A precise technique to localize pulmonary-pleural air leaks.

A bubblegram technique is described for identification of air leaks in persistent pneumothorax by introducing water-soluble contrast medium into the pleural space. The technique proved useful in eight children examined for persistent air leaks.

Primary cerebral germinoma and ventriculoperitoneal shunt metastasis.

A case of peritoneal metastasis via a ventriculoperitoneal shunt 14 months following biopsy and irradiation of an intracerebral germinoma is presented. Metastatic abdominal disease has been controlled at 32 months postmetastatic diagnosis by abdominal irradiation and systemic chemotherapy. The occurrence of ventriculoperitoneal shunt metastases and control of extracranial metastases from intracerebral germinomas are discussed.

Meconium plug syndrome associated with neonatal intussusception.

Two neonates with clinical features of meconium plug syndrome (MPS) were found to have ileocolic intussusception associated with meconium plugs inspissated in the distal ileum. Hydrostatic reduction of the intussusception relieved the intestinal obstruction. Cystic fibrosis was excluded by sweat test and follow-up.

Simplified radioisotope technique for assessing gastroesophageal reflux in children.

There were 125 children with a strong clinical suspicion of gastroesophageal reflux and/or aspiration evaluated with gastroesophageal scintigrams. We found this examination to be more sensitive than the standard barium radiography and highly specific in detecting aspiration of gastric contents. This procedure is simple, safe and more physiologic than other available examinations. However, since gastroesophageal scintigraphy does not delineate anatomic changes as well as barium studies, structural abnormalities of the esophagus and stomach should be evaluated with barium esophagrams and upper GI series.

Technetium-99m biliary imaging in pediatric surgical problems.

The results of scintigraphic imaging of the hepatobiliary system with 99mTc-PIPIDA (IDA derivative p-isopropylacetanilidoiminodiacetic acid) in forty children are reported. 99mTc-PIPIDA imaging is a noninvasive, rapid, safe examination that provides both functional and anatomical information about the hepatobiliary system. Although interpretation is limited by elevated direct serum bilirubin, this agent allows diagnostic information to be obtained with direct serum bilirubin levels up to 8 mg/dl.

Imaging the intracranial complications of Hemophilus influenzae meningitis.

Computed tomography (CT) and radionuclide imaging of the central nervous system (CNS) are important modalities in both the diagnosis and management of CNS complications from Hemophilus influenzae meningitis. Both examinations are sensitive indicators of CNS abnormalities associated with bacterial meningeal infections and are important determinants of the therapy and ultimate prognosis of such complications. Representative findings from CT and radionuclide imaging of children with complications from H. influenzae infection of the meninges are presented.

Ga-67 imaging: Kawasaki disease.

The authors describe a child with Kawasaki disease (mucocutaneous lymph node syndrome), in whom myocardial inflammation associated with the disease was readily detected with Ga-67 imaging.
