[The effect of bismuth subnitrate on cisplatin toxicity].

Bismuth subnitrate (BSN), a bismuth compound medically used for antidiarrheics, was orally administered to see whether it can reduce CDDP nephrotoxicity or not. Thirteen patients aged 19 approximately 60 with ovarian cancer entered this BSN-CDDP trial. A total of thirty three courses of BSN-CDDP treatment was undergone. BSN was administered orally at a dose of 50 mg/kg for five days before CDDP therapy. CDDP was infused for two hours. No vigorous hydration or diuresis was performed. Only 2,000 ml of saline with 20 mEq per liter of KCl was given for post-hydration. The median dose of CDDP was 100 mg/m2. The renal toxicity of BSN-CDDP treatment was minimum. 82% of the courses at the sixth day after the treatment had creatinine clearance levels which were more than 80% of those before the treatment. But twenty-four hour NAG and beta 2-microglobulin excretion were significantly increased. Bone marrow suppression and gastrointestinal disturbance were commonly observed. The results of our study indicate that BSN pretreatment reduces the renal toxicity of CDDP to some extent.

[A case of malignant peritoneal mesothelioma with a good response to treatment].

A rare case of malignant peritoneal mesothelioma in a 49-year-old Japanese woman is presented. The tumor was limited to the peritoneum, and was cytologically and histologically identified as being composed of two components, epithelial and non-epithelial forms. The transition in her CA-125 level suggested that this tumor had good prospects of developing into a malignant mesothelioma. The good prognosis of the patient is considered to be attributed to detecting this tumor in its early stage and to aggressive chemotherapy.

[Undifferentiated carcinoma of the ileum--a case report].

A 64-year-old man was admitted to Nagasaki Citizens' Hospital complaining of an abdominal tumor. Histological diagnosis was undifferentiated carcinoma of the terminal ileum. Multiple metastases of the bones were found after operation, and the patient died approximately 8 months after onset. In this paper, we describe the histological findings of undifferentiated carcinoma with a review of the literature.

[An immunohistochemical study of the distribution of epithelial membrane antigen (EMA) in uterine carcinoma].

Epithelial membrane antigen (EMA) is a component of the human milk fat globule membrane. The production of EMA is increased in both squamous and non-squamous epithelium in a variety of diseases including many neoplasms. Using antihuman EMA monoclonal antibody, an immunoperoxidase stain for EMA was done in normal or neoplastic cervical epithelia and endometrium. Formalin-fixed paraffin-embedded tissues were obtained by either colposcopic biopsy or surgical excision of the uterus from 61 patients. Ethanol fixed imprint smears were obtained from surgically removed uteri from 84 patients. The difference between normal cervical squamous epithelium and dysplasia or carcinoma of the cervix in intensity or patterns of staining for EMA reflected the progression of the neoplastic change in the cervix. The different EMA localization in well differentiated adenocarcinoma and moderately or poorly differentiated adenocarcinoma of the cervix and the endometrium was correlated with histological differentiation.

Fluorimetric high-performance liquid chromatography of prostaglandins and its application to their determination in human seminal fluid.

A highly sensitive and simple high-performance liquid chromatographic method with fluorescence detection for the determination of prostaglandins is described. Prostaglandins are converted into the corresponding fluorescent derivatives by reaction with 3-bromomethyl-6,7-dimethoxy-1-methyl-2(1H)-quinoxalinone in the presence of potassium hydrogen carbonate and 18-crown-6 in acetonitrile. The derivatives are separated simultaneously within 34 min (the total run time per injection, 56 min) on a reversed-phase column (YMC Pack C8) by a stepwise elution with mixtures of acetonitrile, methanol and water and detected fluorimetrically. The detection limits are 10-15 fmol at a signal-to-noise ratio of 5 in a 10-microliter injection volume. Prostaglandins E1, E2, F1 alpha and F2 alpha in human seminal fluids are measured by this method.

A simple electrode for stable recording of pacemaker potentials for ureteral peristalsis from the in vivo canine renal pelvis.

Pacemaker potentials for ureteral peristalsis were recorded in vivo with a fine, flexible acupuncture needle electrode inserted transparenchymally into the renal pelvis of anesthetized dogs. The recorded pacemaker potentials were quite similar, in every respect, to those shown in vitro, suggesting the availability of this electrode in recording and detailed analysis of the pacemaker activity in studies requiring in vivo preparations.

Highly sensitive determination of free polyunsaturated, long-chain fatty acids in human serum by high-performance liquid chromatography with fluorescence detection.

A highly sensitive and rapid high-performance liquid chromatographic method for the determination of free fatty acids including polyunsaturated, long-chain fatty acids in human serum is described. The fatty acids are converted into the corresponding fluorescent derivatives by reaction with 3-bromomethyl-6,7-dimethoxy-1-methyl-2(1H)-quinoxalinone in the presence of potassium carbonate and 18-crown-6 in acetonitrile. The derivatives are separated simultaneously within 60 min on a reversed-phase column (YMC Pack C8) with an isocratic elution using aqueous 72% (v/v) acetonitrile and are detected fluorometrically. The detection limits are 1-3 fmol in a 10-microliter injection volume, which enables precise determination of the free acids including dihomo-gamma-linolenic, arachidonic, eicosapentaenoic, and docosahexaenoic acids present in serum at very low concentrations. The change in the concentrations of the free fatty acids in human sera before and after ingestion of mackerel meat is also described.

Reduced apomorphine sensitivity of dopamine metabolism in rat striatum after repeated administration of methamphetamine.

Rats received daily injections of saline or methamphetamine (MAP; 4 mg/kg/day) for 14 days. Seven days after the completion of this regime, dopamine (DA) and 3,4-dihydroxyphenylacetic acid (DOPAC) levels were measured in the striatum following intraperitoneal injections of either saline plus gamma-butyrolactone (GBL; 750 mg/kg) or apomorphine (2 mg/kg) plus GBL. The saline plus GBL challenges produced no difference in the DA or DOPAC levels between saline- and MAP-treated rats. By contrast, the apomorphine plus GBL challenges produced higher DOPAC levels in MAP-treated rats than saline-treated rats, although they produced no difference in the DA levels between the two groups. These results indicate that apomorphine depresses the striatal DA metabolism less in MAP-treated rats than in saline-treated control rats. Repeated MAP administration might produce this effect through apomorphine subsensitivity of presynaptic DA autoreceptors.

Tardy ulnar nerve palsy caused by cubitus varus deformity.

Clinical features and X-rays of thirty-four cases with cubitus varus deformity were analysed in order to explain the occurrence of tardy ulnar nerve palsy caused by cubitus varus deformity. Cubitus varus cases with tardy ulnar nerve palsy, compared to cases without it, were older at the first visit to the clinic for cubitus varus deformity. There were no other differences between the two groups in clinical features. Computed tomography showed that the olecranon moved to the ulnar side against the olecranon fossa. Operative findings showed that the medial head of triceps brachii shifted medially and covered the ulnar nerve. Both from computed tomography and operative findings we conjectured the possibility that tardy ulnar nerve palsy was caused because the ulnar nerve was compressed by the forward medial movement of the medial head of the triceps brachii caused by cubitus varus deformity.

Congenital anomalies of the upper limb among the Japanese in Sapporo.

Nine hundred and forty three patients with nine hundred and fifty-five congenital upper limb anomalies are classified according to the Swanson's classification. Relative incidence of each type of anomaly, sex incidence, affected side, associated abnormalities and familial history are analysed. The occurrence of congenital upper limb anomalies is also compared with Chinese and Western studies. The characteristic feature in this series is that the incidence of typical syndactyly is not as high as expected. Two problems of Swanson's classification encountered in this analysis are discussed. One of the problems is the relation between polydactyly, syndactyly and typical cleft hand. The other is the relation between brachysyndactyly, atypical cleft hand and transverse deficiency.

Abductor digiti minimi opponensplasty in hypoplastic thumb.

Ten cases of hypoplastic thumbs were treated by abductor digiti minimi opponensplasty in order to restore opposition of the thumb and reform the wasted thenar eminence. In eight of these ten cases operations were combined with multiple Z-plasty or rotation flap to correct the narrowing of the first web space. Adductor plasty using extensor indicis proprius was performed in six cases to restore the stability of the metacarpophalangeal joint of the thumb and ligament reconstruction of the carpometacarpal joint in one hand. In all cases, the transferred abductor digiti minimi was strong enough to abduct the thumb and provide good functional and cosmetic results. We modified Littler's procedure by transferring the origins of abductor digiti minimi muscle from the flexor carpi ulnaris to the palmaris longus tendon. Our modified method gave a better cosmetic appearance than that provided by Littler's method.

Affective disorders in mentally retarded adolescents--report of two cases with lithium treatment.

Two cases of moderately retarded adolescents with affective disorders have been treated for nine and five years, respectively. Rapid cycling episodes were seen in early adolescence and a long duration of episodes appeared in late adolescence. A two-year lithium therapy was able to control them fairly well but did not work as a prophylaxis. Lithium carbonate induced polydipsia or personality change.

[Carcinogenicity in mice by inhalation of benzotrichloride and benzoyl chloride].

Benzotrichloride and benzoyl chloride are suspected to be causative agents of lung cancer and maxillary malignant lymphoma of workers employed in factories producing benzoyl chloride. The present study was undertaken to evaluate the carcinogenicity in mice of inhaling benzotrichloride and benzoyl chloride. Mice inhaled benzotrichloride and benzoyl chloride vapor for 30 min/d for 2 d/wk for 5 months, and each animal was followed for several month without subsequent exposure. Tumor developed in the lung, skin and lymphatic tissues at high incidences in mice inhaling benzotrichloride. By benzotrichloride vaporized at 50 degrees C, the incidence of pulmonary tumors was 53.1% (17/32, p less than 0.001), that of skin tumors was 25% (8/32, p less than 0.02), and that of malignant lymphoma was 25% (8/32, p less than 0.02) observed at the 10th month after exposure. These are significantly higher than that observed in control mice. In mice exposed to benzotrichloride vaporized at room temperature, the incidence of pulmonary tumors was 81.1% (30/37), that of skin tumors was 27.0% (10/37), and that of malignant lymphoma was 10.8% (4/37) observed at the 15th month after exposure. On the other hand, by benzoyl chloride vaporized at 50 degrees C, the incidence of pulmonary tumors was 10.7% (3/28) and that of skin tumors was 7.1% (2/28), but these incidences did not show any significant difference from the controls. These results suggest that the carcinogenicity of benzotrichloride is much higher than benzoyl chloride and that benzotrichloride is the primary cause of malignancies developing among workers engaged in manufacturing benzoyl chloride.
