[Importance of laparoscopy to postoperative treatment of ovarian carcinoma (author's transl)].

Indications in favour of peritoneoscopy in postoperative after-treatment of ovarian carcinomas are proposed on the basis of the authors' own experience. Postoperative laparoscopy in combination with peritoneal cytology and histology appears to be justified in support of differentiated postoperative therapy, since it enables modification of empirical and rigid decions for high-continuity after-treatment of ovarian carcinomas.

[Interference microscopic determinations of dry mass of cell nuclei of normal and dysplastic epithelium, carcinoma in situ and cervical carcinoma cultivated in vitro (author's transl)].

Dry mass of cell nuclei was studied in 34 explant cultures of normal epithelium (8), persisting dysplasias (7), Ca in situ (9) and cervical carcinomas (10) by interference microscopic determination. Dysplasias, Ca in situ and carcinomas can be distinguished from normal epithelium by a significantly higher dry mass value. Such a difference does not exist between dysplasia and Ca in situ. The percentage of dispersion of the dry mass is significantly higher in carcinomas than in all other examined tissues. The biological meaning of these results is discussed. The interference microscopic determination of dry mass of the cell nuclei is a suitable parameter for the characterization of properties of cervical carcinomas and their intraepithelial prestages cultivated in vitro.

[Clinical aspects of trophoblastic tumors].

Trophoblastic tumors are among the rare diseases in this country. The most important clinical aspects are demonstrated by references and personal experiences in the treatment of trophoblastic tumors. In many countries (SU, USA, GB, Japan) the treatment of trophoblastic tumors has been centralized.--The therapeutical strategy is based on histology, stage and other important prognostic factors. Most important is chemotherapy with antineoplastic substances. Surgery and radiation are to be used only under appropriate indication. After treatment the control of the HCG-values by radioimmunassay is of high importance for the early diagnosis of relapse.--All facts demonstrate, that also in the GDR a stronger centralization of the treatment of trophoblastic tumors should be attained.

[Morphology and DNA content of cultures from normal and dysplastic epithelium, carcinoma in situ and carcinomas of the cervix uteri (author's transl)].

Morphology and DNA content were studied in explantate cultures of normal epithelium, persisting intraepithelial atypias (dysplasias and carcinomas in situ) and carcinomas of the cervix uteri. Intraepithelial atypias and carcinomas can be distinguished from the normal epithelium, growing strictly as a monolayer, chiefly by the primary and secondary loosening of the cellular lawn with increased tendency towards overlapping of cells. DNA determinations provide the possibility of demonstrating, also in vitro, deviations of the DNA content of dysplasias, carcinomas in situ and pavement epithelial carcinomas from the normal epithelium. The findings are in accord with the results which we obtained by in vivo studies and underlinions.

Pulse cytophotometric investigations concerning ploidy and proliferation pattern of invasive squamous cell carcinomas of the cervix uteri.

Tumor cell suspensions from 100 biopsies of squamous cell carcinomas of the cervix uteri were investigated by impulse cytophotometry (ICP) after staining with pepsin-ethidium bromide. We estimated the ploidy pattern by comparing the ICP curves with normal diploid material. The originally linearly classified curves were transformed in a logarithmical manner. We found 48 diploid populations and 42 polyploid populations, among them 19 tetraploid tumors. In these the 4c-peak was the highest. In 10 cases there were aneuploid stem lines, mainly lying between 2c and 4c; 2 curves had a hypertetraploid pattern. Helpful for interpretation of the whole curve is the introduction of an index: the "relative mean DNA-content" (DNA). In 15 diploid tumors we found a low proliferating rate, characterized by low 4c-peaks. For interpretation of polyploid cell cycles a completed nomenclature of cell cycle phases was introduced. Comparing our material with chromosome analyses in laterature we found more polyploid, e.g. tetraploid tumors. It can be assumed that the estimation of ploidy pattern solely by DNA measurements has another meaning than by chromosome analyses. The higher ploid peaks in DNA distribution curves (e.g. ICP-karyograms) represent not only true polyploid nuclei but also nuclei which are in a blocked premitotic resting phase (G2 for 4c; G'2 for 8c and so on). The ICP is a valuable method for estimating biological pecularities of tumor cell suspensions.

[Registration of cytostatic effects on cultured cells in vitro and their current significance for individualized chemotherapy of tumors in advanced ovarian carcinoma].

In vitro tests of cell cultures have revealed individual responses of ovarian carcinomas to various cytostatic agents, the most effective ones being cyclophosphamide and trenimon. The role of pretherapeutical sensibilization tests for the chemotherapy of advanced ovarian carcinomas was investigated in a controlled clinical trial as part of a cooperative study. The survival curves of patients treated individually according to the given test result were at any time more favourable than those of a group nonindividually treated with trenimon. No significance between the two curves could be demonstrated on the basis of our preliminary results.

[Studies on the recording of cytostatic effects on organ cultures of squamous cell carcinoma of the uterine cervix].

Organ cultures are a suitable in vitro test--object for pretherapeutical testing of cytostatic resistance on carcinomas of the cervix uteri. The decrease of activity of lactic dehydrogenase and isocitric acid dehydrogenase, determined histochemically, can be used to monitor cytostatic effects at the cultivated tumour. In preparing chemotherapy schedules for advanced cervical carcinomas, cytostatic agents provedly ineffective in vitro should be avoided.

[Impulse cytophotometric studies in cytologically suspicious cervical smears].

187 women with cytologically suspicious cervical smears (Papanicolaou III to V) were investigated by 387 pulse cytophotometric (ICP) measurements. Within the Papanicolaou III group a significantly higher 4c-peak was seen in cases with only one ICP-investigation if we had histologically pathological findings (dysplasia, carcinoma) as contrastes with clinically and cytologically regressive cases. In repeated ICP-investigations we found in group Papanicolaou III (histol.: dysplasia, carcinoma) as well as in group Papanicolaou IV and V a significantly higher mean 4c-height than in only once investigated cases. Within a ICP-series the maximal 4c-height is most representative. The security of diagnosis is raising with repeating ICP-investigations. Here it is a valuable diagnostic aid especially in suspicious cases. All abnormal ICP-findings must be controlled for a long time, even with normal cytological smears. At last, the circumstances causing false negative ICP-results are discussed.

[Flow fluorescence-photometry in ovarian carcinomas with and without effusion].

By flow microfluorometry the DNA distribution curve of the cell nuclei of 39 adnex tumors, mainly ovarian carcinomas, was measured. 47% of the malignant tumor cell populations were diploid, the other ones were heteroploid (i.e. polyploid, tetraploid, aneuploid stem lines). Heteroploid tumors had the worse prognosis. In 8 cases effusions were analysed (7 ascites, 1 pleura). It is possible to diagnose a malignant tumor from effussion fluid by impulse cytophotometry. But in one case a great difference was found between the primary tumor and the ascites fluid, which only shows the DNA distribution of an irritative effusion. The value of impulse cytophotometry for clinical purposes is discussed.

[Pulse cytophotometric and cytologic repeated examinations of cervical secretions as compared with histologic findings (author's transl)].

1625 cervical smears from 397 women were investigated in special cytological consultations. On the same material 693 impulse cytophotometric (ICP) measurements were made, stained with ethidium bromide after pepsination. In 207 patients the diagnosis was certain by histology. The mean height of the 4c-peak (in 0/00 of the 2c-peak) raises with the Papanicolaou grading from 38,2 in Pap. I up to 128,8 in Pap. V. In histological confirmed cases in dysplasia the mean 4c-height was 79,5 and in carcinoma 102.2. If only one ICP investigation was turned to account there are some disagreements between the ICP value and the diagnosis finally resulting from the clinical and cytological course. The differences are considerably diminished by repeated ICP investigations. Therefore ICP measurements are a valuable aid in interpreting cytodiagnostic problems. They often facilitate the choice of further diagnostic and therapeutic procedures.

[Results of cytological and histological studies on the uterine cervix after long-term intake of hormonal contraceptive agents].

Cytologic examinations with 880 women having constantly used oral contraceptives from one to eight years show no agglomeration of epithelial atypias if the primary status was clearly in the negative. Among 669 histologically sure atypias of cervix uteri the part of women using oral contraceptives for at least one year is significantly lower in carcinoma in situ as well as in early invasive carcinoma. The differences in dysplasia are not significant. The results are also discussed with regard to the efficiency of permanent control examinations before and during the use of oral contraceptions. The demand of a further intensification of cytodiagnostic is derived from these examinations.

[The characterisation of malignant ovarian tumors by impulse cytophotometry (author's transl)].

31 malignant adnexal tumors were investigated by impulse cytophotometry. From the DNS distribution curves we interpreted the positions and the height of the peaks in order to get informations about ploidy and proliferation pattern. 45.2% were classified into the diploid, 29.0% into the polyploid and 12.9% into the tetraploid group. 12.9% were aneuploid. Comparing our results with chromosome analyses in the literature we found more tetraploid tumors in our material. For our polyploid curves we assume, that many of the cells in the 8c-peak remain in the corresponding G2'-Phase, not undergoing mitosis.
