[The pathological investigations on termination of early pregnancy with mifepristone].

133 samples of induced early abortions using mifepristone (RU486) in combination with misoprostol (Miso) were examined. There were no significant difference in the outcome and pathological changes between the two groups with varied RU486-Miso dosages. Another 133 samples of abortus (gestational age < or = 49 days) from vacuum aspiration were used as control. The villi of RU486-Miso group showed more extensive and severer degeneration than the villi of the control group under light and electron microscopy. However, whether RU486 affects the trophoblasts directly or in directly is yet in dispute. Further investigation is needed.

Integrins as markers of uterine receptivity in women with primary unexplained infertility.

OBJECTIVE: To assess uterine receptivity in women with unexplained infertility using integrin cell adhesion molecules as markers. DESIGN: Prospective, controlled study design. PATIENTS: Eighty-seven nulliparous women with unexplained infertility and 32 fertile and infertile parous controls. MAIN OUTCOME MEASURE: Immunohistochemical staining for alpha 1, alpha 4, and beta 3 integrin subunits in endometrial biopsies obtained during the window of implantation (days 20 to 24), using the semiquantitative HSCORE by two observers in a blinded fashion. RESULTS: All endometrial biopsies from parous controls contained positive immunostaining for the alpha 1, and beta 3 integrin subunits in glandular epithelium. Some samples from parous controls were missing the alpha 4 subunit. In contrast, compared with parous controls, biopsies from women with unexplained infertility had reduced significantly beta 3 expression, with similar expression of alpha 1 and alpha 4. Two distinct defects in integrin expression were identified: "out-of-phase" samples that lacked beta 3 because of histologic lag (type I defects) and "in-phase" endometrium that still failed to express this integrin (type II defects). These subclassifications accounted for 26% and 39% of the total unexplained infertility group, respectively. CONCLUSIONS: Abnormal endometrial integrin expression was a frequent finding in women with unexplained infertility. These data suggest that defective uterine receptivity may be an unrecognized cause of infertility in this population of women.

[Studies on biological indicator values in diagnosis for lead poisoning].

Blood lead levels, urine lead levels, indicators of porphyrin metabolism and other biological indicators were determined in 330 workers exposed to lead and 100 unexposed controls with national standardized methods for quality assurance. Data were analyzed with modern statistical methods in a microcomputer with SAS software. Criteria values for blood lead and urine lead were formulated with curve fitting and discriminant analysis, and local normal upper limit values were 1.43 and 0.25 mumol/L for blood lead and urine lead, respectively, acceptable upper limit values 1.93 and 0.35 mumol/L, respectively, and diagnostic values for poisoning 3.02 and 0.56 mumol/L, respectively. Correlation between workplace air lead concentrations and biological indicators, between blood lead levels and other indicators, and their specificity, sensitivity, accuracy and underdiagnosis and misdiagnosis were evaluated at the same time to provide a reference database for revising the national diagnostic criteria for lead poisoning.

[Sarcoma of vulva: a clinical retrospective analysis of 10 cases].

OBJECTIVE: To study the characteristics of vulval sarcoma and its relationship between treatment methods and prognostic factors. METHODS: 10 cases of vulval sarcoma were retrospectively reviewed in this paper. Prognostic factors such as tumor volume, grade and treatment methods were analysed. RESULTS: The age-incidence sarcoma of the vulva was 20 years younger than that of carcinoma of the vulva. The treatment of choice was operation combined with chemotherapy or radiotherapy. The effectiveness of chemotherapy or radiotherapy alone was not conclusive. CONCLUSION: Primary treatment with thorough surgery is very important. The prognosis is related with tumor volume, grade and treatment methods.

[Copper needle puncturing for the treatment of cavernous hemangioma].

From the beginning of 1990 to Oct. 1993, we have treated 31 patients with cavernous hemangioma using copper needle puncturing. Three months' to one and half years' follow-up showed positive effects in all cases. The paper describes general and local reaction of the treatment, the changes in blood copper concentration and echocardiography and complications.

[Correction of the secondary nasal deformity in bilateral cleft lip with a new technique].

Bilateral cleft lip often has nasal deformities of various degrees. It is not enough to elongate the nasal columella for such deformities. Based on the thoughts that secondary deformities of the nose and lip should be treated as a whole, the authors introduced a new technique, in which five aspects of the deformities were considered and treated at the same time. Thirteen patients have undergone surgical correction with satisfactory results.

[Late bladder complications following radiotherapy of carcinoma of the uterine cervix].

OBJECTIVE: To analyse the causes, clinical features, treatment and prevention of late bladder complications following radiotherapy of carcinoma of the uterine cervix. METHOD: This clinical retrospective study included 378 cases of irradiation cystitis following radiotherapy for carcinoma of the uterine cervix from February, 1958 to October, 1987. RESULTS: The incidence of irradiation cystitis was 3.2% and was not related to age, FIGO staging and the methods of intracavitary irradiation (conventional or afterloading). It was related to the external appearance of the local lesion; 66.5% of the tumors of patients with late bladder complications was of the exophytic type prior to radiotherapy. 51.1% of the patients developed late bladder complications in a period of 2-5 years after radiotherapy; 16.7% in 5-10 years and only 3.4% within the first years. The patients with irradiation cystitis had received a 10% higher dosage than that routine delivered, as found either in intracavitary irradiation of 39.4% of patients or in external irradiation of 31.6% of patients. Based on the clinical features and cystoscopic findings, late bladder complications can be divided into three types: (1) sudden and temporary hematuria (35.7%); (2) persistent hematuria (63.2%); and (3) vesico-vaginal fistula (1.1%). The 5-year survival rate of patients with irradiation cystitis was 90.4% which was higher than the survival rates of all patients treated by conventional intracavitary irradiation (65.7%) and all patients treated by afterloading intracavitary irradiation (70.4%). 3.7% of patients died of late bladder complications. CONCLUSIONS: To decrease the occurrence of late bladder complications and to obtain long term survival: (1) Dosage to the bladder should be adequate, and over dose to the bladder should by all means be avoided; (2) Patients are to be taught to observe indicated personal care during radiotherapy as well as in the follow-up period; (3) Early institution of proper conservative treatment of cystitis is beneficial.

[A study on the value of vitamin C in treating skeletal fluorosis].

Three hundred and sixteen patients diagnosed as skeletal fluorosis roentgenologically and having no other diseases were divided randomly into 2 groups, matchable in sex distribution, age, functional state and X-ray findings. In the treated group every patient took 200mg vitamin C twice a day, while in the control group placebo tablets. The duration of treatment was 3 months. The therapeutic effect was evaluated by measuring the joint angle and power of gripping and studying the X-ray film and electromyogram. 38 kinds of biochemical parameters were measured simultaneously with a MCA-3 autobiochemical analyser. The results were as follows, (1) The total effective rate in the treated group was 12.84%, chi 2 = 0.556, P > 0.05, so there is no obvious effect in treating skeletal fluorosis with vitamin C. (2) The electromyogram in the treated group showed a recovering rate of 20.00%, chi 2 = 5.9, P > 0.01. It is further shown that there is no effect in treating skeletal fluorosis with vitamin C. (3) 38 kinds of biochemical parameters measured simultaneously and the X-ray findings also showed that vitamin C is not effective. Thus the validity of using vitamin C as the drug of first choice to treat skeletal fluorosis is to be queried.

[A stereologic study on the influence of stent tube pressure on bilio-intestinal anastomotic scar formation].

Biliary tract injury results often in biliary scar stricture causing obstructive jaundice and biliary cirrhosis. Choledocho-jejunostomy with tube stent is the routine to prevent anastomotic stenosis, but how long the stent tube should be in place is controversial and little is known on the influence of the stent tube on anastomotic scar formation. In canine experimental models, bili-intestinal scar was sampled 3 weeks, 3, 6, 9 and 12 months after anastomosis for ultrastructure stereologic image analysis. It was found that the internal pressure of the stent tube exerting on anastomotic stoma inhibits scar formation, and the stent tube should be remained inside the stoma for not less than 9 months.

[Correction of unilateral cleft lip nasal deformity with modified Bardach's technique].

Twenty-nine cases of unilateral cleft lip nasal deformities were corrected with modified Bardach's technique. Overall results were satisfactory. The key to a successful and stable correction of the nasal deformity is to lengthen the columella on the cleft side and to mobilize alar cartilage from its surrounding tissue, creating a symmetric shape and length. The major advantages of the technique are lengthening of the cleft columella and creation of a symmetric and well-projected nasal tip. The surgical technique is described in details. Some considerations before and during operation were discussed.

[Ultracytochemical observation of secretory cells and surfactant-like lamellar bodies in mucosa of eustachian tube].

The distribution of secretory cells and surfactant-like lamellar bodies in mucosa of the guinea pig eustachian tube were studied ultracytochemically. Classified by means of their morphologic characteristics, three types of secretory cells were identified. The dark granulated cells were predominant in the tympanic orifice, the mixed cells were predominant in the isthmus portion, while the light cells were predominant in the pharyngeal orifice. The distribution of the secretory cells in different part of the eustachian tube might play a role in the pathogenesis of middle ear effusions. Surfactant-like lamellar bodies were found in ciliated cells, nonciliated cells and secretory cells of the eustachian tube mucosa. Surfactant-like lamellar bodies (surfactant-like substance) are important in maintaining the physiological function of the eustachian tube, their behavior in normal and pathological states should be further studied.

[Primary malignant lymphoma of the female genital tract clinical analysis of 15 cases].

OBJECTIVE: To review fifteen cases with primary malignant lymphoma of the female genital tract at Cancer Hospital, Chinese Academy of Medical Sciences from 1980 approximately 1994. METHODS: This is a clinical retrospective analysis. There were 7 cases of lymphoma originating in the cervix, 2 in the vagina, 3 in the ovary, 1 in the vulva, endometrium and pelvic cavity respectively. All of the 15 cases were given combination therapy. 3 ovarian lymphoma were treated by surgery and chemotherapy, 2 cervical lymphoma and 1 endometrial lymphoma by surgery, chemotherapy and radiotherapy, 1 pelvic lymphoma by surgery and radiotherapy, others by radiotherapy and chemotherapy. RESULTS: Two patients are living more than 5 years, 3 patients more than 3 years. 4 patients were followed up for less than 2 years and 6 patients relapsed or died. CONCLUSIONS: The prognosis of ovarian lymphoma is worse than that of other genital sites. Combined and sequential chemotherapy-radiotherapy-chemotherapy is the proper treatment sequence for lymphoma of the cervix, vagina and vulva. Extensive chemotherapy may be an appropriate choice for highly malignant pathological type. Radiation should be given early for large cervical lymphoma.

[Study of primary organotypic culture of the chicken auditory sensory epithelia].

Organotypic explants of auditory sensory epithelia-basilar papilla (BP) were prepared from 8-16 days postnatal chicken and cultured in vitro for up to 7 days using primary culture technique. The survival status and histological features of the BP were studied. The results showed that morphologic integrity of hair cells and supporting cells was well maintained in the duration of culture and was comparable to those in vivo. The paradigm anchorage-bottom with bFGF/collagen as a substrate provided the best result, whereas the free-floating method was optimal for a short period of culture within 48 hours. This could be a valuable experimental means for further research of the auditory hair cells in vitro.

[Scanning electron microscopic observation of human vestibular sensory epithelia].

Three-dimensional structures of human vestibular sensory epithelia were studied under scanning electron microscope. The saccular macula showed an "L" pattern and increased in thickness near the striola. The utricular macula appeared like an open shell. Flask and test-tube shaped hair cells were identified as type-I and type-II sensory cells respectively. Stereocilia were arranged in a step like progression; the kinocilium was near the longest stereocilium and looked more flexible than stereocilia. The tallest stereocilia and kinocilia faced away from the striola toward the periphery in the macula sacculi, and toward each other along the striola in the utricular maculae. Polarization directed to the utricle in the lateral crista, and away from the utricle in the anterior and posterior cristae. Otoconia varied in size and shaped like crystals with pyramidal ends.
