Infarctions of the brainstem and cerebellum: a correlation of computed tomography and angiography.

Forty-nine patients were found to have computed tomographic (CT) and clinical evidence for infarction in the brainstem and cerebellum. Seventeen had correlative angiography, of which 15 had a severe occlusive vascular lesion somewhere in the vertebrobasilar system. The correlation between angiographic and CT localization of the infarcts was not good. Inferiorly located infarcts were probably often missed on CT because of basal artifacts. Combined infarctions of the brainstem and cerebellum were common. Nineteen of the 49 patients had an associated mass effect in the posterior fossa; in eight of these, hydrocephalus developed. Although immediate surgical decompression of the posterior fossa has been recommended for treatment of this complication, most of these patients recovered well with careful medical decompression.

Relationship of dystocia transmitting ability with type and production transmitting ability in Holstein bulls.

Relationships of transmitting ability for dystocia with transmitting abilities for type and production in Holstein bulls were estimated. Genetic correlations between dystocia transmitting ability with Predicted Difference (PD) were: milk .00, fat .00, test -.02, and dollars .00. Genetic correlations for transmitting ability of dystocia were -.28 with Predicted Difference Type and -.23 with Type Production Index. Correlation indicated improved conformation will increase dystocia. The relationship of transmitting abilities from Mating Appraisal for Profit with dystocia indicated that scale (size) has a major effect on the type-dystocia relationship. Large size results in both high type classification and increased dystocia. Correlations of rump and center support with dystocia were also in excess of .20.

Venous occlusions of the cavernous area--a complication of crushing fractures of the sphenoid bone.

Four patients who sustained crushing fractures of the sphenoidal bone had angiographic evidence of venous occlusions of the cavernous area. Signs depended upon the type of basal venous and cavernous anatomy. The diagnosis can be made by demonstrating abrupt cutoff of veins in the cavernous region, preferred retrograde flow of the anterior cavernous sinus, direct venous hemorrhage, and venous stasis.

Cervical spinal angiography in midline spondylosis and herniations of intervertebral discs.

Three patients with clinical evidence of cervical myelopathy and degenerative osteoarthritis or herniation of intervertebral disc were studied with angiography. Angiography yielded information regarding the level of the most significant lesion better than did Pantopaque myelography when multiple lesions were present. Angiography also aided in differentiating hard central osteophytic from soft tissue encroachment on the spinal cord caused by herniation of a disc or thickening of the posterior longitudinal ligament. An occasional patient with cervical discogenic myelopathy suggested at multiple levels by myelography may benefit from the localization offered by angiography.

Occlusion of the intradural vertebrobasilar artery.

The diagnosis of occlusion of the intradural vertebrobasilar artery (OIDVBA) was made by means of cerebral angiography in 22 patients. The clinical presentation, course and followup were studied in conjunction with the angiographic findings in each case and the following conclusions made. OIDVBA is not rare. It occurs one-fourth as often as occlusion of the carotid artery. The correct diagnosis is not made clinically before angiography in the majority of patients. Complete visualization of the neck and intracranial vasculature is necessary to document the occlusion. Atherosclerotic thrombosis is the most common type of occlusive lesion. The most common predisposing factors are atherosclerosis, hypertensive cardiovascular disease, diabetes mellitus, and developmental vertebrobasilar hypoplasia. Most patients with occlusion are in the 7th and 8th decades of life and transient attacks of vertebrobasilar ischemia precede the occlusion in one-half of the cases. Emboli usually lodge in the terminal portion of the basilar artery whereas thrombotic occlusions tend not to be located in a characteristic segment. A majority of patients diagnosed angiographically survive their OIDVBA, but most distal occlusions result in death, often following several weeks of coma. In the surviving majority, disturbance of gait, impairment of vision, and symptoms of transient vertebrobasilar ischemia are the most common sequelae.

Diffuse pulmonary hemangiomatosis.

The authors describe three pediatric patients with diffuse pulmonary hemangiomatosis. Virtually identical clinical, radiographic, and pathologic findings were observed in each case. The combination of diffuse interstitial pulmonary infiltration and bloody pleural effusion in a child is pathognomonic. Early recognition of hemangiomatosis can allow more effective evaluation of therapeutic measures.

Time course changes in total lipid content and fatty acid distribution in various tissues of young chickens after feeding rapeseed oil or soybean oil.

1. In chicks fed rations containing 15% rapeseed oil (RSO) or soybean oil (SBO) a gradual increase in the lipid content of heart tissue was found from 1 to 10 days of age. The finding that feeding high levels of RSO to chicks did not cause excessive accumulation of fat in cardiac tissue supports the contention that it is inappropriate to extrapolate from one species to another (e.g. from rats to humans) as has been done by some workers with respects to RSO utilization. 2. Inclusion of RSO in the ration caused an increase in the total omega-9-monounsaturated fatty acid concentration in heart tissue. The increase in C 20:1 fatty acid concentration in heart tissue noted in the RSO-fed chicks suggests beta-oxidation of erucic acid (delta13 22:1) to eicosenoic acid (delta11 20:1). The rapid decrease in the oleic acid (delta9 18:1) concentration of liver tissue of chicks found during the period 1 to 10 days of age suggests the importance of oleic acid as a source of energy for the chicken.

Anaerobic arterial sampling technique in the bovine species.

An arterial sampling technique was used in the unanesthetized bovine species. Samples for acid-base and blood gas analyses were obtained from a branch of the caudal auricular artery and required minimum restraint of the animal. The technique would be useful for research and clinical practice.

Angiographic demonstration of lupus cerebral phlebitis with communicating hydrocephalus.

Two patients with systemic lupus erythematosus, who had striking angiographic abnormalities of venous contour, are presented. Both cases also had communicating hydrocephalus. Postmortem examination of one patient led to a histological explanation of the venous changes observed radiologically. Previous radiological reports have shown involvement of arteries but not veins with lupus erythematosus. The angiographic signs of phlebitis may signify a meningeal reaction which, when combined with ventricular enlargement, should suggest communicating hydrocephalus.

Tumor growth patterns in multiple myeloma.

Serial changes in tumor mass were evaluated in 61 patients with multiple myeloma who had received intermittent courses of melphalan-prednisone until death. The variations in the kinetics of tumor reduction and relapse could be explained by a mathematical model based on two cell populations, one sensitive to and one resistant to chemotherapy. For all responding patients, the median tumor halving-time was 1.3 months and the median doubling time was 2.9 months. The duration of a constant tumor mass during remission was brief in most patients. A larger fraction of resistant cells prior to therapy was associated with a slower tumor doubling-time during relapse. With a constant fractional reduction of sensitive cells and a tumor halving-time of one month or less, all cells sensitive to alkylating agents would be eliminated with 3 years of uninterrupted intermittent therapy.

Occult spinal dysraphism in the geriatric patient.

Three patients in their seventies with occult spinal dysraphism are described. All three had tethered spinal cords, intrasacral meningoceles, and severe degenerative lumbar spondylopathy. The mechanism for the late development of symptomatology is related to an acquired lesion, degenerative spinal stenosis.

Myelographic features of mucopolysaccharidoses: a new sign.

A new mechanism of myelopathic changes in patients with mucopolysaccharidosis is described. Two similar cases had myelographic evidence of marked concentric impingement of the cervical subarachnoid space and cord compression due to thickening of the dura. Myelography is indicated in the evaluation of these patients because surgical decompression may reverse the progressive neurological deficits.

Relationship between plasma lipid concentrations and coronary artery disease in 496 patients.

The relationship between fasting plasma cholesterol and triglyceride concentrations and the frequency and extensiveness of coronary artery disease (CAD) was studied in 496 subjects evaluated for chest pain by coronary arteriography at The Methodist Hospital. One hundred six of the patients had no CAD while 390 had 25% or greater stenosis of one or more major vessels. Ninety-one percent had 75% or greater stenosis of at least one major vessel. Mean age for the group with CAD was 55.7 +/- 8.7 and without disease 49.4 +/- 11.6 (P less than 0.01). Both cholesterol and triglyceride concentrations were higher (P less than 0.001) in the group with CAD. Mean cholesterol concentration in males increased from 195 +/- 36 mg/dl in the group without CAD to 219 +/- 41 in the group with three vessel disease and in females from 207 +/- 40 to 252 +/- 42. A progressive increase in triglyceride values was also detected but was less consistent. At the level of 25% and greater obstruction, the partial correlation coefficients between the number of vessels involved and the cholesterol and triglyceride concentrations, respectively, were +0.201 and +0.181.

Apparatus for controlling temperature of catheter contents in cold environments.

An apparatus was developed to prevent the freezing of catheter contents during blood sampling or liquid infusion into animals maintained at sub-freezing temperatures. The apparatus consisted of a larger, outer set of plastic and copper tubes and a smaller, inner set of tubes. The catheter was passed inside the inner set. Water circulated through the outer set kept the contents of the catheter inside the inner set from freezing. The apparatus was also useful for the rapid cooling of blood to a predetermined temperature while a sample was being withdrawn from an animal.
