Because its extreme rarity, a case of a bilateral chest wall hamartoma is reported in an 7 months infant presenting a moderate thoracic deformation. Roentgenogram reveals a segmentary costal destruction and an adjacent intrathoracic mass. A right thoracotomy is realized and the tumor is removed without difficulties. Diagnosis of a vasculo-cartilaginous hamartoma (other term used: mesenchymoma) is confirmed after the resection of the right lesion, whereas the left one is spontaneously reducing within 7 months. After analysis of 20 previously reported identical cases, we think that diagnosis may be done on clinico-radiological grounds; surgical decision has to take into account the constant benignity, the possible spontaneous regression and the orthopedic sequelae due to an extensive resection.
The authors relate their experience in the treatment of unequal leg length, in children, by was of epiphysiodesis. They realized 54 Phemister's operations and 94 Blount's Among the latters, final sterilization of the epiphyseal cartilage has been noted after 1,5 year and the axis deviations (main complication of these operations) was clearly more frequent and graver. Besides, the estimates preliminary to the epiphysiodesis setted up for the treatment of small inequality proved to be little reliable and little accurate and this difficulty in making accurate prevision is an important snag to the method.
Giant emphysema of a lung lobe has distinctive features. Classically there is overdistension of the affected lung lobe, with one lobe only being involved, and, 50% of cases occur in the newborn infant [1, 4, 8]. The authors describe a particularly severe example with marked mediastinal shift and initially the hemithorax on the side of the lesion was opaque. Angiography was carried out and followed by resection when the infant was 4 months old.
The Authors who have operated on 120 cases of pectus excavatum, describe their own method of surgical correction. It consists of: -- Sternal mobilisation by sternotomies; -- Limited chondreal resections; -- Internal fixation. The Authors emphasize the interest of a good internal fixation which must allow normal breathing during the period of cartilage's strengthening. The use of long flexibles thread pins affords a good stability and flexibility and assures at long term good cosmetic results.
The authors report a long term follow up of 11 children who underwent a Childs-Philips intestinal plication from one to fourteen years ago. One death was caused by this operation. Besides, the tolerance of the Childs-Philips operation seems to be good, the nine other patients present a normal growth without any recurrence of the occlusion. The authors emphasize the interest of this bowel plication in the treatment of occlusion during the childhood.
