Surgical margins in the treatment of nonmelanoma skin cancer and mohs micrographic surgery.

Traditional surgical treatment of nonmelanoma skin cancer includes excision with subsequent evaluation of surgical margins, either via frozen sections intraoperatively or after excision and closure. Accurate communication between surgeon and pathologist regarding the meaning of surgical margins should be confirmed. Recurrences of tumor growth may in part be attributed to asymmetrical tumor growth patterns with extension of tumor in an unanticipated direction. Mohs micrographic surgery is an outpatient procedure that maximizes surgical margin evaluation while minimizing the amount of tissue that must be excised. This article will discuss the concept of surgical margins in excisions of nonmelanoma skin cancer and the role of Mohs micrographic surgery.

Laparoscopic orchiectomy for unilateral intra-abdominal testis.

PURPOSE: A brief description of laparoscopic orchiectomy for undescended testis and a representative case report. METHODS: A 22-year-old Chinese man was admitted for removal of an intra-abdominal testis. Laparoscopic orchiectomy was used to remove the testis at the time of laparoscopic orchiectomy. The patient had an uneventful postoperative course. RESULTS: A concurrent inguinal hernia was discovered during the procedure and repaired. CONCLUSIONS: The use of laparoscopic surgery has proven to be a safe and effective means of treatment for the intra-abdominal testis.

Small-cell undifferentiated carcinoma of neuroendocrine type originating in the gallbladder.

PURPOSE: We report a case of small-cell undifferentiated carcinoma with neuroendocrine (SCUCN) of the gallbladder in a 67-year-old man who presented with suspected cholelithiasis. Treatment included a cholecystectomy and a 4-cycle course of etoposide and carboplatin. CONCLUSIONS: Small-cell undifferentiated carcinoma with neuroendocrine features of the gallbladder is a rare disease with approximately 30 cases reported in the literature. Clinical characteristics include an association with cholelithiasis, an elderly age distribution, a female preponderance, and a correlation with cigarette smoking. It is known to behave aggressively and carry a grave prognosis, with extensive local invasion and early metastasis being characteristic. Medical and surgical therapies exist and have demonstrated best results when used in combination.

Nevus lipomatosus cutaneus superficialis.

Nevus lipomatosus cutaneus superficialis (NLCS) is a rare developmental anomaly characterized by isolated ectopic mature adipose tissue in the dermis. We describe a child with a NLCS present from birth that was growing in size. The lesion was removed by simple excision and has not recurred.

An unusual case of factitious onychodystrophy.

Nail disorders are a common dermatologic problem, and when such disorders are self-induced, the diagnosis and treatment can be challenging. We present an unusual instance of factitious onychodystrophy with associated erythronychia of the nail bed.

Furuncular myiasis secondary to Dermatobia hominis.

Dermatobia homininis is the most common cause of furuncular myiasis in Central and South America. It is diagnosed based on a history of travel to an endemic region and the characteristic cutaneous lesion. We present a 54-year-old patient who presented with both a travel history and cutaneous findings of furuncular myiasis.

Case reports: suicidal ideation in the dermatology clinic.

Many dermatology patients have significant psychiatric comorbidities, including depression and suicidal ideation. In light of the isotretinoin controversy, the relevance of this subject has never been greater. Therefore, it is imperative for dermatologists to understand the appropriate actions to take when faced with a potentially suicidal patient. We present a case of a clinic patient who expressed suicidal intent and discuss necessary steps to take in such a situation.
