Choanal bleeding due to nasopharyngitis.

Twenty-two patients with bleeding from the nasopharynx which due to secondary changes in acute nasopharyngitis were treated at the Fujisaki Hospital. The titers of IgG, IgM and IgA of the viral capsid antigen (VCA), IgG and IgA of the early antigen (EA), and EB nuclear antigen (EBNA) were measured. There was no correlation between season of coryza or changes in season and the onset of nasopharyngitis, except for a slightly greater occurrence during mid summer and mid winter. One of three patients with severe bleeding, underwent posterior nasal packing using a rolled 10 X 10 cm gauze sponge. The majority of cases of bleeding from the nasopharynx were most satisfactorily treated by administration of the ranitidine hydrochloride (Zantac) of a selective antagonist for the H2-receptor of histamine.

Primary ciliary dyskinesia with transverse colon carcinoma.

Primary ciliary dyskinesia, characterized by chronic airway infection, impaired fertility, and situs inversus in half the number of cases, was detected in a 58-year-old Japanese woman, along with carcinoma of the transverse colon. She was of complete situs inversus and subfertile and had chronic bronchitis and nasal polyposis. Ultrastructurally, the cilia of the nasal mucosa lacked dynein arms. This is the first documentation of this disease coupled with cancer.

A platelet-aggregating and hypotensive phospholipid isolated from bovine brain.

A phospholipid that differs from known active lipids and causes potent platelet aggregation and weak hypotension has been isolated from bovine brain. Its platelet aggregating effect on heparinized platelet-rich plasma from rabbits, was at a threshold concentration of about 0.2 nmol ml-1 as phosphorus. The effect was inhibited by CV-3988. The phospholipid was converted by diazomethane treatment to another active lipid that caused short-term hypotension, but not platelet aggregation, rather it inhibited the aggregation of rabbit heparinized platelets induced by platelet-activating factor.

Chemotherapy with cisplatin for AFP-secreting germ-cell tumors of the central nervous system.

Therapeutic results in 17 cases of alpha-fetoprotein (AFP)-secreting tumors of the central nervous system are reported. Twelve of the patients were male and five female. The patients' ages ranged from 5 years to 25 years (mean 13.6 years). The tumors originated in the pineal region in 10 cases, in the suprasellar region in four cases, and in both regions in one case; of the other two tumors, one originated in the basal ganglia and one in the sacrococcygeal region. Extraneural metastases or subarachnoid dissemination occurred in seven cases. Increased AFP titer in the serum or cerebrospinal fluid was verified in 13 cases. The patients were divided into two groups: those treated before (Group A) and those treated after (Group B) the introduction in 1980 of cisplatin as a chemotherapeutic agent. All seven Group A patients died, even after extensive therapy consisting of surgery, radiotherapy, and chemotherapy (ACNU or adriamycin) in some of the cases. The mean survival time following diagnosis for Group A was 23.7 months. Six of the 10 Group B patients died an average of 25.8 months after diagnosis; however, the other four were still alive and well an average of 25.3 months after diagnosis. The tumors responded well to chemotherapy consisting of cisplatin either alone or combined with bleomycin and vinblastine. The initial response rate to treatment in Group B was 100% and five cases had a complete remission, verified by computerized tomography or the serum AFP level. Once the tumor recurred, the response to cisplatin was markedly decreased. It is suggested that an effective therapy must still be sought for recurrent tumors.

[A mild form of brain stem encephalitis due to herpes simplex virus].

A 10-year-old boy had gait and speech disturbances 17 days after the initial symptoms of a fever, headache and cough. Four days later he was admitted to a hospital with mild disturbances of gait, speech, writing, visual acuity, left facial nerve, nystagmus and consciousness. Impairments of cranial nerves (II, III and VII), pyramidal sign and cerebellar sign were noticed. EEG showed generalized slow waves. Auditory brain stem response showed prolongation of the interval between I and V waves and poor differentiation between them. Brain CT could not find any abnormalities. Brain stem encephalitis was diagnosed. Clinical signs and symptoms continued for two weeks when steroid therapy was started and it was effective to improve the disease. He was discharged from the hospital without sequelae. Herpes simplex virus (HSV) type 1 was detected from cells in CSF on admission by fluorescence antibody method. HSV antibody titers in sera changed from 1/8 to 1/64 during three months by complement fixation test. Specific IgG and IgA by enzyme linked immunosorbent assay (ELISA) was high in CSF. Specific antibody in CSF/total antibody in CSF: specific antibody in serum/total antibody in serum for IgG and IgA classes were more than 1. Reports of mild type of HSV brain stem encephalitis seemed to be rare. Our case which was followed for several months carefully would be important to discuss.

[Clinico-pathological studies of CSF dissemination of glioblastoma and medulloblastoma].

Clinico-pathological findings of CSF dissemination which was diagnosed on CT scan, were studied on 13 cases of glioblastoma and 9 cases of medulloblastoma. The type of CSF dissemination and the prognosis of patients were both different between glioblastoma and medulloblastoma. In the former, the dissemination was predominantly in ventricular walls and in the latter, in basal cisterns. The mean survival time after the diagnosis of dissemination is 6 months of glioblastoma as compared with 13 months of medulloblastoma. The Pathological studies show that subependymal and/or subpial infiltration of tumor cells, and thickness of arachnoid membrane by marked mesodermal reaction were demonstrated in cases of glioblastoma. On the contrary, tumor cells of medulloblastoma grow markedly in the subarachnoid space and/or on the ependymal layers. From these pathological findings of CSF dissemination, it will be resulted that the prognosis of glioblastoma is much more poor that of medulloblastoma.

[Anti-tumor activity of human recombinant TNF against human malignant glioma cell lines and combined effect with Hu INF-beta].

Human TNF was detected fairly recently and at present the anti-tumor activity of human recombinant TNF is being examined against various malignant tumors of human origin. In the present study, we report the anti-tumor activity of recombinant human TNF against human malignant glioma cell lines in vitro and in vivo, in addition to its combined effects with HuIFN-beta. The in vitro study was conducted as follows. Thirteen human glioma cell lines were exposed to 100 U/ml TNF, 1,000 IU/ml HuIFN-beta, or both, and the suppression rate was calculated on days 3, 5 and 7. In the in vivo study, nude mice carrying a human glioma cell line, KMS II, in the subcutaneous tissues were divided into groups and drugs were administered intratumorally as described below. 1) control, 2) TNF 5,000 U single administration, 3) TNF 5,000 U, intermittently administered (once/week for two weeks), 4) TNF 5,000 U, continuously administered (3/week for two weeks), 5) HuIFN-beta 50 X 10(4) IU (3/week for two weeks), and 6) combination of 4) with 5). Results of the in vitro study revealed some suppressive effects on proliferation of tumor cells on day 7 in all 13 glioma cell lines examined with 100 U/ml TNF. And also, especially in 8 of 13 cell lines, the suppression rate was more than 30%. The suppressive effects of TNF were augmented by combined use of HuIFN-beta in all cell lines, giving a range of suppression of 67.8 to 99.3%. The in vivo study revealed that the mean tumor weight ratios (control = 100%) on day 19 (the end of the experiment) were as follows; single administration of TNF: 41.3%, intermittent: 46.7%, continuous: 26.7%, HuIFN-beta: 65.9%, combination: 18.5%. Statistical analysis disclosed significant suppressive effects on tumor proliferation between the control group and 3 TNF-administered groups (single, intermittent, and continuous) and that suppression in the continuously administered group was more severe in comparison with the group given single administration. Moreover, it was suggested that combination therapy with TNF and Hu IFN-beta was more effective than a single therapy with TNF only or HuIFN-beta only. From the results described above, it was found that human recombinant TNF had some cytotoxic effects against human malignant gliomas in vitro and in vivo, although the degree of cytotoxicity was not always higher in comparison with the effects of TNF.

[Combination therapy with IFN-beta, ACNU and radiation (IAR) in malignant brain tumors].

In order to analyze the efficacy of combination therapy with Hu-IFN-beta, ACNU and radiation (IAR), nine patients with malignant glioma were treated as a control study. They received 100 X 10(4) IU Hu-IFN-beta daily for seven days intravenously or intratumorally, 3 mg/kg ACNU on day 2 and 5,000-6,000 rads of radiation from day 3. Four out of nine patients showed complete response and one partial response with this IAR therapy. Case 1 was a 64-year-old man who had glioblastoma in the left frontal lobe. Postoperative residual tumors disappeared completely with this therapy. Case 3 was a 8-year-old girl who had an enhanced high-density lesion in the medulla oblongata and pons. After IAR therapy, the high-density lesion was completely vanished and her clinical manifestations of multiple cranial nerve palsy and pyramidal sign were improved remarkably. The major side effects of IAR therapy were mild or moderate myelosuppression, and some patients also showed hepatic dysfunction, mild fever and gastrointestinal toxicities. However, all these side effects were mild and transient and soon recovered to normal levels. These results suggest that IAR therapy is effective and will prolong the survival time of patients with malignant glioma.

[Immunohistochemical study of subependymal giant cell tumor associated with tuberous sclerosis].

Subependymal giant cell tumors from four patients associated with tuberous sclerosis were examined by peroxidase antiperoxidase (PAP) immunohistochemistry for neuron specific enolase (NSE), glial fibrillary acidic protein (GFAP) and S-100 protein. The tumor cells were grouped into three morphological types: ganglion-like pyramidal cells, swollen gemistocytic cells and fibrillated spindle cells. There was little difference in immunohistochemical property between these cell types in each case, suggesting that the tumor cells of different cell types have a same origin. The majority of the tumor cells was intensely stained for NSE in every case. On the other hand, for GFAP none of the tumor cells was stained in two cases, and the minority was weakly stained in the other two cases. The intensity of staining of the tumor cells for S-100 protein varied from case to case. Our findings together with those of others show the definite immunohistochemical characteristics of subependymal giant cell tumors with tuberous sclerosis, i.e., strong positivity for NSE in contrast to their negativity or weak positivity for GFAP. However, these findings do not necessarily suggest that the tumor is of neuronal origin, because in the case of neoplasms the significance of the presence of NSE as indicative of neuronal origin is open to question.

[Clinical evaluation of cefixime in children].

A new beta-lactamase-stable oral cephem antibiotic, cefixime (CFIX), was evaluated for safety, efficacy and pharmacokinetics in children. CFIX was effective in 19 of 20 cases (95%) with bacterial infections. The drug was especially effective against the cases of pneumonia due to beta-lactamase-producing H. influenzae or B. catarrhalis. Pharmacokinetic parameters of CFIX (3 mg/kg) with premeal administration were as follows: Kel 0.328 +/- 0.066 hr-1, T 1/2 2.14 +/- 0.36 hrs, AUC 10.9 +/- 8.7 micrograms X hr/ml, and Vd/F 1.64 +/- 1.42 L/kg. In most of the cases tested, the urinary excretion rate in 12 hours was 5 to 17%. A dose of 3 mg/kg twice daily seems to be adequate for a regular treatment.

[Extraneural metastases of malignant ependymoma inducing atelectasis and superior vena cava syndrome--a case report and review of the literature].

Extraneural metastasis of intracranial ependymoma is a rare pathological entity. Thirty one case reports were traced in the review of the literature and we record one of them. The patient was a 19-year-old male in good health until January 1981 when he was admitted to our hospital with deteriorating mental status. Admission work-up revealed bilateral papilledema, 1-hemiparesis and increased intracranial pressure signs including vomiting. CT scan demonstrated significant abnormality of enhanced mass lesion in the r-temporo-parietal area in which a displacement of the midline structure to the left occurred. R-temporo-parietal craniotomy was performed on the admission day. The globular tumor mass occupied the temporo-parietal area and invaded the cortex. Subtotal resection of the tumor and temporal lobectomy was performed. Microscopic examination of the operative specimen revealed a typical ependymoma pattern. For the next two years, he received operations twice, irradiation (total 14, 170 rads) and various chemotherapy. Two months after the fourth craniectomy, examination revealed scalp overlying the burr opening to be very tense and enlarging as if invaded by the tumor. A large mass occupied the right lateral cervical area and chest X-ray disclosed complete opacity on the right. He gradually developed severe cough and sputum and died two months later on January 1, 1984. At autopsy, the result was that tumor had invaded the subarachnoidal space and subcutaneous area. Extraneural metastases were found to be bronchial lymph nodes, C-4 vertebra, r-cervical lymph node. The histological appearance of these tumors obtained at autopsy was identical to the cerebral tumor.(ABSTRACT TRUNCATED AT 250 WORDS)

Long-term follow-up of patients with optic glioma.

Patients with optic glioma were classified into two types: infant type and childhood. The infant type was usually immature astrocytomas that had an aggressive course and had a poor prognosis. On the other hand, the childhood type was usually mature astrocytomas with a rather benign, self-limiting course, even when the tumor had been left untreated. To make a long-term assessment, 11 patients with optic glioma were studied and 6 long-term survivors with chiasmal tumor invasion, in whom total removal of the tumor had not been feasible, were followed up for 8-12 years. Most of them had a stable course both clinically and radiologically.

Effect of aldosterone antagonists on aldosterone-induced activation of Mg2+ -HCO3- -ATPase and carbonic anhydrase in rat intestinal mucosa.

In previous studies, Mg2+ -dependent, HCO3- -activated ATPase in the brush border and carbonic anhydrase in the cytoplasm of rat duodenal and jejunal mucosa decreased after adrenalectomy. Both enzyme activities increased to near normal levels 4 h after i.p. injection of aldosterone (40 micrograms/kg). These results suggest the possibility that both enzymes in the small intestinal mucosa may be mediators of the action of aldosterone. In the present studies, therefore, the effects of actinomycin D (500 micrograms/kg, i.p.), spironolactone (50 mg/kg, s.c.) and potassium canrenoate (50 mg/kg, s.c.) on aldosterone-induced activation of both enzymes in the upper small intestinal mucosa from adrenalectomized rats were examined to clarify the mechanism of action of aldosterone in enzyme levels. Actinomycin D inhibited carbonic anhydrase activity in small intestinal mucosa from normal rats 4 h after i.p. injection but had no effect on ATPase activity, while two other drugs had no effect on either enzyme activity in normal rats up to 4 h later. Pretreatment with these 3 drugs 1 h before aldosterone administration (40 micrograms/kg, i.p.) to adrenalectomized rats blocked the aldosterone-induced activation of ATPase and carbonic anhydrase in the upper small intestine. On the other hand, adrenalectomy and administration of aldosterone and its antagonists, alone or in combination, had no effect on kidney enzyme activities. These results confirm that Mg2+ -HCO3- -ATPase and carbonic anhydrase are mediators of the action of aldosterone in the upper small intestinal mucosa.
