[Microsurgical treatment of foramen magnum tumors].

OBJECTIVES: To review the radiological features, and results of microsurgical treatment of foramen magnum tumors. METHODS: We retrospectively analyzed the results of microsurgical treatment of 31 patients with foramen magnum tumor. Fifteen patients had pathological schwannomas (neurinomas), 13 patients meningiomas, and 3 patients chordomas. According to tumor extension, MRI findings and microsurgical approaches, the tumors were classified in two types. Type I tumors located at the ventral foramen magnum, including those originated from the posterior fossa with minimal enlargement of the ventral or anterior-lateral (15 patients). Type II tumors located at the posterior or lateral foramen magnum, including those arising from the spinal canal with minimal enlargement of the foramen magnum (16 patients). Surgical approaches to tumors in this series included suboccipital posterior approach (18 patients), extreme lateral or transcondylar approach (10), and suboccipital retrosigmoid approach (3). RESULTS: Total removal was achieved in 25 patients, subtotal removal in 5, and partial removal in 1. No operative death occurred. Type I foramen magnum tumors were totally removed in 9 patients, subtotally removed in 5 and partially removed in 1. All of type II foramen magnum tumors were totally removed. CONCLUSIONS: To totally resect type I foramen magnum tumors, extreme lateral transcondylar approach is an optimal choice. Type II foramen magnum tumors can be totally removed with good prognosis.

[Study on the mechanism of thrombosis by lupus anticoagulant inhibited protein C pathway].

OBJECTIVE: To study the mechanism of thrombosis in systemic lupus erythematosus (SLE) patients with protein C (PC) pathway inhibition by lupus anticoagulant(LA). METHODS: Phosphoethylamine(PE) dependent activated protein C (APC) inhibiting assay (modifed DRVVT) was used to observe the inhibition of APC activity after incubation of IgG from both normal and SLE patients with PE. RESULTS: The activity of anticoagulation of APC was augmented by PE. Proportionally to its concentration, LA could inhibit the PE dependent anticoagulation of APC. The inhibition of LA-IgG on APC in the LA positive SLE patients with thrombosis was stronger than that in LA negative non-thrombosis patients; but IgG has no effect on the APC activity in normal person. CONCLUSION: LA inhibiting the PC pathway by interfering with PE may be the important reason of thrombosis in SLE patients.

[Gene dispersal risk of transgenic plants].

Gene flow is a key problem and top topic in risk assessment and management of transgenic plants. Five major study areas of gene dispersal in transgenic plants were reviewed in this paper, which include gene flow from transgenic plant to its wild species, characteristics of gene flow based on pollen, methods of doing experiment and risk assessment for gene flow, safety standards of risk assessment of transgenic plants, and long-term ecological impact of gene flow in the future. Some areas needed to be studied further were also proposed.

[Total auricle reconstruction with postauricular flap and a Medpor framework].

OBJECTIVE: To introduce a new method and technique of auricle reconstruction. METHOD: The postauricular flap was designed to cover a Medpor framework for auricle reconstruction. The vascular pedicle of the flap was at the temporal part. The operation was performed on 12 patients, of whom 8 patients were with microsomia; 4 patients had total auricle defects. RESULTS: Postoperative follow-up period was 8 months in average. All the 12 patients obtained satisfactory appearance of the reconstructed ears. CONCLUSION: Clinical applications showed several advantages of this method.

[Start-up of full-scale UASB reactors].

The UASB reactors treating high-temperature citric acid wastewater could be started up in the alternation of mesophilic and thermophilic ranges because the local climate changed greatly by seasons. The reactors were started up in mesophilic range, and the total efficiency of the two-stage reactors reached 77%-86%; when the temperature of reactors reached 44 degrees C-45 degrees C, the reactors were operated in thermophilic range, and the total efficiency of the two-stage reactors reached 84%-93%; the reactors were re-operated in mesophilic range after closing about 38 days, and the total efficiency of the two-stage reactors reached 82%-96%. The start-up in the alternation of mesophilic and thermophilic ranges of the full-scale UASB reactors and the characteristics of mesophilic and thermophilic granular sludge were reported in this paper.

[Microsurgical treatment of 86 cases of intracranial aneurysms].

OBJECTIVE: To summarize the microsurgical experience in the treatment of intracranial aneurysms. METHOD: The clinical materials, surgical modes and prognosis of 86 cases of clipped intracranial aneurysms were retrospectively reviewed. RESULTS: Of the 86 cases, 69 recovered, 10 were moderately disabled, 2 severely disabled, and 5 died. CONCLUSIONS: To avoid rerupture, aneurysms should be treated as soon as possible when the diagnosis was established. Proper management of rupture of aneurysms during operation is the key of success. Multiple aneurysms are better to be clipped in one stage. "de novo" aneurysms could be detected early by long-term angiography.

[Study of pefloxacin concentration in blood and sputum in the aged pneumonia patients with impairment of renal function].

OBJECTIVE: To study the drug concentration in blood and sputum, clinical effect and drug toxicity of pefloxacin in the aged pneumonia patients with different degree of impairment of renal function. METHODS: The patients were divided into four groups according to the impairment of renal functions pefloxacin 400 mg/12 h venous inflow, period of treatment is 10 days. Clinical manifestation and experimental index were registered; the drug concentration in blood and sputum was measured with biochemical technique, then compared and analyzed. RESULTS: The drug concentration in blood and sputum in four groups differed from the degree of the impairment of renal function. The concentration of drug in blood and sputum of the normal renal function was close to that in the low-grade impairment of renal function. Their clinical effective ratios were 83%, 80%, bacterium cleanup ratio was 86%. The difference of drug concentration in blood and sputum in the severe impairment of renal function was greater than that in the moderate renal function group, their clinical effective ratios were 66%, 53%, bacterial cleanup ratios were 57%, 36%, adverse reaction was growing along with degree of impairment of renal function. This drug has renal toxicity for moderate and severe impairment of renal function. CONCLUSION: In case of moderate impairment of renal function, prolongation of dose interval should be considered; pefloxacin should be avoided in severe impairment of renal function.

Gynecological procedures under gasless laparoscopy.

OBJECTIVE: To evaluate gasless laparoscopy in treating gynecological conditions. METHODS: With Kirschner wire lifting, transversely or longitudinally introduced into abdominal wall subcutaneously, we exposed the operative field and pelvic organs with routine operative instruments under epidural anesthesia. When a dense pelvic adhesion was confronted, we used the temporarily low-pressured pneumolaparoscopy to dissect it. RESULTS: All procedures were carried out without organ injury, hematoma, or other complications. The operative field can be clearly seen with this method. The mean time for appendagectomy was one hour and for laparoscopic assisted vaginal hysterectomy 80 minutes. CONCLUSION: Gasless laparoscopy avoids the complications which occur in the routine laparoscopy such as subcutaneous or mediastinal pneumatoma, hypercarbonemia, air embolism, cardiopulmonary dysfunction. Our lifting method is simple, convenient, time and money saving, as well as more flexible for treating gynecological conditions.

Factor V Leiden mutation in one family of Chinese origin.

OBJECTIVE: To investigate the factor V Leiden mutation associated with activated protein C resistance (APCR) in Chinese. METHODS: Thirty "normal" individuals and twenty patients with thrombotic disease from Chinese Han Nationality were studied with APTT +/- APC, PCR followed by MnLI restriction enzyme analysis, PCR based direct sequence-specific primers (PCR-SSP) and DNA sequence analysis. RESULTS: In one healthy control, the activated protein C (APC) sensitivity ratio (SR) was found to be significantly lower (0.8) than that in other normal control (> 2.0). This individual was identified to be heterozygous for FV Leiden mutation (Arg506-Gln). His grand-uncle, father, brother and son were also identified to be heterozygous for FV Leiden. The APC resistance was found in 3 other cases of thrombotic diseases, but with no FV Leiden mutation. CONCLUSION: This is the first four generations family case of FV Leiden mutation associated with APCR reported within Chinese ethnic population. It is note-worthy that more FV Leiden or whether other gene defects may be associated with APC resistance and acquired APCR causing thrombosis in Chinese population.

[Realization of a new segmental bioelectrical impedance method for assessing body composition: correlation analysis of human body parameters].

The method of segmental bioelectrical impedance analysis for assessing body composition requires measurement of segmental lengths and cross-sectional areas; thus its complexity is increased. This paper reports the correlation analysis of human body parameters and the linear regressions for predicting segmental parameters by use of gender, height and weight. Accordingly, segmental impedance analysis is simplified.

Establishment of an animal model of chronic atrophic gastritis and a study on the factors inducing atrophy.

OBJECTIVE: To establish a rat model of chronic atrophic gastritis and explore the factors inducing atrophy. METHODS: In accordance with repeated orthogonal design of L8(2(7)), 60% alcohol and 20 mmol/L sodium deoxycholate (served as factor A), 0.05%-0.1% ammonia water (factor B), 0.05% indomethacin (factor C) were given, alone or in combination, to rats in three experiments for 3 months, 6 months or 9 months respectively. Then the rats were dissected, and their pathologic changes of the gastric mucosa were assessed. RESULTS: Typical signs of chronic atrophic gastritis (CAG) were found in all rats which were treated with factor A, B, C alone or in combination for 6 or 9 months. No significant difference of pathologic changes of gastric mucosa was found between the rats treated for 6 months and those for 9 months. No obvious CAG signs were found in the rats treated with factor A, B, C for 3 months. CONCLUSION: Sixty percent of alcohol, 20 mmol/L sodium deoxycholate, 0.05%-0.1% ammonia water and 0.05% indomethacin given to Sprague-Dawley rats for 6 months can successfully establish the animal model of CAG. Prolongation of the model-establishment time is not able to further facilitate the atrophy of gastric mucosa.

[Factors of laccase producing and fermentation conditions by a new white-rot fungus AH28-2].

White-rot fungus AH28-2, a newly isolated strain, produced effectively laccase by induction when grown on a synthetic medium. Aromatic compounds of low molecular weight had an inducing influence on laccase production and its isoenzyme compositions. The using of o-toluidine or syringic acid had the best inducing effect. Cu2+ concentration in medium had distinguished effect on laccase production. Enzyme activity was notably increased by Cu2+ and reached the maximum when Cu2+ final concentration was 5 mumol/L. Mn2+ inhibited the synthesis of laccase. Carbon and nitrogen limitation were not beneficial to laccase synthesis, while high nutrient organic medium was beneficial to the growth of cell and the synthesis of laccase. Using cellobiose as the sole carbon source, the highest level enzyme activity reached 82,923. 7 u/L under the condition of optimum fermentation with ABTS as substrate. This enzyme activity was 2.9-fold higher compared to the reported data on international references in recent years.

[The effect of epidermal growth factor on the pathologic changes of gastric mucosa in SD rats with chronic atrophic gastritis].

OBJECTIVE: To study the effect of epidermal growth factor (EGF) on the pathologic changes of gastric mucosa in rats with chronic atrophic gastritis(CAG). METHODS: The established rat models of CAG were divided into therapy group and control group. The rats in the therapy group received EGF 10 microg/kg subcutaneously (SC), whereas these in the control group the same volume of normal saline SC. 12 weeks later, all rats were killed by cervical dislocation and their gastric mucosa were examined with microscope. RESULTS: The grade of inflammatory cell infiltration in the therapy group was lower than that in the control group (P < 0.01). The thickness of gastric mucosal gland layer was (215.0 +/- 20.7) microm in the therapy group and (139.2 +/- 13.8) microm in the control group (P < 0.01). The ratio of the thickness of gastric mucosal glands and muscularis mucosa(L(1)/L(2)) was 2.70 +/- 0.34 in the therapy group and 1.27 +/- 0.27 in the control group (P < 0.01). The number of gastric glands in 1 mm length of mucosal layer was 26.20 +/- 1.27 in the therapy group and 19.90 +/- 1.78 in the control group (P < 0.01). In the therapy group, the gastric glands were rearranged in order, without signs of malignant proliferation. The width of the expression of proliferating cell nuclear antigen (PCNA ) of gastric mucosa was higher in the therapy group than in the control group [(77.70 +/- 4.16) microm vs (54.40 +/- 4.54) microm, P < 0.01]. CONCLUSIONS: EGF played a therapeutic role in reversing the gastric mucosal atrophy of the rats with CAG. It promoted the expression of PCNA, which induced a protective proliferation of the gastric mucosal lesions in the rats with CAG.

[The clinical importance of persistent elevation of electrocardiographic ST segment in acute myocardial infarction patients after reperfusion therapy].

OBJECTIVE: To observe the clinical importance of persistent elevation of electrocardiographic ST segment in acute myocardial infarction (AMI) patients receiving direct percutaneous transluminal coronary angioplasty (PTCA). METHODS: Thirty patients with a first AMI successfully treated with direct coronary angioplasty were selected. The extent of the ST segment elevation and resolution in electrocardiograms on admission and one hour after direct PTCA was analyzed. All the patients were divided into two groups according to the extent of ST segment elevation and resolution. Group A: resolution of ST segment elevation >or=50%, n = 19; Group B: resolution of ST segments elevation < 50%, n = 11. Low-dose dobutamine stress echocardiogram was taken at (7 +/- 2) days after AMI and echocardiogram checked at 1, 2 and 3 months after AMI. RESULTS: Left ventricular ejection fraction (LVEF) in group A was higher than that in group B at the first week and the first, second and third month after AMI. Aortic blood peak acceleration, cardiac output and cardiac output index of group A were all significantly higher than those in group B at stress. Global wall motion score index (GWMSI) and infarct-zone wall motion score index (IWMSI) in group A were smaller than those in group B at baseline and stress. GWMSI in group A was smaller than that in group B at 1, 2 and 3 months after AMI. There was no significant difference of IWMSI between group A and group B at 1 and 2 months after AMI. IWMSI in group A was smaller than that in group B at the third month after AMI. CONCLUSIONS: The patients in group B had lower left ventricular contractile function and contractile reserve function and less recovery of infarct-zone wall motion than that in group A.

[Immortalization of the rabbit mandibular condylar chondrocyte with SV40].

OBJECTIVE: To establish an immortalized cell line of the mandibular condylar chondrocyte (MCC). METHODS: Reconstructed retroviral vector pLN/SV40 containing the large T antigen and neomycin-resistance genes were transfected into the packaging cells PA317 using lipofectin transfection method. Then the primarily cultured cells derived from mandibular condylar chondrocytes(MCCs) of New Zealand rabbits were infected with the positive retrovirus medium and monoclone was selected with G418. Specific primer of large T antigen was designed and PCR was used to detect the integrating of large T antigen gene. The positive cell clones were subcultured and their growth characteristics were compared with untransfected cells. RESULTS: 10 cell clones of the transduced cells showed the positive gene fragment of 1,196 bp, and 5 of them had been further subcultured for more than 30 passages. One clone of A21 had been passaged for more than 100 times in nearly one and half year, and named immortalized mandibular condylar chondrocyte (IMCC). IMCCs were polygonal-shaped, similar to MCCs; the population doubling time of IMCC and MCC were 30.86 h and 78.95 h respectively. Subculture, freezing and recovering had no effect on cellular shape and proliferation of IMCC. CONCLUSIONS: An immortalized cell cline derived from the rabbit MCC has been successfully established.

Dynamic changes of interleukin-1, interleukin-6 and tumor necrosis factor in intermingled skin graft in burned rats.

OBJECTIVE: To compare the dynamic changes of interleukin-1 (IL-1), interleukin-6 (IL-6), and tumor necrosis factor (TNF) in intermingled skin graft with those in other types of skin grafts in rats. METHODS: A 10%-15% third-degree burn was created in 180 Sprague-Dawley (SD) rats. After removing the scar, skin grafts were performed on the open wounds immediately with autoskin (aus, n=54), alloskin (als, n=54) and intermingled skin (n=36). That is to say, in the intermingled skin graft, a big piece of alloskin (mals) was grafted first, and 3 days later, small pieces of autoskin (maus) wer e embedded in the alloskin. The rest 36 rats were taken as the controls. And the biological activities of IL-1, IL-6 and TNF in graft sheets in each group were detected after skin graft. RESULTS: The levels of IL-1, IL-6 and TNF in the aus group decreased steadily after their initial elevations, whereas in the als group they increased significantly and kept on the peak level in the later phases. In the in termingled group, there appeared a lowest IL-1 level in the mals and a highest one in the maus simultaneously at 7 (4) days (The number out of parenthesis is t he days after transplanting with alloskin sheets, and the number in parenthesis is the days after embedding autoskin sheets in the intermingled skin graft. Similarly hereinafter.) after skin graft (P<0.01), and the high level in the maus abruptly decreased at 14 (11) days after skin graft. At exactly the same phase on day 7 (4), a prominent peaked IL-6 in the mals occurred. In the later phases, the levels of TNF remained relatively low both in the m als and in the maus. From day 7 (4) on, each cytokine fluctuation in the mals synchronized with that in the maus. The longer the post transplantation period lasted, the more the positive cytokine correlated between the mals and the maus. CONCLUSIONS: The low levels of IL-1 and TNF may be important f actors to lighten the intensity of local rejection in the intermingled skin graft. The temporarily peaked IL-6 is both an inducer which induces the production of local IL-1 receptor antagonists and soluble TNF receptors and a signal which indicates a local enhancement of Th(2) cells. The mild rejection process and th e synchronized cytokine level during the later phases suggest a possible chimerism between the mals and the maus.

[Phalloplasty using an abdominal flap with two sources of blood supply (16 cases reports)].

OBJECTIVE: To improve the survival of a reconstructed penis and reduce injury of the donor site at the abdominal wall. METHODS: Sixteen cases of phalloplasty were performed using an abdominal flap based on two sources of blood supply i.e. the cutaneous branches of the inferior epigastric blood vessel and the superficial epigastric blood vessel. RESULTS: All of the operations were successful. CONCLUSIONS: With abundant blood supply and high survival rate and the advantage of easy transposition, the flap is a good option for phalloplasty.
