Arthroscopic lavage compared with nonoperative treatment for traumatic primary anterior shoulder dislocation: a 2-year follow-up of a prospective randomized study.

A prospective randomized study was performed on 30 consecutive patients with traumatic primary anterior shoulder dislocation to compare treatment results of arthroscopic lavage with results of conventional nonoperative treatment. The patients were between 18 and 30 years of age and had no history of shoulder problems. At the 2-year follow-up, 3 (20%) of 15 patients in the lavage group had a redislocated shoulder compared with 9 (60%) of 15 patients in the nonoperative group (P = .03). Two of the patients in the lavage group compared with 6 of the patients in the control group had been operated on or were scheduled for stabilizing surgery. Functional outcome according to the Constant and Rowe shoulder scores did not reveal any significant difference (P = .07) between the groups. However, by the Rowe classification 2 of 15 patients in the lavage group had poor results versus 8 of 15 in the control group, indicating an advantage for the lavage treatment. The study showed that arthroscopic lavage reduced the risk for recurrent dislocation when compared with nonoperative treatment.

Triiodothyronine (T3), insulin and characteristics of 5'-monodeiodinase (5'-MD) in mare's milk from parturition to 21 days post-partum.

It is generally accepted that hormones and tissue growth factors are supplied from mother to neonate via mammary secretion. Among the protein hormones, insulin and prolactin are considered as the most important milk components for neonates. The significance of the thyroid hormones, namely triiodothyronine (T3) generated locally by 5'-monodeiodinase (5'-MD) in the mammary tissues, for the mammary gland itself and for suckling neonates is still under consideration. In the present study the activity of the 5'-MD and the concentrations of T3 and insulin in mare's colostrum and milk during the first 21 days of lactation were measured. Post partum, T3 increased to its highest concentration around day 4 (1.14+/-0.08 nmol/L), then progressively decreased until day 7, reaching a relatively stable concentration of 0.71+/-0.06 nmol/L (overall mean for days 7-21). The colostral insulin concentration, highest on the day of parturition (401.0 +/-24.9 microU/mL), decreased to a nadir value (25.0+/-3.4 microU/mL) on day 5, after which it tended to increase. The mare's milk showed the presence of PTU-sensitive (type I) and PTU-insensitive (type II) 5'-monodeiodinases (5'-MD). Contrary to the classical type II 5'-MD, the mare's milk isoenzyme was inhibited non-competitively by aurothioglucose. A significant relationship (r=0.962, P < 0.01 ) between T3 concentration and 5'-MD activity, from the I st to the 6th lactational day was found, which may indicate a dependence of T3 concentration on the milk 5'-MD activity. The presence of 5'-MD of type II suggests that intra-mammary T3 generation may play a paracrine role supporting lactogenesis. Estimating that 1.8 microg of colostral T3 (0.456 microg/L) is consumed daily by a suckling foal, the T3 hormone action within the intestinal tract cannot be ruled out. This is the first paper to provide evidence of T3 and insulin concentrations, and of T4-5'-monodeiodinases activity in colostrum and milk of the mare.

Bone and joint pathology in fluoride-exposed workers.

Clinical and radiological investigations were performed for 2,258 aluminum workers exposed to fluoride for an average of 17.6 yr (standard deviation = 7.6). Changes in bone and joints were presented in detail in three groups: (1) exposed up to 5 yr (135 cases), (2) exposed from 6-32 yr (1,463 cases), and (3) retired workers (660 cases). A semiquantitative assessment of early fluorosis was introduced. A 20.2% incidence of fluorosis was found, but, according to Roholm, only 1.05% was in stage I. The disease was mainly in the pre-stages of O and OI. A close relationship between the occurrence of fluorosis and the time and degree of fluoride exposure was found. The difficulties in diagnosing skeletal fluorosis result from the questionable sensitivity of the x-ray techniques and from the nonspecificity of the associated symptoms. A quantitative method to assess osteosclerosis and bone structure alteration is needed.

Beat-to-beat QRS amplitude variability during dobutamine infusion in patients with coronary artery disease.

The aim of this study was to investigate if provoked myocardial ischemia induces increased beat-to-beat QRS amplitude variability in patients with angiographically verified coronary artery disease. 15 patients (median age 62 years, range 46-73 years) and 10 healthy controls (median age 25 years, range 22-42 years) were studied. Dobutamine was infused intravenously at a low and at a high dose. The mean low dose of the drug was 10.0 micrograms/kg/min for both patients and controls, whereas the mean maximum dose was 31 +/- 2 for patients and 38 +/- 1 microgram/kg/min for controls. The total QRS amplitude beat-to-beat variance from 12 leads as well as individual variance scores in each single lead were evaluated. Before infusion, the total QRS variance did not differ between patients and controls, nor did the individual variance in 9 of the 12 ECG leads. Dobutamine elicited an increase (p < 0.01) in the total QRS variance, with significantly higher (p < 0.001) total variance in patients than in controls. At the high dose of the drug, the patients displayed significantly higher individual variance values in each ECG lead as well. During dobutamine infusion, 7 of 15 patients developed ST depressions (> or = 0.1 mV in > or = 2 leads) in 12-lead ECG readings. Eleven of 15 patients developed chest pain (grade > or = 3 at the Borg's CR-10 scale). In conclusion, in patients with ischemic heart disease, dobutamine-provoked stress gives rise to increased QRS amplitude beat-to-beat variability, as a sign of electrical instability of the myocardium.

Studies on the renal kinetics of growth hormone (GH) and on the GH receptor and related effects in animals.

Growth hormone (GH) is filtered through the kidney, and may exert effects on renal function when presented via the circulation. Investigations on kidney-related aspects of GH are increasing in number. Using in vitro and in vivo approaches, the present study attempted to provide answers to a number of unresolved or debated issues. In vitro, we detected both GH and type 1 IGF receptors (R) in a porcine renal epithelial cell line. The saturation and down regulation kinetics of the GH-R indicate that it has the properties of a classical GH-R. Furthermore, the simultaneous presence of GH-R and IGF-R on a phenotypically homogeneous cell line suggests the presence of GH-induced auto-/paracrine IGF-1 bioactivity in the kidney. Experiments with isolated proximal rabbit tubules incubated with physiological concentrations of 125I-GH demonstrated a time-and dose-dependent increase in unlabelled GH-displaceable cell-associated radioactivity, lending support to the concept of GH mediating its renal effects via proximal tubular GH-R. Short term administration of GH to rats and humans elicited electrolyte and water retention that may cause edema in adults. In the present study, long term administration of GH to rats caused only a minor increase in serum phosphate levels, with no changes observed in the renal electrolyte clearance. During the first 4 days of GH treatment in rats, no change in plasma renin activity was detected and we were thus unable to confirm the hypothesis that the renin-angiotensin system is responsible for the early phase of GH-associated fluid retention. Pharmacokinetically, when GH was administered to rats with functional disconnection of the kidneys as a model of renal insufficiency, the whole body clearance of GH decreased by ca. two thirds, and was reflected by an increase in the mean residence time and AUCplasma for GH. The plasma half-life, however, was not significantly affected, suggesting that the volume of distribution (Vd) had decreased for the GH administered to the renally compromised animals. A renal contribution to the Vd was visualized as intense radioactive staining in the kidney region on whole body autoradiographs (WBA) of rats dosed with 125I-labelled hGH. The liver region was also intensely stained. Kidney-associated radioactivity was found to be related not only to glomerular filtration, but also to peritubular uptake, since the renal clearance of free GH was found to exceed the GFR.(ABSTRACT TRUNCATED AT 400 WORDS)

B and T lymphocytes in viral hepatitis.

Peripheral blood lymphocytes were investigated in 20 patients with acute viral hepatitis. For assessment of B lymphocytes the rosette test was carried out using sheep erythrocytes coated with antibodies and murine complement. T lymphocytes were recognized by their ability of binding uncoated sheep erythrocytes. In 16 cases the results of EAC test in patients with viral hepatitis resembled those in healthy donors. In 3 cases a significant decrease was observed and in one case a significant rise was found in the number of cells forming EAC rosettes. The T lymphocyte count was within normal range in 19 investigated cases. The results suggest that impaired T lymphocyte reactivity to PHA in acute viral hepatitis is due to a disturbed function of T lymphocytes and not to a decrease in their number. In cases associated with significant hepatocellular damage a reduction in the number of circulating B lymphocytes was observed.

[Expression of surface antigens on lymphocytes stimulated by non- specific mitogens in multiple sclerosis patients].

In studies performed on 30 MS cases, as compared to 16 healthy individuals, significantly higher expression of HLA class II antigens, interleukin 2 receptor and antigens specific for lymphocytes B was observed. In MS patients after stimulation by non specific mitogens increased expression of HLA class II antigens and increased proliferation induced by PMA was found. The obtained results suggest absence of any greater modulation of surface antigens on the lymphocytes stimulated by non-specific mitogens. Our observations indicate that the number of activated T cells, as assessed by expression of Il-2 receptor, is in MS patients relatively low.

[Our first experience with allogeneic and autologous transplantation of hematopoietic stem cells].

The main problems and difficulties in transplantation of haematogenous stem cells are discussed. The problems include: organization of a team of specialists and bone marrow transplantation centre, preparation of the recipient (patient) complications connected with preparation for transplantation, graft taking, graft versus host disease, recurrence of the underlying disease. In the analysis only those patients (8 cases) are included who had received marrow transplantation after chemotherapeutic-radiotherapeutic preparation (3 patients are alive). The first experiences are described also with three autotransplantations of haematogenous stem cells obtained by means of cell separator from peripheral blood. These experiences indicate that it is possible in our country to carry out this highly sophisticated method which requires, however, further improvements and ensuring of adequate personnel and equipment.

[Incidence of antigen expression for HLA antigens A,B,C and DR in patients with atopic diseases in Poland].

Studies of several authors suggest an interdependence of atopic diseases and HLA antigens. Therefore, the authors evaluated an incidence of HLA antigens of classes I and II in patients with atopic disease and healthy individuals. The study involved 127 patients with atopy and 79 healthy blood donors. Anamnesis, results of skin tests and serum levels of allergenic-specific IgE served as the base of atopy diagnosis or exclusion. HLA antigens were assayed with a standard micro-lymphocytotoxic test using two series of standard anti-HLA sera. It enabled to detect 16 locus A-dependent antigens, 25 locus B-dependent and 8 locus C-dependent antigens as well as 8 antigens dependent on locus DR. Only antigen HLA-Bw53 was found more frequently in patients with atopic diseases suggesting its relation to atopy. The results concerning other HLA antigens did not confirm HLA antigens association with atopy suggested by other authors.

[Erythrocyte complement receptor (C3b) expression in multiple sclerosis].

The study was carried out in 121 patients with multiple sclerosis and a control group of 519 healthy individuals. In 13 cases of multiple sclerosis and 12 controls family studies were carried out. Complement receptors (C3bR) on the erythrocytes were demonstrated by the haemagglutination test using human IgG heat-aggregated and guinea pig serum which served as a source of complement. On the basis of haemagglutination intensity and results of radioimmunoassay three phenotypes of complement receptor were isolated: high HH(C3bRh/C3bRh) producing strong agglutination, medium HL (C3bRh/Cbl) producing weak agglutination, and low phenotype LL (C3bR1/Cbl) in which no visible agglutination of erythrocytes was noted. Considerable differences were observed in the distribution of these phenotypes between controls and multiple sclerosis patients in whom the low phenotype (C3bR1/C3bR1) was more frequent. Family studies of controls suggested presence of two codominant alleles C3bRh and C3bR1 with Mendelian inheritance while similar studies of multiple sclerosis patients it was found that e.g. a child with multiple sclerosis whose parents had high C3bR phenotypes, that it were homozygotes for the C3bRh gene, had low phenotype. On the other hand, the high phenotype was found in a child of a female multiple sclerosis patient with the low phenotype (C3bR1/C3bR1). These observations suggest that reduced expression of the complement receptor on the erythrocytes depends in multiple sclerosis on the disease process in the first place, and not on genetic factors.

Insulin and glucose concentration changes in newborn piglets after suckling the colostrum from insulin administered sows.

Immunoreactive insulin (IRI) concentration in sows colostrum has been previously proven to be much higher than that in blood. The experiment was carried out to show the influence of endogenous and added insulin in sows colostrum on insulinaemia and glycaemia of newborn piglets. In colostrum collected from 3 control and 5 experimental sows before loading, basal insulin concentration were 1.595 and 1.365 nM-1, respectively, and calculated for all 8 sows together were 1.451 nM-1 (SEM +/- 0.289). Basal plasma insulin concentrations calculated for 68 healthy piglets before sucking were little differentiated (mean 0.318 +/- 0.044 nM l-1), whereas glucose initial concentrations for those piglets (mean 3.581 +/- 0.275 nM l-1) were highly differentiated. Intramuscular loading of 5 experimental sows with insulin (80 I.U. per animal) caused an increase in the concentration of insulin in colostrum from 1.365 to 3.449 nM l-1 (0.01, P less than 0.02). The mean insulin level (0.313 +/- 0.04 nM l-1) in experimental piglets blood plasma (n = 42) increased significantly to 1.234 +/- 0.07 (P less than 0.001) after suckling by sows loaded with exogenous insulin. Glycaemic response of those two piglets litters was poor but showed a statistically significant increase (P less than 0.001). The glucose concentrations in blood plasma samples of the other three litters did not changes after sucking. The experiment excluded the hypothesis that high level of insulin in colostrum could be the cause of hypoglycaemia in healthy piglets after sucking.

[Evaluation of the stability of vincristine binding to blood platelets by the method of spin labeling of platelet membrane].

N-(1-oxyl-2,2,6,6-tetramethylpriperyd-4-y) maleimide (MSL) labels SH or NH2 groups of blood platelets membrane proteins in covalent bonds. Then EPR spectrum of MSL labeled platelets solutions can be measured. Pre-incubation + of the platelets with vincristine prior to MSL labeling induces SH and NK2 groups blocking, decrease in MSL binding to the platelets membrane and EPR spectrum modification of labeled platelets solution. The vincristine effect on platelets membrane lasts until platelets disintegrate. These data confirm, the stability of vincristine bond with platelets membrane.

Changes of insulin concentration in colostrum and milk of women, cows and sows.

The method of whey extraction in order to determine insulin concentration in colostrum and milk was worked out, checked and applied. The observation of insulin changes showed high and similar insulin concentration in whey samples of women, cows and sows colostrum on the day before and in the day of parturition. One day after delivery hormone concentration maintained high in case of women and sows but in cow colostrum it decreased to 1/12 comparing to the level on the parturition day. During the postpartum following days it was decreasing gradually in the colostrum of all three examined species; the process was very fast in cows, slower in women and the slowest in sows.

Influence of increased environmental temperature on oxidation processes in rat liver mitochondria.

Exposure of rats to elevated temperature of 28 degrees C or 35 degrees C for 3 days six hours daily resulted in a decreased rate of oxidation with succinate or glutamate + malate as substrates, by the mitochondria of liver. The higher decrease was observed in environment temperature of 35 degrees C. There was no change in ADP/O ratio. The activities of NADH: cytochrome c reductase and cytochrome oxidase were stimulated but activities of succinate dehydrogenase and succinate cytochrome reductase were decreased.
