[Tertiary hyperparathyroidism after renal transplantation in man. Treatment by parathyroidectomy and auto-graft. Eight cases (author's transl)].

In a series of 76 functional renal transplantations we noted 8 cases of hyperparthyroidism occurring in periods ranging from 2 months to 4 years. Hypercalcaemia was the most constant manifestation. PTH was raised in the 7 subjects in whom it was measured. Eight patients underwent total parathyroidectomy followed in 7 of them by auto-graft of a fragment of parathyroid into a forearm muscle according to the technique of Wells. The operation was followed in all cases by normalisation in blood calcium levels after the 1st month. Serum PTH levels also returned to normal apart from in one case where renal function was impaired because of rejection of the renal graft. Comparative estimations of PTH in venous blood from the side of the auto-graft and the opposite side showed that the parathyroid graft was functioning.

[Total parathyroidectomy with reimplantation: treatment of choice of hyperparathyroidism secondary to renal insufficiency : report on 24 cases (author's transl)].

Results in a series of 24 total parathyroidectomies with immediate reimplantation in patients with chronic renal insufficiency are described, and the advantages of this method discussed. This technique avoids the complications of sub-total parathyroidectomy, and the very rare failures can be easily treated by local excision if it is velated to recurrence of hyperparathyroidism, or by further reimplantations in cases of hypoparathyroidism. Grafts are possible during a second stage, after several months of freezing, but a limit of 9 months should be suspected. This method is proposed as a routine measure in all cases of parathyroid hyperplasia.

[Tertiary hyperparathyroidism following kidney transplants in man treated by parathyroidectomy and parathyroid autografts. A report on 8 cases (author's transl)].

Hyperparathyroidism developed in 8 of 76 patients two months to 4 years after functional Kidney transplants, the most constant manifestation being a hypercalcemia. Raised PTH levels were present in all 7 patients in whom they were measured. The 8 patients were treated by total parathyroidectomy, followed in 7 cases by an autograft of a parathyroid fragment into the muscle off one forearm, according to the technique described by wells. Blood calcium levels became normal in all cases after one month. PTH levels also returned to normal except in one case where rejection of the Kidney graft caused a reduction in renal function. Comparing PTH levels in the venous blood on the side of the autograft with that on the opposite side demonstrated that the parathyroid graft was actively secreting.

Research on the eventual enzymatic induction activity of cetiedil in rat and man.

Therapeutic consequences of the enzyme induction phenomenon are briefly reminded. This action was investigated: 1.--In the rat: by measuring the P450 Cytochrome content of hepatic microsomes in rats treated with Cetiedil. The results were compared with those obtained with controlled rats in similar conditions who had received either phenobarbital or no drugs at all. 2.--In man: by measuring the urinary elimination of D-glucaric acid and by measuring the plasma half-life of antipyrine before and after Cetiedil. These results in man were compared with similar results obtained before and after treatment with Rifampicin. Cetiedil does not seem to produce any increase in hepatic enzyme activity.

[Teratoma of the neck. A report of one case (author's transl)].

The authors present a rare case of teratoma developed in the cervical region of a female teenager. Comparing this case to the literature, they study the clinical aspects which are different according to the age of the discovery, pointing out the most frequent case of the newborn and they study the histological findings setting out the diagnosis. Prognosis depends on the maturity degree of the tissues; it is more often than not excellent after surgical exeresis with child contrary to the seldom adult cases of which the evolution is quickly distressing.

[A case of relapsing mesenchymatous tumour of the abdominal wall with metastases (author's transl)].

It is classical to consider desmoid tumours as an entity with only a local prognosis, conditioned by the frequency of relapses. In this respect, they are different from other mesenchymatous tumours, fibrosarcomas in particular which have a poorer prognosis. The authors report the case of a 23 years old man with a tumour of the abdominal wall, which in the early stages resembled a desmoid tumour, but the development of metastases suggested a sarcoma. This observation seems to make doubtful the distinction between desmoid tumour and fibrosarcoma, and thus the prognosis of tumours with a desmoid appearance. Under these conditions, this term should only be used for descriptive purposes and has no prognostic value.

[Endométriosis of the small intestine. Report of 4 new cases of stenosis of the small intestine (author's transl)].

The authors report 4 new cases of stenosis of the small intestine of endometrial origin. They emphasize the rareness of these stenosing forms which in most recent series intervened in 1 case of endometriosis out of 1,000 (Martinbeau). They consider the various clinical presentations encountered, asymptomatic forms discovered by chance, forms associated with suggestive gynecological disorders, chronic stenosis of the small intestine or presentation with intestinal obstruction. They consider the diagnostic difficulty in the isolated forms (2 cases) in the absence of pelvi-genital lesions. Among the positive factors, they consider the recrudescence during theperiods of the digestive signs, the macroscopic appearance of the lesions and, above all, the mucosal integrity noted on opening the specimen after intestinal resection. Finally, they consider certain special problems, that of bi-polar intestinal involvement with double localisation, involving the ileum and the recto-sigmoid areas.

[Teutschlaender lipo-calcino-granulomatosis or tumoral calcinosis of Inclan (author's transl)].

The present report describes a typical case of tumoral calcinosis. Differential diagnosis was initially directed towards sarcoma suggested by fibrous connective tissue surrounding cystlike cavities. The course of this disease led to chronic multiple fistulae with secondary infection in spite of two attempts of surgical excision. This case adds further support to the results of earlier reports showing no specific biological abnormalities. The present sutyd indicates otherwise that tumoral calcinosis and lipo-calcino-granulomatosis of Teutschlaender are the same condition. The pathogenesis of this affection remains enigmatic; there is no evidence that the lipidic excess should be the beginning of the process, but this disease must be considered as a distinctive form of calcinosis. The surgical exeresis is the only possible treatment, even if it doesn't prevent recurrence.

[Post-traumatic sequestra of the liver].

The authors report 92 cases of trauma of the liver which survived initial operation and study the special problems linked to hepatic sequestration. They analyse the factors which favour the appearance of these sequestra and the methods of diagnosis using clinical, laboratory, arteriographic and scintigraphic data. Relatively frequent (8,4 p. 100), these complications require fairly early surgical treatment. They are commonly localised to the dome of the liver and this renders their approach difficult. A low postero-lateral thoracotomy with resection of the 10 th or 11 th rib, followed by sequestrectomy and drainage, seems to them ideal approach. The pleuropulmonary sequelae are minimal.
