[Hemolysis test of the five kinds of impeller blood pumps in vitro].

Hemolysis caused by blood pumps is a very important characteristic. In vitro hemolysis test circuits were constructed to operate the model I centrifugal pump, the model II axial flow pump, the magnetic coupling pump and the model I & II spiral mixed pump. The output of all pumps was set at flow 5 L/min, an average pressure of 100 mmHg. Experiments were conducted for 4 hours at room temperature(25 degrees C) with 500 ml fresh anticoagulant sheep blood. Blood samples were taken for plasma free-hemoglobin measurement, and the change in temperature at the pump outlet port was measured during the experiment. Calculate the normalized index of hemolysis (NIH). The results showed that there was no relationship either between the pump rotational speed and the NIH in five types of blood pumps, or between change in temperature at the pump outlet port and NIH. The dynamic fluid field caused by pump design and structure could be the main cause of blood damage.

[Development of a rapid separating incubation method for detection of Escherichia coli O157:H7 in food].

To detect Escherichia coli O157:H7 in food, a selective and differential medium, CTV-MUG-RSMAC agar, was designed. Using this medium, the growth of most interfering organisms could be inhibited, and three main biochemical characteristics, including sorbitol-negative, rahmnose-negative and MUG-negative, to identify E. coli O157:H7 could also be simultaneously recognized. Sorbitol-positive aberrance strains of E. coli O157:H7 could be distinguished on the medium. The procedure of detecting food sample includes adding sample to selective broth and incubating overnight at 42 degrees C, streaking the inoculum to CTV-MUG-RSMAC agar and incubating at 37 degrees C for 24 h, and picking the typical colonies to react with sera for O157 and H7 antigens, and then take biochemical tests. The procedure could be finished within 3 days. This method has been applied to 126 food samples, and E. coli O157:H7 was detected from 3 samples. The results were in coincidence with those using FDA method.

Effect of arsenic trioxide on inhibition of restenosis after rabbit vascular injury and its mechanism.

OBJECTIVE: To investigate the effect and mechanism of arsenic trioxide (As(2)O(3)) on the prevention of restenosis after vascular injury. METHODS: Apoptosis induction of As(2)O(3) on cultured rabbit vascular smooth muscle cells (VSMCs) in vitro was observed. Thirty-two New Zealand white rabbits were randomly divided into 2- and 4-wk study groups, and their controls. 10% As(2)O(3) at 2.5 mg x Kg(-1) x d(-1) or 0.9% sodium chloride was intraperitoneally infused for 3 days before left common carotid arteries were denudated with a balloon. After denudation 2- and 4-wk animals were sacrificed for morphometry and immunohistochemical studies on carotid arteries, and for histopathology on liver and kidney. RESULTS: It was shown via cellular morphology and DNA fragments in electrophoresis that promotion of As(2)O(3) on cultured vascular smooth muscle cell apoptosis was dependent upon its concentration and duration. Compared with the control animals, the mean vascular intimal proliferation areas were reduced in 2-wk study animals (P < 0.05) and no difference was shown in 4-wk (P > 0.05), while the mean vascular luminal areas were all enlarged in both study groups (all P < 0.05). The downregulated bcl-2 expression (all P < 0.05 in 2- and 4-wk) and the upregulated bax expression (P < 0.01 in 2-wk; P < 0.05 in 4-wk) were detected by immunohistochemistry, in comparison with control groups. Gene bcl-2 and bax protein expression were consistent with the suppression of intimal proliferation and the enlargement of luminal areas in corresponding sections. CONCLUSION: As(2)O(3) induces apoptosis of VSMCs and inhibits experimental restenosis effectively after artery injury, via downregulation of bcl-2 and upregulation of bax expression.

[Multiple primary malignant neoplasms].

OBJECTIVE: To investigate the epidemiological and clinical features of multiple primary malignant neoplasms (MPMNs). METHODS: One hundred and twenty-five cases of more than or equal to three primary malignant neoplasms detected in the period of 1958 to 2001 were studied retrospectively in terms of frequency, age of onset, sex ratio, tumor location and the interval between sequential tumors. RESULTS: The overall incidence of MPMNs was 0.090%; for three, four and five MPMNs was 0.078%, 0.009% and 0.001%. The mean age of onset of MPMNs was 64 years. The sex ratio, of male to female, was 1.0:1.2. The mean time interval was 7.1 years, and it reduced from 5.7 to 2.4 years in a descending order of the first, second, third, forth and fifth primary tumors. Three, four and five primary malignant neoplasms were predicated respectively in the large intestine, breast, lung, esophagus, uterus, skin; the large intestine, breast, skin stomach, urinary bladder, uterus; and the large intestine, breast, skin, parotid glands. CONCLUSIONS: The overall incidence of multiple primary malignant neoplasms seems to ascend. The age of onset of MPMNs tends to be increase. The incidence of in women is higher than in men. The time interval from the first to fifth primary tumor is decreasing. The susceptible organs are identical organ, bigeminal organs or organs of the same system.

[Translational control of human interferon alpha 1 gene expression in E.coli].

OBJECTIVE: To increase prokaryotic expression level of IFN-alpha 1C gene through the quantitative theory of translational control and the translational enhancer sequence. METHODS: Stepwise polymerase chain reaction (PCR) was used to alter the 5 terminal cDNA sequence of IFN-alpha 1C in three different grades of base mutation. In this way, the free energy (Delta G) of the secondary structure in translational initiation region (TIR) was decreased gradually. An expression plasmid (pBVE) was constructed to contain the translational enhancer cDNA sequence by modifying pBV220 upstream of the SD region. RESULTS: The expression levels of three kinds of IFN-alpha 1C modified gene were all increased. Furthermore, it presented an increasing trend with decreasing in delta G varying from -50,241.6 to -22,190.0 J/mol. The highest expression was 2.43 x 10(8) U/L, covering twelve times more than its original cDNA. IFN-alpha 1C gene and its modified cDNA was inserted into pBVE as reporting genes. E.Coli cells harbouring pBVE/IFN-alpha 1Cs cDNA produced two to five times more IFN than cells harbouring pBV220/IFN-alpha 1Cs. CONCLUSIONS: pBVE containing translational enhancer is a high level prokaryotic expression vector. The theory of quantitative translational control can effectively be used to enhance the IFN-alpha 1C gene expression level in E.coli.

[Hand-assisted laparoscopic surgery in colorectal carcinoma resection: a report of 14 cases].

OBJECTIVE: To study the application of hand-assisted laparoscopic surgery (HALS) in colorectal carcinoma resection. METHODS: Fourteen patients with colorectal carcinoma underwent resection by HALS. RESULTS: Hand-assisted laparoscopic right hemicolectomy was performed in 6 of 8 ascending colon cancer patients, spending an average of 160 minutes (110 to 220 minutes) for the procedure. The amount of bleeding was 40 approximately 100 ml. Bowel sounds resumed in 36 approximately 72 hours after the operation. The average interval between the operation and patients' discharge was 8 days (7 to 12 days). For 2 patients the surgeons had to resort to laparotomy. All patients received postoperative chemotherapy. Hand-assisted laparoscopic abdominoperineal resection was performed in 5 of 6 patients with lower rectal cancer, with the average duration of 180 minutes (120 to 270 minutes), with bleeding of 40 to 80 ml. Bowel sounds resumed within 24 to 72 hours and the average discharge interval was 14 days (12 to 18 days). The urinary catheter in 6 patients was removed in 7 to 8 days after the operation. All patients with rectal carcinoma received postoperative chemotherapy and radiotherapy. One patient was shifted to laparotomy because of extensive adhesion in the pelvic cavity. Four of 11 patients treated by HALS experienced slight pain in the incision. No other complications were observed in any of the patients. Remote results await further follow-up. CONCLUSION: Hand-assisted laparoscopic surgery in the treatment of colorectal carcinoma, a safe and simple procedure, not only ensures the clearance of tumor, but also reduces operative trauma and hastens recovery.

Penalized weighted least-squares image reconstruction for dual energy X-ray transmission tomography.

We present a dual-energy (DE) transmission computed tomography (CT) reconstruction method. It is statistically motivated and features nonnegativity constraints in the density domain. A penalized weighted least squares (PWLS) objective function has been chosen to handle the non-Poisson noise added by amorphous silicon (aSi:H) detectors. A Gauss-Seidel algorithm has been used to minimize the objective function. The behavior of the method in terms of bias/standard deviation tradeoff has been compared to that of a DE method that is based on filtered back projection (FBP). The advantages of the DE PWLS method are largest for high noise and/or low flux cases. Qualitative results suggest this as well. Also, the reconstructed images of an object with opaque regions are presented. Possible applications of the method are: attenuation correction for positron emission tomography (PET) images, various quantitative computed tomography (QCT) methods such as bone mineral densitometry (BMD), and the removal of metal streak artifacts.

Effects of fiber intake on the blood pressure, lipids, and heart rate in Goto Kakizaki rats.

OBJECTIVE: The prevalence of hypertension in individuals with diabetes mellitus is higher than that in individuals without diabetes mellitus. High fiber intake decreases blood glucose and lipids levels. This study investigated the effects of cereal fiber intake on systolic and diastolic blood pressures in a rat model of type 2 diabetes mellitus. METHODS: Thirty male Goto Kakizaki rats were randomly assigned to one of three groups. Each group was fed a barley diet, a white rice diet, or cornstarch diet. The rats were pair fed for 16 wk. Systolic and diastolic blood pressures were measured once every 4 wk by the photoelectric oscillometric tail-cuff method without anesthesia. RESULTS: The barley diet significantly decreased systolic blood pressure from week 12 and decreased the plasma levels of total cholesterol, triacylglycerol, and low-density lipoprotein cholesterol. There were significant positive correlations between plasma lipids levels and systolic blood pressure. CONCLUSIONS: High fiber intake has beneficial effects on systolic blood pressure and blood lipids levels and suggests that fiber intake should be increased in individuals who have diabetes mellitus to prevent complications.

Effects of barley intake on glucose tolerance, lipid metabolism, and bowel function in women.

OBJECTIVE: The low consumption of grains that are rich sources of dietary fiber may be associated with the increasing prevalence of chronic diseases. This study was conducted to observe the effects of high barley (high-fiber diet) intake on glucose tolerance, lipid metabolism, and bowel function in healthy women. METHODS: Ten women volunteers (20.4 +/- 1.3 y, 19.2 +/- 2.0 kg/m2) signed informed consents and received a standard diet and a barley diet, with each diet for a period of 4 wk with a 1-mo interval (randomized cross-over design). Both diets contained the same amounts of carbohydrate, fat, and protein. RESULTS: The barley intake significantly lowered plasma total and low-density lipoprotein cholesterol concentrations and reduced plasma triacylglycerol concentration. Barley intake also increased stool volume. There was no significant difference in glucose tolerance between diet regimens. CONCLUSIONS: This study demonstrated that barley intake has beneficial effects on lipid metabolism and bowel function and suggests that the intake of a high-fiber food, i.e., barley, should be recommended to prevent chronic diseases.

The inner hair cell synaptic complex: physiology, pharmacology and new therapeutic strategies.

Within the cochlea, the sensory inner hair cells (IHCs), which transduce mechanical displacement of the basilar membrane into neural activity, release glutamate to act on postsynaptic receptor channels located on dendrites of primary auditory neurons. In turn the activity of the postsynaptic auditory dendrites is modulated by a variety of lateral efferent neurotransmitters. This presentation reviews the most recent findings obtained at the IHC synaptic complex with an original technique, namely coupling auditory nerve single unit recordings with multibarrel intracochlear perfusions. Two types of results are emphasized: (1) in physiological conditions, the activity of auditory nerve fibers involves AMPA, but not kainate or NMDA receptors, and (2) this activity is tonically modulated by dopamine, one of the lateral efferent neurotransmitters. With the increasing knowledge of molecular mechanisms involved at the first synaptic complex in the cochlea, it is now possible to envisage local treatments for spiral ganglion neurons. These treatments, available experimentally, may be used in the near future: either to protect spiral ganglion neurons against excitotoxic injury (traumatic and/or ischemic sudden deafness), or to prevent excitotoxic-induced hyperexcitability (probably the starting point of most posttraumatic tinnitus), or to delay neuronal death (neural presbycusis).

Differences between regressive eyes and non-regressive eyes after LASIK for myopia in the time course of corneal changes assessed with the Orbscan.

The aim of this study was to compare regressive eyes with non-regressive eyes after laser in situ keratomileusis (LASIK) for myopia with regard to the time course of biomechanical changes of the cornea and to evaluate the possible roles of these changes in refractive regression following surgery. 15 eyes of 9 patients with refractive regression and 15 eyes of 11 patients with no refractive regression after LASIK for myopia were enrolled in this study. The time courses of central corneal thickness (CCT) and anterior/posterior corneal curvature radiuses (CCR) were measured with Orbscan slit scanning before and 1 week, 1 month and 3 months after LASIK for myopia. The regressive and non-regressive eyes had similar time courses of CCT before and after LASIK, whereas the 2 groups differed in the time courses of corneal shifting movements. In the regression group, the posterior CCR after LASIK was relatively stable (all p > 0.05). By contrast, in the non-regression group, the postoperative posterior CCR at week 1 had a significantly steeper curvature than that at month 3 (p = 0.000). The anterior CCR in the regression group at week 1 had a significantly flatter curvature than that at month 3 (p = 0.002) postoperatively. In contrast, there was no significant change of anterior CCR postoperatively (all p > 0.05) in the non-regression group. Taken together, these data suggest that refractive regression after LASIK might be mainly induced by corneal protrusion rather than central corneal thickening.

Relationship between electrical resistance measurements and microradiographic variables during remineralization of softened enamel lesions.

The purpose of this study was to investigate the relationship between electrical resistance (ECM) measurements and microradiographic variables during remineralization of softened enamel lesions. Sound bovine enamel blocks were demineralized and, subsequently, were remineralized for 3, 4, 5, 7 or 14 days. All the specimens were assessed by ECM and transversal microradiography. The mean ECM values in each group increased after remineralization. The ECM values after remineralization (ECM1), mineral loss (DeltaZ) and maximum mineral content in the surface layer (V(max)) were significantly correlated with the remineralization time. The ECM1 values were also significantly correlated with DeltaZ and V(max). It is suggested that ECM could be used as a device to monitor mineral accumulation as a result of preventive treatment in vitro. However, it may take a much longer time to follow up the remineralization changes compared with demineralization.

Co-morbidity associated with dementia.

PURPOSE: The purpose of this study was to identify common co-morbid conditions associated with dementia subtypes and to evaluate the association of hypertension, diabetes mellitus, atrial fibrillation, congestive heart failure, and anemia with dementia subtypes relative to controls. METHODS: Hospital discharge data were used to identify 15,013 subjects from South Carolina with a diagnosis of dementia between 1998 and 1999. A control group of 15,013 persons without dementia was randomly sampled from hospital discharge records and matched to persons with dementia on the basis of age, race, and gender. Multiple hospitalizations for each patient were merged, and repeated diagnoses during separate hospitalizations were counted once. RESULTS: After adjusting for age, race, and gender, persons with Alzheimer's disease and dementia associated with medical conditions were less likely to be diagnosed with hypertension, diabetes, congestive heart failure, and atrial fibrillation than were controls. Patients with multi-infarct dementia were also less likely to have congestive heart failure, but were more likely to have diabetes. Anemia was not associated with any dementia subtype. CONCLUSIONS: There are distinct differences in comorbid conditions among dementia subtypes. Our research does not support previous studies that suggest a circulatory component to the development of Alzheimer's disease.
