Assessment of the implantation site by transvaginal ultrasonography.

OBJECTIVE: To investigate the implantation site in a singleton pregnancy. DESIGN: Transvaginal ultrasonography (US) was performed at the mid or late follicular phase and/or at very early gestation before 6 weeks. SETTING, PATIENTS: Ultrasound monitoring was performed on 21 women with spontaneous cycles or treated by administration of 2.5 mg/d bromocriptine mesylate (Parlodel; Sando Co., Tokyo, Japan) for occulted hyperprolactinemia at the Fertility Clinic of the Department of Obstetrics and Gynecology at Gunma University Hospital. MAIN OUTCOME MEASURES: The site of the ovary with a growing follicle and/or a corpus luteum of pregnancy, and the location of a gestational sac in longitudinal and transverse views were observed. RESULTS: When ovulation occurred in the right ovary, 12 of the 14 gestational sacs were located on the right wall, 1 on the midwall, and 1 on the left wall. Ovulation in the left ovary resulted in 5 gestational sacs located in the left wall and 2 on the midwall. That is, of 21 gestational sacs, 17 were located on the ipsilateral uterine wall to the ovulating ovary, 3 on the midwall, and only 1 on the contralateral. CONCLUSION: Implantation occurs on the ipsilateral uterine wall to the ovulating ovary.

Effect of postischemic reperfusion on the pancreas.

In an examination of the effect of ischemia and reperfusion on the generation of active oxygen species during pancreatic cell damage, a short-term ischemia and reperfusion model was prepared by the occlusion and reperfusion of both the anterior mesenteric artery and the celiac artery in rats. Following 60 minutes of occlusion plus 7 hours of reperfusion of the anterior mesenteric artery and the celiac artery, the serum concentrations of amylase and lipase rose significantly to 7 and 6 times the respective control values. After 30 minutes of occlusion plus 7 hours of reperfusion, or after 7 hours of occlusion without reperfusion, amylase and lipase levels were not changed significantly. The continuous intravenous infusion of superoxide dismutase (3600 U/kg/hour) in rats receiving 60 minutes of occlusion plus 7 hours of reperfusion suppressed the rise in serum amylase and lipase values to 25 percent of the values in the non-injected group. These results suggest that the active oxygen species which are generated by the short-term ischemia and reperfusion method injure the endothelium and cause hyperamylasemia and hyperlipasemia. Inhibition of the rise in serum amylase and lipase concentrations by pretreatment with a scavenger of active oxygen, superoxide dismutase, suggests that the active oxygen species are involved in the pathogenesis of acute pancreatitis.

[Combined spinal and epidural analgesia for caesarean section].

We compared the combined spinal and thoracic epidural analgesia with 2% lidocaine for caesarean section, with spinal analgesia using 2% lidocaine, tetracaine or dibucaine, and also with lumbar epidural analgesia. The analgesia at high thoracic level could be achieved with combined spinal and epidural analgesia more easily than with others. The frequency of side effects with the combined spinal and epidural analgesia, such as hypotension, decrease of fetal heart rate and so on, was less than that with others. In conclusion, the combined spinal and thoracic epidural analgesia with 2% lidocaine is more useful for caesarean section, than the spinal analgesia or the lumbar epidural analgesia alone.

[Postoperative apnea in preterm infants after inguinal herniorrhaphy].

Preterm infants may become apneic during immediate postoperative period. We studied prospectively postoperative apneic attack in 167 preterm infants after inguinal herniorrhaphy with nitrous oxide-halothane anesthesia. Their mean gestational age was 30 +/- 3 weeks. The mean postnatal age at operation was 14 weeks. The post-conceptual age varied between 36 and 56 weeks. The mean birth weight was 1351 +/- 395 kg. Although many of them had a risk factor of postoperative apneic attack, i.e.a young post-conceptual age (less than 41 weeks), a light weight at operation (below 3000g), only one infant who had received emergency operation had an episode of apneic attack up to 2 postoperative days. For preventing postoperative apneic attack in preterm infants after inguinal hernia, we recommend the use of halothane anesthesia and the attention until a complete awakening.

Induction of tissue factor synthesis in human umbilical vein endothelial cells involves protein kinase C.

Incubation of human umbilical vein endothelial cells with one of the following compounds: endotoxin, recombinant interleukin-1 beta, recombinant tumor necrosis factor alpha, allogenic lymphocyte subpopulations or phorbol ester resulted in significant induction of tissue factor synthesis. Diacylglycerol had the same effect and also enhanced synergistically the induction caused by endotoxin and interleukin-1 beta. Two different inhibitors of protein kinase C, H7 and sphingosine, inhibited tissue factor synthesis at concentrations which did not depress protein synthesis in general, suggesting that protein kinase C is involved in the processes leading to tissue factor synthesis. Cells down-regulated for the tissue factor response to TPA responded essentially normally to endotoxin and interleukin-1 with regard to tissue factor synthesis.

Continuous recording of changes in intracranial pressure using interference echoes of ultrasonic wave: a preliminary report of practicality and clinical evaluation.

We attempted to examine intracranial pressure (ICP) noninvasively using probes receiving interference echoes of ultrasound from thin layers between the skull and the surface of the cerebrum. The reception of echo through bone flaps less than 5-mm thick was good. Normal adults (4) and patients (4) were examined by attaching ultrasound probes to the frontotemporal scalp. Recordings were performed during rest, Valsalva maneuver, hyperventilation, and pumping of a shunt-flushing device. Ultrasound recordings of ICP were compared with those of the scalp using two different time windows. The effect of scalp vessels on the ultrasound recordings of ICP was evaluated. The waves from a pressure transducer and the ultrasound probe were similar. This method was found to be useful for ICP recording.

Prediction and prevention of ovarian hyperstimulation by monitoring endogenous luteinizing hormone release during purified follicle-stimulating hormone therapy.

OBJECTIVE: To elucidate whether early detection of premature luteinizing hormone (LH) release can be useful for prediction and prevention of ovarian hyperstimulation during purified follicle-stimulating hormone (FSH) therapy. DESIGN: Retrospective and prospective study. PATIENTS: Infertile women with polycystic ovarian syndrome. MAIN OUTCOME MEASURES: Correlation between rate of endogenous LH release and incidence of excessive ovarian enlargement. In the prospective study, LH was measured by fluorometric enzyme immunoassay to obtain real-time concentration. Maximal ovarian diameter by ultrasonography. RESULTS: The rate of excessive ovarian enlargement (greater than or equal to 60 mm) in cycles that were treated by a daily administration of purified FSH and accompanied by premature LH release was 83.3%. This rate was significantly higher than that in cycles without premature LH release (24.1%, P less than 0.001). Interruption of FSH administration or alternate day FSH administration in cycles with premature LH release reduced the rate of ovarian enlargement to 45.5% (P less than 0.05). CONCLUSION: Early detection of premature LH release is useful for prediction of ovarian hyperstimulation. Ovarian hyperstimulation can be reduced by modulating the dose of FSH and the interval of administration in cycles with premature LH release.

Diaphragmatic cyst with elevated level of serum tissue polypeptide antigen.

A 58-year-old woman with a mesothelial cyst of the diaphragm showed high level of serum Tissue Polypeptide Antigen (TPA) and cyst fluid TPA. Mesothelial cells of the cyst may excrete TPA and TPA was accumulated in the cysts. The measurement of tumor marker such as TPA of the serum or of the content of the cyst is useful in observation of the clinical course.

Association of immunoreactive-inhibin levels with luteal function.

To evaluate immunoreactive (IR) inhibin as a marker of corpus luteal function, 134 women with natural ovulatory cycles were studied. Serum IR-inhibin levels during the luteal phase were significantly lower in women with low progesterone (P4) levels (less than 10 ng/ml). Endometrial dating correlated more with serum P4 than IR-inhibin levels. All 14 conceiving cycles showed serum P4 levels greater than or equal to 10 ng/ml and IR-inhibin levels greater than or equal to 3.67 IU/ml during the midluteal phase. Furthermore, IR-inhibin levels in the early follicular phase seemed to be associated with lowered P4 levels in the ensuing luteal phase. These results suggest that IR inhibin may be closely related to corpus luteum function.

[Amounts of urinary metabolites of p-chloroaniline and their half lives in a patient with acute poisoning].

The aim of this study was to elucidate the time courses of amounts of urinary p-chloroaniline (p-CA) metabolites during acute poisoning and to establish a suitable method for biological monitoring of exposure to p-CA. Urinary p-CA metabolites which were previously identified in a patient with acute p-CA poisoning were determined quantitatively over time during admission of a patient by high-performance liquid chromatography (HPLC) and by the colorimetric method of diazo-positive metabolites (DPM). The data of urinary excretion of each p-CA metabolite were fitted to two-compartment model of pharmacokinetics. Major urinary metabolites of p-CA were conjugated p-CA and conjugated 2-amino-5-chlorophenol (2-A-5-CP), and the detected amounts of 2,4-dichloroaniline (2,4-DCA) and free p-CA were small. During the rapid phase for the disappearance of each metabolite, half lives of p-CA, 2-A-5-CP and 2,4-DCA were 2.4, 1.7 and 1.7 h, respectively, and during the slow phase, they were 4.5, 3.3 and 3.8 h, respectively. Urinary p-CA occupied about 62% of the total metabolites detected in the urine, 2-A-5-CP about 36%, and 2,4-DCA about 1%. 2-A-5-CP and p-CA were detected in the urine of the patient obtained on days 3 and 4, respectively, after the poisoning, but the amount of DPM after the second day of the poisoning was in the range of that of non-exposed persons.(ABSTRACT TRUNCATED AT 250 WORDS)

Identification of urinary metabolites of human subjects acutely poisoned by p-chloronitrobenzene.

1. Urinary metabolites from human subjects acutely poisoned with p-chloro-nitrobenzene (p-CNB) were identified by g.l.c.-mass spectrometry. 2. Eight substances, namely, a very large amount of N-acetyl-S-(4-nitrophenyl)-L-cysteine, relatively large quantities of p-chloroaniline, 2-chloro-5-nitrophenol and p-chloroformanilide produced by pyrolysis of a substance originating from p-CNB, small amounts of 2-amino-5-chlorophenol and 2,4-dichloroaniline, and traces of p-chloroacetanilide and 4-chloro-2-hydroxyacetanilide, were detected in urine samples. 3. All of the absorbed p-CNB was metabolized prior to excretion, as the parent compound was not found in urine. 4. N-Acetylated metabolites of p-chloroaniline and 2-amino-5-chlorophenol, resulting from p-CNB by metabolism, were found in only one of eight individuals indicating that this pathway is weak or may be absent in some humans. 5. A scheme for the pattern of metabolic pathways of p-CNB is proposed, and chlorination was considered to be a possible novel metabolic pathway.

Effect of intraarterial active oxygen species on the rat pancreas.

To explore the role of active oxygen species in the development and progression of acute pancreatitis, we studied the direct toxic effect on the rat pancreas of active oxygen species: superoxide anions generated by xanthine/xanthine oxidase (X/XO), and hydrogen peroxide (H2O2). After a continuous injection of X (10(-3)M, 0.9 ml/hour)/XO (1 U/ml, 0.3 ml/hour) into the celiac artery supplying the pancreas, hemorrhages and extensive edema developed in the pancreas. The amylase and lipase concentrations in the peritoneal fluid rose to 10.3 and 13.8 times the control values, respectively. The subsequent infusion of superoxide dismutase (SOD, 3600 U/hour) into the external jugular vein completely suppressed hemorrhages, and reduced edema and the amylase and lipase concentrations in the peritoneal fluid. After continuous injection of H2O2 (100 microM, 1.2 ml/hour), via the celiac artery, marked hemorrhages and edema appeared in the pancreas, and the amylase and lipase concentrations in the peritoneal fluid were 11.1 and 17.3 times higher than the control values, respectively. These abnormalities were significantly suppressed by the intravenous infusion of catalase (10 mg/kg/hour) or gabexate mesilate (10 mg/kg/hour). These results indicate that active oxygen species have a direct toxic effect on the pancreas and that free radicals may play an important role in the development of acute pancreatitis.

Endometrial dating in the conception cycle.

OBJECTIVE: To investigate endometrial dating during the conception cycle. DESIGN: Endometrial biopsies of the last half of the luteal phase of conception cycles were dated based on urinary luteinizing hormone (LH) surges. SETTING: Endometrial samples were obtained from women attending the fertility clinic of the Department of Obstetrics and Gynecology at Gunma University Hospital. PATIENTS: One hundred eighty-five women, of whom 15 (8.1%) conceived during this study. MAIN OUTCOME MEASURES: Urinary LH surge, serum progesterone (P) levels, and endometrial dating. RESULTS: All 10 women who conceived showed 2 or within 2 days in-phase endometrial biopsies on days 7 to 11 after the LH surge. In 4 of 5 women biopsied on day 12, an unequivocally delay in the stroma was found, i.e., a persistence of edema and poor development of predecidual reaction. Since serum P levels in conception cycles were significantly higher than in nonconception cycles on days 10, 11, and 12, we interpreted this delay in the stroma as a consequence of conception. CONCLUSION: Endometrial specimens during the last half of the luteal phase of conception cycles are in-phase until day 12. On day 12, gestational hyperplasia causes apparent out-of-phase.

[Anesthetic induction of children with high concentrations of sevoflurane].

The induction and intubation with high concentrations of sevoflurane (4% or 6.4%) were investigated in 180 children with ages ranging from 1 to 6yr 12m. Average induction time (duration between the start of induction and loss of consciousness) with 4% sevoflurane was 56 sec and that with 6.4% was 47 sec. Eight patients out of 90, to whom 4% sevoflurane was applied, could not be intubated due to closure of vocal cord, cough or closure of mouth. The most serious side effect was convulsion during induction, of which incidence was 6% of all patients, and occurred more frequently with 6.4% sevoflurane. The convulsion spread from hand on one side to all over the body, and disappeared spontaneously within 30 sec. There was no significant side effect in cardiovascular system, such as hypotension, arrhythmia or bradycardia. In conclusion, smooth intubation was possible without use of muscle relaxants with high concentrations of sevoflurane. This can be one of the choices for induction of children, though there is a possibility of occurrence of convulsion.

[Perioperative management of uvulopalatopharyngoplasty for the obstructive sleep apnea patient].

The perioperative courses of 10 patients with obstructive sleep apnea syndrome who underwent uvulopalatopharyngoplasty were reported. No hypnotic was administered at night before operation. Premedication consisted of atropine 0.5 mg, meperidine 35 mg, given i.m. 1 hr prior to surgery. No patients developed respiratory depression after premedication. General anesthesia was induced with iv thiopental, and succinylcholine was given to facilitate tracheal intubation. Airway management was technically difficult in all patients. Anesthesia was maintained with N2O-O2-enflurane under controlled ventilation. At the end of the surgery the insertion of a nasopharyngeal airway was performed for the prevention of airway obstruction. In 8 patients it was possible to remove the airway in the next morning. Dexamethasone was administered 3 times to reduce pharyngeal edema during perioperative period. Trachea was extubated when the patient was fully awake and alert. Perioperatively the following points are important in the management of uvulopalatopharyngoplasty. 1. Assessment of airway and the method of airway control. 2. Reduction of pharyngeal edema. 3. Using caution against the postoperative hemorrhage. 4. Avoidance of sedation. 5. Close observation extending into the postoperative period.

[The correct endotracheal tube size for infants and children].

The correct endotracheal tube size was assessed in 600 anesthetized infants and children. The correct tube size was determined by the airway pressure at which a gas leak around the endotracheal tube occurred, when the lungs were inflated with slowly increasing positive pressure. Endotracheal tube size correlated most with body length where the correlation coefficient was 0.973 (P less than 0.01), followed by body weight, tracheal size in X-ray photograph and age. But, there were four correct tube sizes for patients with the same body length.
