A new assay for the evaluation of antiplatelet antibody in idiopathic thrombocytopenic purpura.

A new approach to the study of antiplatelet antibodies is described. Purified IgG from sera or from the culture of splenic ells was radiolabeled with 125I. Incubation of radiolabeled splenic IgG from ITP patients showed significantly greater binding to target platelets when compared to control IgG samples; conversely, the binding to target platelets of 125I-labeled serum IgG from ITP patients did not differ from controls. In each of the six instances when the binding of splenic and serum IgG from the same ITP patients was compared, the splenic samples contained a much higher concentration of platelet-specific IgG. This suggests that intrasplenic platelets in ITP patients are exposed in vivo to high concentrations of antiplatelet antibody and may explain, in part, the importance of the spleen in platelet destruction in this disease. This assay should prove useful in the further study of immune thrombocytopenia.

A survey of women with unplanned exnuptial pregnancies in Brisbane.

The social circumstances of women with exnuptial unplanned pregnancies were surveyed, and the differences between those continuing with their pregnancy and those who decided to terminate their pregnancy were compared. Women who continued their pregnancies were younger, poorer, less educated, and more likely to be employed in unskilled jobs when compared with those who decided to have their pregnancy terminated.

Sterilization: a comparative review.

Publications relating to surgical procedures for sterilization have been reviewed, and the incidences of complications and subsequent pregnancies compared. Laparoscopic sterilization has the lowest incidence of complication, the morbidity rate being lower than that of laparotomy sterilization or hysterectomy, and the mortality rate lower than that of a single pregnancy or taking oral contraceptives for 1 year.

Perinatal death: audit and classification.

Two hundred and thirteen perinatal deaths occurred in a population of 10,539 deliveries over a 4-year period. The associated obstetric complications and circumstances were analysed. The majority of perinatal deaths occurred in fetuses in whom there was a serious malformation, or whose birth weight was less than 800 g. Of the 110 deaths which occurred in normal babies weighing 800 g or more, 54 were antepartum, 5 were intrapartum, and 51 were neonatal. The clinical features surrounding these deaths were classified, and their implications discussed.

Control chart for urinary oestriol excretion during pregnancy.

Calculations to define the distribution of normal urinary oestriol excretion values during pregnancy are presented. The values had a normal distribution, uniform variance and increased linearly with time after logarithmic conversion. The variance could, therefore, be partitioned and a control chart constructed.

A comparison of obstetric morbidity and management between clinic and private patients.

The patterns of morbidity and management among clinic and private patients in a teaching hospital were compared by an audit of computerized case records. The clinic patients were more likely to have a poor obstetric history and antenatal complications, and the private patients were managed more actively. Perinatal mortality in the clinic patients was higher and was related to the higher frequency of low birth weight babies.

A controlled trial of puncture sites for amniocentesis.

A controlled trial to determine a better site for amniocentesis was carried out over a 6-month period, involving 98 obstetric patients. The suprapubic and nuchal puncture site for amniocentesis were compared. The failure rate was found to be lower when the suprapublic site was used, and no significant difference in morbidity was found in the two groups.

Acute traumatic tetraplegia during pregnancy.

A case of acute traumatic tetraplegia during pregnancy resulting in maternal death is presented, and problems of management are discussed. Difficulties experienced by the mother were intractable urinary tract infection and respiratory insufficiency; those in the neonate were prematurity and the effects of drugs administered to the mother.

Typical and atypical ('silent') subacute thyroiditis in a wife and husband.

Typical subacute thyroiditis was diagnosed in a woman. Three weeks later, signs and symptoms of hyperthyroidism developed in her husband. Although the right lobe of his thyroid gland was slightly enlarged, pain and tenderness were absent throughout the course of his illness. The free thyroxine equivalent (FTE) value and the sedimentation rate were elevated; the low uptake of radioactive iodine by the thyroid gland was consistent with "silent" subacute thyroiditis. We postulate that a common etiolgoy, probably viral, was operative in both cases. Nine additional cases of hyperthyroidism with low levels of thyroidal uptake of radioactive iodine are described. The thyroid glands of these patients were normal or slightly enlarged. Antithyroglobulin antibody levels determined in seven patients were not substantially elevated. The clinical course of these patients was characteristic of "silent" subacute thyroiditis. Although the origin of the syndrome remains unclear, the disease is self-limited and therapy, if any, is supportive.

Refresher course for general practitioner obstetricians: report of a survey on needs and expectations.

A survey of 52 general practitioner obstetricians attending a refresher course was carried out, the aims of which were to assess their motivation for attending, their expections of the course, and whether the course fulfilled these expectations. The main motivations for attending were interest and preparation for the Dip Obst RCOG examination. The main expectations were to be acquainted with current developments and to have tuition in practical procedures. The general practitioner obstetricians were generally satisfied with the course; however, they would have preferred a greater emphasis on informal tutorials rather than formal lectures, and practical procedures rather than recent advances in physiology and neonatology.

Elevated platelet-associated IgG in thrombotic thrombocytopenic purpura.

A 41-year--old woman with thrombotic thrombocytopenic purpura (TTP) was treated with prednisone and splenectomy with complete recovery from her illness. Analysis of the patient's platelets for immunoglobulins showed abnormally elevated levels in the range of those seen in thrombocytopenic patients with systemic lupus erythematosus, immune vasculitis, and infectious mononucleosis. This finding is consistent with the concept that the immune system may play a role in the pathogenesis of TTP.
