man with rupture of brachiocephalic artery following esophagectomy. The bilateral common carotid arteries were ligated for bypass-grafting between the right internal carotid artery and the left common carotid artery for 35 min under surface-induced hypothermia at a rectal temperature of 30.7 degrees C. No postoperative neurological deficit developed. The other was 40-year-old woman with a malignant parotid tumor involving left internal carotid artery. The left carotid artery was ligated for 20 min at a rectal temperature of 37.3 degrees C. Cerebral ischemia developed postoperatively. Mild to moderate hypothermia should be reevaluated as one of useful measures to protect the brain from cerebral ischemia.

Release of arachidonic acid via Ca2+ increase stimulated by pyrophosphonucleotides and bradykinin in mammary tumour cells.

The relationship between the increase of intracellular Ca2+ and the release of arachidonic acid by bradykinin and pyrophosphonucleotides was studied in cultured mammary tumour cells, MMT060562. Bradykinin, ATP, UTP and UDP induced an increase of intracellular Ca2+ and the release of arachidonic acid from phospholipids into the extracellular fluid. Release of arachidonic acid was also induced by the application of the Ca2+ ionophore, A23187. Liberation of arachidonic acid by bradykinin and ATP was reduced by mepacrine, a blocker of phospholipase A2 and W-7, a calmodulin antagonist. It is suggested that the increase in cytosolic Ca(2+)-induced release of arachidonic acid occurs through activation of calmodulin-dependent phospholipase A2.

Reduced neck movement after operations for cervical spondylotic myelopathy.

Complaints resulting from reduced neck movements were investigated in 50 patients who had operations for cervical spondylotic myelopathy. Seventy per cent had difficulty in performing 11 basic movements of daily living. Lateral bending or rotation were more difficult than flexion and extension. To look backwards was the most difficult movement. Complaints were highest among those in whom more than three levels of fusion had been carried out.

Evaluation of anterior cruciate reconstruction reinforced by the Kennedy ligament augmentation device. An arthroscopic and histological study.

Fifty anterior cruciate ligament reconstructions with the Kennedy ligament augmentation device were examined arthroscopically and histologically 6 to 61 months after operation. The autogenous segments used were the quadriceps tendon in 13 and the semitendinosus tendon, with or without gracilis, in 37. The arthroscopic findings were rated as good in 31, fair in 15 and poor in 4, and there was correlation between these grades and KT-1000 measurements. The histological findings were graded as good in 17, fair in 26 and poor in 4; these did not correlate with the arthroscopic grades but with the time interval after reconstruction. Maturation of the autogenous augmented segment was found to progress with time, but took more than 3 years to become complete.

Involvement of intracellular calcium ions in the release of the fluorescent dye calcein by cholinergic and alpha-adrenergic agonists from rat parotid acinar cells.

Effects of cholinergic and adrenergic agonists on the secretion of the fluorescent dye calcein were examined to clarify the involvement of calcium ions in the secretion of calcein from acinar cells dispersed from the rat parotid gland. Addition of carbachol (CCh) and noradrenalin (NA), but not isoproterenol (IPR), enhanced the net release of calcein from acinar cells during the subsequent 10 min in a dose range from 10(-8) M to 10(-6) M. The net release of calcein reached a maximum 7 min after the addition of CCh. The release of calcein was suppressed by the simultaneous additions of atropine with CCh, or phenoxybenzamine with NA. Addition of CCh induced a sustained dose-dependent increase in the intracellular levels of calcium ions, ([Ca2+]i). Addition of NA at 10(-6) M increased [Ca2+]i. Phenoxybenzamine completely inhibited the NA-induced increase, but propranolol did not. The removal of extracellular calcium ions did not influence the release of calcein induced by 10(-6) M CCh, but it abolished the sustained increase in [Ca2+]i. The transient increase in [Ca2+]i induced by CCh was observed in the absence of extracellular calcium ions. A calcium ion chelator, 1,2-bis(2-aminophenoxy)ethane-N,N,N',N'-tetraacetic acid (BAPTA) inhibited the CCh-induced release of calcein. The calcium ionophore, A23187 (2.5 x 10(-6) M), but not 10(-3) M dibutyryl cAMP, evoked the release of calcein. It also increased [Ca2+]i. Removal of extracellular calcium ions suppressed the A23187-induced release of calcein.(ABSTRACT TRUNCATED AT 250 WORDS)

Primary large choroid plexus papillomas in the cerebellopontine angle: radiological manifestations and surgical management.

Two cases of primary choroid plexus papilloma originating in the cerebellopontine angle (CPA) are reported. Both papillomas were encapsulated and strongly adhering to a choroid plexus tuft protruding from the foramen of Luschka. Successful removal of the tumour was achieved in each case. Surgical strategy, neuroradiological manifestations, and the differential diagnosis are discussed.

Parasellar Aspergillus granuloma extending from the sphenoid sinus: report of two cases.

BACKGROUND: Sphenoid sinus aspergillosis is a rare disease known to show an aggressive course with high mortality. Early diagnosis, though difficult, is required to prevent lethal fungal meningoencephalitis. CASE REPORT: We describe two cases of parasellar Aspergillus granuloma extending from the sphenoid sinus clinically indistinguishable from intracranial neoplasms. In the first patient, the fungus colony was visualized by computed tomography (CT) and magnetic resonance imaging (MRI) as a calcified concretion and total removal was curative. In the second patient, partial removal and subsequent antifungal therapy had minimal effect. CONCLUSIONS: The prognosis of the patients with this disease depends on prompt surgical treatment before intradural invasion occurs, and CT and MRI are useful diagnostic maneuvers for detecting calcified Aspergillus colonies.

The effects of tibial tunnel placement and roofplasty on reconstructed anterior cruciate ligament knees.

Seventy-five anterior cruciate ligament (ACL) reconstructions augmented with the Kennedy Ligament Augmentation Device were evaluated according to classification of tibial drill-hole position on the basis of the anatomic landmarks of the ACL by two-dimensional radiographic imaging of the fully extended knee. The effects of roofplasty to avoid graft impingement were also assessed. The tibial drill-hole position was classified in relation to the medial intercondylar tubercle on anterior-posterior (AP) view, and in relation to Blumensaat's line (B-line) on lateral view. Arthroscopic evaluation of the ACL and incidence of chronic synovitis as well as Lysholm knee score, manual knee tests, knee extension and flexion angles, and knee tester measurements were performed. The results indicated that the knee joints in which the tibial drill hole was positioned laterally from the medial intercondylar tubercle or in which the tibial drill hole was positioned anteriorly to the B-line showed a tendency to develop more postoperative chronic synovitis. The knees in which the tibial drill hole was positioned anteriorly to the B-line also showed larger AP laxity. There was no difference between the non-roofplasty and roofplasty groups.

A novel protein phosphatase-1 inhibitory protein potentiated by protein kinase C. Isolation from porcine aorta media and characterization.

A novel phosphorylation-dependent inhibitory protein (IP) of porcine aorta myosin light chain phosphatase (PA-MLCP) was purified to homogeneity from porcine aorta media. The molecular mass of IP was 20 kDa. IP phosphorylated by endogenous potentiating kinase (IP-K) inhibited not only PA-MLCP activity, but also that of the catalytic subunit of protein phosphatase-1. The amino acid sequence of a peptide derived from IP phosphorylated with IP-K, RHARVT*VK, shared one of the consensus sequences phosphorylatable by protein kinase C (PKC), where T* was phosphorylated. IP was phosphorylated by PKC and the phosphorylated product inhibited PA-MLCP as strongly as IP phosphorylated with IP-K.

Lower leg fracture with Parkes-Weber syndrome complicated by disseminated intravascular coagulation.

Reports of Parkes-Weber syndrome complicated by disseminated intravascular coagulation (DIC) are rare in the orthopaedic literature. This is a case report of a 23-year-old man who had this syndrome and who sustained a lower-leg fracture complicated by the DIC. Open reduction was not attempted because the DIC worsened after manual reduction. Amputation was rejected by the patient. Three months of continuous infusion of heparin and replacement therapy with fresh frozen plasma was done. Cast immobilization without further reductions was continued. The DIC improved and union of the fractures was observed at 2 years and 3 months after injury.

Regional differences in the contractile and intracellular Ca2+ responses of the guinea-pig vas deferens to neurotransmitters and excess K+.

The contractile responses to various concentrations of noradrenaline (NA), adenosine triphosphate (ATP), acetylcholine (ACh) and excess external K+ in the epididymal, middle and prostatic portions of the guinea-pig vas deferens were investigated by measuring the isotonic contraction, and the intracellular Ca2+ concentration ([Ca2+]i) using fura-2 fluorescence. In the epididymal portion, the contractions evoked by each of these agonists were biphasic, comprising a transient followed by a tonic phase. In the middle portion, NA and ACh evoked biphasic contractions, whereas the ATP-induced contraction was an almost monophasic transient. In contrast, in the prostatic portion, only transient contractions were evoked by ACh and ATP, while the NA-induced contraction was oscillatory. The responses to each of the neurotransmitters in the three portions were not affected by pretreatment with TTX. The maximal responses of tonic contraction in response to each of the neurotransmitters were largest in the epididymal portion, decreased in the middle and were almost absent in the prostatic portion. These regional differences in the contractile properties of the vas deferens were also evident upon stimulation with excess external K+. As with the contractile responses, regional differences in the [Ca2+]i increases were observed during exposure to all the stimulants. It is suggested that the regional differences in the contractile responses of the vas deferens to various neurotransmitters and excess external K+ may involve variation in the mechanisms of Ca2+ homeostasis and in the sensitivity of the contractile apparatus to intracellular Ca2+ ions.

Association between high serum total IgE levels and D11S97 on chromosome 11q13 in Japanese subjects.

The genetic linkage of atopy to chromosome 11q13 through maternally derived alleles has been previously reported. Linkage analysis in Japanese families did not confirm the existence of a major gene for atopy at this locus under the model of autosomal dominant inheritance. However, we observed a significant association between serum total IgE levels and genetic markers at this locus both in 14 Japanese atopic families and in 120 unrelated Japanese subjects. We detected eight alleles at the D11S97 locus and eight alleles in the CA/GT repeat region in the fifth intron of the Fc epsilon RI beta gene. A significantly increased frequency of the D11S97/PstI 0.96 kb allele was observed in the chromosomes of the subjects with high serum total IgE levels both in the family study (p < 0.001) and in the population study (p < 0.05). However, multipoint linkage analysis again did not show any evidence for the existence of a major gene regulating atopy on chromosome 11q13 with location scores to -35 under the model of maternal inheritance. Evidence against linkage was confirmed by the non-parametric linkage analysis, using the affected pedigree member method. Also, there was no substitution of isoleucine for leucine in the fourth transmembrane domain of Fc epsilon RI beta (Leu181), which was reported to be responsible for a subset of atopy in the British population. Therefore, the association of serum total IgE levels with chromosome 11q13 indicates that a gene or genes at this locus may contribute to the expression of high IgE levels in the Japanese population as well as in the British population, but the heterogeneity of the genetic regulation of serum total IgE levels is evident between the two populations.

Gene expressions of oxytocin and oxytocin receptor in cumulus cells of human ovary.

Oxytocin (OT) has been detected in mammalian granulosa-luteal cells during the early stages. The purpose of this study was to explore gene expressions of OT and OT receptor (OTR) in human cumulus cells. Cumulus cells enclosing a mature oocyte were obtained from 6 women undergoing clinical in vitro fertilization and embryo transfer programs. OT and OTR gene expressions were investigated by employing reverse transcription polymerase chain reaction and reverse transcription polymerase chain reaction/single-strand conformation polymorphism methods. OT gene expression in the cumulus cells was positive in 5 women and weakly positive in the remaining patient. The structure of OT mRNA in the cumulus cells was equivalent to that in human hypothalamus. OTR gene expression was also observed in the cumulus cells. This study is the first to describe the simultaneous expression of both OT and OTR genes in human cumulus cells. It is suggested that local OT plays some important roles in fertility through modification of the micro-environment around the oocyte.
