[Value of absorbable stapling in digestive surgery? Experimental study comparing TA metallic stapling, TA absorbable stapling and manual sutures].

Absorbable staples, recently available for TA 55 instrument, has not been yet evaluated in gastro-intestinal surgery. This experimental work was designed to determine the feasibility and the reality of the advantages of the new material: the wound healing process of 3 techniques of intestinal closure (metal staples, Lactomer absorbable staples and polyglactin sutures) were compared. A double Roux en Y loop was performed on 9 dogs: each end was closed by one of these techniques, and recuperated at regular intervals (3 samples by dog). Twelve sutures were examined at 10 post-operative days, 14 at 25 days, and 10 at 130 days. Macroscopic examination showed major inflammatory adhesions around absorbable staples. Two fistulae have complicated absorbable stapling versus 0 with the 2 others techniques. Histologic review showed greater tissue trauma with Lactomer staples than metal staples; intense and prolonged inflammatory reaction was seen around absorbable staples, while metal staples were well tolerated. The difference in design of the 2 staples can explain the differences in the healing process. In spite of the interest for absorbable material, our experimental study shows the present superiority of the metal stapling and prompts improvements of absorbable staples for reliable use in gastro-intestinal surgery.

[Congenital superior mesenteric arteriovenous malformations].

An arteriovenous aneurysm in the region supplied by the superior mesenteric artery was detected fortuitously during abdominal arteriography. The absence of a previous history of injury or operation suggested a congenital origin for this infrequently observed lesion. Logical therapy whenever possible is resection of mesentery and the neighbouring intestine.

[Preoperative vascular loading. A guide for the management of volume expansion in the surgery of pheochromocytoma].

The most acurate fluid replacement is needed for the operative management of patients with pheochromocytoma. Pulmonary capillary wedge pressure monitoring must be incorporated into routine practice for assessing left ventricular filling pressure because discrepancies between central venous pressure and left-sided filling pressure may occur, even in patients apparently free of cardiopulmonary dysfunction. The response to preoperative acute volume loading was used in 11 patients--to detect preoperatively patients in whom fluid infusion was associated with a dangerous increase in pulmonary wedge pressure without improvement in forward flow and--to provide a guide for intraoperative volume replacement. This method confirmed informations suspected from baseline pressure data in one patient: distorsion between right and left ventricular filling pressures. Furthermore it showed an unsuspected ventricular dysfunction in a second patient with normal baseline pulmonary capillary wedge pressure. Both volume and speed of operative fluid challenge were adapted to this preoperative ventricular response.

Adrenal Gland Neoplasms↗

[Risk to the parathyroids in thyroid surgery. Value of parathyroid reimplantation. Apropos of a homogenous series of 350 operations].

Based on a series of 350 thyroidectomies, of which 214 were total or practically total, the risk of postoperative sequelae due to hypoparathyroidism was investigated. The frequency of this complication was particularly high (11.43 p. 100) during the immediate postoperative period, probably due to the amount of thyroid excised in this series. Long-term follow-up showed that only 15 patients (4.29 p. 100) were still being treated with vitamin D derivatives. Parathyroid reimplantation, whether "accidental" during simple thyroidectomy or "as a principle" in jugulocarotid and recurrent nerve gland curettage (25 cases) appears a valid method for avoiding long-term hypoparathyroidal sequelae.

[Thyroid cancer. Prognostic factors].

The outcome of 126 patients with cancer of the thyroid gland was reviewed. Factors affecting prognosis were evaluated and compared with data available in the literature. The two determinant factors were histological type and local extension and metastasis; they correlated closely with age at the time of diagnosis. In this study sex and cervical lymph node involvement were unrelated to survival.

[Peroperative circulatory arrest caused by a tumoral pulmonary embolism following hepatectomy for tumor. Apropos of a case].

The authors report a case of pulmonary tumor embolism during a hepatectomy for hepatoma, leading to a fatal cardiac arrest. This cause of cardiac arrest during operation has never been reported in the literature, but systematic autopsies of patients dying from liver cancer, especially primary or metastatic tumors, shows that the incidence of these embolisms is frequently underestimated. Any anomaly of the hepatic vein appearing in the X-rays implies a danger of intra- or post-operative embolism. Allowance must therefore be made for this in the surgical technique, which should include complete or partial cross-clamping of the vena cava.

[Traumatic rupture of the diaphragm. 67 cases].

As a result of improvements in accident prevention, traumatic rupture of the diaphragm is less commonly observed. Young adults are anatomically more exposed to the injury. Beside the classical factors, falls from great heights and direct traumas must be borne in mind as possible causes. Tears in the diaphragm are only dangerous when accompanied by visceral lesions involving the oesophagus, the large vessels, the heart, etc. Only respiratory symptoms are suggestive of a notoriously difficult diagnosis. Ultrasonography appears to be of value. The prognosis depends on associated lesions, which are frequent. The surgical treatment is usually simple, the only problem being the approach. If no complication occurs, sequelae are minimal.

[Inflammatory pseudotumor of the liver. Unusual course of an unrecognized abscess? Apropos of a case].

The authors report the case of an hepatic tumor occurring in a 46 years old man admitted for a long-way fever. After hepatectomy, the microscopic examination showed fibro-xantho-granulomatous aspects with micro-abscess, tuberculoid nodules and a portal phlebitis. These lesions have been interpreted as an "inflammatory pseudotumor". The tumor had microscopic likenesses with inflammatory pseudotumors of the lung of which an infectious origin is only suggested. In the literature, the authors found only two cases of inflammatory pseudo-tumor of the liver, in which micro-abscess are not mentioned. The feature helps the authors to find some similarities between their tumor and a particular type of granulomatous abscess of the liver.

Diagnosis, Differential↗

[Renodigestive fistulas. Apropos of 3 cases].

Two cases of left renocolic fistula and one case of a double renogastric and left renocutaneous fistula are reported. Diagnostic difficulties are encountered in these affections, their origin being usually from the kidney. Enlarged nephrectomy appears to be the only possible treatment in the majority of cases, the attitude adopted in respect of the digestive viscera involved varying according to local conditions.

[Angiomatous cutaneous lesions revealing arteriovenous fistulas (author's transl)].

Reports of Bluefarb-Stewart syndrome have been infrequent. Two observations of this condition are described. Both patients are young men who presented with extensive angiomatous lesions of the feet, revealing distal arteriovenous fistulas. Surgical treatment was successful in one patient. Diagnostic criteria are discussed. As to pathogenesis the most likely hypothesis is that the arteriovenous fistulas are congenital. Bluefarb-Stewart syndrome can therefore be classified as one of the angiodysplasias.
