One stage reconstruction of intercalated defect of the thumb using the osteocutaneous radial forearm flap.

A radial forearm flap including a segment of the cortex of the radius, based on the distal pedicle of the radial vessels and the cephalic vein, was used to reconstruct an intercalated defect of the thumb in two cases. In one case it was used as an ipsilateral pedicle flap while in the other case it was used as a free microvascular flap. In both cases, the skin flap as well as the segment of bone survived in toto without any evidence of creeping substitution. The functional result was good in both cases. This flap provides thin skin of good texture together with bone for a one-stage reconstruction.

Mullerian duct cyst (utricular cyst): treatment with the transvesical, transtrigonal approach.

The diagnosis of mullerian duct or utricular cyst should be considered in a child with urinary difficulties and a palpable midline, anterior rectal mass. Endoscopic cannulation of the cyst has been found to be the most useful diagnostic test. Infection should be treated with the appropriate antibiotics before definitive treatment. Surgical excision offers the best result. The transperitoneal and posterior parasacral approaches have been described but we favour the transvesical, transtrigonal approach which we find highly satisfactory. The risk of malignancy at a later age is an added indication for surgery.

Microsurgical reconstruction of the upper and lower limbs.

The availability of a wide variety of free tissue transfers has revolutionised and simplified the reconstruction of the upper and lower limbs. These microsurgical procedures will continue to provide alternatives for salvaging limbs, controlling some infections and resurfacing extensive soft and bony tissue defects. Thirteen free tissue transfers consisting of toe pulp, scapular cutaneous and latissimus dorsi myocutaneous flaps were utilised to reconstruct the thumb, hand, soft and bony defects of the lower tibial and heel regions. One latissimus dorsi flap failed because of severe infections.

Circumumbilical incision for pyloromyotomy.

Forty infants with infantile hypertrophic pyloric stenosis had a Ramstedt pyloromyotomy through a circumumbilical incision. Delivery of the pylorus was relatively easy. Mild wound infection occurred in three infants and a further child developed a purulent discharge. There was one instance of abdominal wall dehiscence and all the resultant scars were hardly visible, thus achieving an apparently unscarred abdomen.

Management of xiphopagus conjoined twins with small bowel obstruction.

A set of xiphopagus conjoined twins with prematurity, exomphalos, and intestinal obstruction was separated successfully. Preoperative evaluation included computerised axial tomography, 99mTc-HIDA scan, and barium enema. Major hepatobiliary and gastrointestinal anomalies were encountered. One twin is alive and well today. The other twin died one week postoperatively from sepsis. Postmortem studies showed she had a severe cardiac anomaly incompatible with normal life.

Choledochal cyst--a review of 25 cases.

Twenty-five cases of choledochal cysts operated at the University Hospital between 1969 and 1981 were reviewed. Nineteen of the patients were females. An accurate pre-operative diagnosis of choledochal cyst was made in 10 patients (40%). We have found the 99Tcm-diethyl-IDA (EHIDA) scan to be the safest and most sensitive diagnostic tool. In recent years excision has been performed whenever possible in view of reports of malignancy developing in the cyst wall. It has a lower morbidity and re-operation rate compared to internal drainage. In excision we advise the use of a cuff of the cyst wall for a wider anastomosis. As the biliary tract shrinks and retracts into the porta following excision of the cyst, stricture formation may still occur despite a wide anastomosis.

Epidemiologic analysis of fetal death in utero in Singapore.

Records on 1777 cases of missed abortion and 1001 cases of intrauterine fetal death in three major maternity hospitals in Singapore between 1976 and 1980 were analyzed. The overall incidence of these two conditions was 10.35 and 5.82 per 1000 births, respectively. During the 5-year period, the incidence of missed abortion increased consistently, from 8.05 to 12.76 per 1000 births. In contrast, during the same period, the incidence of intrauterine fetal death declined steadily from 6.44 to 4.68 per 1000 births. Missed abortions occurred more frequently in the early part of the year, especially in the months of March, April and May. The incidence of missed abortion and its correlation with possible environmental factors are discussed.

Maternal plasma estradiol and progesterone levels during therapeutic abortion induced by 16, 16 dimethyl PGE2 p-benzaldehyde semicarbazone ester.

Estradiol and progesterone levels in maternal plasma after administration of 16, 16-dimethyl-PGE2 p-benzaldehyde semicarbazone ester (16,16-PGE2) 6 hourly for abortion were studied in 27 women in the second trimester of pregnancy and in 49 patients with fetal death in utero. Basal maternal plasma levels of estradiol and progesterone measured before prostaglandin injection were significantly lower in cases of fetal death in utero than in the other subjects. After 16,16-PGE2 injection, plasma levels of estradiol and progesterone fell more rapidly and to a greater extent in cases of fetal death in utero than those with normal pregnancy at corresponding gestations; the decline of plasma estradiol levels was quicker and more marked than that of progesterone. Thus, 16,16-PGE2 appears to exert a differential effect on estradiol and progesterone biosynthesis.

Anencephaly: a retrospective analysis in Singapore. 1976 to 1980.

Records of 93 cases of anencephaly from three maternity hospitals in Singapore between 1976 and 1980 were analysed. The incidence was 0.54 per 1000 births. No significant correlation between anencephaly and local seasonal conditions could be found for Singapore.

Hydatidiform mole in Singapore.

A five year retrospective analysis of 198 cases of hydatidiform mole recorded in three maternity hospitals in Singapore is presented. The incidence was 1 in 868 deliveries. The number of molar pregnancies in the first half of the year was higher than in the subsequent six months. The incidence of hydatidiform mole and its correlation with possible environmental factors is discussed.

Liver transplantation from living related donors: review of world experience and its implications for India.

Living related liver transplantation (LRLT) was made possible because of a better understanding of the anatomy of the liver and advances made in hepatic surgical techniques. It was developed to reduce the waiting period for pediatric recipients. In countries like Japan, which do not have brain stem death legislation, LRLT is the only modality available for treating end stage liver disease. The world experience has shown that LRLT has been successfully performed in a variety of conditions leading to acute and chronic liver failure not only in children, but in young adults as well. The initial results of LRLT appear to be better than liver transplantation from cadaveric organs in terms of graft survival and function. Donor safety has been of prime concern. LRLT has tremendous potential in India with or without the brain stem death legislation. Liver transplantation has not been performed in India although the need and expertise for it exists.

Surgical treatment of sebaceous carcinoma of eyelids with clinico-pathological correlation.

Sebaceous carcinoma is the second most frequent malignancy after basal cell carcinoma, affecting the eyelids in Singapore. From 1956 to 1985 25 cases were reported, the pathological slides of which were all reviewed. Most of the tumours arise from the upper eyelids--18/25, (72%). There were 8/25 (30%) with lymph node metastases; 3/25 (12%) died with metastatic disease and one case required exenteration of the orbit.
