Organization of the mouse 5-HT3 receptor gene and functional expression of two splice variants.

The structure of the mouse 5-HT3 receptor gene, 5-HT3R-A, is most similar to nicotinic acetylcholine receptor (nAChR) genes, in particular to the gene encoding the neuronal nAChR subunit alpha 7. These genes share among other things the location of three adjacent introns, suggesting that 5-HT3R-A and nAChR genes arose from a common precursor gene. The alternative use of two adjacent splice acceptor sites in intron 8 creates, in addition to the original 5-HT3R-A cDNA (5-HT3R-AL), a shorter isoform (5-HT3R-AS) which lacks six codons in the segment that translates into the major intracellular domain. This splice consensus sequence is not found in human genomic DNA. In mouse, we demonstrate by RNAse protection assay that 5-HT3R-AS mRNA is approximately 5 times more abundant than 5-HT3R-AL mRNA in both neuroblastoma cell lines and neuronal tissues. We used the Semliki Forest virus expression system for electrophysiological characterization of 5-HT3R-AS and 5-HT3R-AL in mammalian cells. No differences in electrophysiological characteristics, such as voltage dependence, desensitization kinetics, or unitary conductance were found between homomeric 5-HT3R-AS and 5-HT3R-AL receptors. Their properties are very similar to those of 5-HT3 receptors in mouse neuroblastoma cell lines.

Extradural analgesia in labour: complications of three techniques of administration.

We have studied the complications associated with three techniques used to maintain extradural analgesia in labour: midwife top-up doses of 0.25% bupivacaine 10 ml, continuous infusion of 0.125% bupivacaine 10 ml h-1 and patient-controlled extradural analgesia (PCEA) with self-administered 3-ml bolus doses of 0.25% bupivacaine. A significantly higher intervention rate by an anaesthetist was required in the continuous infusion group. There was no difference in the mode of delivery between the three groups, although some women in the continuous infusion group had significantly denser motor block. There was a similar incidence of rectal pressure, unilateral block and missed segments in the three groups. Uneventful hypotension occurred in three women; two receiving PCEA and one receiving continuous infusion. Ten women experienced sensory blocks extending above T7 with no ill effects; seven receiving PCEA and three continuous infusion.

No effect of kinins on DNA synthesis in LNCaP prostate cancer cells.

1. Prostate has kininogenase activity and expresses members of the tissue kallikrein gene family. The present study examined the effect of exogenous and endogenous kinins on growth of LNCaP prostate adenocarcinoma cells. 2. Rate of DNA synthesis was measured by incorporation over 4 h of [3H]-thymidine into a TCA insoluble fraction of LNCaP cells that had been cultured for 24 h. 3. Increased [3H]-thymidine incorporation was seen in response to 10 nmol/L testosterone (+103 +/- 5 s.e.%), dihydrotestosterone (+113 +/- 14%) and R1881 (+64 +/- 10%) (P < or = 0.001; n = 4). 4. In contrast 0.05, 5 and 1000 nmol/L lysyl-bradykinin had no effect (15 +/- 4, 10 +/- 9 and 5 +/- 3 s.e.%, respectively; n = 7). Des-Arg9[Leu8]-bradykinin (a B1 receptor antagonist) and/or D-Arg-[Hyp3,Thi5,8,D-Phe7]-bradykinin (a B2 receptor antagonist), 1 nmol/L, and indomethacin, 5 mumol/L, also had little or no effect. 5. In conclusion, kallidin and endogenous kinins and prostaglandins have little or no effect on DNA synthesis and therefore on the growth of LNCaP cells in comparison to the two-fold stimulation produced by androgens.

Cardiac parasympathetic activity in severe uncomplicated coronary artery disease.

BACKGROUND: Previous studies have suggested that coronary artery disease is independently associated with reduced cardiac parasympathetic activity, and that this is important in its pathophysiology. These studies included many patients with complications that might be responsible for the reported autonomic abnormalities. OBJECTIVE: To measure cardiac parasympathetic activity in patients with uncomplicated coronary artery disease. PATIENTS AND METHODS: 44 patients of mean (SD) age 56 (8) with severe uncomplicated coronary artery disease (symptoms uncontrolled on maximal medical treatment; > 70% coronary stenosis at angiography; normal ejection fraction; no evidence of previous infarction, diabetes, or hypertension). Heart rate variability was measured from 24 hour ambulatory electrocardiograms by counting the number of times successive RR intervals exceeded the preceding RR interval by > 50 ms, a previously validated sensitive and specific index of cardiac parasympathetic activity. RESULTS: Mean (range) of counts were: waking 112 (range 6-501)/h, sleeping 198 (0-812)/h, and total 3912 (151-14 454)/24 h. These mean results were unremarkable, and < 10% of patients fell below the lower 95% confidence interval for waking, sleeping, or total 24 hour counts in normal people. There was no relation between the severity of coronary artery disease or the use of concurrent antianginal drug treatment and cardiac parasympathetic activity. CONCLUSION: In contrast with previous reports no evidence of a specific independent association between coronary artery disease and reduced cardiac parasympathetic activity was found. The results of previous studies may reflect the inclusion of patients with complications and not the direct effect of coronary artery disease itself.

Meconium staining: is it related to maternal smoking?

The present study was undertaken to find if pregnant women's smoking habits were related to the presence of meconium in the amniotic fluid. The medical records of 58 women admitted to the OB service between January 1, 1992, and December 31, 1992, were reviewed. Data from the records was analyzed. Findings indicated that smoking was not related to the presence of meconium in the amniotic fluid. Some women who did not smoke had meconium stained amniotic fluid. Some women who smoked had no meconium staining. A relationship between meconium staining and maternal smoking habits was not supported.

[Criteria and procedures for selection of children with acquired or congenital deafness for cochlear implants in Europe].

The Nucleus Mini System 22 cochlear implant has been implanted in over 2,000 children worldwide. Selection criteria have evolved similar to that for adults since implantation in children began in 1985. The appropriate selection of children for implantation is made by a professional pediatric team which considers audiological and non-audiological issues in determining if the child is a cochlear implant candidate. The main philosophy is to establish if the child has the potential to benefit from a cochlear implant.

[Test results in European children with a Nucleus Mini System 22 cochlear implant].

Over 600 European children have been implanted with Nucleus Mini System 22 Cochlear Implant since 1985. As well as its proven safety und effectiveness it has become increasingly important to monitor the benefit provided postoperatively at regular intervals. The results from 144 European children randomly selected suggest that postlingually deafened children show rapid improvements in performance whilst children deafened before acquisition of speech and language progress at slower but a continuous rate in the first years of experience.

Comparison of the postoperative analgesic and sedative effects of carprofen and papaveretum in the dog.

Forty dogs undergoing a variety of surgical procedures were assigned randomly to one of two groups. All the animals were premedicated with acepromazine (0.05 mg/kg bodyweight) intramuscularly, and anaesthesia was induced with thiopentone sodium, or propofol in the case of lean animals, and maintained with halothane in an oxygen/nitrous oxide mixture using a non-rebreathing circuit. The dogs in group 1 were given papaveretum (0.2 mg/kg) slowly intravenously within 35 minutes of induction of anaesthesia and the dogs in group 2 were given carprofen (4 mg/kg) in the same way. The dogs were scored for sedation and pain by a trained theatre nurse, who did not know which group they belonged to, using a visual analogue scale, at 15, 30, 60, 120, 240 and 360 minutes after the halothane was switched off at the end of the procedure. Nine of the dogs were withdrawn from the trial (eight of them from the papaveretum group) because of inadequate pain relief and these animals were given pethidine (3 mg/kg intramuscularly) which produced adequate analgesia within 15 minutes in all but one case. Carprofen provided profound analgesia which was as effective and of longer duration than that produced by papaveretum, and was associated with significantly less postoperative sedation and a quicker return to the normal conscious state.

Management guidelines in essential hypertension: report of the second working party of the British Hypertension Society.

Several important new issues have arisen in the management of patients with hypertension. A working party of the British Hypertension Society has therefore reviewed available intervention studies on anti-hypertensive treatment and made recommendations on blood pressure thresholds for intervention, on non-pharmacological and pharmacological treatments, and on treatment goals. This report also provides guidelines on blood pressure measurement, essential investigations, referrals for specialist advice, follow up, and stopping treatment.

Unusually compact ribosomal RNA gene cluster in Sphaeronaemella fimicola.

The ribosomal DNA repeat unit of Sphaeronaemella fimicola was found to be a 13.7-kb tandem repeat with a relatively long nontranscribed spacer (NTS) and an unusually compact ribosomal RNA gene cluster. The DNA sequence of an 850-bp PCR amplification product containing the 3' end of the small subunit rRNA (SSrRNA) gene, the 5.8s gene, and the 5' end of the large subunit rRNA (LSrRNA) gene was determined. The putative internal spacers flanking the 5.8s RNA gene could be the shortest yet noted for any fungus, totaling only 102 bases.

Pharmacokinetics of propofol administered by infusion in dogs undergoing surgery.

We have investigated the pharmacokinetics of propofol in seven beagle dogs undergoing surgery for implantation of s.c. tissue pouches. Blood concentrations of propofol were measured using high pressure liquid chromatography with fluorescence detection. After premedication with acepromazine and papaveretum and induction of anaesthesia with propofol 4 mg kg-1 i.v., anaesthesia was maintained with propofol 0.4 mg kg-1 min-1 and 67% nitrous oxide in six dogs. In the seventh dog, the infusion rate was increased to 0.6 mg kg-1 min-1 in order to achieve surgical anaesthesia. The mean elimination half-life was 322.3 (sem 27.0) min, mean volume of distribution at steady state 6.510 (0.524) litre kg-1, mean body clearance 50.1 (3.9) ml kg-1 min-1 and mean residence time 131.6 (8.9) min. The blood propofol concentrations achieved using this standard dosing regimen showed wide variation among individuals. All dogs recovered rapidly from anaesthesia. The mean time to extubation was 7.6 min and to standing unaided was 30.7 min from the end of the infusion.

The effects of halothane and nitrous oxide on the pharmacokinetics of propofol in dogs.

The pharmacokinetics of propofol, 6.5 mg/kg, administered as a bolus dose intravenously (i.v.) were studied in six dogs (group 1). The effect of maintaining anaesthesia with halothane and nitrous oxide in oxygen on propofol pharmacokinetics was also investigated in six dogs undergoing routine anaesthesia (group 2). Induction of anaesthesia was rapid in all animals. Post-induction apnoea was a feature in three of the 12 dogs. The blood propofol concentration-time profile was best described by a bi-exponential decline in two dogs in group 1 and in 3 dogs in group 2, and by a tri-exponential decline in four dogs in group 1 and 3 dogs in group 2. The elimination half-life was long in both groups (90.9 min and 75.2 min, respectively), the volume of distribution at steady state large (4889 and 4863 ml/kg) and the clearance rapid (58.6 and 56.3 ml/kg.min). There were no significant differences between the groups, thus indicating that maintenance of anaesthesia with halothane and nitrous oxide had no effect on the pharmacokinetics of propofol in the dog.
