Vehicular trauma in pregnancy: an algorithm for diagnosis and fetal therapy.

The incidence of motor vehicle accidents resulting in trauma in pregnant women is increasing. The best chance for fetal survival is to ensure maternal survival, so awareness of the types of injuries and their presentation after vehicular trauma is of the utmost importance. An algorithm for the approach to diagnosis and fetal therapy based upon obstetrics and trauma care parameters is hereby presented to the trauma team.

Ultrasonic assessment of fetal colon by comparing ratios of colon circumference to abdominal circumference in normal pregnancy and two abnormal cases. A preliminary study.

Fetal gastrointestinal malformations occur with an incidence of approximately 6/1,000 births and can often be diagnosed in utero by ultrasound. Antenatal knowledge of these abnormalities can potentially decrease fetal and neonatal morbidity and mortality because the appropriate preparations can be made in advance of delivery. Changes in ultrasonic images of the fetal colon were measured by comparing ratios of colon circumference to abdominal circumference in 136 normal pregnancies. Normal values of these parameters are given for gestational age. Two abnormal case reports are included to exemplify the clinical applications.

Menopausal hot flushes and plasma beta-endorphins.

Plasma beta-endorphin levels were measured in five women in physiologic menopause suffering from frequent episodes of hot flushes, recorded objectively by the measurement of finger temperature. Significantly lower levels of plasma beta-endorphin levels were found at the onset of the hot flushes than five to 20 minutes before (P less than .001). After the flush there was a significant rise of plasma beta-endorphin levels at five, ten, and 15 minutes (P less than .002).

The immune system during the pre-cancer and the early cancer period. IL-2 production by PBL from post-menopausal women with and without endometrial carcinoma.

We report on alterations in IL-2 production and cell proliferation following PHA stimulation of peripheral blood lymphocytes (PBL) from stage-I endometrial carcinoma (EC) patients, and on mechanisms involved in these alterations. Our study includes 3 groups: EC patients, post-menopausal women at high risk of developing EC, and age-matched healthy women. IL-2 production was markedly lower in most EC patients than in healthy controls. Varying levels of IL-2 were produced by PBL from women in the high-risk group. The proliferative response of PBL to PHA appeared to correlate with levels of IL-2 production. Our results suggest that macrophages are involved, in part, in the modulation of T-cell functions of EC patients.

Abdominal splenosis mimicking abdominal carcinomatosis.

Abdominal splenosis is an uncommon condition. Since ovarian abdominal carcinomatosis might be discovered unexpectedly and the finding might be similar to it, the scope of the present paper is to draw attention to such a possibility.

Treatment of hypertension during pregnancy with hydralazine monotherapy or with combined therapy with hydralazine and pindolol.

Forty-four consecutive patients referred for treatment because of hypertension (greater than 150/90 mmHg) occurring during pregnancy were randomly allocated to one of two treatment groups, hydralazine alone (n = 21) or hydralazine combined with pindolol (n = 23). Satisfactory blood pressure control (diastolic pressure less than 90 mmHg) was achieved in 86% of patients receiving hydralazine alone and 91% of those on combined therapy. Although the treatment did not lower the overall incidence of hypertensive complications it appeared to delay the onset of such complications until successful surgical intervention was possible. Fetal outcome was similar in both groups and there was no perinatal mortality in this high-risk population. Although blood pressure control was similar in both groups of patients, combined therapy with hydralazine and pindolol can be considered to be superior to hydralazine monotherapy, since in patients treated with the combination the incidence and intensity of troublesome side-effects was markedly lower.

Lumbar epidural analgesia in hypertensive patients during labour.

Continuous lumbar epidural analgesia (L.E.A.) is considered to be the most effective technique for providing pain relief during labour and delivery. A prospective randomized study of 52 patients with pre-eclamptic toxemia in labour was carried out to investigate various aspects of the use of L.E.A. in view of the considerable disagreement concerning its effect. The results were compared to a control group (n = 52) receiving i.v. infusion of Hydralazine (Apresoline) and MgSO4 according to Zuspan recommendations. The results were evaluated statistically. The incidence of neonatal jaundice was higher in the control group (P less than 0.005). Maternal postpartum complications were similar in both groups. There were fewer forceps deliveries in the control group (P less than 0.05). The decrease in systolic BP after L.E.A. is well documented up to 120 min after the first injection of Marcaine (P less than 0.001-0.025), while after 120 min, by which time the Marcaine effect has finished, the systolic BP rises again (time for topping up). The decrease in the average diastolic BP as compared to the control group is statistically significant (P less than 0.001-0.01) until 180 min after initiation of the L.E.A. After that, the average diastolic BP rises again, unless Marcaine is topped up. Our results favour L.E.A. as the obstetric analgesic method of choice in cases of severe pre-eclampsia.

Enterobius (Oxyuris) vermicularis of the pelvic peritoneum--a cause of infertility.

A case of Enterobius (Oxyuris) vermicularis granulomatous disease of the pelvic peritoneum causing pelvic adhesions and infertility is presented and discussed. Since parasitic diseases are common in the developing countries, the infertility team has to bear in mind that a parasitic disease might be the main cause of infertility.

Actual effective CO2 laser power on tissue in endoscopic surgery.

To elucidate the cause of power reduction in endoscopic carbon dioxide (CO2) laser surgery and to define the actual effective power on tissue of a beam delivered through different endoscopes, we designed an experimental model. In passing a CO2 laser beam through a CO2 gas-pressurized chamber, no significant power loss was noted. A correlation between the internal diameter of an endoscope and the power output became evident. Effective power of 92%, 85%, and 48% of the potential 100% was measured when a beam was delivered through single- and double-puncture laparoscopes and a hysteroscope, respectively. These figures are relevant in predicting CO2 laser effect used endoscopically.

Catamenial pneumothorax--a literature review and report of an unusual case.

As already mentioned, only a few cases of CPT were described in the international medical literature, though there probably are some publications in languages unknown to us. It is quite possible that this syndrome is much more common than known to us, but until now there has been a lack of awareness of it. It may be presumed that in some of the cases of women complaining of some discomfort during menstruation, thorough examination would reveal a mild spontaneous pneumothorax which doesn't require any special treatment and is self-resolving; but most gynecologists are not thoroughly aware of this entity. While examining the clinical findings it is possible to demonstrate that this syndrome has its own clinical characteristics, totally different from those of spontaneous pneumothorax in the population as a whole. One of the characteristics of the syndrome is the assumption that women who do not ovulate, as women during the menarche, pregnant women and women taking contraceptive medications, are not subject to CPT. A case described in this review contradicts this assumption, although treatment with ovulatory suppressants, successfully used so far, should not be undervalued. The etiology and pathogenesis of this syndrome was and still is enigmatic. The accumulated knowledge so far does not point to any one etiologic factor. Therefore the suggested treatment, before definitive operative treatment, is so far experimental only, and its chances of success in any particular case are unpredictable.

Adverse reactions to antihypertensive drugs in pregnancy.

In conclusion from a clinical pharmacological point of view it is stressed that side effects from antihypertensive therapy in part is related to dosage. Increased knowledge should make it possible to reduce the rate of side effects. In the individual case the physician always has to compare a number of factors: general condition of the patient, age, adherence to the prescribed therapy, side effects, and other factors in addition to blood pressure values. With all these limitations in mind there is agreement that the benefit of antihypertensive therapy far out-weigh the negative aspects of such treatment.

Human spermatozoa penetration to zona-free hamster ova and the volume of sperm cells.

Thirty-five infertile men were divided into two groups: those with normal spermograms (15 cases) and those with oligospermia (20 cases). Each semen sample was examined by its penetration into zona-free hamster ova and by an improved Coulter counter apparatus for measuring the volume of spermatozoal head. Both groups were further divided into two subgroups: those with normal (above 15%) and those with abnormal penetration assay (0-10%). The relationship between sperm modal size and sperm cell penetration into hamster ova in both groups--the oligospermic and the normospermic--was calculated and was found to be nonsignificant (correlation index: -0.154). It appears that the inability of some human sperm cells to penetrate the zona-free hamster ova cannot be attributed to the spermatozoal size.

Retinal hemorrhages in the newborn following labor induced by oxytocin or dinoprostone.

We examined the fundi of 100 newborns following labor induced by intravenous oxytocin or by oral dinoprostone. Retinal hemorrhage was observed in 40% of neonates in the dinoprostone treatment group, as compared with 28% in the oxytocin treatment group. The accumulation of prostaglandins in the fetal circulation may be responsible for the hemorrhages, which clear spontaneously once the noxious agent is metabolized. Other organ systems must be carefully examined in the neonate to detect other possible untoward effects.

The management of labor in high myopic patients.

The routine in our department for years used to be: prevention of bearing down during the end of the 2nd stage of labor in high myopic parturients, by forceps delivery, with the assumption that this will prevent increased intraocular pressure--thus preventing deterioration or increased damage to the eyes. The goal of this study was to challenge our theory and thus we decided to re-evaluate this approach. Various obstetrical data were recorded in 50 women with myopia from 4.5 to 15.0 diopters who were admitted in labor to our department. Fundus examination was performed in all of them before and after delivery. Various types of retinal degeneration and retinal breaks were observed in most of them at their arrival but no deterioration of these retinal defects was observed in any of the cases at the later examination. In view of our results, it is recommended to let high myopic patients deliver spontaneously.

Nonimmune hydrops fetalis--a rational attitude of management.

A rational attitude of management of the nonimmune hydrops fetalis (NIHF) is stressed in order to achieve an optimal outcome of both the mother and the newborn. Of the two cases reported, both had an underlying cardiac condition. The first was due to rapid supraventricular tachyarrhythmia and responded favorably to intrauterine medication with digoxin and the subsequent delivery of a well baby. In the second case, cesarean section was performed unnecessarily for a fetal indication, since the hydropic infant died soon after birth from a severe cardiac malformation which was not unequivocally diagnosed prior to birth.
