[Nutritional status and quality of life following gastrectomy].

The nutritional status of 18 patients after total gastrectomy was recorded by anthropometric measures, serum protein and immunological investigations. 15 patients underwent gastrectomy due to carcinoma, the others due to benign diseases. Those patients with carcinoma had been free of signs of malignancy for at least 6 months. According to the elected criteria no patient had a normal nutritional status, there were slight to moderate disturbances of the protein metabolism. By mass reduction the body of the gastrectomised patients had adapted to the decreased intake of protein and calories. In this way the body reaches a new balance and remains efficient. The quality of life of most gastroectomised patients is satisfactory. It depends on the incidence of alimentary complaints and the ability of intake of sufficient food. On the basis of these results there are no serious arguments against Graham's oesophagojejunostomy.

[Reservoir capacity of esophagojejunostomy following gastrectomy determined by the radioisotope method].

The substitute reservoir emptying was determined quantitatively in 16 patients with total gastrectomy by means of the radio-isotope method. The continuity was restored in 15 patients by an entero-anastomosis in modification according to Graham and in one patient by esophagoduodenostomy. The investigation was carried out in 11 patients with total gastrectomy in upright position and in 4 patients in lying position. As control group there were used 14 normal subjects. The emptying traces, the radioactive half-life, and the emptying rate show a clear acceleration of the emptying. The behaviour of the emptying returns to normal in lying position. A constant relation of these findings could not be found out compared with the nutritional state or the postprandial complaints.

[Attempted suicide by intake of pholedrine].

It is reported on an attempted suicide of a 21-year old female patient with a lethal dose of 60 to 80 tablets pholedrine longo. A pulmonary oedema could be observed in the patient. This was accompanied by hypotensive crises of blood pressure, which compelled us to use dopamin. Under therapy a stabilization of the behaviour of the patient could be achieved. Pathological liver values were diagnosed. The findings got were discussed and compared with the data of former publications.

Factor XII assay with the chromogenic substrate chromozym PK.

A chromogenic assay for the determination of factor XII using the chromogenic substrate Chromozym PK was evaluated. The assay was linear in the range 10 U/l to more than 200 U/l. Using the assay, the normal range of factor XII (50 healthy volunteers at random) was 136 U/l +/- 27 U/l. Kallikrein-inhibiting concentrations of aprotinin did not influence factor XII. Comparison of the chromogenic with the clotting assay resulted in a correlation coefficient of r = 0.831 (p-value less than 0.001). In patients with deep vein thrombosis, factor XII level was found to be reduced to about 60% of normal activity.

[Preoperative phase of colonic surgery (author's transl)].

The existing possibilities to prepare colon and rectum for surgery are discussed, optimal nutrition just before operation is particularly stressed. The efficiency of oral and parenteral antimicrobial prophylaxis ist pointed out. Apart from contra-indications, orthograde lavage of the bowel is the method of choice.

The lipoprotein abnormality in Tangier disease: quantitation of A apoproteins.

In this study we have determined by radioimmunoassay and double immunoelectrophoresis the total quantities and distributions of A apoproteins in three adult patients affected with Tangier disease (hereditary alpha-lipoprotein deficiency). Compared with normal plasma, the total quantities of apoproteins A-I and A-II in Tangier plasma were determined to be less than 1% and 5-7%, respectively. In Tangier patients, approximately 90% of the apoprotein A-I sedimented when ultracentrifugations of plasma were carried out at density 1.21 g/ml KBr. By contrast, more than 95% of the apoprotein A-II floated under those conditions. In normal plasma, approximately 90% of both apoproteins A-I and A-II is found in the 1.063-1.21-g/ml KBr density fraction. These findings suggest that complete dissociation of A apoproteins occurs in Tangier plasma. This dissociation of apoproteins was confirmed by double immunoelectrophoresis with monospecific antisera. Immunochemical and electrophoretic experiments did not provide evidence for a structural abnormality of apoprotein A-I in these patients, The results taken together strongly suggest that normal high-density lipoproteins are absent from Tangier plasma.

The rheological factor in mucociliary clearance.

The viscoelastic properties of sputum samples obtained from bronchitic patients were measured and correlated with the speed at which the samples were transported by the ciliary action of isolated pharyngeal palates of bull frogs maintained in amphibian Ringers solution at 28 degrees C. The viscoelastic properties were measured at very low shear stress which allowed to express them in terms of Newtonian viscosity and linear elastic modulus, which greatly minimized the variations in such measurements previously experienced with sputum. Samples with lower values of viscosity and moduli (less than 10,000 poises and 15 dynes per square centimeter, respectively) showed faster transport rates in a system where ciliary beat frequency and volume of added mucus were kept relatively constant.

Amino-acid sequence of lac repressor from Escherichia coli. Isolation, sequence analysis and sequence assembly of tryptic peptides and cyanogen-bromide fragments.

The lac repressor from Escherichia coli, composed of four identical subunits with a molecular weight of 37160, was carboxymethylated and fragmented by tryptic digestion and cyanogen bromide treatment. Using ion-exchange chromatography, gel filtration and preparative thin-layer electrophoresis and chromatography 29 of the 30 tryptic peptides were isolated in pure form. Direct Edman degradation and the dansyl-Edman technique were used to determine the sequence of the small tryptic peptides. Special emphasis was put on the sequence determination of the six large tryptic fragments which together account for 177 residues, corresponding to 51% of the repressor subunit with its 347 residues. The large tryptic fragments were analyzed after fragmentation with chymotrypsin, thermolysin and dipeptidyl aminopeptidase I. Thus the sequence of all 30 tryptic peptides could be deduced. The complete sequences of all cyanogen bromide fragments were deduced from peptides obtained by tryptic, chymotryptic and thermolytic digestion of the individual fragments and by automated stepwise Edman degradation of lac repressor and of the large cyanogen bromide fragments. The order of the cyanogen bromide fragments was given by overlapping tryptic peptides. The resulting amino acid composition of the monomer is Asp15, Asn11, Thr18, Ser30, Glu14, Gln27, Pro13, Gly22, Ala44, Cys3, Val33, Met9, Ile17, Leu40, Tyr8, Phe4, Trp2, Lys11, His7, Arg19. The sequence of lac repressor shows no similarities with that of other proteins known to bind to DNA or RNA. The N-terminal 55 residues contain two homologous regions. This part of the sequence which is involved in lac operator binding might have been formed by gene duplication.

Controlled trial of therapy in covert bacteriuria of childhood.

Sixty-three girls with covert bacteriuria were included in a controlled trial of therapy. Recurrent infection in the treated group was common and was not significantly different from the rate of persistent infection in the untreated control group. Two children in each group developed clinical pyelonephritis; the others have remained healthy and all of them have a normal rate of growth. 2 years after diagnosis three of the thirty-four children in the control group and one of twenty-six children in the treated group have radiological evidence of new scars of pyelonephritis. These changes were relatively minor and in both groups of children renal growth was similar to that in normal children. It is suggested that for most of these children therapy is not essential, and that when renal changes occur they are of little or no significance. Prescriptive screening for cobert bacteriuria of childhood cannot be recommended at present.

The amino-acid sequence of lac repressor.

The amino-acid sequence of lac repressor from Escherichia coli has been determined. The sequence contains 347 residues in the subunit single peptide chain. It shows no similarities with the sequences of histones or the known part of beta-galactosidase.
