[Antitumor activity of cisplatin against malignant brain tumors].

Antitumor activity of Cisplatin (cis-diamminedichloroplatinum) against malignant brain tumors was investigated from both experimental and clinical points of view. With glioma and neuroblastoma cell lines we studied inhibition of cell growth was studied and DNA histogram analyzed with flow cytometry to determine its antitumor activity in vitro. At the concentration of 1.25 micrograms/ml, DNA accumulation is S or G1-S phase and mild inhibition of cell growth were demonstrated; at the concentration of 12.5 micrograms/ml, cessation of cell cycle was observed. In the clinical study, four glioma patients were treated by intravenous administration of Cisplatin 10 mg/sqm for 5 days q 4 weekly. There were one complete response, one minor and two stable responses. Three patient with intracranial embryonal carcinoma were treated by cisplatin. One patient showed partial response and the others were stable.

[Effect of MCNU on brain tumors. Part II: Clinical experience with MCNU on malignant brain tumors].

Ten patients with malignant brain tumor (8 cases with glioblastoma, 2 cases with medulloblastoma) were treated with a new water-soluble nitrosourea, MCNU. Objective tumor regression of tumor (CR & PR) on computerized tomography was observed in four patients (2 complete and 2 partial) after MCNU, chemotherapy showing a response rate of 40%. The major side effects of MCNU were mild or moderate myelosuppression, and some cases also showed gastrointestinal toxicities and impairment of hepatic function. However, all these side effects were mild and transient and soon recovered to normal levels. One patient with glioblastoma multiforme recurrence was treated with a high-dose chemotherapy of MCNU (400 mg) associated with autologous bone marrow transplantation. Myelosuppression began to appear from 15th day of MCNU administration and normalized within 30 days afterwards. These results suggest that MCNU therapy is effective for patients with malignant brain tumors.

Primary tracheobronchial amyloidosis. A case report and review of reported cases.

A case of diffuse tracheobronchial amyloidosis in a 70-year-old woman thought clinically at first to have lung carcinoma is reported. Bronchoscopic biopsy was necessary to make a diagnosis. Amyloid was deposited markedly in the subepithelial layer. After Congo-red staining amyloid showed the usual apple-green birefringence under polarizing microscope. Electron microscopic study disclosed typical amyloid fibrillar structure. Amyloid protein of this patient was confirmed to be AL protein by immunoperoxidase study which showed specific staining by the anti-A lambda antiserum. When potassium permanganate reaction was used amyloid protein of this patient resisted to the reaction. We reviewed 19 cases from the previously reported cases of primary amyloidosis of the lower respiratory tract, and gave some description of pathologic and clinical aspects of this disease.

[MCNU effectiveness on brain tumor. Part I: Antitumor activity in vitro on human glioma and neuroblastoma cell lines].

A new water-soluble nitrosourea ( MCNU ) was tested for its antitumor activity against fourteen human glioma cell lines and two neuroblastoma cell lines. Four experiments were performed to determine its antitumor activity: inhibition of cell growth, comparison with ACNU, morphological observation, and analysis of DNA histogram with flowcytometry . Seven out of 14 gliomas (50%) and one neuroblastoma cell lines showed more than 50% inhibition of cell growth in vitro, appearance of giant multinucleated cell morphologically, and DNA accumulation in G2+M and/or S phase of cell cycle in the medium of 10 micrograms/ml MCNU . Antitumor activity and spectrum of MCNU against human brain tumors were almost the same as with ACNU.

Management of massive hemobilia with angiographic embolization.

Two cases of angiographic embolization for the management of massive hemobilia due to hepatico-jejunostomy and percutaneous transhepatic biliary drainage are presented. The clinical features are described together with a short review of the literature. In addition to defining the bleeding source, angiography provides an effective and safe alternative to surgery for the control of hemobilia.

Effect of duodenal juice on pathogenesis of gastric ulcer.

The purpose of the present study was to elucidate the effect of duodenal juice on development of gastric ulcer, in relation to changes of lipid composition and energy metabolism of the gastric mucosa in dogs. For regurgitation of duodenal juice and stagnation of gastric contents in the stomach, the duodenum was constricted below the papilla of Vater, accompanying with pyloroplasty and upper gastro-jejunostomy. Furthermore, to induce ischemia in the gastric mucosa, 0.5 ml of 1% formalin solution was injected into a descending branch of the left gastric artery. Three weeks later, U1 II-III gastric ulcer developed at the formalin injected area with severe gastritis but not with hyperacidity, and the histologic findings were similar to the one of a human gastric ulcer with hypoacidity. On assay of lipid composition in the gastric mucosa, lecithin decreased and both lysolecithin and NEFA increased, showing that lecithin of the gastric mucosa was decomposed by phospholipase A2 of the duodenal juice. In the gastric mucosa, ATP and energy charge decreased, and AMP and lactate increased, indicating that the energy metabolism was led to anaerobic glycolysis. These results revealed that the gastric mucosa becomes very fragile when duodenal juice regurgitates into the stomach and that gastric ulcer may develop even without hyperacidity when the microcirculation is disturbed in this condition.

Brush border Mg2+-HCO-3-ATPase, supernatant carbonic anhydrase and other enzyme activities isolated from rat intestinal mucosa: effect of adrenalectomy and aldosterone administration.

The possible role of Mg2+-HCO3-ATPase, carbonic anhydrase and several other enzymes in rat intestinal mucosa as mediators of the action of aldosterone has been examined. The small-intestinal tract was cut into seven segments, 15 cm each in length and the mucosa was scraped off, homogenized in 50 mM D-mannitol-2 mM Tris-HCl buffer (pH 7.1), differentially fractionated and a crude brush border was obtained. The mucosa from the colon and rectum was combined and used as the large-intestinal sample. Five days after the adrenalectomy, activities of brush border Mg2+-HCO3-ATPase and supernatant carbonic anhydrase from the upper small intestine decreased to about 60 and 40% of normal values, respectively. Activities of Na+-K+-ATPase, beta-glycerophosphatase and succinate dehydrogenase were all decreased. Two and 4 h after i.p. injection of aldosterone (40 micrograms/kg) to adrenalectomized rats, all enzyme activities increased except for Na+-K+-ATPase in the upper small intestine. In contrast, Mg2+-HCO-3-ATPase and carbonic anhydrase activities were unchanged 3 h after i.p. injection of dexamethasone (200 micrograms and 1 mg/kg). The activation of both Mg2+-HCO3-ATPase and carbonic anhydrase by a single injection of aldosterone was blocked by pretreatment with cycloheximide (1 mg/kg). These results suggest that aldosterone may induce the synthesis of enzyme proteins in the intestinal mucosa.

Effects of alcohol diet from the fetal period on lipid metabolism in rats.

The effects of maternal alcohol consumption on lipid metabolism in the litter liver were examined in the rat. Rats reared from their fetal period to adulthood with liquid alcohol diet (fetal alcohol group) did not show any lipid accumulation in the livers. Liver perfusion experiments showed an increased oxygen consumption in the liver to a load of physiological concentration of palmitate. On the contrary, the response of oxygen consumption in fetal control rat livers (reared with non-alcohol liquid diet) to palmitate was found to be depressed. 14CO2 production rate from 14C-palmitate in perfused livers showed a significant increase in fetal alcohol group as compared with controls. No difference in 14C-palmitate incorporation into tissue lipid was found between fetal alcohol and fetal control groups. These data indicate that when rats are treated with alcohol from fetal period they acquire an ability to elevate FFA oxidation in the liver through metabolic adaptation.

Studies on a new nucleoside antibiotic, dapiramicin. I. Producing organism, assay method and fermentation.

A new antibiotic, dapiramicin, has been isolated from the fermentation broth of Micromonospora sp. SF-1917. Since the dapiramicin exhibited essentially no in vitro activity by usual bioassays, two assay methods for dapiramicin were developed, HPLC and a new bioassay by growth inhibition of fungal mycelia on a slide glass. Fermentation of dapiramicin is also described.

[Immunochemotherapy with tegafur and schizophyllan for stomach cancer--report of 2 cases].

Two cases of gastric cancer were treated with immunochemotherapy using Tegafur and Schizophyllan. In one case, a marked reduction in the size of metastatic liver tumor and disappearance of most subjective symptoms were observed. The response continued even 8 months after the initial treatment. In the other case, no marked symptoms except occasional dysphagis have been noted for 2 years and 2 months since diagnosis, although the primary tumor size has remained unchanged. Thus, it is presumed that the combined immunochemotherapy regimen may be useful in the treatment of advanced gastric cancer.

[Anti-tumor activity of ACNU against malignant glioma--a clinical application of in vitro sensitivity test of chemotherapeutic agent].

In vitro sensitivity test of cultured human malignant glioma cells to ACNU[1-(4-amino-2-methyl-5-pyrimidinyl) methyl-3-(2-chloroethyl)-3-nitrosourea hydrochloride] was performed on Falcon 3064 microtestplate. Early culture cells established from 10 clinical cases were examined by means of this method and the correlation between in vitro response and clinical response of tumor to ACNU was studied. In microtestplate assay, it was defined as ACNU sensitive when the tumor growth showed more than 60% suppression when tumor was cultured in the medium which contained 40 meg/ml of ACNU. Clinical responses were evaluated by means of CT scan, being divided into 4 classes, PR (partial response), CR (complete response), NC (no change) and PD (progressive disease). The former two groups were regarded as responder and the latter two as nonresponder. The correlationship between in vitro response and clinical response was higher in resistant group (100%) than in the sensitive group (60%), so the resistant tumor can be checked with this method. For this reason, the use of this assay may prove helpful in the selection of the chemotherapeutic agents.

[Monoclonal antibodies against human brain tumors].

Tumor specific and various tumor associated antigens expressed on human brain tumor cells were reported from many laboratories, which have been identified by xeno, allo or autoantisera, and recently utilizing monoclonal antibodies produced by hybridoma technique. In this article, the results of reports are summarized comparing the studies with conventional antisera and monoclonal antibodies and clinical application of these monoclonal antibodies at present and in near future are described.

[Malignant intracranial lymphoma--clinical and patho-immunological study].

Clinical and patho-immunological study was made on 15 cases of primary intracranial malignant lymphoma. The average age of the patients was 45.2, however older man suffered more than younger one. Male to female ratio was 3:2. The duration between onset to diagnosis was less than 6 months in 13 cases. The common signs and symptoms were hemiparesis (60%) and signs of intracranial hypertension (45%), followed by disturbance of consciousness, visual disturbances, convulsion and signs of inflammation. The location of the lesions was in parietal lobe (44%), temporal lobe (27.8%), cerebellum (22.2%), brain stem and basal ganglia (18%). Multiple lesions were found in 4 cases. CT scan was the most diagnostic method not only to delineate the location of lesions but also to recognize the biological natures of this condition. The radiosensitivity of this tumor was also confirmed by follow-up CT scans during radiotherapy. Most of the lesions were avascular by carotid angiography, but capillary blush at the tumor site was shown in some cases. Laboratory findings revealed that derangement of gamma globulin in the serum and decline of cellular immunity were demonstrated in many cases. Surgical removal followed by whole skull irradiation was made in all cases except one. Remission of neurological signs and symptoms with disappearance of lesions on CT scan was obtained in 7 cases for one to 9 months. All cases except two survival, died during 3 to 28 months (average: 10.1). Survival rate at one year and 2 years was 35.7% and 14.3% respectively.(ABSTRACT TRUNCATED AT 250 WORDS)
