Preoperative embolization of renal tumors--comparison of different methods.

Different techniques and materials for preoperative embolization of renal carcinoma have been assessed in 26 cases. The different methods used and compared were gelfoam fragments (10 cases), cyanoacrylate (4 cases), Gianturco stainless steel coil (3 cases) and the combination of gelfoam and the Gianturco device (9 cases). The method of choice appears to be the combination of gelfoam and the Gianturco device which allows excellent peripheral ischemia with complete occlusion of the main arterial trunks. Embolization is particularly useful in invading and hypervascularized renal tumors supplied by collateral blood vessels.

Acquired supravalvular aortic stenosis.

Severe supravalvular aortic stenosis was discovered at reoperation in 3 children with recurrent stenosis of the left ventricular outflow tract. The lesion consisted of gross thickening of the sinus ridge, most marked at the site of the previous aortotomy. The thickening had shortened the free edge of one or more leaflets and had distorted adjacent commissures. Preoperative angiography had indicated valve stenosis, but little commissural fusion was found at operation and the outflow tract obstruction was relieved by inserting a prosthetic patch in the ascending aorta. Recognition of this iatrogenic pathology is important to avoid supravalvular stenosis after aortic valvotomy and unnecessary valve replacement in children requiring reoperation for recurrent left ventricular outflow tract obstruction.

Effect in vitro of ioglycamide on blood proteins and paraproteins.

No modification of plasma or serum viscosity was observed after in vitro addition of ioglycamide in a series of 160 healthy subjects and 1 467 patients including 33 cases of IgM paraproteinemia, 20 with Waldenström's disease. The labelled contrast medium never bound to paraproteins. These results are contradictory to the statements in the literature, that plasma gel formation occurs in Waldenström's disease.

Sagittal mobility of the normal ankle.

In children and adults, there is a physiological anterior sagittal displacement of the talus which decreases when the examination is repeated in equinus. The displacement occurs when the examination is performed with or without apparatus, with manual stress or with weights. The diminution of the normal sagittal mobility of the talus when the examination is performed with the ankle in the position of equinus, it attributable to the intact anterior talofibular ligament which becomes taut when the ankle is in a position of flexion. A pathological drawer sign will be noted following an isolated tear of the anterior talofibular; under those circumstances sagittal instability is increased and is not appreciably affected by repeating the examination in a position of ankle equinus.
