Trans-synaptically induced bursts in regular spiking non-pyramidal cells in deep layers of the cat motor cortex.

In deep layers of the cat motor cortex, we have investigated the properties of neurons displaying trans-synaptically induced bursts. In in vivo experiments, extracellularly recorded burst neurons were separated into two subtypes based on their dependence on stimulation sites, the medullary pyramid or the ventrolateral (VL) thalamic nucleus, from which bursts of 10-20 spikes were triggered. The spike amplitude attenuation and frequency adaptation during a burst were more prominent in pyramid-dependent burst neurons than in VL-dependent burst neurons. Intracellular recordings in in vivo experiments revealed that pyramid-dependent bursts emerged from a long-lasting depolarization, while each spike during a VL-dependent burst was narrow in half-width and was followed by a fast AHP, similar to fast spiking neurons. In in vitro slice experiments, intracellular recordings were obtained from neurons that displayed a burst of attenuated spikes emerging from a long-lasting depolarization, and were also obtained from fast spiking neurons. They were morphologically recovered to be multipolar cells with sparsely spiny dendrites and local axonal networks, suggesting that they are inhibitory interneurons. The multipolar neurons displaying bursts of attenuated spikes may mediate the recurrent inhibition of pyramidal tract cells.

Experimental study of nerve regeneration in a biodegradable tube made from collagen and polyglycolic acid.

The authors developed a novel bioabsorbable nerve conduit to induce nerve regeneration across long gaps. It is a composite tube made of polyglycolic acid mesh coated with collagen. Seven cats underwent 25 mm resection of the left sciatic nerve. The proximal and distal nerve stumps were inserted into the tube and fixed with sutures to bridge the 25 mm gap between them. Histologic examination 4 months after implantation of the tube showed regeneration of nerve tissue structure, including myelinated axons and Schwann cells, and somatosensory evoked potentials and electromyograms demonstrated functional recovery of the regenerated nerves. Horseradish peroxidase staining revealed restoration of anterograde and retrograde axonal transport. These results indicate that this polyglycolic acid-collagen composite tube is promising as a nerve conduit that provides adequate nerve regeneration.

Use of a newly developed artificial nerve conduit to assist peripheral nerve regeneration across a long gap in dogs.

There is now considerable evidence that peripheral nerves have the potential to regenerate if an appropriate microenvironment is provided. However, there are only a few reports of the successful use of artificial nerve conduits to repair major nerve defects more than 30 mm in length. In this study, we examined nerve regeneration across a long gap in the dog peroneal nerve using a novel artificial nerve conduit developed by our group. The conduit consists of a polyglycolic acid (PGA) collagen tube filled with laminin coated collagen fibers. In 12 dogs, the nerve conduit was implanted across an 80 mm gap in the left peroneal nerve. Three months after surgery, compound muscle action potentials (CMAPs) and somatosensory evoked potentials (SEPs) were detected. Evaluation of locomotor function revealed obvious limping for up to 3 months, but no marked difficulty in walking by 6 months. Microscopic observation of the regenerated nerve segment at 12 months showed numerous myelinated nerve fibers, which were smaller in diameter and enclosed in a thinner myelin sheath than normal axons. These results suggest that our artificial nerve conduit has potential usefulness in enhancing peripheral nerve regeneration, even across large gaps.

Gadolinium-phthalein complexone as a contrast agent for hepatobiliary MR imaging.

Gadolinium-phthalein complexone (Gd-PC) was developed as a hepatobiliary magnetic resonance (MR) contrast agent. Phthalein complexone is one of the iminodiacetic acid derivatives and a structural analogue of bromosulfophthalein. Gadolinium-PC substantially enhanced signal intensity of normal functioning livers on T1-weighted MR images. Contrast enhancement of rabbit liver and gradual accumulation of high intensity bile in the gallbladder were observed after intravenous injection of 0.05 and 0.1 mmol/kg Gd-PC. However, 0.1 mmol/kg of Gd-DTPA caused little effect on liver MR.

Serial MR intensity changes of the posterior pituitary in a patient with anorexia nervosa, high serum ADH, and oliguria.

The posterior pituitary high signal (PPHS) seen on MRI of the sella in normal individuals probably reflects antidiuretic hormone (ADH) granules stored in the posterior pituitary lobe (PPL). We present a case with anorexia nervosa, high serum ADH, and oliguria who underwent three cerebral MR studies over the course of treatment. The first MR examination showed absence of PPHS and early enhancement of the PPL on dynamic MRI. In subsequent MR examinations PPHS became evident in concomitance with clinical improvement. This case suggests that PPHS changes may reflect reaccumulation of ADH granules and that dynamic MR of the PPL may be useful for assessing the vascularity of the PPL and/or the reversibility of its function.

T2 relaxation time of bone marrow water and lipid: correlation with serum ferritin in normal individuals.

PURPOSE: This study investigated the correlation between the iron content of bone marrow and the transverse relaxation times (T2 values) of bone marrow water and lipid on MRI. METHOD: The T2 of the water and lipid fractions of bone marrow was selectively measured in the L1-3 vertebral bodies using the chemical shift misregistration effect on MRI. Results were compared to the serum ferritin levels in 27 healthy subjects. RESULTS: The 1/T2 of the water fraction ws strongly correlated with the serum ferritin, but there was no correlation between the 1/T2 of the lipid fraction and serum ferritin. CONCLUSION: Selective T2 measurement of the water fraction in bone marrow allows precise estimation of the marrow iron content.

FDG-PET in differential diagnosis and grading of chondrosarcomas.

PURPOSE: The purpose of this work was to investigate the usefulness of PET with [18F]2-deoxy-2-fluoro-D-glucose (FDG) in the differential diagnosis and grading of chondrosarcomas. METHOD: Four enchondromas, one osteochondroma, and six chondrosarcomas were prospectively studied by FDG-PET. The significance of the standardized uptake value (SUV) was evaluated in comparison with clinical, radiological, and histopathological findings of the tumors. RESULTS: The SUV ranged from 0.7 to 1.3 (mean 0.96+/-0.22) in benign cartilage tumors and from 1.3 to 3.3 (2.23+/-0.80) in chondrosarcomas. This SUV difference between benign and malignant tumors was statistically significant (p = 0.011) in contrast to considerable overlap of clinical and radiological findings. Among chondrosarcomas, the SUV of one Grade I tumor was 1.3; those of four Grade II tumors were 1.3, 1.9, 2.5, and 3.1; and that of one Grade III tumor was 3.3. CONCLUSION: Our preliminary study showed that FDG-PET could be an objective and quantitative adjunct in the differential diagnosis and grading of chondrosarcomas.

Immunohistochemical studies on the localization of cancer associated antigens DU-PAN-2 and CA19-9 in carcinomas of the digestive tract.

The immunohistochemical localization of DU-PAN-2 antigen and CA19-9 antigen in carcinomas of the digestive tract was studied using an immunoperoxidase technique. Staining for DU-PAN-2 antigen and CA19-9 antigen was observed in 104 (79%) and 96 (73%) of 131 carcinomas of the digestive tract, respectively. Diffuse staining (more than 20% of carcinoma cell stained) for DU-PAN-2 was detected in 14 of 21 (67%) pancreatic carcinomas and 11 of 19 (58%) carcinomas of the biliary tract (including cholangiocarcinoma). Diffuse staining for CA19-9 was detected in 15 (71%) of pancreatic carcinomas and nine (47%) of the carcinomas of the biliary tract. In colon carcinomas, no diffuse staining for DU-PAN-2 was observed, whereas diffuse staining for CA19-9 was found in 41%. There was a positive correlation between the differentiation degree (or grade) of the adenocarcinomas of the colon and the expression of CA19-9, but not that of DU-PAN-2. These immunohistochemical studies showed that DU-PAN-2 antigen is expressed diffusely in most cases of adenocarcinoma of the pancreas and biliary tract and is more specific for adenocarcinomas of the pancreas and biliary tract than CA19-9.

Restoration of gap junctions in the regenerative process of ethanol-induced gastric mucosal injury.

Gap junctions mediate intercellular communication and play an important role in cell proliferation and differentiation. The present study examined the expression of the gap junction protein connexin 32 in rat gastric mucosa during the regenerative process of ethanol-induced gastric mucosal injury. Absolute ethanol (0.5 mL/100 g bodyweight) was administered to rats via gastric intubation. Following the treatment, rats were killed periodically, and gastric tissues were processed for indirect immunofluorescence microscopy using an anti-connexin 32 antibody. Haematoxylin and eosin (H&E), periodic acid Schiff (PAS) and Bromodeoxyuridine (BrdU) staining were also performed in serial sections. Gastric injuries were limited in the mucosal layer. The injury was most severe 1 h after treatment, and was completely resolved by the 4th day after treatment. The number of immunoreactive spots for gap junctions was markedly decreased 1 h after the ethanol treatment. Reappearance of these immunostaining gap junctions occurred with repair of the injury. The reappearance of connexin 32 after injury was delayed in comparison with both the histologic resolution of the injury and the normalization of PAS-stained mucus. In contrast, the time course of reappearance of gap junctions closely paralleled the appearance of BrdU-labelled cells. These results indicate that morphologic repair is different to the recovery of cell maturity and cell proliferation in the regenerative gastric mucosa.

Computerized analysis of EEG background activity in epileptic patients.

Background activity was studied in 128 idiopathic epilepsy patients and 30 normal controls using EEG topography and t-statistic significance probability mapping (t-SPM). In epileptic patients, EEG background activity showed a marked increase in delta, theta, alpha 1, and beta 1, and a decrease in alpha 2 activity as compared with controls. Untreated epileptic patients had a significant increase in delta, theta, and alpha 1 as compared with controls. For epileptic patients treated with antiepileptic drugs (AEDs), the most marked slowing was observed in the polytherapy group, followed by the monotherapy group and then the untreated group. Among seizure types, patients with partial seizures (PS) tended to exhibit more slowing than patients with only generalized tonic-clonic seizures (GTC). Moreover, PS had a right-left asymmetry in alpha 2 and beta 1 activities. In a comparison of AEDs, patients receiving carbamazepine (CBZ) and phenobarbital (PB) showed no significant difference as compared with the untreated group. In contrast, patients receiving valproate (VPA) showed a decrease in slow and fast activities. EEG changes associated with each AED were different in GTC and PS. Patients receiving VPA for GTC showed a decrease in theta and beta 1 activities, but those with PS showed a decrease only in delta activity.

Surgical treatment and prognosis for patients with gastric cancer lesions larger than ten centimeters in size.

In 323 of 1,620 patients with gastric cancer, the lesion was > or = 10 cm in maximum diameter. Studies were done with respect to clinicopathologic features and the prognosis. Patients with tumors > or = 10 cm were younger and women patients were more numerous compared to those with tumors < 10 cm. Tumors were more advanced and noncurative resection was usually done for these patients. A multivariate analysis showed that serosal invasion, liver metastasis, lymph node metastasis, operative curability, tissue differentiation, and extended lymph node dissection to be independent prognostic factors. This retrospective study showed that patients with a larger tumor are at an increased risk for tumor advancement and that they will benefit from curative resection and extended lymph node dissection.

Vasoacting agents flavone acetic acid and hydralazine given in combination enhance antitumor effects under condition of hyperthermia.

The combined effects of flavone acetic acid (FAA), hydralazine (HYD) and hyperthermia on B16 melanoma cells and solid tumor were examined in vitro and in vivo. In vitro, hyperthermia did not enhance the cytotoxicity of the combined use of FAA and HYD. In vivo, growth inhibition of B16 melanoma solid tumor by FAA (150 mg/kg) combined with HYD (5.0 mg/kg) could not be differentiated from that by FAA or HYD alone. Increased antitumor effect was recognized when FAA combined with HYD was used under conditions of hyperthermia. FAA combined with HYD significantly reduced tumor blood flow compared to FAA alone or HYD alone. We thus conclude that the significant reduction in tumor blood flow may play an important role in the enhanced antitumor effects of the combined treatment with FAA, HYD, and hyperthermia.

Postoperative chemotherapy for gastric cancer in the elderly.

Prolongation of the life span and improvement in technology make it more likely that the elderly will be surgically treated and chemotherapy will be prescribed, postoperatively. The relationship between postoperative chemotherapy and survival time after resection for gastric cancer in the elderly was examined in a retrospective review of data on 268 patients over the age of 70 years; all had been treated in the Department of Surgery II, between 1965 and 1987. Of these 268 patients, 161 were prescribed anticancer drugs, postoperatively. Postoperative chemotherapy was more often prescribed for those in the advanced stages of a malignancy. The survival time of those patients was shorter than for those who did not receive these drugs (p < 0.05). In a subgroup of our patients stratified to adjust for stage of the disease, there were no significant differences between the survival rates in cases of stages I, II or IV of the cancer. The survival time of patients with stage III and given chemotherapy improved compared to those not given chemotherapy (p < 0.01). Advanced chronological age is not sufficient justification to limit or withhold treatment with anticancer drugs. We propose that postoperative chemotherapy be prescribed for patients with gastric cancer, regardless of age.

Evaluation of the vertical semicircular canal function by the pendular rotation test: a study on patients with benign paroxysmal positional vertigo.

The pendular rotation test (non-damped) in a head-tilted position, 60 degrees backward and then rotated 45 degrees either to the right or left, was performed in 6 patients with benign paroxysmal positional vertigo. The stimulus mode was amplitude = 360 degrees, frequency = 0.1 Hz, and the maximal speed = 114 degrees/s. By this test procedure, it was possible to evaluate the excitability of vertical semicircular canals. Using an infra-red CCD camera and a personal computer system, the evoked nystagmus was analysed. A statistically significant difference (p < 0.05) in the maximal slow-phase eye velocity of vertical nystagmus was found between those from the anterior semicircular canal and those from the posterior semicircular canal. The excitability of the posterior semicircular canal in the affected ear was found to be lower than that of the anterior semicircular canal.

Computed tomography and 131I-MIBG scintigraphy in the diagnosis of pheochromocytoma.

In order to evaluate the usefulness of computed tomography (CT) and 131I meta-iodobenzylguanidine (131I-MIBG) scintigraphy for the localization of pheochromocytoma, a prospective study was undertaken in 23 patients with possible pheochromocytoma. Seventeen tumors were identified in 13 patients. Two tumors were extra-adrenal. CT was superior for locating tumors in the adrenal glands while 131I-MIBG scintigraphy was more useful in the detection of extra-adrenal pheochromocytoma. Together with adrenal and extra-adrenal pheochromocytoma, the sensitivity for locating the tumor was calculated as 82 per cent (14/17) for CT and 76 per cent (13/17) for 131I-MIBG scintigraphy, respectively. One adrenal tumor (1 cm in size) only was missed by both methods. No false positive CT scans of 131I-MIBG scintigraphy were obtained in the remaining 10 patients who had possible pheochromocytoma but were excluded. These results indicated that CT and 131I-MIBG scintigraphy were both useful tools for detecting pheochromocytoma.

Real-time ultrasonography of thyroid nodules.

Sonographic characteristics were analyzed in 141 histologically confirmed thyroid nodules including 84 benign and 57 malignant tumors. Seven items were examined: Presence of a 'halo'; presence of cystic components; presence of calcification; hypoechogenicity; heterogeneity of the internal echo pattern: irregular or obscured margin; and massive extrathyroidal extension. Only the last-mentioned item appeared exclusively, but not very often, in malignant lesions, and the other characteristics were encountered in both benign and malignant lesions. Although ultrasonography is a method of choice for the evaluation of thyroid nodules, its ability to distinguish benign from malignant lesions is limited.
