The modified MVR leaving the posterior leaflet and its subvalvular tissues intact was performed in 40 patients including 17 of MR, 17 of MSR and 6 of MS from 1981 to 1984 in this institution. Mitral valves were replaced mostly with mechanical valves and using interrupted everting mattess sutures. Moreover, the aortic valve was simultaneously replaced in 13 patients and the tricuspid valve was also treated with annuloplasty, commissurotomy or replacement in 11 patients. There were no hospital deaths and 3 late deaths caused by MOF or unknown cause. Left ventricular rupture as a fatal complication did not occur. High cardiac index and LVSWI with low left atrial pressure were maintained in the immediate postoperative period. Q-d times at the base, mid portion and apex of the left ventricle were measured by 2-D echocardiograms in the follow-up period and were compared with those of patients with conventional MVR. Those at apex and mid portion were significantly shorter in patients with modified MVR than in patients with conventional MVR. These findings suggest that the left ventricular contraction might be maintained better in the former than in the latter. In conclusion, preservation of the posterior leaflet, chordae tendineae and papillary muscles should be preferable in MVR for prevention of both LOS and left ventricular rupture.
A 59 years old male with abdominal aortic aneurysm ruptured into the left common iliac vein was transferred to us with symptoms resembling deep vein thrombosis of the left lower extremity, such as leg pain, swelling and dilatation of the superficial veins. At operation, a Fogarty's occlusion catheter was inserted through the right greater saphenous vein into the inferior vena cava and inflated concomitantly during aortic clamp to prevent pulmonary embolism which may be caused by the dislodged thrombi from the aneurysm, and as well as to control back-flow-bleeding from the central. The fistula (3.0 X 1.0cm) was closed from inside of the aneurysm using the inferior wall of the aneurysm. The blood from the fistula was collected by the Cell Saver and re-transfused to the patient. The abdominal aorta was replaced with a Dacron Y-shaped prosthesis. The postoperative course was uneventful. CTR on chest X-ray subsided from 51% to 42%, cardiac output normalized from 11l/min to 6l/min, and symptoms resembling the deep vein thrombosis disappeared.

A 32-year-old man developed abdominal pain and the paradoxical hypertension after aortic coarctation repair. Abdominal angiography revealed widespread changes of the branches of the superior mesenteric artery and the celiac axis. After the conservative treatment, both the abdominal pain and the angiographic changes disappeared.

The pharmacological activities of haloperidol decanoate in several behavioral and biochemical tests for neuroleptics were pursued to validate its clinical mode of action. Haloperidol decanoate itself was about 40 times less potent than haloperidol in inhibiting 3H-spiperone binding to rat striatum in vitro and its direct application into the cerebral ventricle of mice showed no effect on brain concentrations of dopamine metabolites, homovanillic acid (HVA) and 3,4-dihydroxyphenylacetic acid (DOPAC). When injected intramuscularly, haloperidol decanoate exhibited various behavioral and biochemical effects together with prolonged accumulation of the parent drug haloperidol in dopamine-rich areas of the brain, but there was a clear difference in the time-course patterns of the effects. Conditioned avoidance responses in mice and rats were suppressed for a long period of days in the time-course pattern parallel to the elevation of brain haloperidol levels; the effect reached the peak a couple of days after the treatment and remained there over a week with gradual recovery towards 1 month after the injection. On the other hand, tolerance developed as rapidly as within 7 days to the antagonistic effects on apomorphine-induced stereotyped behaviours in mice and rats and to prolactin-releasing activity in rats. Biochemically, there was a regional difference in the brain in the time-course of effect of haloperidol decanoate. Namely, the initial increase in dopamine metabolites was no longer seen in the striatum on day 21, whereas it still persisted in the frontal cortex. Thus, the present study has confirmed on the one hand the assumption that haloperidol decanoate is a pro-drug which itself has no neuroleptic activity and validated on the other hand the clinical results that it exhibits long-lasting antipsychotic activities with minimum and tolerable extrapyramidal and prolactin-related side effects.

This report concerns a clinical evaluation for a newly devised hollow fiber oxygenator, Capiox II. It functions on a one-pump system, and is simple to set up and operate. This equipment was used for 118 patients undergoing cardiac surgery at Tokyo University Hospital from February 1982 through February 1983. The gas transfer capacity proved to be satisfactory. The employment of an air-oxygen blender prevented overoxygenation, and reasonable levels of PaO2 and PaCO2 were demonstrated with a FiO2 0.7, V/Q ratio 0.7 at normothermia. The destruction of platelets was much less with the use of this oxygenator, as compared to findings with the BOS-10. Hemolysis by Capiox II appeared to be lower than that by BOS-10, but the difference was not statistically significant. Differences were distinct in the amount of microbubbles; strikingly, no bubble was evidenced in Capiox II by the ultrasound bubble detector, during general procedures. We conclude that Capiox II is of excellent clinical value, and should be used especially for infants as well as adult patients with possible long perfusions. The merits and demerits of this equipment are given attention.
Complex carbohydrates in premalignant lesions of mouse submandibular gland tumors were examined by the lectin-peroxidase conjugate method. Peroxidase-conjugated lectins of PNA, RCA-1, DBA, SBA, UEA-1 and WGA were used to detect specific sugar residues of complex carbohydrates in premalignant lesions during experimental carcinogenesis. Marked reduction of PNA and SBA bindings occurred in duct-like structures and cystic lesions which were transformed from granular convoluted tubule cells. Premalignant lesions bound slightly to PNA, RCA-1, DBA, SBA and WGA and manifested increased UEA-1 binding. Squamous metaplastic epithelia of premalignant lesions manifested increased binding to PNA, RCA-1 and SBA as compared to those of duct-like structure and cystic epithelia.
Peroxidase-conjugated lectins were used for the histochemical detection of carbohydrates in experimental carcinomas of mouse submandibular glands. Induced carcinomas, 43 lesions from 25 cases, were examined histochemically with galactose-binding lectins (PNA and RCA-1), N-acetyl-galactosamine-binding lectins (DBA and SBA), a fucose-binding lectin (UEA-1), and a N-acetyl-glucosamine-binding lectin (WGA). In non- or slightly keratinized squamous-cell carcinomas, the lectin binding of PNA, RCA-1, DBA, SBA, and WGA was weak in tumor epithelia, and UEA-1 binding was slight. In highly keratinized squamous-cell carcinomas, lectin binding was increased in tumor epithelia, but no reaction was noted in completely keratinized regions. Desquamated materials in lumens of tumors gave an intense stain with lectins. Stromal connective tissue, including collagen fibers and basement membranes stained intensely. Lectin binding to submandibular carcinomas was different from binding to granular convoluted tubules and the striated ducts of the normal submandibular gland.
