[Importance of the proliferating trichilemmal cyst].

The biological behavior of proliferating trichilemmal cysts is commented on, based on two cases. Two female patients, 71 and 70 years of age, developed solitary monstrous tumours of the scalp in the course of repeatedly recurring trichilemmal cysts. The biological behaviour of both was marked by infiltration of the galea aponeurotica, and in the latter case, invasion of the calvaria and the venous sinuses of the brain with a lethal outcome. Histologically, the differentiation of the cyst walls ranged from pseudoepitheliomatous hyperplasia to areas largely lacking differentiation, indicating a trichilemmal carcinoma. Therefore, a more cautious evaluation of proliferating trichilemmal cysts, commonly underestimated as pseudocarcinomatous, is indicated. The second case in particular shows that on occasion an invasive carcinoma can arise in a trichilemmal cyst. Radical surgery can not only prevent recurrence with loss of differentiation, but also enable a better histological evaluation of the deep-lying tumour tissue.

[ Pilar sheath acanthoma - a benign follicular hamartoma].

A new case of pilar sheath acanthoma is reported. This tumor has been identified in 1978 and shows characteristic histological features: a central sinus with keratinous material, a pore-like opening to the surface, keratinization of the epidermoid type and projecting epithelial tumor lobes containing small keratin cysts resembling incomplete pilar differentiation.

[Thyroiditis from the surgical viewpoint].

1296 Patients were treated for thyroid disorders in the Surgical Clinic of the University of Graz between 1975 and 1981. In 3.5% thyroiditis was diagnosed, 75% struma lymphomatosa Hashimoto and 25% subacute thyroiditis de Quervain. 75% of all cases were classified only by histological postoperative examination. Suspected malignancy leads in most cases to operative treatment, but there are also indications because of the inflammatory lesion. We do not believe that thyroiditis should be treated surgically as thyroid carcinoma, neither provides the low incidence between carcinoma and thyroid inflammation any radicality for preventing malignant disorders.

[Corresponding behaviour of the epithelium and stroma of the cervix uteri].

Different types of epithelium in the human cervix are regularly found within circumscribed areas and obviously respect predetermined borderlines. The determination theory may explain these findings and could in consequence lead to the hypotheses of stromal regulation of the epithelium. Corresponding morphological changes of epithelium and associated stroma are evaluated as another relevant aspect. More or less obvious borderlines between two morphologically different environments are found in the stroma underlying epithelium with distinct borders. Such stroma changes were seen to be correlated to benign and neoplastic alterations of both squamous and cylindrical epithelium.

[Vitamin A and carotene in thyroid diseases].

From 190 goitrous patients (106 euthyroid, 53 hyperthyroid, 31 hypothyroid) serum levels of vitamin A and carotene were obtained. The serum levels of vitamin A were significantly decreased in both hyperthyroidism and hypothyroidism, the serum levels of carotene in hypothyroidism only. Remarkably, vitamin A levels almost never drop to subnormal values in hyperthyroidism. There is evidence, that a sufficient dietary protein supply enables the liver cell to produce enough amounts of retinol binding protein and prealbumin to overcome the increased clearance observed in hyperthyroid conditions.

Auriculotemporal (Frey) syndrome in late childhood: an unusual variant presenting as gustatory flushing mimicking food allergy.

Auriculotemporal or Frey syndrome is characterized mainly by recurrent episodes of facial gustatory flushing and/or sweating, limited to the cutaneous distribution of the auriculotemporal nerve. Although relatively common in adults following injury to the auriculotemporal nerve or parotid disease, the condition has rarely been reported in children. Moreover, in childhood, auriculotemporal syndrome has been described mainly in infancy and early childhood as a sequel of perinatal birth trauma resulting from assisted forceps delivery. We report a 13-year-old girl with a 2-month history of recurrent, painless, preauricular gustatory flushing without sweating, initially suspected to be a food allergy. Detailed inquiry revealed a history of a bicycle accident with mandibular condyle fracture 7 years prior to the onset of symptoms. Our patient demonstrates an unusual presentation of auriculotemporal syndrome in late childhood as gustatory flushing mimicking food allergy. Awareness of this variant is essential for prompt recognition, thus avoiding unnecessary laboratory tests, especially as this condition usually resolves spontaneously.

Ocular melanoma: epidemiology, clinical presentation and relationship with dysplastic nevi.

PURPOSE: Ocular melanoma is a rare entity compared to cutaneous malignant melanoma. We examined the frequency of the tumor in a defined geographic region, its clinical presentation and its relationship with dysplastic nevi in 136 patients. METHODS: 136 patients (64 men and 72 women; mean age 61.7 years, range 20-92 years) with ocular melanoma were treated at the University Hospital of Graz between June 1996 and December 2001. 129 had primary uveal melanoma in one eye (117 choroidal melanomas, 11 melanomas of the ciliary body and 1 of the iris), 2 patients had uveal melanoma in both eyes, 4 patients had conjunctival melanoma and 1 patient had a melanoma of the lacrimal sac. Epidemiology, history, potential risk factors, clinical presentation and relationship with dysplastic (= atypical) nevi were documented. RESULTS: 48 patients (35.3%) showed more than five dysplastic nevi, compared to only 1.2% in the general population (chi(2) test: p < 0.001). 5 (3.7%) had additional cutaneous melanoma and 7 (5.1%) had a family history of melanoma. The lifelong risk for the occurrence of an additional primary cutaneous melanoma was 2.9%, which is significantly higher than the usual estimate of 1% for the general population. CONCLUSIONS: Patients with primary ocular melanoma have an increased risk to develop cutaneous melanoma and should therefore be examined regularly by dermatologists.

[Histological investigation of the micromorphological effects of the application of a laser needle--results of an animal experiment].

In an experimental animal study (Sus scrofa domesticus) we investigated the effects of the new technique of laser needle stimulation (wavelength: 685 nm; energy density: 4.6 kJ/cm2 per point; application duration: 20 min). The results revealed changes in microcirculatory parameters of the skin resulting in an increase in blood flow. However, the quality and intensity of the laser light did not induce micromorphological alterations in the skin.

Langerhans cells in epithelial skin tumors. A quantitative immunohistological and morphometric investigation.

In the present study we investigated whether there is a correlation between Langerhans cell density, peritumoral infiltrate or intraepithelial T-lymphocyte density within 17 epithelial skin tumors using immunohistologic and morphometric methods. We found a significant difference between seborrheic keratosis, basal cell carcinoma, and squamous cell carcinoma. No correlation was found between the Langerhans cell density and the number of intraepithelial T lymphocytes or between Langerhans cell density and the peritumoral infiltrate. A significant (inverse) correlation was found between mean nuclear area of the epithelial tissue and the number of Langerhans cells. These data might indicate that the number of Langerhans cells does not influence the extent of the anti-tumoral immune response, but that there is an association between the differentiation state of the epithelium and Langerhans cell density.

Hexadecylphosphocholine inhibits invasion of mouse T-cell lymphoma cells in two different invasion assays.

Hexadecylphosphocholine (HePC), an ether lipid analogue, is a new antineoplastic drug which has been shown to exert a remarkable antiproliferative effect in vitro and in vivo. The signal transduction pathway and the phospholipid synthesis are thought to be the main putative molecular targets of HePC, yet the exact mechanism of action is still unclear. To investigate the antiinvasive activity of HePC on a mouse T-cell lymphoma cell line (BW-O-Li1), we used a type I collagen gel and devitalized dermis as substrate to evaluate the migration of BW-O-Li1 after exposure to HePC. BW-O-Li1 cells were exposed for 24 h to a non-cytotoxic (10 microM) as well as to cytotoxic concentrations of HePC. Afterwards, BW-O-Li1 cells were seeded on top of a reconstituted collagen gel layer or pippeted into a steel ring placed on the dermal site of a devitalized dermis. Lymphoma cells, which invaded the collagen layer were counted by light microscopy, invasion into devitalized dermis was measured by an image analysis system. Compared to unexposed cells, invasion into the collagen gel differed significantly even at 10 microM HePC, whereas the absolute number of invading cells, independently of the HePC concentration, showed no difference in the amount of counted cells. Migration into devitalized dermis was significantly reduced for 10 microM and 40 microM HePC. These data show that complementary information can be obtained by application of the two invasion assays and that the antiinvasive effect of HePC emerges at non-cytotoxic concentrations of the substance.
