To clarify the changes of vasoactive amines associated with acute hepatic failure, ammonia, tryptophan, serotonin (5-HT) and histamine in the blood and liver were studied in dogs (n = 22) of each three group of acute hepatic ischemia; occlusion of hepatic artery (controls), occlusion of hepatic artery and portal vein (THI), and portocaval shunt with THI (PCS + THI). These biochemical changes were studied in each group at six time intervals: Preocclusion, 15 and 30 minutes postocclusion, and 30, 60 and 120 minutes after release of occlusion. A rapid rise of blood ammonia levels was observed in groups of THI and PCS + THI after occlusion. Blood 5-HT increased in postocclusion of both controls and THI. However, a decrease of 5-HT was observed in PCS + THI. Hepatic 5-HT also increased after occlusion in THI and PCS + THI as compared with a decrease in controls. Plasma histamine rose significantly in all groups after the occlusion. These data demonstrated that the changes of vasoactive amines in hepatic ischemia and/or splanchnic pooling appeared to affect microcirculation of the liver and play a role of pathogenesis of hepatic failure after hepatic ischemia.

The potential of diagnostic and therapeutic applications of a system using hematoporphyrin derivative (HpD) and laser photoradiation for the localization and treatment of canine lung cancer experimentally induced in the bronchi was studied. Seven cases of canine invasive squamous cell carcinoma were used in this experiment. A dose of 2.5-3.0 mg/kg body weight HpD was administered intravenously in each dog forty-eight hours prior to examination for fluorescence. The violet light source was a krypton ion laser beam. The beam was transmitted via a quartz fiber inserted through the fiberoptic bronchoscope, and an image intensifier was used to detect the fluorescence from the bronchial foci. An argon dye laser was used for treatment of the foci with red light. Six of seven dogs with squamous cell carcinoma revealed fluorescence at the foci. Conversely, the bronchi of controls, consisting of normal dogs, showed no fluorescence. Three dogs with squamous cell carcinoma received treatment. The therapeutic system consisted of photoradiation with an argon dye laser beam also transmitted via a quartz fiber inserted through the fiberoptic bronchoscope and a dose of 2.5-3.0 mg/kg body weight HpD was administered intravenously 72 hours before the procedure. All carcinoma cases showed complete responses.

Investigations in an experimental model system and in clinical material indicate that squamous bronchial carcinoma is regularly preceded by the occurrence of bronchial cells with DNA values exceeding significantly the normal 4c region. The studies also show that cytomorphological methods do not allow the discrimination between milder forms of atypia and nonspecific cellular alterations. We describe a combined cytomorphological and cytochemical diagnostic procedure which facilitates the distinction between atypical squamous cells and nonspecific cellular changes in the bronchial tree.

Intracranial granuloma in childhood is considered to be a very rare disorder. A three-year-old child is described who had bilateral intracranial granulomas originating from the ends of bilateral subdural-peritoneal shunts. Serial computerized tomographic examinations revealed variable and peculiar findings. Since subdural-peritoneal shunt is used in neonate, intracranial granuloma must be considered as a potential complication of unremoved subdural-peritoneal shunt.
Membranous obstruction of the inferior vena cava (MOVC) is a rare clinical entity. The incidence is higher in Japan and Africa than that in USA and Europe. MOVC is considered to be different from the Budd-Chiari syndrome. There are differences in clinical manifestation and pathologic changes in the liver. Findings of the liver revealed cirrhosis in all patients but in one. Ascites and esophageal varices could be observed in 79% and in 58% of the patients, respectively. According to Sugiura's classification, they consisted of Type Ia in 63%, Type Ib in 11%, Type II in 11%, and Type III in 16%. Portopulmonary shunt by splenopneumopexy was successfully performed on 19 patients with MOVC in this clinic, of which diagnosis was established by cavography.
Cytochemical determinations of the amount of protein in the cell nucleus together with the amount of DNA give a background for judging the growth activity of the individual cell. A series of human breast carcinomas of different malignancy grades was studied, and it was shown that judged by their types of DNA profiles, the tumors of low malignancy grades were characterized by growth-arrested cells while the majority of cells in highly malignant tumors were intensely growth activated. This indicates that determinations of proteins in the nucleus together with DNA determinations can be used to better judge malignancy grades in individual breast carcinomas, which is of considerable clinical importance.

The ratio of protein to amount of DNA in the nucleus was cytophotometrically determined by means of a combined Feulgen-naphthol yellow S staining procedure. Quiescent normal lymphocytes, bronchial epithelial cells and mammary epithelial cells all showed ratios that were significantly lower than in the proliferating counterparts of the different cell types. Hence, combined determinations of DNA and nuclear protein make it possible to determine the fraction of growth-arrested cells (G0 cells) in cytologic preparations.
CT scan demonstrates the invaluable information about the parenchymal lesions of head injuries. The parenchymal lesions were classified into 6 categories; 1) isodensity without mass effect: I(-), 2) isodensity with mass effect: I(+), 3)high density: H, 4) high-low density complex: H-L, 5) low density: L, 6) diffuse cerebral swelling: DCS. Glasgow coma scale (GCS) and outcome scale (GOS) were international practical scales for the evaluation of severity and prognosis of severe head injuries. One hundred and seventy-four cases with severe head injury were analysed. I(+), H and H-L were common findings in the group of GCS 3-6, and I(-) was in GCS 7-12 and GCS 13-15. H and H-L were not related with GCS. DCS was most common in GCS 7-12. Acute epidural hematoma was frequent in the group of GCS 13-15, and acute subdural hematoma was in GCS 3-6. The prognosis was significantly poor in the group of GCS 3-6, with the mortality of 72 percents. On the other hand, the prognosis was quite good in GCS 7-12 and GCS 13-15. There were few reports about the traumatic subarachnoid hemorrhage (SAH). SAH was one of the important risk factors in severe head injuries and it was frequently associated with I(+), H and H-L. The prognosis of the patients with SAH was most unfavorable in the presence of I(+). Number of analyses were reported about the traumatic intraventricular hemorrhage (IVH). IVH was also one of the powerful risk factors and this was commonly associated with H and H-L. The prognosis of the patients with IVH was very poor. Finally, the groups of the patients, whose prognoses turned out to be unexpected results from GCS on admission, were analysed. First, the age was an important factor. In the patients, whose prognoses were good in spite of low GCS, I(-) was a mostly common finding, while SAH, IVH and obscured cisterns, esp. basal and quadrigeminal, were less common. In the patients, whose prognoses were poor despite of favorable GCS, H and H-L were common findings. SAH and IVH were also common. The poor prognosis was induced by secondary systemic complications, such as pneumonia and meningitis, etc.
