Patterns of walker use and walker injury.

Infant walkers have been described as a cause of unexpected trauma in the first year of life. We conducted a prospective study to determine the mechanism and pattern of walker injuries presenting to an urban pediatric teaching hospital. We also studied the patterns of walker use. All injuries sustained by infants in baby walkers during a 1-year period were reviewed. The 139 injuries included 29 fractures. The most severe injuries were caused by falls down stairs; these falls accounted for 123 of the injuries. Stair gates were present in one third of all falls. At follow-up 2 months later, two thirds of the children had been in the walker subsequent to the injury. One third were still in the walker at 2 months after their initial injury. Less than half of the homes that had not had stair gates in place had acquired them at the time of follow-up. Baby walkers represent a cause of significant injury in the infant population; studies of warning labels and anticipatory guidance are needed.

Small bowel biopsy for malabsorption: comparison of the diagnostic adequacy of endoscopic forceps and capsule biopsy specimens.

Biopsy specimens of the small bowel were obtained from 40 patients suspected of having malabsorption. Four different techniques were used at a single session--namely, endoscopic biopsy of the descending duodenum using paediatric and standard size forceps and suction capsule biopsy of the descending duodenum and the proximal jejunum. Specimens were compared for size, adequacy, and ability to confirm or exclude mucosal abnormality. Fourteen patients had villous atrophy. In all patients four biopsy specimens were obtained with paediatric endoscopic forceps and four with standard endoscopic forceps. No capsule biopsy specimen was retrieved from the duodenum in three patients and from the jejunum in five patients. Specimens were considered to be adequate in 36 patients when paediatric forceps were used, in 39 when standard forceps were used, in 28 on duodenal capsule biopsy, and in 32 on jejunal capsule biopsy. This study indicates that the most reliable method for diagnosing or excluding villous atrophy is endoscopic forceps biopsy of the descending duodenum, provided that at least four specimens are obtained with standard size forceps.

Evaluation of sponging to reduce body temperature in febrile children.

A study was conducted to evaluate the efficacy of sponging as a way of reducing body temperature in febrile children. Of 130 children 73 received antipyretic medication and sponging and 57 received antipyretic medication alone. No difference in temperature reduction was noted between the two groups. It is therefore suggested that sponging be abandoned as a mode of temperature reduction in febrile children whose increased temperature is due to an infectious process.

"Nursemaid's elbow" in infants six months and under.

Six instances of subluxation of the radial head ("nursemaid's elbow, pulled elbow") in babies in the first 6 months of life are presented. In four cases, the pulled elbow was produced in a previously unreported fashion.

Comparison of weight, depth, and diagnostic adequacy of specimens obtained with 16 different biopsy forceps designed for upper gastrointestinal endoscopy.

The influence of endoscopic forceps variables (size, design, and make) on biopsy specimen weight, depth, and diagnostic adequacy has been studied in vitro on normal human stomach, and in vivo at endoscopy in dog and in patients. Greater pressure during the biopsy procedure yielded significantly heavier, deeper, and histologically more acceptable specimens. Standard sized forceps (diameter 2.4 mm) and those with ellipsoid cups consistently produced larger specimens than the smaller 'paediatric' forceps (1.8 mm) and those with round cups. Deeper specimens were also obtained using the standard forceps. In vitro and in vivo in the dog, the standard sized forceps also produced specimens of greater diagnostic adequacy than the paediatric forceps. At endoscopy in patients, however, we could not detect any influence of the size, shape, and presence of forceps spike or fenestration on the diagnostic adequacy of the specimens.

Transitional cell carcinoma of the upper renal tract in rubber workers.

We report two patients where transitional cell carcinoma of the upper urinary tract developed after occupational exposure to known carcinogens, in the rubber industry. Although carcinoma of the bladder is well recognized in this context, to our knowledge, this association has not been reported previously.

Comparison of hepatic elimination of different forms of cholecystokinin in dogs. Bioassay and radioimmunoassay comparisons of cholecystokinin-8-sulfate and -33-sulfate.

The influence of hepatic transit on the ability of exogenous cholecystokinin-8-sulfate and -33-sulfate (CCK-8 and CCK-33, respectively) to stimulate gallbladder contraction and exocrine pancreatic secretion, as well as on the peripheral plasma concentration of each agent, was evaluated in five conscious dogs with pancreatic and gallbladder fistulas and complete portacaval transposition. The gallbladder pressure increments after portal administration of CCK-8 (0.125, 0.25, 0.50, and 1.0 microgram/kg per h for 5 min) were diminished by 36, 45, 39 and 25%, respectively, in comparison with those obtained with systemic administration of identical doses of CCK-8 (P less than 0.05). In a subsequent experiment, the integrated pancreatic juice volume, bicarbonate, and protein secretion were diminished by 22, 32, and 48%, respectively, during a 30-min infusion of CCK-8 (0.10 micrograms/kg per h) into the portal venous system, in comparison with the results obtained with systemic administration of CCK-8 (P less than 0.05). In contrast, the gallbladder pressure and pancreatic exocrine secretory responses to portal administration of CCK-33 did not differ significantly (P greater than 0.05) from the results obtained with systemic administration of CCK-33. Radioimmunoassay for CCK-8 in plasma showed that the integrated CCK-8 value during portal administration was significantly lower (P less than 0.05) than it was during systemic administration. The results for CCK-33, however, did not vary, whether it was given by a systemic or portal route (P greater than 0.05). Thus, the present study demonstrates that CCK-8 is partially inactivated by the liver whereas CCK-33 is not, which suggests that CCK-3 in the circulation may play a significant role in the physiologic regulation of the gallbladder and exocrine pancreas.

Facial profileplasty by liposuction extraction.

Facial profileplasty can include the removal of abnormal fat deposits in the face and neck as an individual begins to age and, in many cases, the correction of chin projection to achieve a better neck-chin contour line. In this report we retrospectively analyze the complications and results from 142 patients undergoing facial liposuction alone or in combination with chin augmentation or rhytidectomy during a follow-up period of 14 months. The liposuction technique offers very low morbidity and is superior to conventional excisional methods.

Widespread mucous metaplasia of the urinary bladder with nephrogenic adenoma.

We describe widespread metaplastic changes observed in nonexstrophic urinary bladder mucosa in a 32-year-old man. Extensive areas of the luminal surface were covered by two types of metaplastic epithelium. One type was bilayered while the other was single layered with columnar cells of varying electron density as well as mucus-secreting cells. An area of adenomatoid metaplasia was also observed.

Idiopathic hydronephrosis. Light microscopic features and pathogenesis.

A histological study of surgical specimens of idiopathic hydronephrosis demonstrated an abnormal muscle arrangement at the ureteropelvic junction in 18 of 26 cases. We believe this abnormality to be the cause in these cases. At this junction, the muscle bundles, instead of displaying the normal interwoven (braided) pattern, are arranged into an outer circular and an inner longitudinal layer. This abnormal muscle arrangement can be attributed to local failure of the physiological uncoiling during growth and development.

Correlation of in vitro measurements of contractility of the gallbladder with in vivo ultrasonographic findings in patients with gallstones.

The objective of this study was to examine the in vitro responsiveness to cholecystokinin-8 of gallbladder muscle strips from patients with gallstones and to correlate the findings with preoperative ultrasonographic studies of gallbladder contractility. The response of gallbladder muscle strips to cholecystokinin-8 of patients with noncontracting gallbladders was significantly reduced in comparison to that of patients with gallstones whose gallbladders contracted in response to fat.

Amorphic lenses: a mobility aid for patients with retinitis pigmentosa.

An amorphic lens, designed by William Feinbloom, was tested on 69 patients with retinitis pigmentosa. The results showed that one-half of the patients experienced immediate improvement in mobility and one-third of the patients fitted with the amorphic lens reported (after wearing the lenses for 2 weeks) improved mobility compared to their mobility before using the lens.

Adolescents: why they can be so obnoxious.

This paper describes examples of adolescent behavior which parents and school personnel find obnoxious. It attempts to explain why intelligent, reasonable and well-adjusted teenagers periodically behave in this way, as something that is developmentally inevitable given the uneven development of competencies in several important areas. Adolescent incompetencies in four areas (cognitive, social cognition, moral judgment, and knowledge of the world) are discussed and exemplified by resulting behavior (including the current Cabbage Patch doll fad). Suggestions for dealing positively with the sometimes difficult and exasperating behavior of adolescents are offered.

Bound-cation exchange affects the lag phase in actin polymerization.

The delay or lag phase at the onset of polymerization of actin by neutral salt is generally attributed to an actin nucleation reaction. However, when nucleation is circumvented by the use of phalloidin-stabilized nuclei, a lag phase persists when Ca2+-containing actin is polymerized with MgCl2. Pretreatment of actin with ethylene glycol bis(beta-aminoethyl ether)-N,N,N',N'-tetraacetic acid (EGTA) and/or Mg2+ shortens or eliminates this lag phase, suggesting that exchange of the actin-bound divalent cation occurs during this nucleation-independent lag phase. Measurement of the actin-bound cation initially and after brief incubation with EGTA/Mg2+ directly verifies that Mg2+ has replaced Ca2+ as the actin-bound cation, producing a highly polymerizable Mg2+-actin species. Bound-cation exchange prolongs the lag phase in actin polymerization and probably explains what has been termed the monomer activation step in actin polymerization.

Submental liposuction extraction with hard chin augmentation.

Liposuction techniques utilizing fat extraction sculpturing of the neck and simultaneous chin augmentation are described herein. In young persons, extraction is carried out in the submental areas. In older patients, who have diffuse fat accumulations and less skin tone, extraction includes the submental and submandibular areas, along with dry tunneling to the lateral borders of the sternocleidomastoid muscle and inferiorly to the suprasternal notch. When the skin tone is poor, with excessive redundancy, combined rhytidectomy with both open and closed liposuction is performed. With liposuction submental sculpturing , necessity for chin augmentation is critical to tauten the submental skin and to give a better neck-chin angle. Hard chin implants are preferred over the soft gelatinous types. We utilized a hard acrylic implant ( Rish ) in all cases. Results obtained by the dual approach of submental fat suction sculpturing and hard chin augmentation have been excellent.
