Isotope loopography, a new test: comparison with standard loopography and its relationship to renal function in patients with ileal conduit urinary diversion.

Reflux associated with the classic ileal conduit has been implicated as possibly contributing to long-term renal deterioration. When demonstrated on a standard loopogram reflux occurs under highly non-physiologic conditions. A new method of radioisotope loopography performed under physiologic conditions was devised and compared to the standard loopogram. The standard loopogram is more of an indicator for the capacity for reflux. The isotope loopogram is a more specific indicator for reflux that is potentially detrimental to renal function. Approximately 50 per cent of ureteroileal anastomoses that permit reflux with conventional loopography do not do so under the physiologic conditions of the isotope loopogram.

Construction of hemibladders and cutaneous vesicostomies for bilateral renal function studies. A new technique in Macaca arctoides.

A surgical technique for the construction of hemibladders and permanent cutaneous vesicostomies in Macaca arctoides is described. Serial studies of renal function were performed in sedated monkeys before and 1 or more months after division of the bladder. The results of the studies confirm our previous observation that renal function in monkeys is comparable to that in man and show that (i) the surgical procedure is successful in the majority of instances; (ii) simultaneous determinations of renal function of the right and left kidneys are comparable to function observed before dividing the bladder; and (iii) studies of function of the two kidneys may be repeated for 18 months or more and as often as three times a week. We conclude that the procedure provides an opportunity for prolonged observation of changes in renal function and morphology of a single experimentally manipulated kidney compared to (i) the normal function and morphology of the kidney before inducing disease and (ii) the function of a normal contralateral kidney.

Submucosal fibromatosis: prostatism in a young boy?

A seven-year-old boy was seen with a urodynamically significant, obstructing, posterior urethral mass. It was believed to represent either a fibromuscular prostatic enlargement or a nonpedunculated congenital polyp. The differential diagnosis is discussed. Transurethral resection of the mass resulted in decreased voiding pressure and relief of symptoms.

Pulmonary dysfunction after lymphangiography.

Pulmonary function tests were done on 15 patients before and after bipedal lymphangiography. Lymphangiography was associated with an approximate 10 per cent reduction in lung volume. This phenomenon persisted for 48 to 72 hours. The probable mechanism for this reduction is closure of terminal airways or alveolar ducts or both and is reversible by beta-adrenergic drugs. The reduction does not correlate with the severity of pre-existing pulmonary disease. The degree of pulmonary impairment can be assessed roughly by measurement of the vital capacity. An operation can be performed safely 3 to 5 days after lymphangiography.

Traumatic rupture of a renal cell carcinoma.

After sustaining very minor trauma, a man presented with flank pain and fever. A complete clinical and radiological evaluation led to exploration for suspected ruptured renal carbuncle with perinephric abscess. A ruptured renal cell carcinoma was found. Differential diagnostic considerations include renal tumor, abscess, cyst, and hydronephrotic kidney. Radical nephrectomy is indicated if the contralateral kidney can sustain life. The patient shows no sign of recurrence or metastases 4 months following antibiotic treatment and nephrectomy followed by chemotherapy and radiation of the renal bed.

Adenocarcinoma↗

Malignant hemorrhagic renal cyst with occult neoplasm.

An asymptomatic hemorrhagic renal cyst was benign at angiography, nephrotomography, cystography, and cytologic examination. Malignancy was found at surgery. Malignancy cannot be excluded in a hemorrhagic cyst which is benign by all other criteria.

Economics of academic medical consultations.

For the urology faculty at the University of New Mexico School of Medicine, consultations for hospitalized patients produced too little revenue and took to much time. To test faculty perceptions of the consultation process, a study was conducted and is reported in this article.
