Significance of pharyngeal colonization with aerobic gram-negative bacilli in elderly institutionalized men.

The prevalence of pharyngeal colonization with aerobic Gram-negative rods was studied on three occasions during one year in 68 elderly males resident on two long-term care wards. For the three surveys, pharyngeal carriage was identified in 43%, 43%, and 37% of residents. Each resident tended to remain either colonized or noncolonized. Pharyngeal colonization was associated with resident incontinence, the development of infection, and increased mortality during the year of observation.

Correlation between prostaglandin and thromboxane levels and clinical signs in cyclosporine A-treated guinea pigs with acute experimental allergic encephalomyelitis.

Prostaglandin E, prostaglandin F2 alpha and thromboxane B2 levels were measured in the spinal cord and cerebellum of guinea pigs with acute experimental allergic encephalomyelitis treated with Cyclosporine A in a schedule which delayed the onset of the disease and completely suppressed it in 40% of the animals. A correlation was found between the eicosanoid levels in the central nervous system and clinical signs of the disease. Where lesions were completely suppressed the levels of eicosanoids were normal. The number of lesions found correlated well with the clinical state of the animals.

Antimicrobial cream susceptibility testing.

Utilizing a recently described rapid needle extrusion method for determining the sensitivities of burn wound isolates, we have compiled data on the resistance patterns of over 250 isolates from our burn unit. Major isolate groups were Staphylococcus aureus, Pseudomonas aeruginosa, various Enterobacteriaceae and Enterococci. Excellent correlation was exhibited between the inhibitory zone sizes and the minimal inhibitory concentration for 120 organisms tested. Utilizing the needle extrusion sensitivities to facilitate the selection of the burn creams, a significant reduction in the microbiologic flora of the burn wound was noted. The utilization of this technique in the selection of burn creams deserves controlled trials to assess whether changes in topical therapy might alter clinical outcome.

Randomized, double-blind comparison of cefotaxime, cefoxitin, cefazolin or placebo as prophylaxis during gastric, small bowel or complicated biliary surgery.

In a double-blinded trial, 142 patients were randomized to receive placebo, cefazolin, cefoxitin or cefotaxime during high-risk gastroduodenal, biliary tract or small bowel surgery. Of the 125 evaluable patient trials, postoperative wound infection or intra-abdominal sepsis developed in 14/29 (48%) of placebo controls, 8/24 (33%) cefazolin, 7/37 (19%) cefoxitin, and in 2/35 (6%) of cefotaxime recipients. When patients undergoing vagotomy and pyloroplasty with prior cimetidine therapy were excluded, cefotaxime was superior to cefoxitin prophylaxis (2/26 versus 7/28, P = 0.014). Cefotaxime may be useful as a prophylactic agent in high-risk upper gastrointestinal surgery.

Consumption of coffee or tea and symptoms of anxiety.

The relationship of consumption of coffee or tea to self-reported symptoms of anxiety is examined with data from the detailed examination component of the National Center for Health Statistics Health and Nutrition Examination Survey. Among this nationwide sample of 3,854 respondents, there was no significant association between consumption of coffee or tea and symptoms of anxiety.

Autoimmunity in ocular and generalised myasthenia gravis.

Restricted ocular myasthenia gravis (OMG) and generalised myasthenia gravis (GMG) have been shown to differ in a number of respects. In OMG, antiacetylcholine receptor, antistriational and antinuclear antibodies were rare relative to their frequency in GMG. In contrast, antithyroid antibodies and a history of thyroid disease were much more prevalent in OMG than in GMG, OMG was not associated with the female predominance seen in GMG and appeared to be relatively common in some races rather than others. It is suggested that different pathogenetic mechanisms are responsible for these two forms of MG.
