[EEG-slowing after high-dose Methotrexate with citrovorumfactor-rescue. A spectralanalytic study (author's transl)].

In 23 treatment courses of high-dose Methotrexate (MTX) with Leukovorin-rescue for osteogenic sarcoma serial EEG-examinations before and after MTX-infusion were done. In 11 course Vincristine had been administered additionally. Frequency-analysis of EEG-background activity was performed by computer-based FFT. At the end of the MTX-infusion EEGs were unchanged, compared with the findings before start of treatment. At 24 and 48 hours after start of infusion, there was a slight but statistically significant slowing, recognizable from an increase of the theta/alpha-ratio (median + 48%, range -7% to + 373%) and a drop of dominant frequency (median-8%, range +12% to -53%). There was a stronger trend towards normalisation in patients, not having received Vincristine. The greatest changes were found in a patient, whose serum-MTX-concentration at 24 hours exceeded the upper therapeutic limit. The EEG-findings are discussed as an equivalent of a subclinical MTX-encephalopathy. Acute encephalopathies with severe EEG-changes and morphological changes on CT have been observed in MTX-intoxications.

[Micro-invasive cancer of the cervix. The diagnosis and indications for treatment (author's transl)].

The histological criteria for diagnosing micro-invasive carcinoma of the cervix and for deciding the treatment which should theoretically follow these criteria have been reviewed from the literature. To make a diagnosis of micro-invasive carcinoma it is necessary to have an adequate biopsy specimen and at least a cone that has been taken to include healthy material. There is a need to have a classification that can easily be used by pathologists and the following different stages of early epithelioma are therefore proposed: widespread, spreading-incipient, early micro-invasive and an established micro-invasive. The risk of metastasis, although it is slight, should only be considered in the established micro-invasive stage. This may make extended surgical treatment justified.

Acute monocytic leukemia complicating combined-modality therapy for localized childhood Ewing's sarcoma.

Rapidly fatal acute monocytic leukemia occurred in an 11-year-old by 33 months after the beginning of irradiation and chemotherapy for non-metastatic pelvic Ewing's sarcoma. At autopsy, no recurrent primary disease was seen. An analysis of this case together with a review of the literature suggests therapy-related leukemogenesis. Thus, the decline in mortality rate for childhood cancer may be accompanied by an increased incidence of second neoplasms in cured children having the potential of a normal life span.

Some new aspects on damages in the organ of Corti after pure tone exposure.

(1) The loss of hair cells after pure tone exposure at the critical level of intensity shows an exact correlation to the frequency, the time of sound exposure, and time of recovery as regards the quality and quantity. (2) The total extent of the damage of sensory cells has to be estimated higher than the hair-cell loss as observed under the light microscope. Although an improvement of the function of the organ of Corti was determined electrophysiologically from the 5th to the 10th day of recovery, the hair-cell degeneration was progredient during this period. (3) The hair-cell damage is apically more strongly marked from the area of damage than basically. (4) Depending on the experimental conditions there are considerable degenerative alterations in the nerve fibres and in the connective apparatus of the organ of Corti besides hair-cell losses and damages.

[Acute leukemia in a 13-year old boy with a synchronous occurrence of lympho- and monoblasts (author's transl)].

The synchronous occurrence of blasts of the lymphatic and myelogenous lineage in acute leukemia has been rarely observed. Reviewing individual case reports it is suggested that malignant transformation already starts at the level of the stem cell. We are reporting a case of a 13-year old boy who initially showed morphological features of acute lymphatic leukemia. Following 14 days of therapy the assumption of a pure lymphatic lineage has to be changed in favour for monoblasts which are now occurring.

[Implications of terminal deoxynucleotidyl transferase activity in various forms of childhood leukemia (author's transl)].

Terminal deoxynucleotidyl transferase (TdT) was examined in mononuclear peripheral blood cells (pB) and bone marrow (BM) specimens of 63 children with acute leukemia (AL). The enzyme activity in normal specimens (pB, BM) was below 0.2 U/10(8) cells; whereas, 49 of the 52 children with acute lymphoblastic leukemia (ALL) at diagnosis showed an activity in the range 1.2--60 U/10(8) cells. 2 of the remaining 3, devoid of TdT activity, were found to be B-cell leukemia. Patients with acute non-lymphoblastic leukemia (ANLL) were generally TdT-negative. Elevated level of TdT activity was detected in only one of 11 children with ANLL. In one patient with acute leukemia two distinct populations of cells with lymphoblastic (87%) and myeloblastic characteristics were evident. The clinical course and cell marker studies were consistent with the interpretation of a defect at the level of the common stem cell giving rise to a TdT-positive lymphoblastic cell population at diagnosis and, following the initial ALL-therapy (4 weeks), a predominant TdT-negative myeloblastic population. TdT as a marker for the modulation of chemotherapy was examined in the remission phase of the disease. Of the nine patients in the first 3 months of remission, one was found to have elevated level of TdT activity (2.5 U/10(8) cells). These data define the usefulness of TdT in the classification of acute leukemia.

[Trophoblastic pseudotumour of the uterus (author's transl)].

When perforation of the uterus occurred while a therapeutic termination of pregnancy was being carried out in a 25-year-old woman it was shown that the monomorphic trophoblastic pseudotumour was invading the myometrium. It was not secreting HCG when it was diagnosed and it had not recurred 19 months after the diagnosis and hysterectomy.

[Clinical and biological studies of an hybrid S/Stanleyville II hemoglobin (alpha 2 78 Asn replaced by Lys beta 2 6 Glu replaced by Val) (author's transl)].

A report of a doubly heterozygous case for both haemoglobins: Hb Stanleyville II (alpha 78 replaced by Lys) and Hb S (beta 6 Glu replaced by Val) with Hb hybrid S/St II. A 29-year-old woman from Zaïre was found to have four haemoglobins: Hb A, Hb Stanleyville II, Hb S and Hb hybrid S/St II. The clinical and hematological effects of this combination were similar to those of heterozygous sickle cell anaemia. Substitution of Lysin for Asparagin at residue alpha 78 reduced the tendency to polymerization and increased mechanical stability. These findings demonstrated involvement of this site in intermolecular interactions and explained the moderate severity of the sickle cell anaemia syndrome.

[The occurrence of a toxigenic strain of Fusarium tricinctum in connection with disease in a dairy cow].

The fungus Fusarium tricinctum, producing T-2 toxin, was isolated from mouldy maize silage. T-2 toxin was identified by TLC and by a bioassay. Consumption of mouldy silage resulted in a disease in a dairy cow, which had to be killed. The patho-anatomical findings obtained in the slaughtered cow generally suggested trichothecene poisoning. It was derived from these findings that the disease was due to the presence of trichothecenes in the feed.

[Screening for diabetes during pregnancy (author's transl)].

Screening for diabetes, or more commonly the study of maternal glucose tolerance, a routine procedure during pregnancy. The first step consists of assessing the diabetic risk for the patient. Screening tests should be applied early if risk factors for diabetes are present, and/or at a later stage in all pregnant women. The initial test should always be provoked hyperglycemia by the intravenous route. According to the value of the glucose assimilation coefficient K, the test will be repeated using the oral route. Screening can only be effective if applied to all pregnant women.

[Acute myelogenous leukemia 11 years after successful treatment of Hodgkin's disease. A contribution to the problem of secondary tumors].

Acute myelogenous leukemia 11 years after successful treatment of Hodgkin's disease: A contribution to the problem of second malignancies. The occurence of a therapy resistant acute myelogenous leukemia 11 years after successful treatment (operation, radiotherapy, polychemotherapy) of Hodgkin's disease is described. While this second malignancy was rarely seen in the era of minimal or no therapy of Hodgkin's disease, it is nowadays described more often. The possible causes of this second malignant tumor are discussed. Although modern therapy of Hodgkin's disease should not be abandoned from fear of second malignancies, any change in primary treatment must consider not only acute toxicity but also the occurence of late second malignant tumors. Long term follow-up of all patients treated with radiotherapy and/or polychemotherapy is necessary.
