Report: conference on key issues in post-graduate psychiatric education in Canada.

At a national conference in 1985, the Royal College guidelines for post-graduate training in psychiatry in Canada were reviewed. The topics considered included geriatrics, consultation-liaison, chronic care, out-patient, and psychotherapy training, as well as pre-licensure requirements and examinations. The recommendations will be used by the Royal College in reformulating its requirements for psychiatric residency programs.

Steroid regulation of Na+/K+-ATPase activity in the rat kidney: effect of a new 19-nor-progestagen.

The effects of Nomegestrol acetate (17 alpha-acetoxy-6-methyl-19-nor-4,6-pregnadiene-3,20-dione), a new 19-nor-progesterone derivative, on renal Na+/K+-ATPase activity were assessed in normal and adrenalectomized rats, and compared with the stimulatory or inhibitory actions produced by other steroids. This compound displayed an inhibitory effect which was similar to, but smaller than, that induced by progesterone and quite distinct from the stimulation produced by 19-nor-progesterone and corticosteroids. In addition, unlike progesterone, it did not antagonize the effect of aldosterone in adrenalectomized rats. This result, together with previous in-vivo and in-vitro observations on this compound indicates that additional modifications introduced in the molecular structure of 19-nor-progesterone produces a potent progestagenic substance virtually devoid of effects on renal Na+/K+-ATPase activity and sodium loss in urine.

[Molecular interaction between synthetic spirobutenolides and C1 esterase normal inhibitor].

Previously we have found that spirobutenolide synthetic derivatives show an inhibiting effect on C1 esterase activity. In contrast to these compounds some derivatives in the same series reveal an esterase activity when they are added to a C1 esterase preparation in presence of normal serum. The present work shows that this effect reflect an interaction with natural inhibitor C1 INH.

Pharmacological profile of the new pituitary-gonadic suppressant 17 beta-acetoxy-5 alpha, 17 alpha-pregn-2-ene-20-yne.

17 beta-Acetoxy-5 alpha, 17 alpha-pregn-2-ene-20-yne (TX 380) strongly suppresses the hypophyso-gonadic axis at central and peripheral levels. Depending on the dosage and on the hormonal status at the time of treatment, this combined inhibition results in major disturbances of the estrus cycle and in impaired ovulation. TX 380 shows minimal androgenic activity. It exerts no effect on the fetal female urogenital tract during gestation. TX 380 possesses only weak estrogenic properties, and does not promote endometrial proliferation in the female rabbit. Moreover, it markedly opposes the action of progesterone. Given this pharmacological profile, TX 380 stands as a candidate drug in the treatment of gynecologic affections, such as endometriosis and benign breast disease, and as a potential contraceptive agent.

Pharmacological studies on stress-induced renin and prolactin secretion: effects of benzodiazepines, naloxone, propranolol and diisopropyl fluorophosphate.

Stress-induced renin and prolactin secretion was investigated using a conditioned emotional response paradigm. Three minutes after placement in a chamber the rats received an electric shock to their feet via the grid floor, then were immediately returned to their home cage. This procedure was repeated for 3 consecutive days. On the fourth day, instead of receiving an electric shock, they were removed after 3 min and sacrificed by decapitation. Control rats were treated identically with the exception that shock was not administered at any time. There was a significant increase in plasma renin activity and prolactin level in the stressed rats. The administration of the antianxiety drugs chlordiazepoxide (10 mg/kg i.p.) or midazolam (0.125-2 mg/kg i.p.) blocked the stress-induced increase in prolactin levels but not the stress-induced rise in plasma renin activity. Administration of the beta-blocker propranolol (1 mg/kg i.p.) inhibited, but did not completely block, stress-induced rise in plasma-renin activity and had no effect on stress-induced prolactin secretion. The opiate antagonist naloxone (0.1-10 mg/kg i.p.) and the acetylcholinesterase inhibitor diisopropyl fluorophosphate (0.5 mg/kg i.p.) did not block stress-induced renin or prolactin secretion. It is concluded that stress-induced prolactin secretion is regulated by a benzodiazepine-mediated mechanism and that stress-induced renin but not prolactin secretion is mediated in part via beta-receptors.

Boundary and intimacy.

Personal boundaries are essential for psychological stability. In psychopathology, they may be too porous, as in the case of borderline personalities, or too rigid, as in the case of narcissistic and paranoid personalities. A developmental model which could explain abnormal boundaries would postulate neglect producing porous boundaries, and intrusiveness producing rigid boundaries. Case material is presented in which patients with narcissistic personality traits had grown up with an intrusive, controlling mother, and without a father to provide a buffer. This led to an inability to tolerate intimacy in adult relationships. The transference of both patients reflected their extreme sensitivity to impingement on their boundaries. Such patients suffer from inner emptiness because of their inability to incorporate positive experiences.

Altered dopaminergic modulation of sympathetic nervous system activity in idiopathic edema.

Hormonal and mean arterial pressure responses to posture and isometric handgrip exercise were examined in 6 women with idiopathic edema and 10 age- and weight-matched normal women before and after 5-7 days of administration of the dopamine agonist, bromocriptine (2.5 mg three times a day). Edema patients demonstrated greater orthostatic weight gain, greater upright epinephrine values, and greater supine and upright norepinephrine values than did the control group. However, supine and upright plasma dopamine levels were similar in the two groups. In edema patients there was a greater supine and posture related norepinephrine and epinephrine to dopamine ratio than in normal controls. These abnormalities were not corrected by treatment with bromocriptine. Supine and upright plasma norepinephrine and epinephrine levels were decreased following bromocriptine treatment in normal subjects but not in edema patients. These data are consistent with the concept that there is decreased dopaminergic regulation of sympathetic nervous activity in patients with idiopathic edema.

Prostacyclin bioassays using inhibition of platelet aggregation and relaxation of rabbit coeliac artery.

Bioassays of prostacyclin by inhibition of velocity and maximum of both irreversible and biphasic platelet aggregation, and by relaxation of the potassium-contracted isolated superfused rabbit coeliac artery have been undertaken. The use of each parameter is possible within a range of PGI2 concentrations, which have been determined for each of the five parameters studied. The optimal concentrations of PGI2 for a bioassay have also been determined, corresponding to each range.

[The endometrium of the immature rabbit stimulated by an estrogen and treated with a progestogen. Scanning electron microscopy study].

The endometrium of rabbits, treated by the usual pharmacology methods designed for the measurement of the pseudogestagen effect, was studied by scanning electron microscopy. Estrogen stimulation was followed by a multiplication of the number of ciliated cells. Treatment with progesterone lead to a decrease in the numbers of microvilli and to the appearance of rounded bulges which increased in numbers as the progesterone dose level increased. These changes were quite close to those observed in post menopausal women under estro-progestogen treatment.

[The endometrium of intact or castrated rats. Effects of hormonal stimulation. A scanning electron microscopy study].

The endometrium of female rats during the estrous cycle and of female ovariectomised rats under hormone stimulation, has been studied by scanning electron microscopy. The endometrium underwent marked cyclical changes; the development of microvilli was noted before ovulation, and secretory phenomena started during proestrus but remained discrete throughout the cycle. The rat endometrium was also characterised by the fact that ciliated cells were scarce. Following ovariectomy, the endometrial surface pattern was similar to that of the menopausal woman endometrium. After estrogen or estroprogestogen stimulation, by the methods usually employed in endocrine pharmacology, the general endometrial pattern was very different to that observed during the reproductive cycle in rats.

Anti-thrombotic effect of very low doses of acetyl salicylic acid in rats.

The anti-thrombotic effect of acetyl salicylic acid (ASA) was studied using the following experimental model: a silk thread was inserted in an extracorporeal shunt between the right carotid artery and the opposite jugular vein. A thrombus developed around the thread which was weighed at 10 minutes intervals. ASA inhibits the development of the thrombus at a dose of 3.1 mg/kg. When the dose was increased the anti-thrombotic effect disappeared. These results suggest that ASA may be an effective antithrombotic agent in man if the dosage is based on pharmacological data.

Basics of videodisc and optical disk technology.

This article outlines basic videodisc and optical disk technology. Both optical and capacitance videodisc technology are described. Optical disk technology as a mass digital image and data storage device is defined and briefly compared with other established information storage media including magnetic tape and microforms. The article includes a look into the future of videodisc and optical disk.

Information providers and videodisc/optical disk technology.

Although industrial and consumer videodisc technology has been available since 1978, information providers are now just beginning to explore the possibilities of using videodisc and optical disk technology as publishing media. Firms involved in the design, development, and delivery of information products and services are looking at these technologies as new business opportunities and distribution channels. The areas in which information providers are using videodisc and optical disk technology are covered briefly.
