Results of surgical treatment in children with bladder exstrophy.

A series of 42 children (30 boys and 12 girls) underwent surgery for bladder exstrophy between 1972 and 1989. Primary bladder closure was performed in 11 patients and was successful in 6. Four of these children are about 2 years old and so it is not yet possible to assess their continence. Ureterosigmoidostomy was performed in 35 children, one of whom was converted to cutaneous ureterostomy. Follow-up ranges from 2 to 20 years (average 9 years 3 months) and 50% of the patients are symptom-free. The most frequent problems were acidosis and urinary tract dilatation. Other complications, such as hypokalaemia and pyelonephritis, were seldom seen. Although ureterosigmoidostomy has some disadvantages, our patients have adapted well and lead a normal life.

[Comparative light microscopic, scanning-electron microscopic and electron microscopic studies of the effect of experimental interventions by surgical scalpel, electrocautery and CO2-laser beam in the oral cavity].

Tissue effect interventions by means of surgical scalpel, elecrtokauter and CO2-laser ray in the mouth cavity of 20 white rats has been examined. According to their light microscopical examinations both the laser and the electrokauter caused thermal injuries taking place in typical zones while by the scalpel the cut surface in rendered ragged. The band-width of the thermoinjury caused by the kauter is a multiple of that caused by the laser. It has been proved by means of scanning electronmicroscopical examinations that interventions by means of laser result in sharp wound borders, the wounds cut by means of kauter are characterised by the presence of a great number of carbonized specks while by means of the scalpel a mechanical tear of the tissues is brought about. By means of electronmicroscopical examination the characteristics of the typical thermoinjured zones are described. It has been established that in the case of laser the injury of the ultra-structure extends to 400 microns while in case of electrokauter it reaches a width of 1500 microns. The excellent haemostatic effect brought about by the thermoeffect by means of the laser, in contrast to the broad thermoinjured zone caused by the kauter, is obtained at a very mild thermoinjury.

[Chronic gastritis and the presence of Helicobacter pylori in patients with non-ulcerative dyspepsia].

After definition in the group of the non ulcer dyspepsia (NUD) can be counted all those patients at whom beside the dyspeptical complaints, the radiological and endoscopical examinations didn't show ulcerative changes. The authors made biopsy 550 times on the occasion of 1390 gastroscopical examination (39.5% of the cases). The histological examination showed chronic gastritis in 372 cases (26.7% of all the examinations, 67.6% of the histological examinations). At this group of patients the dyspeptical complaints gave the principal indication of gastroscopical examination. Also it was examined the presence of dysplasia and intestinal metaplasia beside the different severity grade of chronic gastritis. The presence of Helicobacter pylori (Hp) was examined by histological methods. Hp positivity was noticed in 16.4% in the upper group. The authors made Hydrogen-breath examinations in 34 cases between patients with NUD. The results of Hydrogen-breath examinations also raise the multifactorial nature of the NUD. On the basis of examinations chronic gastritis and CP infection can form subdivisions in the heterogenic group of patients with NUD. For exacter judgement of Hp pathogenicity are needed further and wide-spread examinations. The authors would like to call the attention to the indispensability of the biopsy during the gastroscopical examination.

[Comparative study on wound healing in the oral cavity following experimental surgery using a scalpel, electrocauterization and CO2 laser beam].

In the mouth cavity of white rats lesion was created by means of surgical scalpel and electrokauter the course of the wound healing was observed on basis operations of various types (coagulation, vaporization, excision) as well as scalpel and electrokauter the course of the wound healing was observed on basis of histological examination of biopsies taken on the second, fifth, tenth, twentieth and fortieth day. It was verified that the wound healing is excellent after all three types of laser surgical intervention and is significantly more favourable than in case of electrokauter. The course of healing is finished during 3 weeks after coagulation and vaporaziation, and during 4 weeks after excision. The new epithelium layer became thinner than the original while the submucosa became twice as thick. The laser treated surfaces are protected by coagulation proteins, the wound edges are sharp, the amount of the cicatrice tissue formed after the wound healing is less than after the employment of the scalpel and the electrokauter.

[Increased numbers of large granular lymphocytes in the peripheral blood in selected inflammatory rheumatic diseases].

In six patients with selected inflammatory rheumatic diseases the authors found increased numbers of LGL in the peripheral blood. Concurrently in these patients an increase of lymphocytes with the CD57 sign was found. In one patient with rheumatoid arthritis more detailed analysis revealed a reduced number of lymphocytes with sign CD16. In this patient in the peripheral blood cells with signs CD3+ and CD57 were found and the function of NK cells was markedly reduced. The clinical course of the disease in the patients was severe, in particular in SLE, where in three patients the disease was associated with the development of lupus glomerulonephritis. The patients were treated by immunosuppressive therapy. The authors reflect on the causes of the increase of LGL cells and do not rule out the possible presence of a serious immunoregulatory disorder between T and B lymphocytes.

Immunohistochemical analysis of estrogen and progesterone receptors in benign breast diseases.

Surgical specimens of 62 patients with fibrocystic disease, 39 with fibroadenoma and 11 with intraductal papilloma were evaluated with ER-ICA and PgR-ICA methods. Positive nuclear staining was only observed in the epithelial components. The myoepithelium, myofibroblasts, fibroblasts and the so called apocrine epithelium, however, did not contain steroid receptors. Some steroid receptor negative cases occurred among the atrophic and non-proliferating fibrocystic diseases. In the proliferating benign alterations, particularly in those considered preneoplastic, e.g. radial scar, ductal and lobular atypical epithelioses, such as carcinoma lobulare in situ and florid intraductal papilloma there was a positive staining for estrogen receptor and progesterone receptor in the epithelial cells. Authors' results seem to serve as a theoretical basis for the hormonal influence of precarcinomatous cases and to explain the success of antisteroid therapy in patients with fibrocystic disease.

[Removal of kidney calculi in children by extracorporeal shock wave therapy].

ESWL treatment for renal calculi was used in 19 occasion in 16 children aged 6-15 years. 13 children were stone free 3 months after the treatment. All of the stones were located in the renal calices or in the renal pelvis. The possibilities of the procedure, as well as its effects were examined with the use of urography, ultrasound, isotop studies and magnetic resonance imaging.

Correlation between the birth prevalence of isolated hypospadias and parental subfertility.

Factors associated with the occurrence of isolated hypospadias have been studied. We previously reported a secular trend association between hypospadias occurrence and progestagen use by the Hungarian population. Further study does not support that secular trend association. The use of progestagens for the treatment of reproductive problems is a very complex issue, and there are other factors relating to male and female fertility that differentiate the hypospadias families from the controls. It appears that families with problems of subfertility are at increased risk for the occurrence of hypospadias. Our data support more recent studies, which do not demonstrate an association between the occurrence of hypospadias and the administration of progestagens in humans.

The state of the testicle and the epididymis associated with exstrophy of the bladder in undescended testes.

In undescended tests, the large number of testicular dysplasias/hypoplasias and of that of the fusion abnormalities of the epididymis, and the joint occurrence of the two, can achieve even 50%. The frequency of the above anomalies were studied in undescended testes associated with exstrophy of the bladder. Bilateral undescended testes were observed in 5 out of 26 boys born with vesical exstrophy. In 3 of them bilateral orchiopexy had already been performed. In 5 of the 6 operations intact testicles and intact epididymis were found. Fusion abnormality was observed only in one case, but the testicle was intact also in that case. This can be attributed to the fact that in exstrophy of the bladder, the testicles have failed to descend not due to the insufficient effect of fetal androgen, but due to anatomical causes. This is also supported by the fact that where the disorder of descension has been caused by mechanical factors, there was a much lower ratio of testicular-epididymal fusion abnormality.

Nephrolithotomy in childhood by extracorporeal shock-wave lithotripsy.

Extracorporeal shock-wave lithotripsy (ESWL) performed on 19 occasions in 6--15-year-old nephrolithic children is reported. The stones were present in the calyceal ends in the renal pelvis. The possibilities of treatment and its effects were examined by urography, ultrasound, camerarenography and magnetic resonance imaging procedures. After three months 13 children are free of stones.

Family study of natural killer cell activity in C1q-deficient patients with systemic lupus erythematosus-like syndrome: association between impaired natural killer cell function and C1q deficiency.

Impaired natural killer (NK) cell activity has been found in patients with systemic lupus erythematosus (SLE)-like syndrome. The mechanism by which NK cell function is impaired in SLE patients is not quite clear. We report here a family study of NK cell activity in C1q-deficient patients with SLE-like syndrome. In both SLE-active and SLE-inactive stages of the disease, NK cell function was significantly impaired when compared with the healthy controls (10.6 +/- 2.3% and 16.9 +/- 4.8% to 34.7 +/- 9.6%, p less than 0.025). On the other hand, differences in NK cell cytotoxicity between SLE-active and SLE-inactive members of the family were not statistically relevant (p less than 0.1). Further, we found no correlation between NK cell activity and clinical or laboratory values, except for a positive correlation between function of NK cells and C1q and CH50 values, respectively (rs = 0.93, 0.01 less than p less than 0.02). To our knowledge, this is the first report on a notable association between impaired NK cell activity and C1q deficiency. The type of inheritance of C1q deficiency in this family is also discussed.

Phenotypic variability of chemically induced primary rat mammary tumors.

Rat mammary tumors induced by DMBA (7,12-dimethylbenz(a)anthracene) or MNU (N-methyl-N-nitrosourea) were compared for frequency of histological types. Total tumor incidence in 50-day-old rats (Groups 3, 4, 5) was about 100% independently of the rat strain and carcinogen. There were found no distinct histological tumor types between DMBA and MNU carcinogenesis, although the distribution of fibroadenomas, adenocarcinomas and sarcomas varied markedly among rat groups. In 300-day-old female Wistar rats (Group 1) treated with DMBA, fibroadenomas and adenocarcinomas showed an incidence of 58% and 42% respectively. In 50-day-old rats (Group 3) the proportion of adenocarcinomas increased up to 72% of total DMBA tumors. MNU carcinogenesis induced adenocarcinomas in 98% of total tumors in the Lewis rat strain (Group 5), while only 53% in Wistar rats (Group 4). The rest of tumors were sarcomas occurring in opposite ratio to adenocarcinomas. The relatively high susceptibility of connective tissue to MNU as compared with mammary epithelium was due to the mode of MNU administration and seemed to be strain dependent. Both DMBA and MNU carcinogenic systems are valuable experimental models of mammary tumor. The cell phenotypes of the resulting tumors can be predicted with high probability by the choice of dose regimen of carcinogen and the route of its application.

[Experimental study of the direct effect of CO2 laser beam on the oral mucosa].

Interferences were made with a high-energy carbondioxide laser on the tongue and buccal mucosa of albino rats. Tissue destructions -- and cryodestruction reported in the literature -- were comparatively evaluated. At the edge of the laser cut a 30 micron carbonization zone occurred, beyond this a 150 micron necrotic zone and an 800 micron transition zone were observed. These were irreversibly damaged territories with coagulated blood vessels. The surrounding was hyperaemic and edematous. The application of higher energy brings about less tissue damage with narrower zones. A similar but more widely damaged destruction occurs after electrocauterization. Here, irreversibly damaged areas are 5 to 8-times wider than those found with laser. The largest damaged area was found after cryodestruction. It is thus concluded that the use of a high-energy carbondioxide laser results in the smallest irreversibly damaged tissue area. This explains the clinical advantages of the laser knife: coagulation during cutting, reduced postoperative pain and edema, small scar and good functional and aesthetic restitution.

Possible applications of the "Manuflex" external fixing instrument in small animal surgery.

An external fixing instrument (minifixateur externe) recently developed for use in humans and marketed by the name of "MANUFLEX", its accessories, possible applications and fields of indication are described. The apparatus has been patented in Hungary and an international patent has been applied for. In the second part of the paper the application of the external fixateur for treating bone fractures of dogs is reported, together with the practical experience gained with it so far. As the clinical trials gave favourable results and since the instrument is available for veterinary practitioners at a reasonable price, MANUFLEX may become a gap-filling apparatus in the instrumentarium of small animal surgery.

[Development of the dental arch and the palate from a prosthodontic point of view].

The replacement of missing teeth by a fixed restoration in the juvenile dentition presents various problems. One of them is that the growth and development of the entire masticatory organ continues until around the 18th year of age. It was the goal of a longitudinal study to find out when a fixed tooth replacement can be used in an adolescent without impairing the final development of the jaw bones. For this reason the transverse and sagittal dimensions of the dental arches of male adolescents aged 14 to 18 years were measured every year, and the palatal surfaces were measured in the first and last year of the study. The values determined for the dental arches remained constant. The mean values and deviations revealed no significant differences. From a prosthodontic point of view the development of the dental arches and the palate may be considered as concluded.
