The results of carotid angiography in 304 cases of cerebral hemisphere infarction have been reviewed. Carotid occlusion was demonstrated in 19% and atheromatous vessel wall change in a further 28%. Hypertensive subjects showed a significantly lower prevalence of carotid occlusion, but a similar prevalence of vessel wall change. There was more likely to be angiographic abnormality if there was a neck bruit or if the ECG showed ischaemic changes. The findings support the concept that the pathogenesis of cerebral infarction differs in normotensive and hypertensive patients.

Cognitive function was evaluated in 11 patients with Huntington's chorea, using eight subtests of the WAIS, and their performance was compared with that previously reported for a normal aged population. The pattern of impairment was similar in the two groups. Tests of immediate memory were given to nine of these patients, and to 10 healthy but aged subjects. In both groups, there was a significant correlation between the performance of individuals in these tests and their IQ, suggesting that the choreic patients had no selective impairment of immediate memory. The clinical and psychological records of the 11 patients revealed no evidence of focal deficits, and the majority of patients were fully orientated and retained insight to their condition. These observations are discussed in relation to the pattern of intellectual impairment seen in other presenile dementias.

Suprathreshold fundus lesions produced by ruby and argon laser photocoagulation were studied within 24 hours by light and electron microscopy. It was shown that damage was maximal in the outer retina in all ruby laser lesions and extramacular argon laser lesions. In both monkey and human, inner retinal damage occurred independently of outer retinal damage in macular lesions produced by the argon laser. In lesions produced by equal energy, inner retinal damage was more severe in humans than in monkeys. In both species outer retinal damage was less severe in the foveal than the parafoveal region and this disparity was greater in humans than in monkeys. These findings are important to the therapeutic use of argon laser energy for mascular disease. In particular, absortion of energy in the inner retina reduces the energy available in the treatment of subretinal lesions in the foveal area, and causes unwanted neuroretinal damage. The higher sensitivity to argon laser irradiation of the human fovea compared with the monkey fovea, has not been appreciated when defining laser safety limits.
Computerized axial tomography using the EMI scanner as a new method of using x-rays in diagnosis. The technique displays intracranial and orbital structures in the transverse plane. The appearances of normal EMI Scans are described and correlated with cerebral and orbital anatomy seen in transverse section.
