Diagnosis and management of asymptomatic renal cell carcinomas in von Hippel-Lindau syndrome.

Renal cell carcinomas occurred in 6 of 17 subjects with the von Hippel-Lindau syndrome (HLS). In 4 patients, the tumors were discovered by abdominal computed tomography (CT) during a prospective study. Abdominal CT is recommended as a safe and accurate method for screening patients with HLS for renal cell carcinoma. Renal cell carcinoma in HLS tends to be multifocal and bilateral. Because of this, it is recommended that the tumors be treated by surgical enucleation rather than by more extensive surgical procedures which might ultimately lead to the need for dialysis.

Culture of peripheral monocytes in vitro in patients with renal cell carcinoma: a possible prognostic indicator.

An in vitro assay has been applied in 23 patients with renal cell carcinoma. A mononuclear cell-rich fraction was cultured from peripheral blood of patients with renal cell carcinoma. The number of monocytes maturing into macrophages was quantitated over a period of 7 days. Monocyte maturation was significantly lower in patients with renal cell carcinoma than in normal individuals. Of 10 patients tested both pre- and post-nephrectomy, 8 patients showed clinical improvement after surgery. In all 8, the posttreatment macrophage yield improved significantly from the pretreatment values to approach normal values. In 2 patients found to have metastasis postoperatively, the macrophage yield continued below normal levels. Our results suggest that the in vitro maturation of macrophages from mononuclear cells may parallel clinical events.

Basal cell carcinoma of penis.

Basal cell carcinoma, common in the head and neck region, rarely occurs on the penis. We report here a case of primary basal cell carcinoma of the penis in a fifty-five-year-old man, apparently representing the eleventh reported case. Review of the literature indicates that most penile basal cell carcinomas were located on the shaft and that the age, race, and other clinicopathologic features were comparable to basal cell carcinomas of other sites. The rarity of penile basal cell carcinomas is probably due in part to the low exposure of this region to sunlight and other predisposing factors. Interestingly, of the 9 reported cases of penile basal cell carcinoma, with available information, 6 were associated with local conditions which could have played a predisposing role.

Primary neoplasms of the ureter.

The role of tobacco as a carcinogenic agent should be considered in ureteral carcinoma. In our study 77 per cent of the patients had a history of smoking, with an average of 50 packs a year consumption. Various studies have shown that the prognosis for ureteral carcinoma is related primarily to the grade and stage of the disease at the time of treatment. Therefore, it is imperative that the clinician maintain a high index of suspicion. This index of suspicion should be heightened in any patient with lower tract urothelial tumors. In addition to endoscopic followup excretory urography should be done on an annual basis, with retrograde studies when necessary. Ureteral brush biopsies or tissue obtained with the Dormia basket may be diagnostic. A Dormia stone basket was used in 3 patients in our study and sufficient tissue for diagnosis was obtained in each case. No complications were encountered. The results obtained in treating a primary leiomyosarcoma of the ureter are noteworthy in that complete remission of pulmonary metastases was obtained after the patient received a course of doxorubicin hydrochloride and dimethyl-triazeno imidazole carboxamide.

Maturation of monocytes in patients with renal cell carcinoma.

Peripheral blood leukocytes were cultured from patients with renal cell carcinoma and compared with those from normal volunteers. The number of macrophage precursor cells recovered was 17.99 +/- 11.65 X 10(3) cells per ml of blood in patients with renal cell carcinoma, whereas the number recovered in normal individuals was 72.0 +/- 20.0 X 10(3) cells per ml of blood. The low macrophage yield in patients with renal cell carcinoma was not associated with serum factors, and there was no difference in the number of monocytes in the mononuclear cell preparations. Therefore, there seems to be an intrinsic defect in the macrophage maturation process in vitro in patients with renal cell carcinoma.

Clotting predisposition in carcinoma of the prostate.

To delineate factors that might increase the risk of thromboembolic events in patients on estrogen therapy for prostatic carcinoma a study was undertaken of various coagulation parameters. A correlation was demonstrated between advanced stages of prostatic carcinoma and depression of the serum antithrombin III activity. The addition of estrogen therapy resulted in further depression. Since low levels of serum antithrombin III activity have been associated with thrombosis this observation may have clinical relevance.

Leiomyosarcoma of urinary bladder.

Leiomyosarcoma of the bladder remains a rare diagnosis with poor therapeutic results. The available data continue to support surgical intervention as the mainstay of therapy, although other modalities have been employed with varying success. A case report and review of the literature with current treatment alternatives are presented.

Computed tomography in the diagnosis of renal carcinoma complicating Hippel-Lindau syndrome.

Four patients on long-term follow-up for Hippel-Lindau syndrome (HLS) are presented. All had undergone nephrectomy for renal carcinoma; in 2, carcinoma subsequently developed in the remaining kidney. Because of the tendency of these tumors to develop bilaterally, wide local resection is preferable to radical nephrectomy if the tumors can be diagnosed when small and noninvasive. CT is currently the most accurate method of regularly screening patients with HLS for asymptomatic renal carcinoma.
