Serum levels of epinephrine and norepinephrine in cirrhotic patients with portal hypertension.

OBJECTIVE: To investigate the role of catecholamines in the pathogenesis of portal hypertension. METHODS: Serum epinephrine (E) and norepinephrine (NE) concentrations in the peripheral vein, portal vein and superior vena cava (SVC) were measured by high pressure liquid chromatography in 38 portal hypertensive patients, 28 idiopathic hypertensive patients and 34 controls, respectively. RESULTS: Peripheral venous E concentrations in portal hypertensive patients and controls were 57.5 +/- 37.4 ng/L and 23.5 +/- 11.2 ng/L, respectively (P < 0.01), and peripheral venous NE concentrations were 451.1 +/- 381.2 ng/L and 183.0 +/- 83.3 ng/L, respectively (P < 0.01). Compared with controls, peripheral venous E (54.9 +/- 39.9 ng/L vs 23.5 +/- 11.2 ng/L, P < 0.01) and NE (524.3 +/- 219.9 ng/L vs 183.0 +/- 83.3 ng/L, P < 0.01) in idiopathic hypertensive patients were also significantly increased. E and NE in SVC, portal vein and peripheral artery in portal hypertensive patients were also increased, but only the elevation of E in SVC was statistically significant (207.2 +/- 55.4 ng/L vs 83.7 +/- 46.7 ng/L, P < 0.05). CONCLUSION: Our results reveal significant metabolic disorders of E and NE in portal hypertensive patients, which may play an important role in the pathogenesis of portal hypertension.

Alpha-1 adrenergic receptors in liver plasma membranes of cirrhotic patients with portal hypertension: a quantitative study.

OBJECTIVE: To investigate the changes of alpha-1 adrenergic receptors in cirrhotic liver and the relationship between the changes and the pathogenesis of portal hypertension. METHODS: The concentration and affinity of alpha-1 adrenergic receptors in the liver plasma membranes of posthepatitic cirrhotic patients with portal hypertension were quantitatively measured using radioligand binding analysis. RESULTS: Compared with 8 controls without hepatic pathological changes, the maximal binding capacity (Bmax) of 9 posthepatitic cirrhotic patients decreased (129.1 +/- 12.0 vs 142.1 +/- 14.1 fmol/mg protein, P > 0.05), dissociation constant (Kd) values increased (0.3945 +/- 0.0974 vs 0.2382 +/- 0.0548 nmol/L, P < 0.01), and the receptor maximal content (RMC) decreased (417.4 +/- 76.8 vs 739.9 +/- 167.6 fmol/g liver, P < 0.01). CONCLUSIONS: The decreased concentration and affinity of alpha-1 adrenergic receptors may play an important role in the metabolic disturbances of catecholamines often seen in some cirrhotic patients, and have implications in the pathogenesis of portal hypertension.

[Effect of intravenous injection of interleukin-8 on mouse hematopoietic progenitor cell mobilization].

OBJECTIVE: To study the mobilization effects of recombinant human IL-8(rhIL-8) on mouse peripheral blood hematopoietic progenitors (HPCs). METHODS: A daily single dose of rhIL-8 was intravenously injected into normal or G-CSF-treated mice for two days. The number of colony-forming unit-Mix (CFU-Mix) in peripheral blood was analyzed by colony assay, while the number of c-kit+ Sca-1+ HPCs was examined by immunofluorescence staining. In addition, colony forming unit of spleen (CFU-S) and marrow repopulating ability (MRA) cells were examined by transplanting lethally irradiated mice. RESULTS: Single dose of rhIL-8 (30 micrograms) significantly increased the numbers of CFU-Mix[(194 +/- 61) x 10(3)/L] and c-kit+ Sca-1+ cells [(3186 +/- 517) x 10(3)/L] in the peripheral blood as compared with that of normal saline controls [(7 +/- 3) x 10(3)/L and (644 +/- 341) x 10(3)/L] at 15 minutes after intravenous injection. G-CSF could also increase peripheral CFU-Mix and c-kit+ Sca-1+ cells to (790 +/- 48) x 10(3)/L and (10,729 +/- 2339) x 10(3)/L, respectively, after subcutaneously injection for two days, this increase was further augmented by rhIL-8 injection. IL-8 alone or in combination with G-CSF could significantly increase the number of CFU-S and MRA cells in circulating blood. CONCLUSION: IL-8 alone or in combination with G-CSF could significantly mobilize circulating HPCs.

[Single-tube nested polymerase chain reaction technique in detecting Mycobacterium tuberculosis DNA].

OBJECTIVE: To study the sensitivity and specificity of single-tube nested polymerase chain reaction (SN-PCR) technique in detecting Mycobacterium tuberculosis DNA in paraffin-embedded tissues. METHOD: BCG DNA and the paraffin-embedded tissues of 30 cases with typical tuberculous lymphadenitis were examined to detect the IS6110 specific insertion sequences DNA of Mycobacterium tuberculosis complex by general PCR(G-PCR), double-tube nested PCR (DN-PCR) and SN-PCR techniques. RESULT: Fifteen fg or more BCG DNA could show positive results by DN-PCR and SN-PCR techniques, while 480 fg by G-PCR method. The positive rates of G-PCR, DN-PCR, and SN-PCR in detecting Mycobacterium tuberculosis DNA in 30 cases with tuberculous lymphadenitis were 43%, 100%, and 100% respectively, all of which were significantly different (P < 0.01) from that of the acid-fast staining method (10%). Significant differences in the positive rates also existed between G-PCR and DN-PCR, G-PCR and SN-PCR (both P < 0.01), while the positive rates of DN-PCR and SN-PCR were found same. CONCLUSION: The sensitivities of nested PCR techniques in detecting Mycobacterium tuberculosis are significantly higher than that of G-PCR, whereas the sensitivities as well as the specificities of SN-PCR and DN-PCR are the same. Thus SN-PCR seems more practicable.

[Significance of hypercoagulation index in treatment of allergic purpura by Huoxue Huayu decoction].

OBJECTIVE: To explore the therapeutic mechanism of Huoxue Huayu Decoction (HXHYD) in treating allergic purpura (AP) and the significance of hypercoagulation index (HCI). METHODS: Patients of AP were divided into two groups. On the basis of conventional treatment of western medicine, the HXHYD was given to the treated group additionally, and the cure rate, change of hypercoagulation index of patients were compared with those of the control group treated by western conventional treatment alone. RESULTS: The cure rate of the treated group was 86%, higher than that of control group (48%) significantly. HCI of the treated group was lowered markedly after treatment, as compared with that before treatment, the difference was significant (P < 0.01). CONCLUSIONS: By the comprehensive effect of promoting anticoagulation activity, maintaining the integrity of vascular endothelia and reducing aggregation of platelets, HXHYD could reduce the HCI and improve hypercoagulative state of AP patients, so as to elevate the cure rate significantly.

[Influence of huanglian and berberine on the erythrocytic osmotic fragilitas of experimental glucose-6-phosphate dehydrogenase deficiency in rats].

OBJECTIVE: To make clear the influence of Huanglian (Coptis chinensis) and berberine on the erythrocytic osmotic fragilitas of glucose-6-phosphate dehydrogenase(G6PD) deficiency in rats, so as to infer whether Huanglian and berberine have medicamentosus hemolysis effects. METHOD: Huanglian decoction and berberine solution are applied to the erythrocytes of G6PD deficiency in rats through external and oral administration and the erythrocytic osmotic fragilitas is measured. RESULT: Only at an excessively high dose can the medicines raise the erythrocytic osmotic fragilitas of G6PD deficiency in rats. Berberine and Huanglian have similar effects, but berberine is slightly weaker. CONCLUSION: Focus on the erythrocytic osmotic fragilitas, at a general dose, Huanglian can not cause hemolysis. Huanglian and berberine are of more safety to normal erythrocytes.

[In vivo color measurement of 410 healthy maxillary anterior teeth].

OBJECTIVE: In order to measure and analyse the color characteristics of intra-oral vital teeth objectively and accurately. METHODS: The fiber-optic spectrophotometer (FMC-9204, Kunmin, China) was used for measurements of 410 healthy maxillary anterior teeth of 70 people aged from 18 to 70 years old who lived in Kunmin, China. RESULTS: The results were expressed by CIEL*A*B* systems. Statistical analysis showed that: L* of natural teeth is related to age positively, and the regressive equation is L* = 42.74 + 0.14 x age. Value of A* is related to age negatively, the regressive equation is A* = 1.73 - 0.03 x age. The color of the maxillary anterior teeth was also related to tooth position. The central incisor has the highest value of L*, but the canine is the darkest. Besides, the canine is redder, yellower, and more saturated. CONCLUSION: These results may help to make the optic character of oral prosthetic material well matched with the natural teeth.

[Portacaval shunt with H-grafts of small diameter in treating cirrhotic patients with portal hypertension].

OBJECTIVE: To evaluate the effectiveness of portacaval shunt with H-grafts of small diameter in the treatment of cirrhotic patients with portal hypertension. METHOD: 48 patients cirrhotic with portal hypertension were randomized into three groups: portacaval shunt with H-grafts of 8 mm diameter with external ring reinforced (20 patients), proximal splenorenal shunt (17), and side-to-side portacaval shunt (11). The preoperative liver function in Child's grade and the history of variceal bleeding were the same in the three groups. RESULT: The postoperative decrease in portal pressure was comparable in the three groups (0.83 +/- 0.31 kPa, 0.81 +/- 0.50 kPa, and 1.02 +/- 0.45 kPa, P > 0.05). There was no significant difference in postoperative rebleeding rate during the follow-up for 15-28 months, and in hospital mortality between the three groups. However, postoperative encephalopathy developed much less in patients undergoing portacaval shunt with H-grafts than those receiving side-to-side portacaval shunt (5.0% vs. 36.4%, P < 0.05). Postoperative transfemoral portography performed within one month showed shunt patency in 95% of the 20 patients undergoing H-graft shunting. B-mode ultrasonography follow-up of up to 15 months also showed shunt patency in all 19 survivals receiving H-graft shunt. CONCLUSION: This procedure shows less trauma, lower postoperative encephalopathy rate, while as effective in preventing recurrent variceal bleeding as the traditional portosystemic shunt.

[Effect of somatostatin on portal and hepatic hemodynamics and portal venous pressure in cirrhotic patients with portal hypertension].

OBJECTIVE: To investigate the effect of somatostatin on hepatic and portal hemodynamics and portal venous pressure in cirrhotic patients with portal hypertension. METHOD: Colorful doppler ultrasonography was used to measure the diameter and maximum hemokinetic velocity of portal vein and hepatic vein before and after intravenous administration somatostatin, and thus their hemokinetic volume was calculated respectively. Among 15 surgical patients, changes of the portal venous pressure were observed via a catheter before and after the administration of somatostatin. RESULT: One hour and 1.5 hour after the administration, the pressure of portal vein significantly decreased from 2.77 +/- 0.26 kPa to 2.42 +/- 0.27 kPa and 2.40 +/- 0.27 kPa, respectively (P < 0.01). The systolic pressure, diastolic pressure and heart rate showed no significant changes. After the administration, the sectional area of portal vein decreased by 7.28% but without significance. The maximum hemokinetic velocity of portal vein significantly decreased by 18.96% (from 19.72 +/- 7.75 cm/sec to 15.98 +/- 7.26 cm/sec, P < 0.01). The mean total hemokinetic volume increased from 1786.22 +/- 926.37 ml/min to 1836.17 +/- 844.24 ml/min, but without significance (P > 0.05). CONCLUSION: Continuous instillation of somatostatin via peripheral vein can decrease the portal venous pressure in cirrhotic patients with portal hypertension. The mechanism may be the decrease of hemokinetic volume caused by the decrease of hemokinetic velocity of the portal vein.

[Immunohistochemical research on brain stem injury: expression of heat shock protein 70 in rat brain after traumatic brain stem injury].

The aim of this study was to determine the expression of heat shock protein 70 (HSP70) as molecular chaperones, following traumatic brain stem injury(TBSI). The expression of HSP70 was detected in respective brain regions of rats 1 h, 3 h, 6 h, 12 h, 24 h after brain stem stabbing wound. Numerous HSP70 immunoreactivity positive blood vessel endothelioid and glia cells were found in cerebral, cerebellar cortex and hippocampus after 1 h, with H.E stain unchanged. In the area near the focal of brain stem injury, HSP70 immunoreactivity positive neurons increased significantly after 1 h of injury, reaching the highest level after 3 h, much higher than in other areas. HSP70 immunoreactivity positive cells still existed after 24 h of injury. It was suggested that TBSI cause the defectiveness of protein structure in brain, and induce HSP70 proteolysis of denature protein including misfolding or aggregate. TBSI can be diagnosed regionally by the increasing of HSP70 immunoreactivity positive neurons in the focal of brain stems.
