Cognitive functioning in albino children.

Twelve albino children with IQs in the normal range were psychometrically assessed and compared to a control group of equally visually handicapped children who were non-albino. A striking and consistent pattern of cognitive functioning emerged in the albino group (a large verbal/performance discrepancy in favour of verbal skills) which could not be attributed to their poor visual acuity. This unusual pattern of intellectual functioning may be related to the unique neuroanatomy and neurophysiology recently demonstrated in people with albinism.

Stimulus-secretion coupling in bovine parathyroid cells. Dissociation between secretion and net changes in cytosolic Ca2+.

The relationship between the concentration of cytosolic free Ca2+ ([Ca2+]i) and secretion of parathyroid hormone (PTH) was investigated in isolated bovine parathyroid cells using the fluorescent Ca2+ indicator, quin 2. Increasing the concentration of extracellular Ca2+ from 0.5 to 2.0 mM caused a 3-fold increase in [Ca2+]i (from 183 +/- 4 to 568 +/- 21 nM) which was associated with a 2-4-fold decrease in secretion of PTH. Decreasing extracellular Ca2+ to about 1 microM caused a corresponding fall in [Ca2+]i to 60-90 nM. Extracellular Ca2+-induced changes in [Ca2+]i were not affected by omission of extracellular Na+. Depolarizing concentrations of K+ (30 mM) depressed [Ca2+]i at all concentrations of extracellular Ca examined, and this was associated with increased secretion of PTH. Ionomycin (0.1 or 1 microM) increased [Ca2+]i at extracellular Ca2+ concentrations of 0.5, 1.0, and 2.0 mM, but inhibited secretion of PTH only at Ca concentrations near the "Ca2+ set point" (1.25 microM). In contrast, dopamine, norepinephrine (10 microM each), and Li+ (20 mM) potentiated secretion of PTH without causing any detectable change in [Ca2+]i. The results obtained with these latter secretagogues provide evidence for a mechanism of secretion which is independent of net changes in [Ca2+]i. The phorbol ester 12-O-tetradecanoyl phorbol 13-acetate (TPA) did not alter [Ca2+]i or secretion of PTH at low (0.5 mM) extracellular Ca2+ concentrations. At 2.0 mM extracellular Ca2+, however, TPA (20 nM or 1 microM) depressed [Ca2+]i and potentiated secretion of PTH. The addition of TPA prior to raising the extracellular Ca2+ concentration reduced the subsequent increase in [Ca2+]i. The results show that the effects of TPA on secretion in the parathyroid cell are not readily dissociated from changes in [Ca2+]i and suggest that some TPA-sensitive process, perhaps involving protein kinase C, may be involved in those mechanisms that regulate [Ca2+]i in response to changes in extracellular Ca2+.

The other problems of alcoholics.

Today's alcoholism treatment populations are characterized by increasing heterogeneity. Alcoholics vary with regard to other substance use and dependence, psychiatric/psychological problems and neuropsychological functioning. The implications of such heterogeneity are explored with regard to conceptualization of alcoholism, assessment, and individualized treatment planning.

Cranio-maxillary fixation using supra-orbital pins.

This paper compares the forces which external frames, used for cranio-maxillary fixation, can resist. Clinically, several frames using supra-orbital pins have been proven satisfactory. By quantifying the resistance forces, a more objective assessment of their effectiveness is made. The resistance to backward displacement of the maxilla is measured and compared for 3 frames. Two frames are used for comparison of resistance to occlusal forces.

The rabbit as an experimental model for regional chemotherapy. 1. Intra-arterial hindlimb infusion.

Regional delivery of chemotherapy to a tumour or tumour-bearing region has pharmacokinetic advantages over the systemic route. The applications of an animal model for regional drug delivery are outlined. The technique for intra-arterial infusion in the rabbit hindlimb is described. The use of the implantable VX2 rabbit carcinoma as a model for solid human tumours may be studied by this method. Pharmacokinetic data obtained with the model allow comparison between systemic and regional routes of delivery. The distribution of the cancerostatic plant toxin ricin following regional delivery has been investigated using this experimental model.

The rabbit as an experimental model for regional chemotherapy. 2. Isolated perfusion of hindlimbs.

Regional chemotherapy to a tumour is most commonly delivered by intra-arterial infusion. An alternative method of regional drug delivery, isolated perfusion, may be used where anatomical considerations permit. The technique of isolated perfusion in the rabbit hindlimb is described. The use of this model with the implantable rabbit VX2 carcinoma allows estimation of drug uptake by normal tissues, primary tumour and popliteal lymph node metastases. Correlation of such data with blood flow measurements enables targeting of new cytotoxic agents to be evaluated. The effect of perfusate composition on tissue uptake of such an agent, the plant toxin ricin, has been determined.

Response patterning in goldfish: single alternation in a Pavlovian aversive paradigm.

Single alternation behavior was studied in a Pavlovian aversive shuttle-response situation using goldfish. Independent groups of fish were given either a fixed interval between alternating reinforced (R) and nonreinforced (N) trials or were given differential temporal information between trials. Although all groups exhibited higher response probabilities on reinforced than on nonreinforced trials, goldfish receiving a short interval (10 s) following R trials and a long interval (60 s) following N trials (R10N60) demonstrated significantly superior alternation performance compared with subjects receiving a fixed intertrial interval (R35N35) or a long interval following R trials and a short interval following N trials (R60N10). The alternation performance exhibited by the R10N60 group was shown to be equivalent to that of subjects receiving standard color discrimination training. However, the alternation performance in group R35N35 showed a great deal of within-subject variability and raises questions concerning the elusive nature of alternation using Pavlovian conditioning procedures.

Capgras syndrome--the many avenues to delusional misinterpretation.

The case of a 47 year old woman with Capgras' Syndrome is presented. Despite her misinterpretation of acoustic and visual clues, multiple investigations and extensive neuropsychological testing failed to implicate an organic basis upon which to explain her delusional belief. The authors conclude that single explanatory hypotheses--psychological or neurological--may preclude an appreciation for the heterogeneity of the syndrome.

Distorted cognitive maps: college students' misperceptions of nation size.

On a questionnaire 256 college students reported a number of distorted views concerning the sizes of nations in the world and these distortions appeared to generalize to geographic regions. Considering that individual cognitive maps are commonly used to assess new information about the global environment, these inherent distortions could significantly bias the students' views of the world.

Penetrating neck trauma: diagnostic studies in the asymptomatic patient.

The selective management of penetrating neck trauma implies an attempt to individualize care and minimize unnecessary surgical exploration. In asymptomatic patients, diagnostic studies are performed in an attempt to exclude clinically unrecognized injuries. This review of 100 consecutive cases of penetrating neck trauma assesses the role of ancillary diagnostic studies in 53 patients selectively managed. The diagnostic yield from a combination of angiography, fluoro-esophagography, and aerodigestive tract endoscopy was 22.6% (12 of 53). However, only five patients (9.4%) actually benefitted from ancillary diagnostic studies, in that angiography documented clinically unrecognized injury. In an effort to avoid the indiscriminate use of ancillary diagnostic studies, a selective management plan based on anatomic zones of injury is provided.

Biochemical and morphological characterization of a plasma membrane-enriched fraction from bovine parathyroid cells.

A fraction of enriched plasma membranes from bovine parathyroid cells has been prepared by differential centrifugation. Biochemical characterization shows that this fraction has a specific activity enrichment of 7.2-fold in ouabain-sensitive Na+-K+ ATPase, and 3.5-fold in 5'-nucleotidase. Less than 4% of the total mitochondria and lysosomes are present within the plasma membranes, while microsomal contamination accounts for 14% of total specific activity. Parathyroid hormone radioimmunoassay also reveals the presence of some secretory granules within the plasma membrane fraction. The characteristic morphological aspect of the unusual surface membrane is shown by freeze-fracture electron microscopy. In the enriched pellets, vesicles identified as having a plasma membrane origin have variable sizes, and 50% show an inside-out conformation. Even though the plasma membrane fraction described herein is not absolutely free from contamination by other subcellular components, this protocol represents the first attempt to purify surface membrane from parathyroid tissue and provide the starting material for understanding, at a molecular level, the properties of extracellular Ca2+ regulation and its coupling with secretion of parathyroid hormone.

Some further studies on the prediction of experimental colds in volunteers by psychological factors.

An analysis of the incidence of colds in volunteers, exposed experimentally to various rhinovirus and influenza virus challenges, confirmed an earlier report that assessments of virus shedding show a higher degree of infection by rhinovirus in Introverts than they do in Extroverts. The influenza trials however did not show such an effect. In addition, people with higher scores on a widely used self-report inventory of mild psychoneurotic symptoms tended to show more evidence of increased nasal secretion after infection by either type of virus. The specific scale showing the latter effect most clearly was that of Obsessional Symptoms. Taking account of these psychological factors greatly increased the sensitivity of a study of the beneficial effects of interferon.

Reporting the patterns of local medical practices with cancer patients to community hospital physicians: the methodology.

This is a presentation of the methods chosen to present initial data to the physician site committees in the Minneapolis-West Metro Community Hospital Cancer Program. This data will be presented at approximately six-month intervals so that guidelines can be modified, to maximize the effectiveness of the physician management guidelines. The reporting system has been designed to allow each hospital to look at its own data as compared to the aggregate total of all the hospitals (but never another individual hospital), and to allow more detailed studies within a local hospital using the usual quality assurance procedures. The goal is to help in meeting the American College of Surgeons requirement of accredited cancer programs of continued surveillance of the cancer treatment given and the results in hospitals, and to document the effect of the Community Hospital Cancer Program upon cancer patient management.
