[Geometric relationship of axial crown contour and gingiva contour of normal second mandibular premolars].

OBJECTIVE: To investigate the relationship of axial crown contour and surrounding healthy gingiva contour. METHODS: Mandibular second premolars were selected as the object of study. Three cross-sections were made through crown and gingiva of the model tooth. Curve functions of axial buccal crown and gingiva contour of each section were acquired by means of curve fitting, and the angle of emergence profile was calculated, which represents the degree of undercut. RESULTS: There is unique relationship between axial crown and gingiva contour individually. This character of tooth is similar to its contralateral tooth. The emergence profile angles are in the range of 40 degrees-110 degrees, 84.1% of the angles are in range of 70 degrees-110 degrees, 15.9% in 40 degrees-70 degrees, that is to say natural teeth are inclined to be of less undercut. CONCLUSION: The geometric relationship between crown contour and gingiva contour is so characteristic that contralateral tooth should be the only guidance to replace lost tooth.

[Effect of Saikosaponin-d on up-regulating GR mRNA expression and inhibiting cell growth in human leukemia cells].

OBJECTIVE: To study the effect of Saikosaponin-d (SSd) on the expression of glucocorticoid receptor (GR) mRNA and cell growth in HL60 cells. METHODS: Antiproliferation effects of SSd on HL60 cells were determined by 3H-thymidine incorporation. Cell cycle analysis was also performed by flow cytometry. GRmRNA was analyzed by northern blot analysis. RESULTS: After 48 hours treatment with 10 micrograms/ml SSd, 3H-thymidine incorporation decreased in HL60 cells and the effect was time and dose dependent. The flow cytometry analysis showed HL60 cells were arrested at G0/G1 phase. The expression of GR mRNA increased. CONCLUSION: SSd exhibits the inhibition effects on cell growth in HL60 cells and could up-regulate the expression of GR mRNA in HL60 cells.

[Observation on short-term effects of Angelica injection on chronic obstructive pulmonary disease patients with pulmonary hypertension].

OBJECTIVE: To study the effects of 25% Angelica sinensis injection on hemodynamics, endothelin-1 (ET-1), angiotensin-II (AT-II), endogenous digitalis-like factor (EDF), pulmonary function and arterial blood gas in the patients with chronic obstructive pulmonary disease (COPD) complicated pulmonary hypertension. METHODS: Sixty COPD patients complicated with pulmonary hypertension in remission stage were randomly divided into two groups, 30 cases in each. The Angelica group and the control group were treated with Angelica sinensis injection and 5% glucose injection (250 ml, intravenous dripping per day for 10 days) respectively. It was designed to investigate the changes of hemodynamics, ET-1, AT-II, EDF, pulmonary function and arterial blood gas. RESULTS: The levels of mean pulmonary arterial pressure (mPAP), pulmonary vascular resistance (PVR), blood ET-1, AT-II and EDF were reduced by (18 +/- 5)%, (27 +/- 8)%, (20 +/- 6)%, (36 +/- 9)%, (38 +/- 11)% respectively, and PaO2 was increased in Angelica group (P < 0.05 or P < 0.01). There were insignificant differences of the above parameters in the control group, and no changes of pulmonary function in both groups. CONCLUSION: Twenty-five Percent of Angelica injection can improve pulmonary hemodynamics through influencing the metabolism of ET-1, AT-II and EDF as well as increase PaO2 of the body.

[Simultaneous pancreas-kidney transplantation for the treatment of type I diabetes with end-stage renal disease].

OBJECTIVE: To demonstrate whether simultaneous pancreas-kidney transplantation is practical for the treatment of Type I diabetes with end-stage renal disease. METHODS: Eight cases of combined pancreas-kidney transplantation were performed in our institute. The age ranged from 35 years to 48 years (average 43.46 years). All cases were diagnosed as type I diabetes with end-stage nephropathy, two cases with blindness due to retinopathy. The case history ranged from 2 years to 22 years. Pancreas allograft was placed in the right iliac fossa with pancreas exocrine drainage to bladder, whereas renal allograft the in left iliac fossa. Initial immunosuppression regimen is quadruple. RESULTS: Seven patient could be insulin free after transplantation, with normal fasting blood glucose. One patient received insulin treatment for 40 days after operation. One patient survived 1 year and 9 months after transplantation with normal functioning pancreas allograft and kidney allograft. Four patient survived 2 months with normal allograft function. Fasting blood glucose was between 4.5 and 6.2 mmol/L; Cr was between 53 and 106 micromol/L. Diet control was not necessary. Two patient died of encephalorrhagia, and 1 pancreatic vascular thrombosis and necrotic pancreatitis. CONCLUSIONS: Combined pancreas-kidney transplantation is feasible to treat type I diabetes with end-stage nephropathy. Functional pancreas allograft could be procured in the present condition. Pancreatic exocrine is drained to bladder. The diagnosis of acute rejection could be made earlier by detecting urine amylase. Heparin should be added in order to prevent pancreatic thrombosis.

[Effect on keratocyte-mediated collagen degradation by Pseudomonas aeruginosa].

OBJECTIVE: To study the pathogenesis of cornea melting (ulceration) by pseudomona (P) aeruginosa for instruction of clinical treatment. METHODS: Type I collagen gels with or without suspended keratocytes were incubated for 24 hours under medium containing sterile P. aeruginosa culture broth. Native collagen fibrils were removed from the media by ultrafiltration. The ultrafiltrates were then hydrolyzed, and the amount of hydroxyproline was measured spectrophotometrically. The effect of a synthetic matrix metalloproteinase (MMP) inhibitor, Galardin, on collagen degradation was also examined. RESULTS: P. aeruginosa broth induced type I collagen gel degradation directly. In the presence of keratocytes, degradation by P. aeruginosa broth was enhanced. Galardin significantly reduced the amount of collagen degraded by P. aeruginosa culture broth, no matter keratocytes were present or not. CONCLUSION: P. aeruginosa culture broth directly degrades type I collagen and also increases keratocyte-mediated collagen degradation. The result is helpful to the clinical treatment of cornea melting caused by P. aeruginosa, and the mechanism should be further studied.

[A clinical investigation on cataract surgery with 2.8 mm incision].

OBJECTIVE: The results of phacoemulsification which was performed through a 2.8 mm clear cornea tunnel incision and foldable intraocular lens (IOL) implantation were analyzed. METHODS: On 126 eyes with cataract of 105 cases, phacoemulsification with foldable IOL implantation was randomly performed. The results of postoperative vision, refraction, corneal edema, anterior chamber flare and the rate of corneal endothelial cell loss were retrospectively summarized. RESULTS: The uncorrected visual acuities of postoperative 1 day, 3 days, 1 week, 2 - 3 months, >or= 0.5 were in 102 eyes (81.0%), 108 eyes (85.7%), 112 eyes (88.9%) and 112 eyes (88.9%), respectively. The mean diopters of astigmatism at postoperative 3 days, 1 week, 1 month and 3 months were (1.09 +/- 0.51) D, (1.02 +/- 0.47) D, (0.88 +/- 0.52) D and (0.81 +/- 0.62) D, respectively. The mean diopters of astigmatism at postoperative 1 week, 1 month and 3 months compared with that before the surgery had no significant difference (P > 0.05). The comparison between the preoperative astigmatism and that at postoperative 3 months showed that only about 0.08D of change existed. The postoperative corneal edema and anterior chamber flare were mild, the anterior chamber configuration remained well, and the rate of endothelial cell loss was 11.9%. CONCLUSIONS: The surgical incision is small and easy to perform. Postoperatively, the reaction is mild, the change of astigmatism is small in degree, the best visual acuity can be obtained at the early stage and the refractive status is stable, thus the duration for restoration is greatly shortened.

[The epidemiological study of HIV infection among paid blood donors in one county of China].

OBJECTIVE: To observe the prevalence of HIV infection among paid blood donors and risk factors. METHODS: Selected the residents between 18 to 50 years old from two villages in one county of China in 1997. We observed the prevalence of HIV infection and keep following them to 2000. RESULTS: Nine hundred sixty-three residents were investigated and the prevalence of HIV was 1.8% (17/963). There were 88 paid blood donors among 963 interviewed subjects, the HIV prevalence was significantly higher (17.0% or 15/88) than those who didn't have the experiences of paid blood donations (0.2% or 2/875). Only one person out of 668 who were followed from 1997 to 2000 has converted to sero-positive, the incidence of HIV infection is 0.06 per 100 person-years. Nearly half of sero-negative and sero-positive people have the experiences of muscle injection. Among those with HIV-positive spouse, we found no one converted to sero-positive. The CD(4) count of those sero-positive people declined every year. HIV subtype and C2-V3 region sequence analysis were made for five HIV-positive people in 1998, the result indicated that all of them were infected with HIV-1 subtype B' and the genetic divergence rate was 2.4%. CONCLUSION: There was a high prevalence of HIV among paid blood donors in some regions, and paid blood (plasma) donations were the main risk factor for HIV infection. Although no evidence suggests that during the follow-up period the epidemic of HIV infection has spread locally, it still shall take measurements to prevent further transmission of HIV through paid blood donors.

[The changes in serum antibody level after immunization with HFRS vaccine].

OBJECTIVES: To observe the changes in serum antibody level after mass immunization with vaccine against hemorrhagic fever with renal syndrome (HFRS) and to evaluate its efficacy and effectiveness in the prevalent areas. METHODS: Healthy people aged 16 to 60 years in the villages were recruited as study subjects, excluding those suffered from HFRS previously, going out for more than nine months and those with contraindications, and were randomly allocated into immunization and control groups with 10,460 and 16,159 persons, respectively. Specific IgG antibody was determined with indirect immunofluorescent assay (IFA) and neutralizing antibody (NA) was determined with micro CPE method. RESULTS: Two weeks after the full-course immunization, sero-conversion rate for IFA reached 100% in those sero-negative before immunization, with a 95% confidence interval of 96.3 - 100.0%, and that for NA 44.4%, with a 95% CI of 22.0% - 69.0%. Geometric mean titer (GMT) were 72.1 and 4.6 for IFA and NA, respectively. Booster immunization was provided for them one year later. Positivity of IFA and NA was 28.6%, 83.3%, 75.0%, 53.1%, 22.6% and 14.8%, 55.6%, 35.0%, 31.3%, 26.0%, before booster immunization, two weeks, one year, one and a half years, two years after booster immunization, respectively. CONCLUSION: HFRS vaccine had good immunogenicity, but its duration of serum antibody sustenance was relatively short.

[Telomerase activity in human non-small cell lung cancer (NSCLC)].

OBJECTIVE: To investigate the activity of telomerase in human non-small cell lung cancer (NSCLC) and the correlation of telomerase activity with the cancer development. METHODS: Totally 48 cases of NSCLC tissues were studied accompanied with 7 cases of normal lung tissues and 12 samples of lung tissues adjacent to the tumor. The telomerase activity was detected using Telomerase PCR ELISA kit. Statistics study included chi(2)-test and the exact probabilities. The significance level chosen was P value less than 0.05 (P < 0.05), and all tests were two-sided. RESULTS: 75% (36/48) of NSCLC tissues possessed the expression of telomerase activity and only 8.33% (1/12) of tissues adjacent to tumor and none of the normal lung tissue enabled to express telomerase activity. The statistical difference was significant (P < 0.05). No significant difference was found between different histological types of NSCLC, however, the frequency of telomerase expression differed significantly between tumors of well/mordeately differentiated and poorly differentiated; stage I and stage II/III. CONCLUSION: Telomerase activity might be a good marker in estimating the occurrence, development and diagnosis of NSCLC.

[Analysis of the long-term results of late hand function reconstruction in patients with severe electrical injury of the wrists].

OBJECTIVE: To evaluate and analyze the long-term results of all the operation mordalities practised at present of late hand function reconstruction in patients with severe electrical injury of the wrists. METHODS: Forty-seven cases with late electrical injury of the wrists were followed up and comprehensively analyzed in terms of the long-term results of morphological reconstruction and functional restoration. RESULTS: In addition to cutaneous tissue repair and functional training of the joints of the wrists, the flexor digitorum reconstruction with auto free tendon grafting gave a poor result (only 33.4% of the cases with function above medium level). In one case, the wrist function was rebuilt by compound plantaris tendon free flap with leg deep facia and posterior tibial artery, resulting in the restoration of thumb flexor function, with shorter treating time and less postoperative adhesion. Median and ulnar nerves were repaired with free auto nerve grafting with no obvious effects. Two cases were repaired by compound tissue grafting of sural nerve and deep facia carried by small saphenous veins with uncertain result. In another 3 cases, with the aid of retrograde evoked potential (REP), a proximal anastomosis of nerves was done with fairly good result. CONCLUSION: Insufficient blood supply and scar formation were the major causes of poor results of the tendon and nerve grafting. It was recommended that the graft should carry sufficient blood supply to improve the blood supply of the recipient bed in order to improve the therapeutic effects. The selection of proximal nerve segment should include the functional examination.

[Caspase 3 in the cytokines induced acute leukemia cell apoptosis].

OBJECTIVE: To explore the variation and significance of caspase 3 activity in the process of G-CSF and GM-CSF regulated NB4 cell apoptosis. METHODS: Cell morphology, flow cytometry and DNA fragmentation assay were used to demonstrate the apoptosis induced by G-CSF and GM-CSF. Caspase 3 activity was detected by spectrofluorometry, and its inhibition test by AC-DEVD-CHO. RESULTS: G-CSF could induce NB4 cell apoptosis, while GM-CSF could not. Caspase 3 activity was significantly higher in the apoptotic cells than in control cells. The G-CSF induced NB4 cell apoptosis was efficiently inhibited by AC-DEVD-CHO. CONCLUSIONS: G-CSF and GM-CSF exert different effects on the apoptosis of leukemic cells. G-CSF induced apoptosis of leukemic cells through activation of caspase 3.

[Transplantation of free scapular flap to repairing injury of faciocervical region].

OBJECTIVE: To sum up transplantation of free scapular flap to repairing injury of faciocervical region, 103 cases in past 16 years were reviewed. METHODS: The scapular flap contained the ascending, descending, and transverse branches of the circumflex scapular artery. The range of the flap was as following: across the scapular spine to the acromion in upper edge, to the level of anterior iliac spine in lower edge, to the bilateral middle axillary line in both sides. RESULTS: The clinical result was good. Fifty eight cases were followed up for 3 months to 10 years, the function of the faciocervical region was recovered with satisfied contour. In part of the patients, a second-stage operation was performed to make the flap thinner. CONCLUSION: The maximal range of the flap can be 45 cm x 16 cm. For its sufficient blood supply, it should be of no influence on survival of the flap if the window is open in suitable site. Because of its large range of donating region and sufficient blood supply, it is fit for children.

[Effects of pinyangmycin, dexamethasone and sodium morrhuate injection on treatment of cavernous hemangioma in maxillofacial regions].

OBJECTIVE: To evaluate the effects of Pinyangmycin, Dexamethasonum and Sodium morrhuate injected homochronouly on treatment of cavernous hemangioma in maxillofacial regions. METHODS: The medical records of 115 patients with cavernous hemangioma in maxillofacial regions between September, 1996 and September, 1998 were reviewed. Fifty of the patients were men, and 65 were women, with a ratio of 1:1.3. The age of the patients was from 4 months old to 45 years old. 73 of the hemangioma bodies were sited in maxillofacial regions, 42 were sited in oral cavity. The sizes of the lesions varied from 1 cm x 1 cm to 5 cm x 8 cm. Treatment procedure: 1. Preparation: 8 mg Pinyangmycin was dissolved in 5 mg/ml Dexamethasonum and in 4 ml of 1% procain which were ready for use; 2. Injection: 1-5 ml (Pinyangmycin 1.6-8.0 mg) of mixed solution was first injected to the hemangioma, then followed by an injection of 0.5-2.0 ml Sodium morrhuate according to local appearance, size of the tumor and patient's age. Generally, the amount of each injection should not be over 8 mg Pinyangmycin. If the tumor did not subside after an injection, another injection might be repeated every 5-7 days, and this process might be repeated 3-5 times. The total dose of Pinyangmycin should not be over 40 mg. RESULTS: 115 patients were followed up for 6-24 months. 81 of 115 (70.43%) cases were recovered, and 30 (26.09%) cases were basically recovered. 4 (3.48%) cases got improvement. The recovery rate were 96.52%, and the total efficiency was 100%. CONCLUSION: This method may be a safe, simple and effective therapy to cavernous hemangioma in maxillofacial regions.
