Itch following photochemotherapy for psoriasis.

Itch was measured quantitatively as nocturnal scratch in 12 patients with psoriasis treated with 8-methoxypsoralen and UVA and in 7 treated with dithranol. Three of those treated with PUVA showed an increase in nocturnal limb movement which was mostly due to itch but partly due to restlessness. There was little change in nocturnal limb movement in patients treated with dithranol.

Effect of growth hormone administration: reciprocal changes in serum 1 alpha,25-dihydroxyvitamin D and intestinal calcium absorption.

The mechanism by which GH stimulates intestinal calcium absorption was explored. In this study, seven children with idiopathic GH deficiency were studied before and during GH therapy while they were maintained on the same metabolic diet. After 5-14 months of GH administration, fractional intestinal calcium absorption increased from 0.364 +/- 0.114 to 0.449 +/- 0.159 (P less than 0.01), and serum 1,25-dihydroxyvitamin D decreased from 63.7 +/ 19.3 to 46.7 +/ 17.8 pg/ml (P less than 0.025). Serum immunoreactive parathyroid hormone, urinary cAmp, serum 24,25-dihydroxyvitamin D, and 25-hydroxyvitamin D did not change significantly. The data suggest that GH does not directly influence parathyroid hormone or vitamin D metabolism. The possibility that relative intestinal resistance to 1,25-dihydroxyvitamin D exists in patients with GH deficiency is considered.

The benzodiazepines--use and abuse.

The benzodiazepines, discovered and developed during the past 20 years, have become one of the most widely used group of drugs in the world at the present, some have suggested too widely used. The extent of therapeutic use is examined relative to the incidence of disorders associated with anxiety and it is concluded that, in general, the benzodiazepines are used conservatively by doctors rather than overused, and that their current consumption is not excessive relative to the level of emotional morbidity in the community. However, as with all psychoactive substances, dependence, both psychological and physical, can occur. This is of the alcohol-barbiturate type, though of less severity. The incidence of dependence under both therapeutic and abuse circumstance is quantified and found to be very low. The social significance of this level of dependence is examined relative to both the overall reliance of society on pleasure-giving activities, and the puritan ethics still applicable to mental illnesses. Conclusions are drawn about appropriate action that might be taken to reduce dependence.

Comparison of photochemotherapy and dithranol in the treatment of chronic plaque psoriasis.

A two-centre trial has been carried out on 224 patients with chronic plaque psoriasis randomly allocated to treatment with a standard dithranol regimen of 8-methoxypsoralen and long-wave ultraviolet light (P.U.V.A.). Lesions in 91% of the 113 in the P.U.V.A. group cleared satisfactorily compared with 82% of 111 in the dithranol group, but clearing took longer (34.4 +/- 1.8 S.E. days) with P.U.V.A. than with dithranol (20.4 +/- 0.9 S.E. days). P.U.V.A. treatment took less patient-time and nurse-time and was more convenient and acceptable to the patients. Patients in whom lesions had failed to clear with dithranol, and some who had needed methotrexate for control, responded satisfactorily to P.U.V.A. A few patients who had failed on P.U.V.A. were treated with dithranol and responded to it. There is a case for the use of P.U.V.A. for patients who would otherwise require methotrexate and those who cannot be managed successfully with dithranol. There is also no reason to withhold P.U.V.A. in patients of 60 years or above with chronic plaque psoriasis. However, despite its superiority in terms of cost and patient acceptability, P.U.V.A. cannot be recommended as the first line of treatment for patients with uncomplicated, dithranol-responsive plaque psoriasis until there is more information on relapse-rate and toxicity.

Cytokinetic studies of crypts in convoluted human small-intestinal mucosa.

Forty-seven peroral biopsy specimens of duodenojejunal mucosa showing convolutions were obtained from patients with a variety of clinical disorders. These mucosal samples were divided into three groups according to the extent of the convolutions and the severity of the accompanying histopathological changes; the cytokinetic status of the crypts in the three groups was then analysed and compared. Those in which the mucosae were predominantly or totally convoluted (group 3) showed the most notable cytokinetic changes: crypts were hyperplastic and crypt cell production rate was markedly increased compared with the other two groups and with morphologically normal control mucosae. In the case of one patient with mucosal changes of group 3 severity, additional studies were carried out using vincristine to produce metaphase arrest. The cell cycle time of 27 hours was greatly shortened compared with a control value of 45 hours.We find that the presence of convolutions in small-intestinal mucosal biopsy specimens is accompanied by an increase in the rate of cell production from the crypts, which is presumably related directly or indirectly to the rate of loss of mature enterocytes from the surface of the mucosa. Furthermore, an increase in the proportion of convolutions may reflect an increase in the rates of cell production and cell loss. In the group 3 convoluted mucosae the cytokinetic profile of the crypts resembled that of some of the flat avillous coeliac mucosae previously studied although the rates of cell production did not reach the levels attained by the most productive of the flat coeliac mucosae.

Regulation of gonadotropin secretion in Turner's syndrome.

To determine the role of body fat in regulating secretion of luteinizing hormone and follicle-stimulating hormone, we measured both at 20-minute intervals for 24 hours in eight children with Turner's syndrome. The 24-hour mean luteinizing hormone levels varied from 20.2 to 70.5 mlU per milliliter. Total body weight, total body fat and percentage of body fat showed a significant negative correlation with the 24-hour mean luteinizing hormone concentrations (P less than 0.01). The 24-hour mean follicle-stimulating hormone concentrations ranged from 60.4 to 229 mlU per milliliter, with a significant negative correlation between total body fat and percentage body fat and the 24-hour mean concentrations (P less than 0.05). These negative correlations were not mediated by estrogens or androgens.

Dermatoglyphics in dermatitis herpetiformis.

In a dermatoglyphic study of 101 patients with dermatitis herpetiformis, comparison was made with their normal relatives and several random normal series. The results of the several comparisons are quite consistent, considering the nature of the data, for the all point to an attenuation of qualitative and quantitative digital and palmar traits in the patients. It is argued that these differences are associated with the disease itself, and that some of the female relatives have an inherited tendency to the disorder, but do not express it. The findings suggest the involvement of genetic factors in the aetiology, and possibly intrauterine environmental influences as well.
