The prognosis for patients referred with suspected acute myocardial infarction.

A follow-up investigation of the prognosis of 381 patients admitted with suspected acute myocardial infarction (AMI) has been carried out in respect of later AMI or death. During hospitalization the patients were divided into groups with particular attention to patients with no demonstrable myocardial infarction but with ischaemic heart disease (non-AMI) and patients with confirmed AMI. All patients were subjected to follow-up for 43 months (range 37-54). The mortality from cardiovascular causes after four years was 26.2% of 130 non-AMI patients and 25.8% of AMI patients. The majority of new infarctions were found in the AMI patients, but with even increase in both groups, 50% occurring within the first 12 months. The groups were studied with regard to earlier manifestations of ischaemic heart disease and heart failure during hospitalization, without any difference being observed. Due to the poor prognosis the question is raised whether non-AMI patients as a group should be offered prophylactic therapy.

Propyl ether. I. Interaction with the sensory irritant receptor.

The sensory irritating response of propyl ether, measured as a decrease in respiratory rate in mice, faded within one minute. The threshold was 620 p.p.m., calculated from the concentration-response curve, which had a low slope. The sensory irritating level depressing the respiratory rate to 50 per cent (RD-50 = 89,000 p.p.m.) could not be reached with saturated vapour at 20 degrees. The thermodynamic activity corresponding to the RD-50 was about 0.6, a rather high value. The low slope and high thermodynamic activity may be explained by steric hindrance of the oxygen atom and thereby decreasing the receptor-activating capacity and/or the stability of the active receptor conformation. A specific effect beyond the receptor offers another explanation.

Propyl ether. II. Pulmonary irritation and anaesthesia.

After cessation of the sensory irritation response, described in part one (Nielsen et al. 1985), a mixed response due to pulmonary irritation and anaesthesia appeared, resulting a decrease in respiratory rate and tidal volume. The corresponding thresholds were 1490 p.p.m. and 1280 p.p.m., respectively, obtained from the last half hour of a four hours exposure period. As the decrease in respiratory rate and tidal volume was found to be correlated, the per cent decrease in respiratory minute volume could be approximated from the square of the respiratory frequency. A threshold limit value (TLV) about 100 p.p.m. based on pulmonary irritation and anaesthesia was proposed. No impurities in the technical grade ether influenced the responses.

Carbon isotopic fractionation in heterotrophic microbial metabolism.

Differences in the natural-abundance carbon stable isotopic compositions between products from aerobic cultures of Escherichia coli K-12 were measured. Respired CO2 was 3.4% depleted in 13C relative to the glucose used as the carbon source, whereas the acetate was 12.3% enriched in 13C. The acetate 13C enrichment was solely in the carboxyl group. Even though the total cellular carbon was only 0.6% depleted in 13C, intracellular components exhibited a significant isotopic heterogeneity. The protein and lipid fractions were -1.1 and -2.7%, respectively. Aspartic and glutamic acids were -1.6 and +2.7%, respectively, yet citrate was isotopically identical to the glucose. Probable sites of carbon isotopic fractionation include the enzyme, phosphotransacetylase, and the Krebs cycle.

Interaction of alcohol and tobacco as risk factors in cancer of the laryngeal region.

The aim of this study is to present risk assessments for the combined effect of alcohol and tobacco in cancer of the larynx. The case control study included all newly diagnosed laryngeal cancer patients under the age of 75 in Denmark during the years 1980-2. Four age and sex matched controls were selected using the municipal person registry in which the case was listed. Ninety six per cent of all cases and 78% of controls participated in the study, which is based on 326 cases and 1134 controls. Information on alcohol consumption and tobacco use was obtained by means of mailed questionnaires. For all laryngeal cancers as well as for the subgroups concerning cancer of the glottis and supraglottis alcohol consumption and tobacco use were found to be important risk factors. The effect of joint exposure was greater than the effect predicted from the sum of effects of each factor acting separately. Thus the combined effect follows a multiplicative rather than additive model.

Effect of combined alcohol and tobacco exposure on risk of cancer of the hypopharynx.

The effect of moderate to heavy exposure to alcohol and tobacco on the risk of cancer of the hypopharynx is estimated in a case-control study. All incident hypopharyngeal cancer patients in Denmark who were under 75 years of age were compared with incident cases of laryngeal cancer and population sampled controls. The ascertainment of cases and controls took place during March 1980 to March 1982. Thirty-two patients with cancer of the hypopharynx, 321 patients with cancer of the larynx, and 1141 population sampled controls participated in the study. The effect of combined exposure to alcohol and tobacco seemed to be multiplicative rather than additive, a finding which is similar to that in laryngeal cancer patients. However, as there were only small numbers, only major deviations from the additive or multiplicative model for interaction would be detectable.

Self-reported data on spontaneous abortions compared with data obtained by computer linkage with the hospital registry.

In a study of occupational causes of spontaneous abortions, based upon self-reported data and data from the hospital registry, evidence of differential misclassification was noted. Among those exposed a larger proportion of the self-reported spontaneous abortions were identified in the hospital registry, compared with what was found in the control group. This could be due to recall bias of the questionnaire data masking an effect of exposure, or a lower threshold for hospitalization among those exposed vis-à-vis controls, which would exaggerate the effect of exposure, if any. The analysis tended to support the idea of a less accurate recall of spontaneous abortions among controls, especially for abortion that occurred more than 3 years before the questionnaires were sent out. A second questionnaire was sent out to a subset of the participants 3 1/2 years after the first questionnaire. 17% reported fewer spontaneous abortions in this second questionnaire compared with the situation in the first questionnaire, for the period 1973 to 1980.

Effects of electronic fetal monitoring on rates of early neonatal death, low Apgar score, and cesarean section.

Using data from the University Hospital in Odense, Denmark, we have studied the effects of electronic fetal monitoring on rates of early neonatal death (death of liveborn within first week of life), low five-minute Apgar score (less than 7), and cesarean section. Approximately 22 000 births occurring in 1974 - 82 were studied, of which 13% were electronically monitored. Monitoring was significantly associated with an elevated cesarean rate in low-risk labors and certain high-risk labors, but appeared not to increase the cesarean rate in all high-risk labors. In high-risk labors, monitoring was associated with an approximate 40% reduction in rates of low Apgar score and early neonatal death, but neither reduction was statistically significant. The associations of monitoring with elevated cesarean rate declined significantly over the study period. These results accord closely with studies conducted in the United States, indicating that monitoring is of benefit in high-risk labors, but leads to excessive cesarean deliveries in low-risk labors. Our study additionally indicates that the effect of monitoring on the cesarean rate has been declining over time.

Residual retroperitoneal tumour tissue in patients treated for metastatic non-seminomatous testicular germ cell tumours: an immunohistochemical investigation.

The morphology of resected residual retroperitoneal tumour tissue from 18 patients treated with a combined chemotherapy regime for advanced testicular non-seminomatous germ cell tumours was studied. In five cases (28%) the resected tissue comprised only fibrous tissue and in ten cases (56%) only mature teratoma (T) was present. Embryonal carcinoma (EC) with yolk sac tumour (YST) differentiation was found in addition to T in one case and in two cases the resected tissue comprised pure EC. In all patients with residual T, T had also been present in the primary tumour. Resected tissue containing T was investigated for the presence of various marker proteins, including alpha-1-antitrypsin (A1 AT), carcino-embryonic antigen (CEA), ferritin (FER), lactoferrin (LF), and pregnancy-specific beta-1-glycoprotein (SP1), in addition to the well-established markers for germ cell tumours, alphafetoprotein (AFP) and human chorionic gonadotropin (HCG). AFP and HCG were present in only two cases. A1 AT and CEA were demonstrated in various amounts in epithelial structures in 11 out of 11 cases with T, while FER was found in ten and LF and SP1 in seven cases. Since A1 AT, CEA and LF were also found in the secreted material within the lumen of the teratoid structures, aspiration of cystic fluid for demonstration of these proteins in addition to AFP and HCG is recommended for diagnostic assessment. CEA and SP1 are suggested for localization and treatment of tumour tissue with the recently-developed methods using specific antibodies which are either radiolabelled or conjugated to anti-neoplastic drugs.

Epornitic of papova-like virus-associated disease in a psittacine nursery.

Of 45 fledgling psittacine birds being raised in an avian nursery, 14 died over a 6-week period. At necropsy, several birds were found to have a moderate amount of subcutaneous hemorrhage over the crop and across the dorsum. Histologically, all birds had hepatic necrosis. Large pale to lightly basophilic intranuclear inclusions were seen in the spleen of 6 birds and in the liver of 5 birds examined. Ultrastructural evaluation demonstrated intranuclear virus particles resembling the papovavirus incriminated as the agent responsible for budgerigar fledgling disease.

A gamma-detecting probe for radioimmune detection of CEA-producing tumors. Successful experimental use and clinical case report.

The detection of tumors with radiolabeled antibodies against CEA is possible; however, current nuclear medicine scanning cameras rarely detect tumors smaller than 2 cm in diameter. One of the limitations to tumor detection is the inability to place a detecting camera near a deeply seated intra-abdominal tumor. A hand-held gamma-detecting probe, suitable for intraoperative use, was designed to locate radioactive tumors. Experimental work with CEA-producing colon tumor xenografts in nude mice suggests this probe is more sensitive than external scanners in detecting small tumors. A case report documents the clinical use of this new intraoperative probe.

Organochlorine compounds in human breast fat from deceased with and without breast cancer and in a biopsy material from newly diagnosed patients undergoing breast surgery.

Epidemiological studies have related the incidence of mammary cancer to the dietary intake of fat and/or meat. Since organochlorine compounds (e.g., polychlorinated biphenyls (PCB) and DDT (and its metabolite DDE] are accumulated in the adipose tissue it was tempting to suggest a relationship between levels of PCB and "DDT" (i.e., DDT + DDE) in breast fat tissue and the occurrence of mammary cancer. To elucidate this theory, the organochlorine levels of 14 breast fat tissue samples from breast cancer patients and similar samples from 18 deceased mammary cancer patients were compared to that of 21 similar samples from noncancer patients and finally to adipose tissue samples from 35 non-cancer autopsy specimens. No significant differences were traced. Thus it seems that the accumulation of PCB and DDT measured in breast fat tissue do not relate to the occurrence of mammary cancer.

Welding and cancer of the larynx: a case-control study.

The purpose of the study was to examine the occupational history of laryngeal cancer patients, and especially their exposure to welding. The investigation was conducted as a case-control study where all newly diagnosed patients less than 75 yr of age with cancer of the larynx in Denmark during March 1980 to March 1982 were selected as cases. For each case, four age- and sex-matched controls were identified from the municipal person register in which the case was listed. Data were collected partly by means of questionnaires and partly by abstracting information from the medical records of cases. Workers exposed to welding fumes had a slightly increased risk of cancer of the larynx, most predominantly of cancer of the subglottic area.
