Chronic lymphocytic leukemia with gamma chain cytoplasmic inclusions.

A case of lymphocytosis diagnosed clinically as chronic lymphocytic leukemia is described. The lymphocytes possessed cytoplasmic inclusions that contained IgG-kappa immunoglobulin. Studies of immunoglobulin synthesis showed that the cells synthesized but did not secrete the immunoglobulin. The migration on poly.acrylamide gels of the IgG heavy chain was anomalous, and it is proposed that this interfered with its normal secretion and allowed the development of cytoplasmic inclusions within the cell. These findings are unique when compared with previous studies of immunoglobulin biosynthesis by normal or leukemic peripheral blood lymphocytes.

The radioimmunoassay of 3,3',5' - triiodothyronine (reverse T3) in unextracted human serum.

A specific, sensitive and simple double antibody radioimmunoassay for total serum 3,3',5'-triiodothyronine (reverse T3, rT3) in small volumes of unextracted human serum is described. High titre antisera were raised in rabbits using dl=rT3 or l-rT3 conjugated to bovine serum albumin. The selected antisera cross reacted less than 0-003% with triiodothyronine (T3) and 0-14% with thyroxine (T4). A stable high activity rT3 tracer was prepared by iodination of 3,3"-diiodo-l-thyronine by the chloramine-T method, and purified by column chromatography on Sephadex LH-20. Binding of rT3 to endogenous serum proteins was blocked by including 8-anilino-1-naphthalene sulphonic acid (ANS) in the assay. Mean rT3 levels in healthy euthyroid adults are 0-27 nmo1/1 (range 0-21-1-18); in cord sera 3-67 nmo1/1 (range 2-30-7-45) and in amniotic fluid taken during the second trimester 4-70 nmo1/1 (range 2-22-8-00). In approximately half the hypothyroid subjects, rTO levels were undetectable ( less than0-05 nmo1/1) and in the remaining subjects the mean rT3 level was 0-10 nmo1/1 (range 0-05-0-25).

Measurement of regional cerebral blood volume by computerized axial tomography.

Computerized tomography has been used for the estimation of regional cerebral blood volume in a group of 18 patients. Sodium iothalamate was injected intravenously (1.75 ml/kg) to increase the absorption of x-rays in the cerebral circulation. A mean value of 5.7 ml/100 g was found for the cortex and 5.1 for tha thalamus.

Computerized tomography (the EMI Scanner): a comparison with pneumoencephalography and ventriculography.

Computerized tomography, using the EMI Scanner, allows the diagnosis of cerebral atrophy or hydrocephalus to be made with the same degree of accuracy as conventional neuroradiological methods. Ventricular measurements made on EMI scans have been compared with those from pneumoencephalograms and ventriculograms. A range of normal ventricular measurements for the EMI scan is suggested.

Cerebrovascular carbon dioxide reactivity and conductance in patients awake and under general anesthesia.

Paired cerebral blood flow measurements at two levels of arterial carbon dioxide (CO2) tension were made in 36 awake patients and 85 patients under general anesthesia to measure the percent change in flow per mm Hg change in PaCO2 (CO2 reactivity). CO2 reactivity was significantly greater in the generally anesthetized (5.96) than in the awake group (3.65). In both groups, a linear correlation was found between mean percent CO2 reactivity and conductance (the reciprocal of resistance), up to a level of conductance of 0.85 in awake and 0.95 in unconscious patients. When the CO2 reactivity of 18 awake patients and 59 unconscious patients was compared with that of respective control groups, only 11 patients were found to have abnormal CO2 reactivities for their conductance levels.

Computer subtraction in regional cerebral blood-volume measurements using the EMI-Scanner.

A computerized subtraction technique has been described to measure regional cerebral blood volume (rCBV) using the EMI-Scanner in a group of 13 patients. Sodium iothalamte was injected intravenously (1-75 ml./kg) to increase the absorption of X rays in the cerebral circulation. Significant regional differences in CBV were shown, values in the frontal and temporal regions being lower than the mean hemisphere value (4-9 +/- 0-7) and higher in the occipital region. The left hemisphere showed a significantly higher CBV when compared with the right. Measurements of CBV in the cortex showed no regional variation, but the mean cortical value of 6-0 +/- 1-8 was significantly higher than the hemisphere mean.

Survey of dentists on the impact of the United Auto Workers/auto industry dental plan.

Survey responses showed little change in dental office staffing and paperwork as a result of the UAW/auto industry program. Only slight differences between respondents in the four states were apparent; except for Michigan, where the greatest increase in staff and additional clerical hours was reported. The differences that did exist might be related to the volume of UAW/auto industry business in each area and the dentists' opinions of the carriers they relate to most frequently. The responses did not reveal a clear indication of success or failure of the plan by virture of any category evaluated.

A comparison of computer asisted tomography (EMI scanner) with conventional neuroradiologic methods in the investigation of patients clinically suspected of intracranial tumor.

Computerized tomography has been used as the initial radiologic investigation among 212 intracranial tumor suspects. Ninety-five of these patients were ultimately shown to have tumors as a result of all their investigations. Eighty tumors were clearly defined by EMI scanning, four further scans were suspicious but not unequivocally abnormal and six scans showed complications of the tumor rather than the lesion itself. Thus, only five scans were falsely negative in the presence of a tumor. The false positive and false negative rates for EMI scanning have been compared with conventional techniques and computerized tomography shown to compare favourably with angiography or encephalography.

Comparison of E.M.I. and radioisotope imaging in neurological disease.

A comparison has been made between the information provided by isotope imaging--before and after the introduction of E.M.I.-scanning--and E.M.I.-scanning itself. E.M.I.-scanning is superior to isotope imaging (also called scintigraphy) with the gamma camera and the rectilinear scanner in the amount and quality of information provided.

Indications for angiography and surgery in carotid artery disease.

The results of angiographic investigation of 211 patients suffering from transient cerebral ischaemic attacks in the carotid territory have been reviewed. The greatest proportion of patients with carotid stenosis and who were referred for endarterectomy were those with a neck bruit ipsilateral to the affected hemisphere. Ocular involvement (amaurosis fugax) was associated with a still higher prevalence of angiographic abnormality, and carotid occlusion was commonest in this group. The implications for the management of patients with transient ischaemic attacks are discussed.

Cerebral blood flow in dementia.

Twenty-four patients of comparable age, blood pressure, and degree of dementia were classified by an "Ischemic Score" based on clinical features into "multi-infarct" and "primary degenerative" dementia. Regional cerebral blood flow (CBF) was measured by the intracarotid xenon 133 method. Both groups showed a decreased proportion of rapidly clearing brain tissue (largely gray matter). Cerebral blood flow per 100 gm brain per minute was normal in the primary degenerative group but low in the multi-infarct group. This suggests the blood flow is adequate for metabolic needs of the brain in patients with primary degenerative dementia but inadequate for those with multi-infarct dementia. There was no correlation between degree of dementia and CBF in the primary degenerative group but an inverse relationship existed in the multi-infarct group. Reactivity of blood vessels to reduction of arterial carbon dioxide pressure was normal in both groups.
