Chemical and biochemical studies in fetuses affected with Nieman-Pick disease type A.

Chemical and biochemical studies were performed on two unrelated fetuses affected with Niemann-Pick disease type A, following abortion at about the 19th week of gestation. Abortion was performed as a consequence of previous findings, in amniotic fluid cell cultures, that sphingomyelinase activity was completely absent. Phospholipid analyses of various organs of the fetuses, spleen and liver were the organs mostly affected. Interestingly enough considerable accumulation of sphingomyelin was found in the placenta. The brain was the only organ in which sphingomyelin storage could not be proved. In addition to sphingomyelin a slight accumulation of cholesterol was noticed. Deficiency of sphingomyelinase activity measured at pH 5.0 was the general characteristic of the affected tissues. It is concluded that the accumulation of sphingomyelin in various organs throughout the body of fetuses affected with Niemann-Pick disease is suggestive of the essential role of the enzyme sphingomyelinase and its biochemical maturation, even during the early stages of gestation.

The importance of cuff width in measurement of blood pressure in pregnancy.

Blood pressure was measured indirectly in 218 normotensive primigravidas with arm circumferences ranging from 21.7 to 36.1 cm using three standard blood pressure cuffs. It was found that the medical profession's lack of awareness as to the proper use of cuffs in prenatal care may involve 40% of the patients--20% of these have a small arm circumference, will not show a true blood pressure reading, and may fail to get the proper treatment. On the other hand, 20% of these patients who have a rather large arm circumference, may be grouped among those who need special care and attention, and may be given unnecessary treatment--a fact which may bring about clinical complications, unnecessary expense, incorrect statistical data and a misleading demographic impact.

Menopausal age in various ethnic groups in Israel.

The effects of various parameters on age at menopause have been investigated in five ethnic groups in Israel comprising East European, West European, North African, Israeli and other Middle Eastern (Mediterranean) women, respectively. The data were acquired by means of anonymous questionnaires and were programmed for 1770 women. Correlation coefficients between various variables and age at menopause revealed three variables which have a straight correlation, vis. obesity index, number of children, and years of amenorrhoea (during the reproductive years). The years-of-smoking variable has an inverse correlation with age at menopause. East Europeans have the highest age at menarche. Two-way analysis of variance has shown that the obesity index, years of amenorrhoea, number of children and years-of-smoking parameters are individually more important than ethnic origin. The finding that the age at menopause is highest in the North African group is explained by the higher incidence in this group of high parity, a greater number of amenorrhoea, obesity, and low cigarette consumption. Since many habits (such as smoking, diet, use of contraceptive pills, multiple partners and marital obligations) are subject to frequent change in the modern world, it is of the utmost importance to repeat such a study every few years.

Malignant cervical teratoma of the fetus.

Cervical teratomas have been defined and one new case is now added to the 116 cases previously reported. We have been able to find only six reports of malignant teratoma in the literature so far. The problem of diagnosis and obstetric management are discussed briefly. The incidence of appearance of the tumor is described and the likelihood of hydramnios or of acute obstructive symptoms of the trachea and esophagus at birth are discussed. The differential diagnosis includes cystic hygroma, congenital goitre, bronchial cyst, lymphangioma, neuroblastoma, parotid tumor and carcinoma of the thyroid. The object of this review is to present a synoptic description of these tumors, their clinical manifestations, management, treatment and prognosis, so that it may afford the attending obstetrician a guideline in understanding this rare entity.

The Neugebauer-Le Fort operation: a review of 118 partial colpocleises.

The Neugebauer-Le Fort operation or partial colpocleisis is a safe, simple and rapid procedure for the repair of uterine prolapse in elderly women who are poor surgical risks. The operation was performed in 188 women with total procidentia. Good anatomic results were obtained in 90.7% of the women. Late complications were rare, including recurrence of prolapse (3 patients), and urinary incontinence (12 patients). Immediate postoperative complications were not common. The main disadvantage is the inaccessibility of the uterus in case of postmenopausal bleeding (2 patients). Partial colpocleisis has a place in the armamentarium of the gynecologist treating isolated cases of prolapse of the uterus, and may be indicated in the elderly women with total procidentia who is a poor surgical risk an in whom general anesthesia is contraindicated, and who is certain she wants to forego sexual intercourse.

The use of biological dressings in radical vulvectomy.

The use of biological dressing techniques accelerates recovery from radical vulvectomy combined with lymphadenectomy. Our earliest interest in the field arose as a result of the morbidity rate which ensued subsequent to surgical therapy of cancer of the vulva. The technique involved in 8 such procedures is described, and its applicability illustrated with photographic plates and review of the literature.

Secondary cervical perforation by the Copper-T intrauterine device.

Five cases of cervical perforation have been observed in women with a Copper-T IUCD, over a period of 5 yr. In all cases, the distal end of the stem (vertical limb) of the T had perforated through the cervical wall into one of the fornices of the vagina. Only 2 cases had symptoms leading to the discovery of the device, while the other 3 were discovered accidentally or on routine examination.

XY gonadal dysgenesis associated with hGH and gonadotrophin deficiencies.

A girl of remarkably short stature, referred for investigation with the diagnosis of gonadal dysgenesis and the finding of a male karyotype, proved to be deficient in growth hormone and gonadotrophin secretion, and was treated with growth and sex hormones. It was concluded that this case demonstrates an apparently casual coincidence of pituitary insufficiency with XY gonadal dysgenesis, evidently the first to be reported.

Ovarian abscess associated with incomplete abortion and intrauterine contraceptive device.

A 28-yera-old woman with an intrauterine device (IUD) was admitted to hospital with signs of incomplete abortion. The IUD was removed and curettage revealed necrotic infected decidua with no trophoblastic cells or chorionic villi. The patient subsequently developed multiple abscesses in the right ovary. The mechanism of development of ovarian abscesses in patients with IUD or with infected incomplete abortion is discussed.
