[Prostatic specific antigen in the serum in prostatic cancer].

Prostatic specific antigen (PA) level was determined with a Wako test kit (Japan) for prostatic cancer and others. The incidence of abnormal values of PA in untreated prostatic cancer, was 50, 50, 80, and 100% for stage A1, C (pN0, NX), D1 and D2 cancers, respectively. Grade was not related to the level of PA. Prostatic hypertrophy, prostatitis and urinary stone showed a false positive rate of 52, 18 and 0%, respectively. The level of PA was not correlated to those of prostatic acid phosphatase (RIA). In 31% of the cases, the elevated PA decreased 4 weeks after start of endocrine treatment. Elevated PA in low grade cancer was not normalized as much as that in high and moderate grade cancers. The positive rate of PA in the serum of reactivated patients was significantly higher than that of the patients with cancer under good control by endocrine treatment.

[A case of traumatic renal artery thrombosis].

A case of traumatic renal artery thrombosis is reported. The patient is a 14-year-old girl with the chief complaint of hypertension. She was admitted to our hospital about a month after a traffic accident. An IVP demonstrated nonvisualization of the right kidney and complete occlusion of the right renal artery was revealed by aortography. Right nephrectomy was performed, because atrophy of the right kidney was marked. A week after the operation, her blood pressure became normal. We present our case and review the literature.

[A case of renovascular hypertension due to chondrosarcoma arising from lumbar vertebrae].

A 35-year-old man visited our clinic with the chief complaint of headache and an upper abdominal mass. Renal vein renin activity, drip infusion pyelogram, GT scan and selective renal angiogram demonstrated a retroperitoneal tumor and right renovascular hypertension. Thus tumor extirpation and right nephrectomy were performed. After nephrectomy, blood pressure was corrected within the normal range. Histologically the tumor was diagnosed to be chondrosarcoma and there was no lesion in the wall of the right renal artery. This case had secondary chondrosarcoma arising from the first and second lumbar vertebrae and produced renovascular hypertension due to compression of the tumor.

[Current status of male contraceptive operations--questionnaire at vasectomy].

To investigate the current status of male contraceptive operations, the applicants for vasectomy were surveyed. The average age of the husbands was 37.3 years and that of the wives was 33.4 years. The number of children they had was 2.4 on the average. The duration of marriage ranged from 3 to 37 years and the average was 10.9 years. Fifty-eight percent of the patients visited our clinic of their own motive. Few of them had sought advice of a specialist before. Twenty-two percent of the wives had had an experience of natural abortion, whereas 58% of them had had artificial abortions. Methods of contraception which have been taken were condoms for 47 couples, Ogino's method (rhythm method) for 12, IUD for 11, pills for 6 and so on. Eight couples had not practiced contraception before. Forty-nine percent of the husbands were office workers, which was much higher than the rate for the whole working population in Japan.

Proliferation of Dunning R-3327 rat prostatic adenocarcinoma and of its sublines (CUA and CUB).

Proliferating cells in tumors of the androgen-dependent R-3327 and its partially dependent and autonomous sublines, CUA and CUB, were examined. Two types of cells, myoepithelial cells and large stromal cells, in the R-3327 have a high proliferating ability, as estimated by thymidine uptake. These cells were also observed in the normal dorsolateral prostate of rats, the lobes from which R-3327 originated. CUA is a squamous carcinoma, which proliferates at the border of the epithelial layer and large stromal cells. The tumor cells of CUB appear as clusters of large stromal cells which have a strong potential for dividing. Therefore, R-3327 contains two types of dividing cells, and their uncoordinated proliferation may be responsible for causing the different features seen in the sublines.

Changes in the volume and histology of retractile testes in prepubertal boys.

Testicular development was studied in prepubertal boys with retractile testes. Testicular volume, the diameter of the seminiferous tubules and the number of spermatogonia in the tubules were decreased in cases of unilateral retractile testis, when compared with values for the contralateral normally descended testis. On the other hand, in patients with a unilateral retractile testis and contralateral inguinal testis, there was no difference in the developmental parameters between the two testes. These results suggest that the retractile testis has developmental failures characteristic of a cryptorchid testis and therefore requires orchiopexy.

A case of 48,XXYY Klinefelter's syndrome with incurable skin ulcer.

A 40-year-old male was referred to the Chiba University Hospital for hypogonadism. He showed many of the phenotypic features of Klinefelter's syndrome. Cytogenetic studies revealed a 48,XXYY karyotype. The significance of the association of this karyotype with incurable skin ulcer is discussed.

Histochemical study of R1881-binding protein, prostatic acid phosphatase, prostate-specific antigen, and r-seminoprotein in prostatic cancer.

A histochemical study on R1881-binding protein, prostatic acid phosphatase (PAP), prostate-specific antigen (PA) and r-seminoprotein was conducted. These parameters and histologic grades were compared to each other and with the responsiveness to endocrine therapy. A good correlation was found between histologic grade and presence or absence of R1881-binding protein in the tissue. Presence or absence of R1881-binding protein as well as histologic grade correlated well with responsiveness to endocrine therapy. Positive staining patterns of PAP, PA, and r-seminoprotein in the tissues were similar to each other, particularly between PA and r-seminoprotein. However, no correlation was found between the histologic grade and ratios of PAP, PA or r-seminoprotein-positive cells. Nor was any correlation seen between the ratio of these marker-positive cells and responsiveness to endocrine therapy.

Effect of androgen on ornithine decarboxylase activity in androgen-dependent mouse mammary tumor (Shionogi Carcinoma 115) and its androgen-independent subline (CS 2).

The activity of ornithine decarboxylase in androgen-dependent mouse mammary tumor (Shionogi Carcinoma 115) was reduced to 25% by castration of tumor-bearing mice and restored to the normal level 12 h after administration of testosterone or 5 alpha-dihydrotestosterone. Administration of estradiol-17 beta to the tumor-bearing castrated mice also stimulated the enzyme activity while progesterone and cortisol had little effect. On the other hand, the enzyme activity was affected by neither castration nor androgen injection to CS 2, which is a subline of SC 115 and completely independent of androgen for growth. The inhibition of ornithine decarboxylase activity in SC 115 by injecting alpha-difluoromethylornithine did not affect the enhancement of RNA polymerase I activity by androgen, showing independent elevation of the levels of the two enzymes by androgen.

Properties of progestin-binding protein in benign hypertrophic human prostate.

RU 27987 is a new ligand for progesterone receptor and binds in high affinity to nuclei of target tissues of progesterone. Using this compound, progestin-binding components in the benign hypertrophic human prostate were studied, and compared with those examined with R 5020, a conventional ligand, in the study of progesterone receptor. In cytosols, the binding affinity of RU 27987 was higher than that of R 5020, and the number of maximum binding sites for RU 27987 seemed to be large but correlated well with those of R 5020. The binder for RU 27987 sedimented at 8.6 S, and the binding was specific to progestational steroids, indicating that binding properties of this binder in the cytosols are identical to those for R 5020. Although there was no binding with R 5020 in the nuclear extract, a small amount of specific binding with RU 27987 was detected. However, the cytosol bound with RU 27987 was not retained in DNA Sepharose and no specific binder for RU 27987 in the nuclear extract was observed in a sucrose density gradient centrifugation. From these observations, it was assumed that the nuclear binding observed was attributable to contamination of the cytosolic binder. The results obtained in the present study suggest that the progestin-binding component in the benign prostatic hypertrophy is not the progesterone receptor but a high affinity binder for progestins whose physiological role is not clear at present.
