Arthrodesis of the knee with intramedullary nail fixation.

Thirty-three patients had an arthrodesis of the knee by means of an intramedullary nail introduced through the greater trochanter. Fifteen of the procedures were done for a failed knee arthroplasty; eight had failed because of infection and seven, because of aseptic loosening. Twenty-nine of the thirty-three knees united three to four months after the first attempt at arthrodesis and three united after technical errors were corrected. One knee had a broken nail and a non-union; this was still untreated at the time of writing. Four nails broke: three in the line of fusion and one in the line of an infected supracondylar pseudarthrosis of the femur. No new infections developed after the arthrodesis. Three patients had had an infection and a chronic fistula before the arthrodesis, and the fistulae healed six, fourteen, and eighteen months postoperatively. In another patient, who had had infection and necrosis of the skin preoperatively, the wound healed in six months. All of these knees healed without an additional major operation. The functional result was satisfactory in all patients. After the arthrodesis, seventeen of the thirty-three patients needed less aid when walking, and no patient needed more aid. Fusion of the knee with a long intramedullary nail can be safe and effective, even in the presence of infection, if the revision is performed properly and certain technical principles are followed. It is especially important to establish good contact between the resected bones.

Serum carbonic anhydrase III, an enzyme of type I muscle fibres, and the intensity of physical exercise.

The effects of 30 min running with stepwise increasing intensity (exhaustive, energy demand approx. 50----100% of VO2max), 60 s supramaximal running (anaerobic, greater than or equal to 125% of VO2max) and 40-60 min low-intensity running (aerobic, 40-60% of VO2max) on serum concentration of muscle-derived proteins were studied in 5 male and 5 female elite orienteerers. S-Carbonic anhydrase III (S-CA III) was used as a marker of protein leakage from type I (slow oxidative) muscle fibres and S-myoglobin (S-Mb) as a non-selective (type I + II) muscular marker. The fractional increase in S-CA III (delta S-Ca III) was 0.37 +/- 0.09 (mean +/- SEM, p less than 0.001), 0.10 +/- 0.05 (N. S.) and 0.46 +/- 0.09 (p less than 0.001) 1 h after exhaustive, anaerobic and aerobic exercise, respectively. The corresponding values for delta S-Mb were 1.45 +/- 0.36 (p less than 0.001), 0.39 +/- 0.13 (p less than 0.01) and 0.67 +/- 0.18 (p less than 0.001). The value for the delta S-CA III/delta S-Mb ratio was 0.68 +/- 0.03 after the aerobic exercise, but only 0.25-0.26 (p vs. aerobic exercise less than 0.001) after the two high-intensity forms of exercise. Since type I fibres of skeletal muscle are known to be responsible for power production during low-intensity exercise, whereas fibres of both type I and type II are active at higher intensities, the delta S-CA III/delta S-Mb ratio may depend on the recruitment profile of type I vs. type I + II fibres.

Air bacterial and particle counts in total hip replacement operations using non-woven and cotton gowns and drapes.

Air bacterial and particle counts were obtained in a conventionally ventilated operating theatre, during 8 operations for total hip replacement performed using synthetic non-woven fabrics as drapes for the patients and gowns for the staff (trousers and stockings were of conventional cotton material), ('non-woven' group), and in 8 corresponding operations using conventional cotton fabrics ('cotton' group). No significant difference between the groups with regard to air bacterial counts was observed. The use of cotton fabrics was associated with substantially higher particle counts, probably due to particles from the cotton textiles themselves. No significant correlation was observed between the bacterial and particle counts, indicating that these came from different sources and that the particles were mainly of sterile origin. Thus, the benefit of the synthetic, non-woven fabrics in hip replacement surgery, when these fabrics are used to cover theatre staff only partially in the form of gowns, seems questionable with regard to the reduction of air bacterial counts. On the other hand, the surgical textiles of non-woven material improve the purity of the operating theatre since unlike cotton fabrics they do not produce and disperse particles in the air.

The significance of residual stenosis after decompression for lumbar spinal stenosis.

Thirty-one patients with degenerative or combined degenerative and developmental lumbar spinal stenosis diagnosed by myelography and at subsequent surgery were thoroughly examined by computed tomography (CT) to reveal the completeness of the decompression and its correlation with the outcome. Complete elimination of the stenosis was achieved in 10 cases (32%) of whom 9 (90%) showed improvement. Various combinations of residual stenosis (some type of stenosis of the lateral part of vertebral canal at the laminectomy sites and also central stenosis outside the laminectomy field) were observed in 21 cases (68%). Of these, 14 (67%) had nearly complete, subtotal decompression with minimal or some residual stenosis, 7 (33%) had only minor decompression of the most constricted part of the spinal canal with a considerable amount of residual stenosis. The latter groups showed improvement in 8 cases (57%) and 6 cases (86%), respectively. Three patients (10%) with increased postoperative symptoms had only minimal or some residual stenosis. The significance of these findings is discussed.

Decompressive surgery in the tendinitis and tear stages of rotator cuff disease.

Eighty-six shoulders with rotator cuff disease (54 with tendinitis and 32 with rotator cuff tear stage) operated on to relieve the impingement exerted by the coracoacromioclavicular arch on the rotator cuff were analyzed. The median follow-up time was 5.0 years. Partial resection of the anterior undersurface of the acromion, excision of the coracoacromial ligament or of the lateral end of the clavicle (and most commonly a combination of all three measures), were the methods used to achieve decompression. The final results show a substantial improvement in 83% of the cases. The results were related to the extent of acromial resection, preoperative range of the painful arc, and age ; but no statistically significant prognostic signs could be determined. There was some evidence, however, that a wide painful arc and extensive resection of the acromion were associated with a poor outcome. The indications for clavicular resection remain somewhat uncertain. Clavicular resection combined with acromioplasty seemed to give somewhat fewer good results, but the procedure is nevertheless indicated in cases with subacromial impingement associated with acromioclavicular osteoarthrithis. Decompressive surgery proved to be as effective in the tendinitis stage of the disease as in the case of rotator cuff tear. Conclusive operative release of shoulder impingement in rotator cuff disease is a useful procedure in patients with painful arc symptoms resistant to conservative treatment. The result can be improved by refinement of both indications and techniques.

Collagen synthesis and proteolytic activities in rat skeletal muscles: effect of cast-immobilization in the lengthened and shortened positions.

The activities of prolyl 4-hydroxylase (PH) and galactosylhydroxylysyl glucosyltransferase (GGT), both enzymes of collagen biosynthesis, were measured in the gastrocnemius, soleus, and tibialis anterior muscles of rats after bilateral cast-immobilization of the muscles in lengthened and shortened positions for one and three weeks. The activities of muscular proteolytic and hydrolytic enzymes cathepsin D (CD), beta-glucuronidase (beta-GU), alkaline protease (AP), and the rate of acid autolysis (AA) were also studied. The biochemical results were compared to the morphologic changes by light microscopy. Compared to the results for a control group, there was a decrease of 37% and 53% in the specific PH activity of shortened gastrocnemius and soleus, respectively, after three weeks of immobilization. The corresponding decrease in GGT of the shortened gastrocnemius was 47%. At the same time, PH and GGT in the lengthened plantarflexors were at the control level. The proteolytic activities of the shortened plantarflexors were generally higher (CD by approximately 30%, beta-GU of gastrocnemius by 81%, AP of soleus by 63%, AA of gastrocnemius by 34%, and AA of soleus by 56%) than those of the lengthened ones. Light microscopy of the shortened muscles showed numerous atrophic fibers, but no pronounced inflammatory response in the disused muscle tissue. The results suggest that increased proteolysis and wasting of muscle tissue during cast-immobilization is associated with adaptive responses in the metabolism of the muscular collagen network.

Examination of the sprained ankle: anterior drawer test or arthrography?

The accuracy of the anterior drawer test for the diagnosis of recent lateral ligament tears in the ankle was evaluated in a series of 192 patients using surgical or arthrographic findings for reference. Considerable overlapping of results was obtained in ankles with and without ligament tear. Twenty-eight per cent of the anterior talofibular ligament tears, and 38% of the combined anterior talofibular and calcaneofibular tears were not detected, and single and combined tears could not be differentiated. It is concluded that the anterior drawer test is too unreliable as a basis for any decision regarding surgical treatment of a recent sprain. Therefore, arthrography is recommended as the method of choice in such cases of recent ankle sprain, where the need of surgery has to be supported by X-ray analysis.

The value of axial view in the radiography of shoulder girdle--experiences with a new modification of positioning.

A series of 117 patients underwent radiography of the shoulder girdle, performed using a modification which allowed the examination of also severely injured patients in the axial view. The findings in the frontal and axial views were compared. A total number of 161 pathological findings were discovered; out of them 30 were visible only in the frontal view and 49 only in the axial view. The series indicates that the axial view can be taken painless in patients with recent injury, and that the frontal and axial views considerably supplement each other.

Effect of immobilization on collagen synthesis in rat skeletal muscles.

The activities of prolyl 4-hydroxylase (PH) and galactosylhydroxylysyl glucosyltransferase (GGT), both enzymes of collagen biosynthesis, and the concentration of hydroxyproline (Hyp) were measured in the soleus, gastrocnemius, and tibialis anterior muscles of rats after cast immobilization in the middle position for 1 or 3 wk. The specific activity of PH decreased by 54 and 70-75% (P less than 0.001) in the soleus muscle after 1 and 3 wk, respectively, the corresponding decreases in GGT activity were 43 and 47% (P less than 0.001). A less pronounced decrease in the activities of these enzymes was observed in gastrocnemius and tibialis anterior muscles. The Hyp concentration in the soleus increased during the first week of immobilization but began to decrease thereafter, and the total muscular Hyp content was reduced after immobilization for 3 wk. The results suggest a marked inhibition of muscular collagen synthesis during immobilization. Electrical stimulation of the sciatic nerve partially prevented this disuse atrophy and the decreases in PH, GGT, and Hyp in the tibialis anterior muscle but not in the gastrocnemius or soleus muscles.

Prevention of postoperative urinary retention after total hip arthroplasty in male patients.

The efficacy of phenoxybenzamine in preventing postoperative urinary retention after total hip arthroplasty was investigated in a double-blind placebo-controlled study on 60 consecutive male patients with obstructive urinary symptoms. The patients were randomized into two groups, one being given 10 mg phenoxybenzamine orally and the other a placebo immediately after the termination of the operation and 8 and 16 hours later. Only 10% of the patients in the phenoxybenzamine group were affected by retention compared with 48% in the placebo group (p less than 0.01). It is concluded that phenoxybenzamine effectively prevents urinary retention after total hip arthroplasty in male patients with obstructive urinary symptoms. The drug is recommended for routine prophylactic use with patients not presenting with impaired cerebral circulation or serious coronary heart disease, to prevent infection leading to bacteraemia or other complications of urinary catheterization.

Studies on structure-activity relationship of gossypol, gossypol ethers and three naphthaldehydes in the inhibition of spermatozoal metabolism.

Gossypol tetramethyl ether [C30 H24 O2(OCH3)4] and gossypol hexamethyl ether [C30 H24 O2(OCH3)6], which in contrast to gossypol are stable compounds, were tested for their ability to depress fructose degradation in fresh human sperm cells. Both ethers inhibited spermatozoal fructolysis, yet less effectively than did the parent compound. A synthetic compound, O-hydroxylnaphthaldehyde, and two commercially available preparations, 1- and 2-naphthaldehydes, were also tested under the same experimental conditions. These preparations represent about half of the gossypol molecule and possess a reactive aldehyde group in their molecules. Their inhibitory effect on fructose degradation in fresh human sperm cells, however, was considerably smaller than that of gossypol itself. It thus appears that the whole ring structure of gossypol rather than the intact aldehyde group is required for an effective inhibition of spermatozoal energy metabolism.

High incidence of low serum vitamin D concentration in patients with hip fracture.

The study of 82 consecutive hip fracture patients (22 males and 60 females) and 185 various controls showed that low serum 25-OH-vitamin D3 (25-OD-D3) concentration was common in the hip fracture patients, who in addition were old, incapable of independent daily life, had poor dietary habits, reduced nutritional status, and spent insufficient time in sunlight. The most usual mechanisms of trauma were minor indoor falls. Osteoporosis was common and associated with low 25-OH-D3. Hypocalcemia was more common in hip fracture patients than in controls. Increased serum alkaline phosphatase was not a specific feature in hip fracture patients. The results suggest that vitamin D supplementation is indicated in elderly disabled people.

Stress fractures of the sesamoid bones of the first metatarsophalangeal joint in athletes.

Over a period of 11 years 15 cases of stress fractures of the sesamoid bones of the first metatarsophalangeal (MTP) joint were treated in athletes. The mean age of the patients was 22.3 years, and there were 9 males and 6 females in the series. All patients were athletes, who began to suffer from the symptoms during training without any trauma. Eight fractures were located in the medial, six in the lateral sesamoid bone, and in one case both sesamoids were affected. The diagnosis was performed on the basis of the history, symptoms, clinical examination, and radiological, or isotope scanning findings. Ten of the patients were treated conservatively by prescribing an avoidance of excessive physical activity and better training shoes. In five cases surgical excision of the fragmented painful sesamoid bone was performed. There were no complications in the series and the athletes could start gradually training 6-8 weeks after the operation. The histology showed fibrotic non-union at the fracture site and supported the diagnosis of stress fracture. Three of the conservatively treated athletes had mild symptoms in intensive training, others had a good end result.

Disk-space hypodensity in CT: the first radiological sign of postoperative diskitis.

Five patients with recent postoperative diskitis were examined with plain lumbar films and/or multidirectional tomography and CT. Three patients with normal recovery after lumbar disk herniation operation served as controls of the CT findings. Plain films and tomography showed intervertebral disk space narrowing and end plate erosions two and one-half to four weeks after the onset of symptoms. CT detected significant hypodensity of the affected disk spaces as compared to the controls. This was the first recognizable sign of diskitis in three patients and appeared as early as ten days after the onset of symptoms.

Postoperative intervertebral discitis.

Discitis is a rare complication of disc operation. During a 10-year-period 1100 patients were operated for lumbar disc herniation and only eight patients developed postoperative discitis. The symptoms began 3 weeks after the operation and the most prominent clinical feature was back pain with muscle spasm. The sedimentation rate was highly elevated but the body temperature was slightly elevated for only a few days. Typical radiographic findings were narrowing of the affected disc space and end-plate resorption 3-4 weeks after the initial symptoms. In the early phase, CT showed hypodense disc material in the affected disc space, which may be the first radiological sign of discitis.

Consequences of synovectomy of the knee joint: clinical, histopathological, and enzymatic changes and changes in 2 components of complement.

The state of 36 knee joints of 32 rheumatic patients was evaluated after surgical synovectomy, the follow-up period varying from 2 to 39 months. Synovitis was suspected by arthroscopy in 67% and verified histologically in 52%, although in a milder form than at the time of synovectomy. The regenerated synovial tissue was less permeable to serum proteins and contained lower activities of lysosomal enzymes than the excised synovial tissue. Although the number of leucocytes decreased in the joint fluid after synovectomy, the activities of lysosomal enzymes were lowered only slightly, and there was no change in the amount of C3 and C4 components of complement pathway.

Polarographically detected effect of ultrasound and irradiation on DNA solution.

Aqueous solutions of DNA were sonicated, irradiated, or sonicated and irradiated. The effect of the treatment was detected by pulse-polarography. The polarographic response of irradiation depended on the concentration of DNA. It was found that the effects of sonication and irradiation were non-additive. The reason for this is discussed briefly.
