Mortality and career radiation doses for workers at a commercial nuclear power plant: feasibility study.

Career radiation doses for 8,961 male workers at the Calvert Cliffs Nuclear Power Plant (CCNPP) were determined for both utility (n = 4,960) and contractor (n = 4,001) employees. Workers were followed from the time of first employment at CCNPP (including plant construction) to the end of 1984 (mean follow-up = 5.4 y). Plant operation began in 1975. The mean duration of employment was 1.9 y at CCNPP and 3.1 y in the nuclear industry. Career radiation doses were determined from dosimetry records kept by the utility company and the U.S. Nuclear Regulatory Commission (NRC). For all exposed workers, the average career dose was 21 mSv and was higher for contractor (30 mSv) than utility (13 mSv) workers. Career doses were also higher among those employed in the nuclear industry for greater than or equal to 15 y (111 mSv) and among workers classified as health physicists (56 mSv). Cumulative doses of greater than or equal to 50 mSv were received by 12% of the workers; the maximum career dose reported was 470 mSv. The availability of social security numbers for practically all employees facilitated record-linkage methods to determine mortality; 161 deaths were identified. On average the workers experienced mortality from all causes that was 15% less than that of the general population of the U.S., probably due to healthier members of the population being selected for employment. Our investigation demonstrates that historical information is available from which career doses could be constructed and that, in principle, it is feasible to conduct epidemiologic studies of nuclear power plant workers in the U.S. Although difficult, the approach taken could prove useful until such time as a comprehensive registry of U.S. radiation workers is established.

Bidirectional stimulation of the white-opaque transition of Candida albicans by ultraviolet irradiation.

Most strains of Candida albicans are capable of switching spontaneously and at high frequency between a number of phenotypes distinguishable by colony morphology. The switching frequency of Candida albicans strain WO-1 between two predominant phenotypes, 'white' and 'opaque', and a minor phenotype, 'fuzzy', increased dramatically with low doses of ultraviolet irradiation that killed less than 20% of the population. The ultraviolet irradiation effect continued to be expressed over many generations as evidenced by stimulated sectoring. Ultraviolet irradiation stimulated switching in both the white-to-opaque and opaque-to-white direction, suggesting that a common mechanism functions in both directions.

The incidence of Listeria monocytogenes in raw milk from farm bulk tanks in north-east Scotland.

Over 180 farm bulk milk tanks were tested for the presence of Listeria monocytogenes on three separate occasions which included periods when cows were grazing and confined inside on a silage diet. The incidence of L. monocytogenes contamination was low, ranging from 3.8% in the summer samples to 1.0% in October. The level of contamination was estimated to be lower than one L. monocytogenes bacterium per ml in positive samples, as most required cold enrichment of 10-20 ml volumes before recovery. The distribution was sporadic; only one farm gave positive isolations on all three sampling occasions, one other on two, and all others were from different farms. No correlation between the presence of L. monocytogenes and hygiene standards or the feeding of silage was found.

Improved pattern of rate responsiveness with dynamic slope setting for the QT sensing pacemaker.

We have recently described the electrophysiological basis of a new algorithm for the QT (TX) sensing rate responsive pacemaker. By using the new software program running on the standard programmer it has been possible to simulate the new algorithm in ten patients with complete heart block (seven patients had implanted TX units and three were paced with an external TX pacemaker) during routine exercise testing. In this way a single-blind, intra-patient comparison of the pattern of pacing rate change using both the existing and new algorithms was possible. In nine out of the ten cases the time taken to increase the pacing rate from 70 to 80 bpm was reduced significantly when the new algorithm was used (P = 0.037). Additionally, the correlation between the atrial and ventricular rates in those patients with normal sinus node function (seven patients) was determined. In all cases we have observed a significantly improved correlation between the atrial and ventricular paced rates during exercise with the new algorithm (P less than 0.001).

The 'happy puppet' syndrome of Angelman: review of the clinical features.

Thirty six children with typical features of Angelman's syndrome, including global developmental delay, ataxia, episodes of paroxysmal laughter, seizures, and microcephaly were studied. The series included three sibships of three affected sisters, two affected brothers, and two affected sisters, respectively. The facial appearance is characterised by a prominent jaw, a wide mouth, and a pointed chin. Tongue thrusting is common. The movement disorder consists of a wide based, ataxic gait with frequent jerky limb movements and flapping of the hands. Tone is variable in the limbs with normal reflexes, and the plantar responses are usually flexor. The syndrome is being diagnosed more often, and attention is drawn to its diagnostic aspects.

A database of genetically determined neurological conditions for clinicians.

Progress is reported in the establishment of a database of genetically determined neurological conditions. There are now 1,300 conditions and 3,400 references store on the hard disk of a microcomputer. The database is searched, and a differential diagnosis obtained, by choosing a small number of signs and symptoms from a comprehensive list of cardinal features.

TRH pharmacokinetics and nerve stimulation evoked prostatic fluid secretion during TRH infusion in the dog.

The pharmacokinetics of TRH have been determined in man and the rat as well as other species whose serum contains TRH degrading enzymes, TRH pharmacokinetics have not been determined in the dog. This species is unusual in that its serum contains little or no TRH degrading activity. TRH pharmacokinetics were determined from measurements of plasma TRH concentrations during and following a 1-h infusion of TRH, 20 micrograms.kg-1.min-1, into dogs bearing prostatic urethral cannula. During TRH administration prostatic fluid secretion was evoked by hypogastric nerve stimulation and the content of TRH immunoreactive material in prostatic fluid was determined. Prostatic fluid was also collected after hypogastric nerve stimulation in dogs that did not receive TRH infusion. The TRH plasma profile in TRH-infused dogs exhibited two-compartment characteristics with an initial half-time of 10.4 +/- 5.4 (mean +/- SD) min, a terminal half-time of 69.4 +/- 24 min, and a metabolic clearance rate of 7.64 +/- 4.48 ml.min-1.kg-1. These findings suggest that the half-time of TRH is longer, and its metabolic clearance rate is less, in the dog than in humans or rats. TRH administration was not associated with altered nerve induced prostatic fluid secretion. Prostatic fluid samples contained TRH immunoreactive material regardless of whether they were collected after epithelial cell stimulation with pilocarpine, or after hypogastric nerve stimulation. In contrast to previous results with vasoactive intestinal peptide, TRH does not acutely alter prostatic fluid secretion. As has been reported for prostatic tissue extracts, immunoreactive material is present in prostatic fluid and appears to arise from epithelial cells.

The incidence of Listeria monocytogenes in farm bulk milks in NE Scotland.

Raw milk samples (c. 20 ml) were taken from approximately 180 farm bulk tanks in the NE of Scotland on 3 occasions over a period from August 1987 to February 1988 and tested for the presence of Listeria monocytogenes. The maximum incidence of L. monocytogenes (3.8%) was found in the August sampling, with incidences of 1.02% and 2.7% in October and February. Direct enrichment of 0.1 ml was negative and only 2 of the total of 14 positives were obtained by direct enrichment using 1.0 ml of milk, the remainder requiring cold enrichment over a period up to 8 weeks. These findings are similar to two recent USA surveys which suggest that is positive milks the number of L. monocytogenes per ml is in the order of 1 organism per ml.

Siblings of children with severe handicaps.

Interviews with 24 children ages 9 to 13 about life with their younger siblings who had severe handicaps revealed a consistently high level of involvement, strong feelings of responsibility, and an emphasis on positive aspects of family life. Hardships were not denied, but these children appeared to be faring better than children studied in earlier research, before schooling for children with handicaps and other supports for their families were mandated.

Predicting success in football.

Clinical interviews were held with the rated most successful starting players for each position on a college football team. Mental strategies associated with major sensory systems were determined for each player through observation of eye movement patterns and players' usage of sensory based words. Mental strategies for motivation, creativity, belief, learning, decision making, and memory were assessed through these observations. Both offensive and defensive winners have a high visual lead. The auditory sensory modality is LEAST utilized. Creativity and decision making are dominated by the visual sense. Mental strategies for motivation, belief, and memory are balanced between visual and kinesthetic sensory modalities. In future studies clinical observations suggestive of sensory based mental strategies can be combined with empirically validated personality test data to increase predictive power in the selection and placement process with college and professional football players. This should reduce costs of selection and placement errors caused by hitherto undetected psychological factors.

Pyridine nucleotides and phosphorylation potential of rabbit corneal epithelium and endothelium.

In order to validate in situ corneal redox fluorometry, the redox state and phosphorylation potential of freeze trapped rabbit corneal epithelium and endothelium were studied using quantitative histochemical methods. The results were compared with noninvasive measurements using an optically sectioning fluorometer microscope. Enucleated rabbit eyes were either frozen in Freon-12, cooled by liquid nitrogen or exposed for 1 hr in 1 mM NaCN to block oxidation and then freeze trapped. Corneas were sectioned, freeze-dried, samples of individual layers dissected, weighed, and analyzed for: NADH, NAD+, NADPH, NADP+, ATP, ADP, and Pi. The aerobic epithelium showed a ratio for NAD+/NADH of 1.85 +/- 0.08 (9). In anoxia this ratio decreased to 1.06 +/- 0.07 (8). The NAD+/NADH ratio of aerobic endothelium was 3.25 +/- 0.28 (6); in anoxia this ratio was 0.68 +/- 0.14 (5). The values of phosphorylation potential ATP/(ADP X Pi)M-1 were: 447.9 +/- 40.2 (9) in aerobic epithelium, 378.2 +/- 24.7 (5) in anoxic epithelium; 308.4 +/- 25.2 (7) in aerobic endothelium and 225.4 +/- 19.1 (5) in anoxic endothelium. Aerobic-anoxic transitions alter the concentrations of NADH and NAD+ but did not affect the concentration of NADPH and NADP+. The microhistochemical data indicate that the redox state of rabbit epithelium is less sensitive to hypoxia than the endothelium. This difference between the two limiting layers is reflected in alterations of phosphorylation potential induced by hypoxia. The similarly high efficiencies of both layers in maintaining relatively high ATP levels during histotoxic hypoxia is most likely a result of compensatory ATP generation by enhanced glycolysis.

Comparison of HIV-antibody prevalence in patients consenting to and declining HIV-antibody testing in an STD clinic.

All patients attending a sexually transmitted disease (STD) clinic were offered voluntary, anonymous human immunodeficiency virus (HIV)-antibody testing and counseling as part of routine clinical evaluation. During a three-month evaluation period, 82% of patients accepted HIV testing. Testing was accepted equally by men and women and by heterosexual and homosexual men. Black men were more likely to refuse testing than men from other ethnic groups. Eight (0.7%) of 1146 STD clinic patients tested were infected with HIV. A blinded study of 237 patients who refused testing identified nine (3.8%) additional HIV-infected patients. Men who refused testing were 5.3 times more likely to be infected than men who accepted testing. Male homosexuals and black and Hispanic men who refused testing were 7.3 and 8.8 times, respectively, more likely to be infected with HIV than were their counterparts who accepted testing. Human immunodeficiency virus testing and counseling should be routinely offered to STD clinic patients. Male STD clinic patients, especially homosexual or minority men, who refuse voluntary HIV testing should be counseled regarding reducing their risk for HIV transmission.

The relationship between plasma vasopressin and changes in coagulation and fibrinolysis during hip surgery.

To investigate whether vasopressin (aVP) could have a role in the regulation of coagulation and fibrinolysis during hip surgery, venous blood samples were taken for assay of FVIII:C, FVIII R:Co, vWF:Ag, fibrinopeptide A (FPA), euglobulin clot lysis time (ECLT), high molecular weight fibrin breakdown products (XL-FDP) platelet aggregation in whole blood and aVP from seven patients undergoing elective hip surgery. Samples were taken at set points over the operative period. FVIII:C increased during the operation from a geometric mean of 0.7 iU/ml pre-operatively to 1.09 iU/ml (p less than 0.05) post-operatively. vWF:Ag and FVIII R:Co rose in a similar manner. PAA (10(6)/ECLT2) rose from 12 units pre-operatively to 167 units (p less than 0.001) at prosthesis cementing, and post-operatively fell to subnormal levels. FPA increased from 13 pmol/ml to 58 pmol/ml (p less than 0.05) at prosthesis cementing, and fell to 9 pmol/ml post-operatively. Plasma XL-FDP rose from 115 ng/ml pre-operatively to 456 ng/ml at skin closure (p less than 0.05). Plasma aVP rose from 0.5 pg/ml pre-operatively to 40 pg/ml (p less than 0.01) at division of the femoral neck. There were no changes in platelet aggregation using 1.5 microM ADP. The results demonstrate activation of coagulation and fibrinolysis during the operative procedure. The mechanisms involved in these changes are complex, but the results support the hypothesis that aVP has effects on factor VIII and fibrinolysis similar to those described for abdominal surgery.

Biomaterials for facial bone augmentation: comparative studies.

Presently no material is available which is entirely satisfactory for facial bone augmentation. These studies examine several of those already in clinical use, made from various polymers in solid, porous, and woven forms. Homograft bone has also been studied, as an implant material. All materials were used in situations for which they are currently recommended clinically. Bioglass (Bioglass is a trademark of the University of Florida) implants, which are suggested for clinical use, have been studied in the same model and results show that their surface activity provides a more satisfactory immobilization, both in the short and long term, than does the tissue ingrowth on which most of the other materials depend. Results show that in this model as well as in clinical practice, porous and woven materials provoke in tissues a continuing cellular response which will always compromise long-term clinical success. Autograft bone has associated morbidity and is unpredictable with respect to its incorporation into host tissue and persistence at the site. Bioglass, however, was immobilized successfully at both hard and soft tissue interfaces without the need for porosity, could be satisfactorily shaped in the operating room, and, in addition, had the bonelike hardness which is not provided by any other available material.
