[Monotherapy of epileptic children with monitoring of the serum levels of anticonvulsive drugs].

In 46 children with partial and primarily generalized seizures monotherapy was started with amizepine, luminal, mysodin or phenytoin. The treatment was monitored determining serum concentrations of the drugs. Monotherapy was effective in 74% of cases. In 89% of them the duration of treatment was one year, at least. In some children the values of amizepine, luminal and mysodin concentrations below the therapeutic level were effective. The evolution of electroencephalographic changes failed to correspond strictly to the clinical results of the treatment.

[Effect of gamma-hydroxybutyrate on EEG pattern in epilepsy. Preliminary report].

Investigations were carried out in 50 patients with epilepsy who had been subjected previously to careful clinical examination. For the investigations the preparation Gamma-OH EGIC was used in doses of 0.03 g/kg of body weight in rapid intravenous injection. The preparation modified rapidly the EEG tracings which were divided into 4 phases: 1) phase of initial resting record, 2) disappearance of alpha rhythm with increase of medium and low-voltage theta rhythm, 3) high-voltage phase resembling sleep activity with prevalence of delta rhythm, 4) phase of desynchronization--return of normal activity. Out of 50 epileptics in 31 cases temporal-lobe epilepsy was present, in 15 the seizures and EEG changes were generalized, in 4 cases focal non-temporal-lobe epilepsy was recognized. Administration of Gamma-OH to epileptics caused in 100% of cases a modification of the initial tracings and in 84% activation of EEG changes mainly with activation of focal slow-wave activity; in cases of minor absence attacks the drug always induced paroksysmal activity. The preparation is safe and non-toxic and may be used in outpatient clinics. It seems that activation of EEG changes in epileptics by Gamma-OH administration may be due to disturbances of the equilibrium between the structures activating and inhibiting the reticular formation of the brain stem and cerebral cortex.

[Effect of brietal and gamma-OH on the bioelectric activity of the cerebral cortex and deep temporal structures in SEEG studies in patients with epilepsy].

Eleven SEEG investigations were analysed in patients with temporal epilepsy after separate intravenous injection of Brietal and gamma-OH. The authors discuss a modification of the background activity and pathological seizure discharges in the cortex and deep structures of the temporal lobes. Attention is called to an evident increase of the quantitative and qualitative elements of excitation or manifestation of seizure activity not present previously. The activating effect of the these preparations on the pathological bioelectric activity without presence of clinically observable seizures suggests the possibility of existence of inhibitory mechanisms at the levels below the cortex and limbic system, probably in the reticular system of the brain stem. It was found that in the used doses the administration of both preparations was completely safe. Brietal and gamma-OH are good preparations facilitating more accurate diagnosis of epilepsy.

[Effect of electric stimulation of the amygdaloid body and hippocampus on bioelectrical activity recorded from these structures and from skull surface in epilepsy].

The authors discuss the morphology of records of spontaneous and stimulated bioelectric activity in an analysis of selected 15 EEG records using electric stimulation of deep temporal structures using relatively low parameters of stimulation (optimal: 2--3 V, 50 Hz, 1 msec, duration 5 sec). Attention is called also to the fact that seizure activity is absent in leads from skull surface during long-lasting bioelectric seizure activity in deep cerebral structures. It is concluded that induction of afterdischarges and analysis of the directions of their spread may be of practical importance from the point of view of diagnosis and therapy. The authors state also that induction of bioelectric afterdischarges in deep temporal structures, particularly in the hippocampus caused in these patients no disturbances of consciousness and memory.

[Class II HLA antigens in patients with multiple sclerosis from Warsaw and Gdansk regions].

An investigation of class II histocompatibility antigens was carried out in patients with definite diagnosis of multiple sclerosis (MS) according to Poser's criteria. There were 42 patients from Warsaw and 98 patients from Gdańsk. Histocompatibility antigens were assayed with microlymphocytotoxicity test. 6 antigens on locus DR and 1 antigen on locus DQ were evaluated. The antigen frequencies were compared in the two groups using the chi-square test. The antigen HLA-DQw1 occurred significantly more frequently in MS patients compared to healthy control group. Antigens HLA-DR2 and DR3 occurred more frequently in patients from Gdańsk than Warsaw region.
