A transarterial approach of electrical ablation of atrioventricular junction in a dog model: comparison of the effects between high and low energy shocks.

To analyze the effectiveness of a transarterial catheter technique for electrical ablation of the atrioventricular junction, 30 mongrel dogs were studied by means of synchronized electrical shock between the catheter adjacent to the noncoronary cusp and a metal plate behind the dog's back using a standard cardioversion unit. These dogs were classified into two groups according to the energy delivered. The high energy group received more than 100 joules (group A) and the low energy group received from 20 to 60 joules (group B). Complete atrioventricular block was induced by a single shock in all dogs. In group A, ventricular premature beats appeared in all dogs; ventricular fibrillation and ventricular tachycardia appeared immediately in half (6/12) after electrical ablation. No ventricular dysrhythmias occurred in group B. Temporary right ventricular pacing was also performed in 10 out of 12 dogs in group A after electrical ablation. In contrast only one dog required pacing in group B. The cycle length of the subsidiary pacemaker rhythm was essentially identical in both groups. The QRS duration of the subsidiary pacemaker rhythm in group A was significantly longer in group B (P less than 0.01). The extent of myocardial damage induced by electrical ablation in group B was more localized than those in group A. However, the histological lesion representing the granulation tissue with necrosis and slight chronic inflammatory cell infiltration, was identical between both groups A and B. It was concluded that this technique of low energy electrical ablation of the atrioventricular junction adjacent to the noncoronary cusp via a transarterial approach was useful in producing an experimental model of chronic complete AV block.

Human endothelial cell growth factors derived from thyroid anaplastic cell carcinoma.

Human endothelial cell growth factors were partially isolated from transplanted thyroid anaplastic cell carcinoma propagated in nude mice. Purification was monitored in human umbilical cord vein endothelial cell cultures by (3H)-thymidine incorporation into DNA. Crude tumor extracts with 0.1 M Tris-HCl, pH 7.2 were fractionated by ammonium sulfate precipitation. The active materials precipitated by 35% to 50% ammonium sulfate were further purified by Bio-Rex 70 (Bio-Rad, Richmond, Calif.) cation exchange chromatography. Active fractions eluted by 0.5 M to 0.7 M NaCl were further analyzed via heparin-sepharose affinity chromatography. This resulted in separation of one major peak eluted by 0.9 M to 1.2 M NaCl and confirmed to promote human umbilical cord vein endothelial cell proliferations, and three other peaks. The molecular weight determination of the most active fraction performed by high-pressure liquid chromatography utilizing TSK 2000 gel column indicates 41,000 and 19,000 daltons for the active materials, respectively. Our results demonstrate that the angiogenesis properties of a solid tumor (thyroid anaplastic cell carcinoma), are not composed of a simple mechanism in vivo.

Constrictive pericarditis with electrocardiographic evidence of right ventricular hypertrophy.

We report the findings in a patient with constrictive pericarditis who had ECGs resembling right ventricular hypertrophy without right ventricular pressure overload, and we provide a possible explanation for the cause of this abnormality. Right ventriculography demonstrated a dilated and hyperkinetic outflow tract and compressed free wall with calcified pericardium. The derangement of the configuration of the right ventricle was considered to be the genesis of this electrocardiographic abnormality.

Predominance of squamous cell carcinoma and rarity of adenocarcinoma of maxillary sinus among Japanese.

The reasons for the prevailing predominance of maxillary sinus squamous cell carcinoma and the rarity of maxillary sinus adenocarcinoma among Japanese were investigated in a cooperative study at two institutions. There was little difference in the histological diagnostic criteria. It is hypothesized that the predominance of squamous cell carcinoma and the rarity of adenocarcinoma of the maxillary sinus can be related to the extensive use of softwood in the Japanese furniture industry, to the high prevalence of chronic sinusitis among Japanese in the past or to the popularity of cigarette smoking among Japanese.

Adrenoceptors in experimental hypertension.

Cardiac, aortic and renal alpha- and beta-adrenoceptors were examined in three types of experimental hypertension in rats. In spontaneously hypertensive rats (SHR), cardiac alpha 1-adrenoceptor concentration was increased and cardiac and aortic beta-adrenoceptor concentrations were decreased compared with Wistar-Kyoto rats (WKY). A similar decrease in cardiac beta-adrenoceptor concentration was also shown in two-kidney, one clip (2K, 1C) and deoxycorticosterone acetate(DOCA)-salt hypertensive rats. Renal alpha 1- and alpha 2-adrenoceptor concentrations were increased in SHR, but not in 2K, 1C and DOCA-salt hypertension. In contrast, renal beta-adrenoceptor concentration was increased in 2K, 1C and DOCA-salt hypertension, but unchanged in SHR. The observed increase in cardiac alpha 1- and renal alpha 2-adrenoceptor concentrations may partly contribute to the elevation of blood pressure in SHR.

Age related alteration in levels of keratan sulfate in sera of orthopaedic patients.

Using the monoclonal antibody to the keratan sulfate(KS), we modified the assay system for detecting of serum KS levels (Thonar, et al. Arthritis Rheum. 28:1367-1376, 1985). This method is based on the enzyme-linked immunosorbent assay (ELISA). The dogfish KS was most effective and sensitive, in this system. KS levels were measured in 45 patients with no complaints of arthralgia, no history of arthritis and no remarkable change in the laboratory data, including erythrocyte sedimentation rates, rheumatoid factor, uric acid and C-reactive protein. Age related changes were apparent in serum levels of KS. Age related catabolism in normal articular cartilage and serum KS levels will aid to estimate the turnover of articular proteoglycan.

[Clinical analysis of patients with hepatocellular carcinoma with non-A, non-B hepatitis virus].

An analysis of 242 patients with a hepatocellular carcinoma (HCC) has shown that 65 patients without a hepatitis B virus (HBV), alcohol abuse, or a transfusion history and 17 posttransfusion patients had a mean death age of 65.5 +/- 10.2 and 62.1 +/- 8.4 years respectively. The average age of 65 patients was 8.5 years older than those with a HBV and without a alcohol abuse history. Of 17 patients, 3 with a HBV had a mean death age of 55.3 +/- 10.8 years and the interval time from transfusion to a HCC was significantly shorter than in other post-transfusion patients.

[Evaluation of a modified latex agglutination inhibitory reaction method for the quantitation of urinary estriol in pregnant women].

This study evaluated a modified latex agglutination inhibitory reaction method for the quantitation of urinary estriol in women during normal and abnormal pregnancy. The urinary levels of estriol in pregnant women measured by this method correlated with urinary and serum levels measured by radioimmunoassay. The urinary level of estriol rose during pregnancy and reached a peak at the time of delivery, after which it returned to the control level in a few days. In contrast, the urinary level of estriol in women with abnormal pregnancy did not rise during pregnancy. This method is useful to detect impaired function of the placenta in pregnant women.

[Two cases of inflammatory pseudotumor of the lung].

Two cases of inflammatory pseudotumor of the lung were reported. Case 1. A 68 year-old man was referred to our hospital because of an abnormal shadow on chest X-ray. He had a history of pneumonia in the right upper lobe five months before. The chest X-ray film revealed a coin lesion in the right upper lung field (S1), the same segment as the previous pneumonia. Although RPF and INH were administered for three months, the shadow did not change, and cough and sputum continued. In order to confirm the diagnosis, open thoracotomy was performed and microscopic findings of the resected tumor showed inflammatory pseudotumor; proliferation of fibrous tissue with infiltration by inflammatory cells (plasma cells, lymphocytes and a few neutrophils). Case 2. A 35 year-old man was admitted to our hospital because of an abnormal shadow on chest X-ray, i.e. a coin lesion with vascular indentation in the left lower lung field (S8). A wedge resection including the mass was performed, and histopathologic examination revealed inflammatory pseudotumor, or plasma cell granuloma; proliferation of fibrous connective tissue with infiltration by predominantly mature plasma cells. Both patients have been doing well after the operation. We also reviewed 46 cases reported in the Japanese literature and discussed various aspects of this disease. Of 10 patients who had a history of previous respiratory tract infection, 8 had histopathologic features with various inflammatory cells, including many lymphocytes, as in our Case 1. Although the common etiology of inflammatory pseudotumor is obscure, we suppose that in some cases the lesion may be a result of post-inflammatory repair process.

[Significance of the confirmatory test of enzyme immunoassay and western blotting method in detection of anti-HTLV-1 in sera].

In this study, we directly compared the sensitivity of the detection of anti-human T-cell leukemia virus type-1 (anti-HTLV-1) antibody in serum by enzyme immunoassay (EIA), gelatin particle agglutination method (PA), and Western blotting method (WB). Of 69 cases studied, 6 cases positive by WB were negative by both PA and EIA methods. Of these 6 cases, 5 cases were interpreted to be positive by the confirmatory test of EIA (inhibitory rates of greater than 50%). Thus, the results of the confirmatory test were most consistent with those of WB methods. We also determined the levels of anti-HTLV-1 antibody in sera from the babies born of anti-HTLV-1 positive mothers at various months of age by the above three methods. As ages progressed, a parallel decline in the levels of anti-HTLV-1 antibody was shown by the three methods. However, at the time when sera became negative by both PA and EIA methods, the antibody was still detected by WB method. The study concludes that both WB and confirmatory test of EIA are more sensitive indicators of HTLV-1 infection.

[Usefulness of ceftazidime in intractable respiratory tract infections].

Usefulness of ceftazidime (CAZ) was studied in 56 cases of intractable respiratory tract infections. CAZ was administered at a daily dose of 2-4 g in 2 divided doses by intravenous drip infusion for 3-15 days. 1. Analysis was carried out in 38 cases and the following result was obtained. Efficacy rate was: 68% (17/25) in pneumonia, 60% (3/5) in chronic bronchitis and 67% (4/6) in secondary infections in chronic respiratory disease cases, and the overall efficacy rate was 63.2% (24/38). 2. In bacteriological study, 68.2% (15/22) of eradication rate was obtained. Against Staphylococcus aureus, eradication was obtained in all strains (4 strains). Against Pseudomonas aeruginosa, eradication occurred in 4 strains out of 10, and decrease in number in 2 strains. 3. As for adverse effects, mild hepatic disorder was observed in 2 cases (3.6%) out of 56. 4. From the above result, CAZ is considered to be very useful when used as monotherapy for aged patients and in the treatment of severe and intractable infections accompanied by underlying diseases.

Selective effector coupling of muscarinic acetylcholine receptor subtypes.

Attempts have been made by means of recombinant DNA technology to understand the molecular basis of the functional heterogeneity of the muscarinic acetylcholine receptor (mAChR). Molecularly defined mAChR subtypes have been produced from the cloned DNAs in Xenopus oocytes and NG108-15 neuroblastoma-glioma hybrid cells as transient and stable expression systems, respectively, and agonist-induced cellular responses have been examined. The results obtained provide evidence that mAChR subtypes are selectively coupled with different effector systems, albeit not exclusively.

[Luminol-dependent chemiluminescence of peripheral neutrophils in asthmatic patients].

To evaluate the role of the neutrophil in the pathophysiology of bronchial asthma, we examined the luminol-dependent chemiluminescence of peripheral neutrophils (CL) in steroid-dependent asthmatic patients during an acute attack (n = 18), during stable period (n = 10), and in normal controls (n = 21). When stimulated with formyl-methionyl-leucyl-phenylalanine (fMLP 0.1 microM), the CL of patients in acute attack was lower than that of normal controls. (5.29 +/- 1.25 vs. 9.29 +/- 0.84 relative light units p less than 0.01) Among 18 patients, we could examine 10 patients during the stable period after receiving steroids. The CL of those 10 patients during the stable period was significantly higher than their CL during acute attacks, when evaluated by the paired t-test (p less than 0.05), and was not very different than in normal controls. When stimulated with opsonized zymosan (5 mg/ml), CL was similar in the three groups. We also examined the CL of peripheral neutrophils of normal persons after the preincubation with serum of patients during acute attack and stable period. When stimulated with fMLP, CL after preincubation with serum of patients during acute attack was lower than CL after preincubation with the serum of patients during the stable period. It was suggested that the respiratory-burst activity of peripheral neutrophils in acute asthmatic attack was reduced to the chemotactic stimulus of fMLP.

[A case of cardiac sarcoidosis with various electrocardiographic change].

The patient was a 61 year old woman of Japanese who was in good health until May 1983, when she came to our hospital because of the visual disturbance. The chest X-ray film showed bilateral hilar lymphadenopathy and a transbronchial lung biopsy revealed sarcoid granuloma. She received systemic steroids (10 to 40 mg/2 days of prednisolone) for visual disturbance from August 1983 to May 1986. Then steroid therapy was suspended temporarily. In August 1987, she was readmitted because of syncopal episodes. She was noted to have irregular bradycardia and the electrocardiogram revealed atrioventricular block and paroxysmal polymorphous ventricular tachycardia. Initially cardioversion to sinus rhythm was accomplished with direct current electric shock and temporary pacemaker was inserted and subsequently a permanent transvenous demand pacemaker implanted. The patient then became asymptomatic. Coronary arteriography revealed no significant organic stenosis and left ventriculogram showed a left ventricular aneurysm of the inferior wall of the left ventricle. A transvenous percutaneous right ventricular endomyocardial biopsy revealed non-specific myocardial degeneration with slight interstitial fibrosis. Thallium-201 myocardial perfusion scans revealed the inferior left ventricular defects. We conclude the syncopal episodes and electrocardiographic abnormalities of this case were due to massive myocardial sarcoidosis which caused left ventricular aneurysm.

[Adoptive immunotherapy by intra-arterial infusion of ATLAK or Allo-TLAK cells in patients with head and neck cancer].

For clinical application of adoptive immunotherapy, it is necessary to prepare a sufficient number of autologous tumor specific effector cells. A large amount of peripheral blood lymphocytes was obtained by leukapheresis using a Heamonetics V50 pheresis system. Autologous tumor- and lymphokine-activated killer (ATLAK) cells were induced by autologous mixed lymphocyte tumor cell culture (autologous MLTC) and further activation with recombinant interleukin-2 (rIL-2). Another problem was the difficulty of obtaining a sufficient number of highly activated effector cells to reach the target tumor tissue. Direct infusion of effector cells into a feeding artery was effective for cell accumulation in the target. ATLAK cells were infused into maxillary artery in 4 patients with maxillary squamous cell carcinoma. The results indicated that the therapy was effective for reduction of the tumor mass. After immunotherapy, surgery was performed and the tissues were histologically examined. Degenerated tumor cells and intensive infiltration by mononuclear cells and macrophages were seen in the surrounding fibrous tissue. However, the quantity of fresh autologous tumor cells available from open biopsy was limited. Allogeneic cultured tumor cell line was used as stimulator of lymphocytes instead of autologous tumor cells. The killing activity of the allogeneic tumor and lymphokine activated killer (Allo-TLAK) cells was significantly induced against the autologous tumor cells. Antitumor effect was observed in 5 out of 9 patients. Side effects were minor, such as slight fever and blood eosinophilia, which may be due to the rIL-2 function. These results indicate that this method of therapy is an effective form of adoptive immunotherapy.

Administration of elastase blocks the formation of fragmented elastic fibers in aorta of rabbit.

Fragmented elastic fibers are known to be associated with atherosclerotic lesions, angitis and age-related changes of the vessel wall. We produced fragmented elastic fibers in the thoracic aorta by immunizing rabbits with elastin peptides. Fragmented elastic fibers in the media were associated with degenerative smooth muscle cells, and the number of fine elastic fibers beneath the endothelium was decreased. The lesion presented here resembled age-related changes of the vessel wall. We also evaluated the influence of elastase administration on the formation of this lesion. In rabbits receiving elastin immunization with simultaneous intraperitoneal administration of elastase, fragmented elastic fibers were apparently decreased and the appearance of the elastic fibers resembled that of normal rabbits. These data suggest the possibility that administered elastase blocks the formation of fragmented elastic fibers in the aortic wall.
