Recombinant human granulocyte-macrophage colony stimulating factor (rh GM-CSF) after bone marrow transplantation.

Bone marrow transplantation improves the chances of survival in a variety of hematological malignancies. However, infectious complications during the post-transplant phase contribute significantly to morbidity and mortality. To reduce the duration of granulocytopenia, which is approximately 20 days after BMT, in this study patients with ALL, relapsed or high-grade NHL, relapsed or refractory HD, or Neuroblastoma stage III/IV, were given rh GM-CSF to assess the effects on hematological and immunological reconstitution after conditioning therapy and BMT. The results of 9 patients are presented. After autologous BMT and subsequent rh GM-CSF therapy, a peripheral blood neutrophil count of 500/microliters was reached within 8-12 days, i.e., between 7 and 10 days earlier than would have been expected without rh GM-CSF. Furthermore, it appeared that rh GM-CSF was useful in case of insufficient bone marrow regeneration post autologous transplant. The influence of rh GM-CSF after allogeneic BMT is not yet clear. Further studies will be necessary to evaluate the potential of this promising new drug after BMT.

Phagocytic function of polymorphonuclear leukocytes and the RES in endotoxemia.

The reticuloendothelial system (RES) and the polymorphonuclear leukocytes (PMNs) are thought to play a major role in defense against sepsis. Disturbances in the function of these two phagocytic systems during a septic event is associated with the development of lung capillary injury. Endotoxemia is said to lead to similar changes. Our study examined the function of the RES and PMNs after bolus injection of endotoxin (2 micrograms/kg BW) in a standardized sheep model. For up to 24 hr after endotoxin, blood samples were drawn and PMN function was followed by chemiluminescence, chemotaxis, and adherence as well as the phagocytosis and killing of bacteria. RES function was determined by the blood clearance of a labeled Tc99 colloid. We found an increase of RES clearance directly after endotoxin. Chemotaxis, phagocytosis, and killing were reduced. Adherence was increased. Chemiluminescence peak maximum (CLPM), representing the metabolic activity of the PMNs, was initially increased but shortly thereafter showed a significant decline (at 1 hr: 0.52 +/- 0.13 X 10(6) cpm with P less than 0.05 compared to baseline). The chemiluminescence peak time (CLPT), a measure of membrane receptor function, was significantly reduced (10.0 +/- 2.2 min with P less than 0.001 compared to baseline). Endotoxin led to a reduction of intracellular PMN functions (phagocytosis, killing, CLPM) within 1 hr. Membrane localized functions (adherence, CLPT) were increased. The changes in PMN function might be the reason for the development of lung capillary injury, in spite of undisturbed RES clearance.

Inhibitory effect of tetrachloro-p-hydroquinone and other metabolites of hexachlorobenzene on hepatic uroporphyrinogen decarboxylase activity with reference to the role of glutathione.

Exposure of rats to HCB caused a dose-dependent depletion of GSH. Chlorophenolic and sulfur-containing metabolites of HCB incubated with GSH-free rat liver cytosolic protein drastically diminished the UROD activity. In addition, HCB also exhibited inhibitory potency. The most effective compounds studied were TCH and its oxidation product, chloranil. Incubation of liver cytosolic protein and of GSH with HCB and its metabolites yielded results that suggested interaction between the compounds and cell constituents--an interaction that may cause inhibition of the hepatic UROD activity in the HCB-exposed organism.

Influence of beta adrenergic blockers and their combinations with anticholinergic drugs on epileptiform activity in rabbit hippocampus.

The influence of alpha- and beta-receptor antagonists combined with drugs which act as antagonists at muscarinic and nicotinic receptor sites on epileptic focus produced by topic penicillin application was investigated. The beta-receptor blocking agent caused a faster and more intense activity of epileptogenic focus in comparison with activity in control animals, but the number of seizures decreased relatively for 25%. N-receptor antagonists diminished the facilitating effect of the beta-receptor antagonist on focal activity. Combination of m- and beta-receptor antagonists did not influence the spike activity of the focus. Treatment with alpha-receptor antagonists enhanced spikes and number of seizures only insignificantly. The combination of alpha- and n-receptor blocking substances does not influence the development of the epileptic process. The alpha-receptor antagonist together with the m-receptor blocking drug induces a facilitation of spike activity.

Elimination of porphyrins and their precursors in rats pretreated with hexabromobenzene.

The elimination times of porphyrins and their precursors and of hexabromobenzene (HBB) itself were studied in female rats given 15 mg HBB by stomach tube every other day for 4 months. The concentrations of HBB in the blood, liver and adipose tissue were in the ratio 1:1.5:25, 24 hr after the last dose. Two weeks after the end of treatment, HBB was no longer detectable in the tissues. In animals given a single oral dose of 16.6 mg HBB/kg body weight, HBB was no longer detectable in adipose tissue 12 days after dosing. The half-life of HBB in adipose tissue was about 2.5 days in the animals given HBB for 4 months, and at the end of the treatment the concentrations of porphyrin in the liver, urine and faeces were increased to about 1000, 600-700 and 60-70 times the control values. The amounts of delta-aminolaevulinic acid and porphobilinogen in the urine of treated animals were 6-7 times those in controls. After the end of HBB treatment, it took almost 1.5 yr for delta-aminolaevulinic acid and porphobilinogen excretion to return to normal. Nearly 2 yr were needed for complete elimination of the accumulated liver porphyrins.

The heterogeneity of the nucleus raphes dorsalis in albino rats. A fluorescence histochemical and microelectrophoretic study.

In albino rats, the nucleus raphes dorsalis (NRD) of the midbrain was investigated using a sensitive fluorochroming procedure to detect indolamines in combination with a microelectrophoretic method to demonstrate the isoenzyme pattern of acetylcholinesterase (AChE). Based on the fluorescence histochemical appearance, 3 types of indolaminergic cells were identified in the various areas of the NRD. According to data of the literature and in agreement with our own results, the morphological heterogeneity of the NRD was shown. Significant differences of AChE specific zymograms of NRD regions (subgroups) demonstrate that organization of NRD is heterogeneously proved not only by morphological results but also in respect of enzyme constitution.

Multiple nodules on the tongue of a patient with seizures.

Systemic cysticercosis with cerebral involvement is a serious disease that may be encountered with greater frequency in the southwestern United States because of the proximity to areas of endemic disease. Because oral lesions can occur, the dentist and dental specialists, by recognizing and performing biopsies of lesions in patients suspected of having the disease, can provide a valuable service.

Amebic infections in asymptomatic homosexual men, lack of evidence of invasive disease.

A survey for enteric infections in 140 asymptomatic homosexual men who attended a community clinic revealed a high prevalence of infection with Entamoeba histolytica (27.1 per cent) and Giardia lamblia (15.7 per cent). In contrast, the prevalence of elevated indirect hemagglutination (IHA) titers (greater than or equal to 1:128), which indicate invasive amebiasis, was low (5.7 per cent). Our findings suggest that only a limited amount of invasive amebic disease is occurring in this group of homosexual men.
