[Usefulness and limitations of transesophageal echocardiography in anesthetic practice].

Transesophageal echocardiography (TEE) has been used as a monitor of cardiovascular function and as a diagnostic tool in anesthetic practice. TEE is the only available monitor to detect anatomical abnormalities such as of wall motion as well as valvular abnormalities. Doppler TEE has wider diagnostic functions. TEE is a very sensitive monitor to detect myocardial ischemia by recognizing wall motion abnormalities and loss of systolic wall thickening. Preload defined by left ventricular end-diastolic volume may not correlate with left ventricular end-diastolic pressure or pulmonary capillary wedge pressure (PCWP) when left ventricular compliance changes such as after coronary artery surgery. PCWP can be misleading when transmural pressure across cardiac chambers are undetermined such as in patients with cardiac tamponade or those on high positive end-expiratory pressure. In these situations, TEE is a powerful tool to link physiologic parameters and anatomy. TEE is also a very sensitive monitor to diagnose air embolism during cardiac surgery and neurosurgery in sitting position. There are, however, several shortcomings such as its cost, "too much sensitivity", requirement of some experience, interobserver variability, and so on. The computer-assisted on-line analysis would greatly augment usefulness of TEE. When these shortcomings are overcome, TEE would be one of the most important monitors in anesthetic practice.

[Methodological consideration on evaluation of lung cancer screening program: one-step process and two-step process].

A reasonable method to evaluate lung cancer screening program is to measure the reduction in death rate from lung cancer among those randomly allocated to screening program (one-step process). Alternative method is two-step process; step 1 is to measure what proportion of lung cancer patients, can be detected through screening and how early the screening can detected lung cancer patients, and step 2 is to delineate effectiveness of therapy following early detection. Advantages and disadvantages of the two processes were described in view of statistical power and sample size. In addition, a workplace-based screening program using chest X-ray files was mentioned as an example of step 1 of two-step process. The example also included economical consideration of the screening.

[Antitumor effect of IL-2 entrapped in liposomes on rat hepatoma, AH-66].

The human recombinant interleukin 2 (IL-2) was entrapped in liposomes, and the therapeutic effects of the liposomes containing IL-2 (Lip-IL-2) were experimentally studied using the rat hepatoma strain, AH-66, maintained in donryu rats and challenged subcutaneously in the inguinal region. The peri-tumor injections of Lip-IL-2 (15 X 10(4)/kg units for the IL-2 dose) significantly inhibited the tumor growth as determined from the relative mean tumor weight, although no therapeutic effects were observed when the unentrapped IL-2 or liposomes containing saline was administered rats in the same way as the injections of Lip-IL-2. They also prolonged the survival time of rats. The studies of serum IL-2 values after i.v. or s.c. injections of Lip-IL-2 revealed that IL-2 was released gradually from the liposomes containing IL-2 into the circulation. As the result of the tumor tissue staining of the immunoperoxidase 18 hrs after the peri-tumor injection of IL-2, it was shown that a number of macrophages infiltrated into the tumor tissue and degenerated tumor cells were observed adjacent to those macrophages. It is suggested that Lip-IL-2 is useful as an antineoplastic agent in the immunotherapy and that the therapeutic effects of Lip-IL-2 would be related to both the slow release of IL-2 and the cytotoxicity on the tumor cells mediated by the macrophages.

Dealing with the proportional hazards assumption when using the proportional hazards model with a single independent variable.

In order to examine the proportional hazards assumption implicit in the proportional hazards model, three types of simulations were carried out on two groups of hypothetical survival data. Simulation 1 assumed that the hazard rate for one group was higher than that for the other group but that they were not proportional. Simulation 2 was an example of hazard rates crossing each other. Simulation 3 demonstrated the effect of the extent to which the hazard rates crossed. The test statistic, Z:PH, used for testing the proportional hazards assumption was calculated, along with the logarithm of the hazard ratio with its standard error. Although the increase in number of subjects was found to favor detections of violations of the proportional hazards assumption, cases such as those in Simulation 1 were considered acceptable for clinical use. Cases such as those in Simulation 2 were not suitable for the proportional hazards model. Simulation 3 suggested that cases with a substantial degree of unproportionality could be detected to violate the proportional hazards assumption, even when groups of 100 subjects each were involved in the analysis.

[Hemorrhagic gastric ulcer associated with consumption coagulopathy in the dissecting aortic aneurysm--a case report].

A case of the hemorrhagic gastric ulcer and post-operative stomal ulcer associated with the consumption coagulopathy in the dissecting aortic aneurysm is presented. A 68 year old man, who had diagnosed as having the dissecting aortic aneurysm (DeBakey III) in 1985 and had been attending to our hospital as an outpatient since then, was admitted to our hospital because of the hemorrhagic gastric ulcer on July 3rd, 1987. Although the wide resection of stomach was performed after the admission, the hemorrhagic stomal ulcer was developed 4 weeks after the operation. The administration of heparin in addition to antacids to improve the consumption coagulopathy, having been caused by thrombus formation in the aneurysm, was effective in control of the hemorrhage and also making the stomal ulcer scarred. It is suggested that ischemia of the stomach mucosa, which seemed to have been induced by the dissecting aortic aneurysm, was responsible for the development of the ulcers in this case and the hemorrhage from them was caused mainly by the consumption coagulopathy derived from thrombus formation in the aneurysm.

Biphasic contraction induced by ouabain in human umbilical arteries.

The effects of nifedipine on the contraction induced by ouabain were investigated in helically cut human umbilical arterial strips. Ouabain (10(-7)-10(-4) M) induced a rapid transient (early) contraction and a persistent slow (late) contraction. Both effects were dose-dependent. The early contractions were inhibited by nifedipine (5 x 10(-9) and 10(-7) M) and by Ca2+-free solution containing 0.2 mM EGTA but the late contractions were not altered by nifedipine. The results suggest that ouabain could facilitate the transmembrane Ca2+ entry through voltage-sensitive Ca2+ channels.

Molecular cloning of the cDNA for the large subunit of the high-Ca2+-requiring form of human Ca2+-activated neutral protease.

A nearly full-length cDNA clone for the large subunit of high-Ca2+-requiring Ca2+-activated neutral protease (mCANP) from human tissues has been isolated. The deduced protein, determined for the first time as an mCANP, has essentially the same structural features as those revealed previously for the large subunits of the low-Ca2+-requiring form (muCANP) [Aoki, K., Imajoh, S., Ohno, S., Emori, Y., Koike, M., Kosaki, G., & Suzuki, K. (1986) FEBS Lett. 205, 313-317] and chicken CANP [Ohno, S., Emori, Y., Imajoh, S., Kawasaki, H., Kisaragi, M., & Suzuki, K. (1984) Nature (London) 312, 566-570]. Namely, the protein, comprising 700 amino acid residues, is characterized by four domains, containing a cysteine protease like domain and a Ca2+-binding domain. The overall amino acid sequence similarities of the mCANP large subunit with those of human muCANP and chicken CANP are 62% and 66%, respectively. These values are slightly lower than that observed between muCANP and chicken CANP (70%). Local sequence similarities vary with the domain, 73-78% in the cysteine protease like domain and 48-65% in the Ca2+-binding domain. These results suggest that CANPs with different Ca2+ sensitivities share a common evolutionary origin and that their regulatory mechanisms are similar except for the Ca2+ concentrations required for activation.

[Physiological significance of plasma free and conjugated dopamine in healthy volunteers--with special reference to sympathetic nerve activity].

In order to elucidate the physiological significance of plasma dopamine, blood pressure, pulse rate (PR), plasma concentrations of free or conjugated dopamine (free or conjugated pDA), noradrenaline (pNA) and adrenaline (pAd) were measured in 9 healthy volunteers. Blood sampling for the measurements was performed at a basal condition maintaining a supine position for 60 minutes, after twenty minutes 60 degrees head-up tilt (tilt) and an intravenous infusion of 1000 ml 0.9% saline for 2 hours. Following tilt, mean values in diastolic and mean blood pressure, PR, pNA and pAd were significantly increased, while free, conjugated and total pDA were decreased. On the other hand, saline infusion yielded significant decreases in hematocrit, pNA, free, conjugated and total pDA, but blood pressure, PR and pAd remained at the same level. Free/conjugated pDA ratio did not change during tilt or saline infusion. The basal value of free, conjugated or total pDA did not significantly correlate with blood pressure, PR, pNA or pAd, respectively. Furthermore, no significant correlations between the changes in pDAs and hemodynamic parameters, pNA or pAd by tilt or saline infusion were observed. From these results, it was suggested that plasma free or conjugated dopamine in physiological conditions may not be released from sympathetic nerve endings or adrenomedullary glands. Further investigations are needed to clarify the physiological significance of plasma dopamine in humans.

The urokinase type of plasminogen activator in cancer of digestive tracts.

The amounts of urokinase (UK) antigen and tissue plasminogen activator (t-PA) antigen were determined in plasma, urine and tissues of patients with cancer of digestive tracts. Urinary levels of UK, but not those of t-PA increased in patients with cancer, and generally decreased after the removal of cancer by operation. Urinary UK levels kept increasing in patients with recurrence of cancer or with metastasis into liver or peritoneum. Plasma levels of t-PA, but not those of UK decreased in patients with cancer. When the amounts of UK were compared in cancer tissues and their adjacent normal mucosal layer, cancer tissues always had higher levels of UK, but t-PA levels were same between tumor tissues and normal mucosa. The results suggest that the type of plasminogen activator was UK-type in cancer of digestive tracts.

The pathophysiological role of water-sodium balance and renal dopaminergic activity in overweight patients with essential hypertension.

Studies were conducted to evaluate the role of water-sodium balance and renal dopaminergic activity in the hypertensive mechanisms of overweight patients with essential hypertension (EHT). The body mass index (BMI) was correlated positively with mean arterial pressure, plasma volume, extracellular fluid volume, or total exchangeable sodium and negatively with plasma noradrenaline concentration or plasma renin activity in patients with EHT. Fractional excretion of sodium (FENa) was significantly lower in overweight patients than in normal weight patients with EHT. Hypotensive effect of sodium restriction or the natriuretic response to infused dopamine was more remarkable in overweight patients with EHT than in normal weight patients with EHT. Urinary excretion of free dopamine (UDA) was correlated positively with simultaneously measured urinary excretion of sodium or FENa and negatively with the natriuretic response to dopamine infusion. In addition, UDA was positively correlated with the BMI in normal weight patients with EHT, whereas the relation between the UDA and the BMI was significantly negative in overweight patients with EHT. These findings suggest that the expansion of body fluid volume and sodium might result from the blunted natriuretic ability due to an attenuation of the renal dopaminergic activity in overweight patients with EHT. The expansion of body fluid volume and sodium may play an important role in the hypertensive mechanisms of overweight patients with EHT.

Smoking among junior high school students in Nagoya, Japan.

A questionnaire survey was administered in January 1982 to 3090 junior high school boys and girls in Nagoya, Japan. The proportion of current regular smokers who smoke at least one cigarette per week was 3.6% for boys and 0.5% for girls. By the third grade this had increased to 6.9% and 1.2% respectively for each sex. Multivariate analysis of associated social psychological factors revealed that peer smoking was most strongly related to individual smoking status. Parental smoking, sibling smoking, educational aspiration, and attitudes toward anti-smoking legislation for minors were also significantly related to smoking status. The fostering of resistance to social pressure to smoke, family involvement, strict execution of the law, and increased experiences of success in academic, social and physical activities should be considered in smoking prevention programmes for this age group.

Beta 1- and beta 2-adrenoceptor-mediated relaxation responses in peripheral arteries from spontaneously hypertensive rats at pre-hypertensive and early hypertensive stages.

Beta-adrenoceptor-mediated relaxation of isolated arteries from spontaneously hypertensive rats (SHR) was studied. The relaxation responses of the femoral arteries of pre-hypertensive SHR (PHSHR) to isoprenaline (ISO) and dibutyryl cyclic AMP were less than those of the age-matched tissues of rats of the Wistar-Kyoto strain (WKY). The responses of the mesenteric arteries of PHSHR to ISO and noradrenaline (NA) but not fenoterol (FEN) and forskolin were diminished when compared to those from the WKY tissues. The diminished relaxation response to ISO was observed in the SHR arteries with and without endothelial cells as compared to similar arteries from WKY. Relative potency ratios of ISO:FEN:NA and the Schild plot data for atenolol and butoxamine suggested that there were only beta 1-adrenoceptors and predominantly beta 2-adrenoceptors mediating relaxation in the femoral and mesenteric arteries, respectively. There was no difference in beta-adrenoceptor subtypes between the SHR and WKY arteries. The evidence in this study suggests that beta 1-adrenoceptor-mediated relaxation was diminished in the femoral and mesenteric arteries of PHSHR.

Hypertensive effects of calcium infusion in subjects with normotension and hypertension.

The hemodynamic effects of acute hypercalcemia induced by calcium infusion, 7.5 mg/kg per h for 1 h, were investigated in normotensive and hypertensive subjects, aged 43-83 years, in the absence and presence of the calcium antagonist verapamil. Calcium infusion raised blood pressure from 125 +/- 14/62 +/- 6 to 143 +/- 13/69 +/- 7 mmHg (mean +/- s.d.) and from 173 +/- 15/81 +/- 6 to 202 +/- 23/92 +/- 9 mmHg with an increase in concentration of serum calcium from 2.2 +/- 0.1 to 3.2 +/- 0.2 mmol/l and from 2.2 +/- 0.1 to 3.2 +/- 0.1 mmol/l in normotensives (n = 20) and hypertensives (n = 12), respectively. In hemodynamic studies, the calcium infusion significantly raised blood pressure and total peripheral vascular resistance and decreased heart rate with no change of cardiac output, in normotensives (n = 8) and hypertensives (n = 8). The absolute increases in both blood pressure and vascular resistance were significantly higher in hypertensives than in normotensives, but the per cent increases were not significantly different in both groups. The per cent increases in blood pressure and vascular resistance were negatively correlated with increasing age. The infusion of verapamil at the rate of 0.15 mg/kg per h inhibited the increases in both blood pressure and vascular resistance during the calcium infusion in normotensives (n = 6) and hypertensives (n = 6). In conclusion, the calcium infusion raised blood pressure because of elevated vascular resistance, and the effects of calcium on blood pressure were inhibited by the calcium antagonist.(ABSTRACT TRUNCATED AT 250 WORDS)

The effect of ouabain on pressor responses to infused noradrenaline in patients with essential hypertension.

The relationship between changes in the pressor response to infused noradrenaline induced by intravenous injection of ouabain, an Na+,K+-ATPase inhibitor, and plasma renin activity and plasma ionized calcium was examined in 16 normotensive subjects and in 16 patients with essential hypertension. These patients were divided into 11 normal-renin and five low-renin essential hypertensives. The pressor response was significantly greater in low-renin hypertensives than in normotensives and normal-renin hypertensives. Following the injection of ouabain, the pressor response was significantly increased with no change in basal levels of blood pressure, plasma noradrenaline concentration, plasma calcium and plasma parathyroid hormone in both normotensives and essential hypertensives. The pressor response to noradrenaline was negatively correlated with levels of plasma noradrenaline and calcium after the injection of ouabain as well as before the injection in normotensives and essential hypertensives. The regression line between the pressor response and that of plasma noradrenaline or plasma calcium was significantly shifted towards a higher pressor response in normotensives, but not in essential hypertensives. The changes in the pressor response to noradrenaline induced by the injection of ouabain was significantly smaller in essential hypertensives, particularly in low-renin hypertensives, compared with normotensives. These results suggest that: (1) ouabain increases the pressor response to noradrenaline; (2) this increase is related to calcium metabolism; (3) endogenous Na+,K+-ATPase inhibitor(s) might be elevated in essential hypertensives; and (4) an increase in endogenous Na+,K+-ATPase inhibitor might, therefore contribute to an enhanced noradrenaline response in essential hypertensives, particularly in low-renin hypertensives.

Antagonism of alpha 1-adrenoceptor-mediated vascular contraction by urapidil in isolated arterial strips of spontaneously hypertensive rats.

Antagonism of alpha 1-adrenoceptor-mediated vascular contraction by urapidil was examined in helical strips of femoral and mesenteric arteries isolated from 13-week-old Aoki spontaneously hypertensive rats (SHR) and age-matched normotensive Wistar-Kyoto (WKY) rats, since this agent has antihypertensive effect through antagonizing peripheral alpha-adrenoceptors. Schild plot analyses clearly demonstrated the existence of only alpha 1-adrenoceptors in these arteries from both strains. Therefore, it is possible to demonstrate alpha 1-adrenoceptor blocking effects of nonselective alpha-adrenoceptor antagonists as well as selective alpha 1-adrenoceptor antagonists. Urapidil antagonized the alpha 1-adrenoceptor-mediated vascular contraction in a competitive fashion. The pA2 value for urapidil against alpha 1-adrenoceptors was not significantly different between SHR and WKY rats. The addition of 10(-5) M norepinephrine (NE) produced a sustained contraction in a SHR femoral artery, whereas in a WKY rat femoral artery this agonist produced a transient contraction followed by a sustained relaxation. Urapidil elicited a dose-dependent relaxation with a IC50 value of 6.50 in the SHR femoral artery precontracted with NE. In the presence of 3 x 10(-7) M timolol, a beta-adrenoceptor antagonist, femoral arteries from both strains exhibited similar magnitude of contraction in response to the stimulation with 10(-5) M NE. Under these conditions, urapidil elicited a similar extent of relaxation between SHR and WKY rats. On the other hand, the addition of 10(-5) M NE produced a sustained contraction in mesenteric arteries from both SHR and WKY rats. The contraction expressed as a ratio to the maximum developed by KCl depolarization was significantly greater in SHR than in WKY rats. In these arteries, the relaxing effect of urapidil was more evident in SHR than in WKY rats. Contractile responses to NE and relaxing effects of urapidil were not affected by timolol. These results suggest that urapidil effectively antagonized enhanced alpha 1-adrenoceptor responses seen in SHR arteries.
